Since 1981, the Federal Railroad Administration (FRA) has annually prepared a National Inspection Plan (NIP) whose purpose is to summarize regional efforts to improve railroad transportation safety. The research concluded on the following tasks including the problems, conclusions and recommendations associated with these tasks is summarized: (1) the preparation of the 1983 NIP, with recommended procedures for improving future NIPs; (2) the development of an outline for the 1984 NIP, including a methodology for the allocation of inspection resources and other specialized regional activities; (3) the management and development of the 1984 NIP; and (4) the development of an instruction manual to be used in the preparation of future NIPs.

State Departments of Transportation (S-DOT's) periodically allocate budget for safety upgrades at railroad-highway crossings. Efficient resource allocation is crucial for reducing accidents at railroad-highway crossings and increasing railroad as well as highway transportation safety. While a specific method is not restricted to S-DOT's, sorting type of procedures are recommended by the Federal Railroad Administration (FRA), United States Department of Transportation for the resource allocation problem. In this study, a generic mathematical model is proposed for the resource allocation problem for railroad-highway crossing safety upgrades. The proposed approach is compared to sorting based methods for safety upgrades of public at-grade railroad-highway crossings in Tennessee. The comparison shows that the proposed mathematical modeling approach is more efficient than sorting methods in reducing accidents and severity. Copyright © 2012 Elsevier Ltd. All rights reserved.

Many people are involved in accidents every year at railroad crossings, but there is no suitable sensor for detecting pedestrians. We are therefore developing a ubiquitous stereo vision based system for ensuring safety at railroad crossings. In this system, stereo cameras are installed at the corners and are pointed toward the center of the railroad crossing to monitor the passage of people. The system determines automatically and in real-time whether anyone or anything is inside the railroad crossing, and whether anyone remains in the crossing. The system can be configured to automatically switch over to a surveillance monitor or automatically connect to an emergency brake system in the event of trouble. We have developed an original stereovision device and installed the remote controlled experimental system applied human detection algorithm in the commercial railroad crossing. Then we store and analyze image data and tracking data throughout two years for standardization of system requirement specification.

Diesel exhaust has been suspected to be a lung carcinogen. The assessment of this lung cancer risk has been limited by lack of studies of exposed workers followed for many years. In this study, we assessed lung cancer mortality in 54,973 U.S. railroad workers between 1959 and 1996 (38 years). By 1959, the U.S. railroad industry had largely converted from coal-fired to diesel-powered locomotives. We obtained work histories from the U.S. Railroad Retirement Board, and ascertained mortality using Railroad Retirement Board, Social Security, and Health Care Financing Administration records. Cause of death was obtained from the National Death Index and death certificates. There were 43,593 total deaths including 4,351 lung cancer deaths. Adjusting for a healthy worker survivor effect and age, railroad workers in jobs associated with operating trains had a relative risk of lung cancer mortality of 1.40 (95% confidence interval, 1.30-1.51). Lung cancer mortality did not increase with increasing years of work in these jobs. Lung cancer mortality was elevated in jobs associated with work on trains powered by diesel locomotives. Although a contribution from exposure to coal combustion products before 1959 cannot be excluded, these results suggest that exposure to diesel exhaust contributed to lung cancer mortality in this cohort. The cabin safety demonstration on board an aircraft is one of the methods to provide safety information for passengers before aircraft takeoff. However, passengers' enthusiasm toward safety demonstrations is normally low. Therefore, the study of passengers' perception toward safety briefings on board an aircraft is important in increasing the safety awareness for the travelling public on commercial aircraft. A survey was distributed to measure the perceptions of Middle Tennessee State University (MTSU) faculty and staff, Aerospace students, and international students who have traveled in the last year. It was generally found that watching the cabin safety demonstration before aircraft takeoff was believed to be important for passengers. However, the attention to the safety demonstration remained low because the safety briefings were not good enough in terms of clear communication, particularly in the recorded audio demonstration and the live safety demonstration methods of briefing.

A significant portion of railroad infrastructure exists in areas that are relatively remote. Railroad crossings in these areas are typically only marked with reflective signage and do not have warning light systems or crossbars due to the cost of electrical infrastructure. Distributed sensor networks used for railroad track health monitoring applications would be useful in these areas, but the same limitation regarding electrical infrastructure exists. This motivates the search for a long-term, low-maintenance power supply solution for remote railroad deployment. This paper describes the development of a mechanical device for harvesting mechanical power from passing railcar traffic that can be used to supply electrical power to warning light systems at crossings and to remote networks of sensors via rechargeable batteries. The device is mounted to and spans two rail ties such that it directly harnesses the vertical displacement of the rail and attached ties and translates the linear motion into rotational motion. The rotational motion is amplified and mechanically rectified to rotate a PMDC generator that charges a system of batteries. A prototype was built and tested in a laboratory setting for verifying functionality of the design. Results indicate power production capabilities on the order of 10 W per device in its current form. This is sufficient for illuminating high-efficiency LED lights at a railroad crossing or for powering track-health sensor networks.

Randomized clinical trials are commonly overseen by a Data and Safety Monitoring Board comprised of experts in medicine, ethics, and biostatistics. Data and Safety Monitoring Board responsibilities include protocol approval, interim review of study enrollment, protocol compliance, safety, and efficacy data. Data and Safety Monitoring Board decisions can affect study design and conduct, as well as reported findings. Researchers must incorporate Data and Safety Monitoring Board oversight into the design, monitoring, and reporting of randomized trials. Case study, narrative review. The Data and Safety Monitoring Board's role during the comparative pediatric Critical Illness Stress-Induced Immune Suppression (CRISIS) Prevention Trial is described. The National Institutes of Health-appointed CRISIS Data and Safety Monitoring Board was charged with monitoring sample size adequacy and feasibility, safety with respect to adverse events and 28-day mortality, and efficacy with respect to the primary nosocomial infection/sepsis outcome. The Federal Drug Administration also requested Data and Safety Monitoring Board interim review before opening CRISIS to children below 1 yr of age. The first interim analysis found higher 28-day mortality in one treatment arm. The Data and Safety Monitoring Board maintained trial closure to younger children and requested a second interim data review 6 months later. At this second meeting, mortality was no longer of concern, whereas a weak efficacy trend of lower infection/sepsis rates in one study arm emerged. As over 40% of total patients had been enrolled, the Data and Safety Monitoring Board elected to examine conditional power and unmask treatment arm identities. On finding somewhat greater efficacy in the placebo arm, the Data and Safety Monitoring Board recommended stopping CRISIS due to futility. The Defense Nuclear Safeties Board (DNFSB) issued Recommendation 2000-2 on March 8, 2000, concerning the degrading conditions of vital safety systems, or systems important to nuclear safety, at DOE sites across the nation. The Board recommended that the DOE take action to assess the condition of its nuclear systems to ensure continued operational readiness of vital safety systems that are important for safely accomplishing the DOE's mission. To verify the readiness of vital safety systems, a two-phased approach was established. To contribute towards an understanding of hospital board composition and to explore board oversight of patient safety and health care quality in the English NHS. We reviewed the theory related to hospital board governance and undertook two national surveys about board management in NHS acute and specialist hospital trusts in England. The first survey was issued to 150 trusts in 2011/2012 and was completed online via a dedicated web tool. A total 145 replies were received (97% response rate). The second online survey was undertaken in 2012/2013 and targeted individual board members, using a previously validated standard instrument on board members' attitudes and competencies (the Board Self-Assessment Questionnaire). A total of 334 responses were received from 165 executive and 169 non-executive board members, providing at least one response from 95 of the 144 NHS trusts then in existence (66% response rate). Over 90% of the English NHS trust boards had 10-15 members. We found no significant difference in board size between trusts of different types (e.g. Foundation Trusts versus non-Foundation Trusts and Teaching Hospital Trusts versus non-Teaching Hospital Trusts). Clinical representation on boards was limited: around 62% had three or fewer members with clinical backgrounds. For about two-thirds of the trusts (63%), board members with a clinical background comprised less than 30% of the members. Boards were using a wide range and mix of quantitative performance metrics and soft intelligence (e.g. walk-arounds, patient stories) to monitor their organisations with regard to patient safety. The Board Self-Assessment Questionnaire data showed generally high or very high levels of agreement with desirable statements of practice in each of its six dimensions. Aggregate levels of agreement within each dimension ranged from 73% (for the dimension addressing interpersonal issues) to 85% (on the political). English NHS boards largely hold a wide range of attitudes and

Concern about the quality and safety of health care persists, 10 years after the 1999 Institute of Medicine report To Err is Human. Despite growing awareness of quality and safety risks, and significant efforts to improve, progress is difficult to measure. Hospital leaders, including boards and medical staffs, are accountable to improve care, yet they often address this duty independently. Shared responsibility for quality and patient safety improvement presents unique challenges and unprecedented opportunities for boards and medical staffs. To capitalize on the pressure to improve, both groups may benefit from a better understanding of their synergistic potential. Boards should be educated about the quality of care provided in their institutions and about the challenges of valid measurement and accurate reporting. Boards strengthen their quality oversight capacity by recruiting physicians for vacant board seats. Medical staff members strengthen their role as hospital leaders when they understand the unique duties of the governing board. A quality improvement strategy rooted in synergistic efforts by the board and the medical staff may offer the greatest potential for safer care. Such a mutually advantageous approach requires a clear appreciation of roles and responsibilities and respect for differences. In this article, we review these responsibilities, describe opportunities for boards and medical staffs to collaborate as leaders, and offer recommendations for how boards and medical staff members can address the challenges of shared responsibility for quality of care.

Dozens of people are killed every year when they fall off of train platforms, making this an urgent issue to be addressed by the railroads, especially in the major cities. This concern prompted the present work that is now in progress to develop a Ubiquitous Stereo Vision based system for safety management at the edge of rail station platforms. In this approach, a series of stereo cameras are installed in a row on the ceiling that are pointed downward at the edge of the platform to monitor the disposition of people waiting for the train. The purpose of the system is to determine automatically and in real-time whether anyone or anything is in the danger zone at the very edge of the platform, whether anyone has actually fallen off the platform, or whether there is any sign of these things happening. The system could be configured to automatically switch over to a surveillance monitor or automatically connect to an emergency brake system in the event of trouble.

Health and diseases on board ships may depend on water. Interventions to improve the quality of water may bring to significant benefits to health and water stores/supply and should be controlledto protect health. This paper has reviewed the main regulations for the control of water safety and qualityon board ships and presents some practical recommendations for keeping water healthy and safe in passenger and cargo merchant ships. The main international regulations and guidelines on the topic were analysed. Guidelines forWater Quality on Board Merchant Ships Including Passenger Vessels of Health Protection Agency, World Health Organisation (WHO) Guide to Ship Sanitation, WHO Guidelines for Drinking Water Quality, WHO Water Safety Plan and the United States Center for Disease Control and Prevention Vessel Sanitation Program were examined. Recommendations for passenger and, if available, for cargo ships were collected and compared. Recommended questionnaire: A questionnaire summarising the main information to collect for assessingthe enough quality of water for the purposes it should be used on board is proposed. The need of havinga crew member with water assessment duties on board, trained for performing these activities properlyis discussed. Water quality on board ships should be monitored routinely. Monitoring should be directedto chemical and microbiological parameters for identifying possible contamination sources, using specifickits by a designed crew member. More detailed periodic assessments should be under the responsibility ofspecialised personnel/laboratories and should be based on sample collection from all tanks and sites of waterdistribution. It is important to select a properly trained crew member on board for monitoring water quality.

The Energy Conservation Projects to benefit the railroad industry using the Norfolk Southern Company as a model for the railroad industry has five unique tasks which are in areas of importance within the rail industry, and specifically in the area of energy conservation. The NIU Engineering and Technology research team looked at five significant areas in which research and development work can provide unique solutions to the railroad industry in energy the conservation. (1) Alternate Fuels - An examination of various blends of bio-based diesel fuels for the railroad industry, using Norfolk Southern as a model for the industry. Themore » team determined that bio-diesel fuel is a suitable alternative to using straight diesel fuel, however, the cost and availability across the country varies to a great extent. (2) Utilization of fuel cells for locomotive power systems - While the application of the fuel cell has been successfully demonstrated in the passenger car, this is a very advanced topic for the railroad industry. There are many safety and power issues that the research team examined. (3) Thermal and emission reduction for current large scale diesel engines - The current locomotive system generates large amount of heat through engine cooling and heat dissipation when the traction motors are used to decelerate the train. The research team evaluated thermal management systems to efficiently deal with large thermal loads developed by the operating engines. (4) Use of Composite and Exotic Replacement Materials - Research team redesigned various components using new materials, coatings, and processes to provide the needed protection. Through design, analysis, and testing, new parts that can withstand the hostile environments were developed. This is the tenth Annual Report to the Congress describing Department of Energy activities in response to formal recommendations and other interactions with the Defense Nuclear Facilities Safety Board (Board). The Board, an independent executive-branch agency established in 1988, provides advice and recommendations to the Secretary of Energy regarding public health and safety issues at the Department's defense nuclear facilities. The Board also reviews and evaluates the content and implementation of health and safety standards, as well as other requirements, relating to the design, construction, operation, and decommissioning of the Department's defense nuclear facilities. During 1999, Departmental activities resulted inmore » the closure of nine Board recommendations. In addition, the Department has completed all implementation plan milestones associated with three Board recommendations. Hospital boards, those executive members charged with developing appropriate organisational strategies and cultures, have an important role to play in safeguarding the care provided by their organisation. However, recent concerns have been raised over boards' ability to enact their duty to ensure the quality and safety of care. This paper offers critical reflection on the relationship between hospital board oversight and patient safety. In doing so it highlights new perspectives and suggestions for developing this area of study. The article draws on 10 interviews with key informants and policy actors who form part of the 'issue network' interested in the promotion of patient safety in the English National Health Service. The interviews surfaced a series of narratives regarding hospital board oversight of patient safety. These elaborated on the role of trust and intelligence in highlighting the potential dangers and limitations of approaches to hospital board oversight which have been narrowly focused on a risk-based view of organisational performance. In response, a need to engage with the development of trust based organisational relationships is identified, in which effective board oversight is built on 'trust' characterised by styles of leadership and behaviours that are attentive to the needs and concerns of both staff and patients. Effective board oversight also requires the gathering and triangulating of 'intelligence' generated from both national and local information sources. We call for a re-imagination of hospital board oversight in the light of these different perspectives and articulate an emerging research agenda in this area.

Scaffold is widely used in high work-places inside and outside of a building construction site. It is inexpensive and is installed and dismantled easily. Although standards and ledgers of a steel tube and coupler scaffold are installed in a regular distance, the distances of transoms are not equal in some places. Sometimes a working platform or a board is absent in the corner of scaffold. This may cause safety accidents because a foothold is not stable on the transoms. An adjustable safety board and a rotational safety board are suggested in this paper. The adjustable board consists of two footholds. The small one is inserted into the large one. The rotational board covers not only right angle but also acute or obtuse angles. These safety boards for scaffold help to decrease safety accidents in construction sites.

Hospital Board Oversight of Quality and Patient Safety: A Narrative Review and Synthesis of Recent Empirical Research

Context Recurring problems with patient safety have led to a growing interest in helping hospitals’ governing bodies provide more effective oversight of the quality and safety of their services. National directives and initiatives emphasize the importance of action by boards, but the empirical basis for informing effective hospital board oversight has yet to receive full and careful review. Methods This article presents a narrative review of empirical research to inform the debate about hospital boards’ oversight of quality and patient safety. A systematic and comprehensive search identified 122 papers for detailed review. Much of the empirical work appeared in the last ten years, is from the United States, and employs cross-sectional survey methods. Findings Recent empirical studies linking board composition and processes with patient outcomes have found clear differences between high- and low-performing hospitals, highlighting the importance of strong and committed leadership that prioritizes quality and safety and sets clear and measurable goals for improvement. Effective oversight is also associated with well-informed and skilled board members. External factors (such as regulatory regimes and the publication of performance data) might also have a role in influencing boards, but detailed empirical work on these is scant. Conclusions Health policy debates recognize the important role of hospital boards in overseeing patient quality and safety, and a growing body of empirical research has sought to elucidate that role. This review finds a number of areas of guidance that have some empirical support, but it also exposes the relatively inchoate nature of the field. Greater theoretical and methodological development is required if we are to secure more evidence-informed governance systems and practices that can contribute to safer care. PMID:24320168

Railroad trespassers are persons on railroad property whose presence is prohibited or unlawful. Most trespassers are walking along or across railroad tracks. In 1997, fatalities to railroad trespassers became the leading cause of railroad-related deaths in the United States. In 1998, 513 persons were injured and 536 persons were killed while trespassing (Federal Railroad Administration, unpublished data, 1999). This report presents three incidents in which trespassers were injured or killed and summarizes a study of fatal and nonfatal injuries to railroad trespassers in Georgia from 1990 through 1996.

Historical estimation of diesel exhaust exposure in a cohort study of U.S. railroad workers and lung cancer.

We have previously shown an elevated risk of lung cancer mortality in diesel exhaust exposed railroad workers. To reduce exposure misclassification, we obtained extensive historical information on diesel locomotives used by each railroad. Starting in 1945, we calculated the rate each railroad converted from steam to diesel, creating annual railroad-specific weighting factors for the probability of diesel exposure. We also estimated the average annual exposure intensity based on emission factors. The U.S. Railroad Retirement Board provided railroad assignment and work histories for 52,812 workers hired between 1939-1949, for whom we ascertained mortality from 1959-1996. Among workers hired after 1945, as diesel locomotives were introduced, the relative risk of lung cancer for any exposure was 1.77 (95% CI = 1.50-2.09), and there was evidence of an exposure-response relationship with exposure duration. Exposed workers hired before 1945 had a relative risk of 1.30 (95% CI = 1.19-1.43) for any exposure and there was no evidence of a dose response with duration. There was no evidence of increasing risk using estimated measures of intensity although the overall lung cancer risk remained elevated. In conclusion, although precise historical estimates of exposure are not available, weighting factors helped better define the exposure-response relationship of diesel exhaust with lung cancer mortality.

The economic efficiency and competitiveness of environment-friendly rail transportation depends on safety, availability and maintenance of single highly loaded structure components. Until now these components have been changed in fixed maintenance intervals irrespective of any usage related conditions. With the knowledge and evaluation of the component conditions, life cycle costs can be reduced by means of optimized maintenance and/or "fit for purpose" design. For example, rail-bound vehicle wheel sets are among the most highly stressed travelling gear components of the bogie. if such a component fails, a serious accident may occur. For this reason, a health monitoring system based on the interpretation of ultrasonic sound signatures has been developed. First, the ultrasonic waves generated by an artificial defect on the outer wheel tread of a railroad wheel towards an acoustic sensor, placed inside the hollow shaft of the railroad axis were simulated with a EFIT (Elastodynamic Finite Integration Technique). The results achieved proved that relevant signals can be found in a frequency range up to 300 kHz. Based on this a diagnostic unit was designed and built for application under rotation conditions, which consists of a piezo-electric sensor, primary electronics, an analog-to-digital converter, a digital signal processor, a trigger unit, and a telemetric transmitter. This diagnostic unit was integrated in the hollow shaft of a railroad wheel axis, a component of a special laboratory test rig. Algorithms which allow for the rotation-synchronized processing of acoustic signals were implemented into the rotating diagnostic unit. After successfully completing a campaign for this test rig, a second test was performed inside the wheel/railroad simulation test rig of the Deutsche Bahn AG under railroad-like conditions. The data generated inside the hollow shaft of the railroad wheel axis by the diagnostic unit were telemetrically transmitted to an industrial computer

Chronic Obstructive Pulmonary Disease Mortality in Diesel-Exposed Railroad Workers

Diesel exhaust is a mixture of combustion gases and ultrafine particles coated with organic compounds. There is concern whether exposure can result in or worsen obstructive airway diseases, but there is only limited information to assess this risk. U.S. railroad workers have been exposed to diesel exhaust since diesel locomotives were introduced after World War II, and by 1959, 95% of the locomotives were diesel. We conducted a case–control study of railroad worker deaths between 1981 and 1982 using U.S. Railroad Retirement Board job records and next-of-kin smoking, residential, and vitamin use histories. There were 536 cases with chronic obstructive pulmonary disease (COPD) and 1,525 controls with causes of death not related to diesel exhaust or fine particle exposure. After adjustment for age, race, smoking, U.S. Census region of death, vitamin use, and total years off work, engineers and conductors with diesel-exhaust exposure from operating trains had an increased risk of COPD mortality. The odds of COPD mortality increased with years of work in these jobs, and those who had worked ≥ 16 years as an engineer or conductor after 1959 had an odds ratio of 1.61 (95% confidence interval, 1.12–2.30). These results suggest that diesel-exhaust exposure contributed to COPD mortality in these workers. Further study is needed to assess whether this risk is observed after exposure to exhaust from later-generation diesel engines with modern emission controls. PMID:16835052

This is the seventh quarterly report on the progress of activities addressing safety issues associated with Hanford Site high-level radioactive waste tanks that contain ferrocyanide compounds. In the presence of oxidizing materials, such as nitrates or nitrites, ferrocyanide can be made to explode in the laboratory by heating it to high temperatures [above 285{degrees}C (545{degrees}F)]. In the mid 1950s approximately 140 metric tons of ferrocyanide were added to 24 underground high-level radioactive waste tanks. An implementation plan (Cash 1991) responding to the Defense Nuclear Facilities Safety Board Recommendation 90-7 (FR 1990) was issued in March 1991 describing the activities thatmore » were planned and underway to address each of the six parts of Recommendation 90-7. The governance of patient safety is a challenging concern for all health systems. Yet, while the role of executive boards receives increased scrutiny, the area remains theoretically and methodologically underdeveloped. Specifically, we lack a detailed understanding of the performative aspects at play: what board members say and do to discharge their accountabilities for patient safety. This article draws on qualitative data from overt non-participant observation of four NHS hospital Foundation Trust boards in England. Applying a dramaturgical framework to explore scripting, setting, staging and performance, we found important differences between case study sites in the performative dimensions of processing and interpretation of infection control data. We detail the practices associated with these differences--the legitimation of current performance, the querying of data classification, and the naming and shaming of executives--to consider their implications. © 2015 The Authors. Considering chance in quality and safety performance measures: an analysis of performance reports by boards in English NHS trusts.

Hospital board members are asked to consider large amounts of quality and safety data with a duty to act on signals of poor performance. However, in order to do so it is necessary to distinguish signals from noise (chance). This article investigates whether data in English National Health Service (NHS) acute care hospital board papers are presented in a way that helps board members consider the role of chance in their decisions. Thirty English NHS trusts were selected at random and their board papers retrieved. Charts depicting quality and safety were identified. Categorical discriminations were then performed to document the methods used to present quality and safety data in board papers, with particular attention given to whether and how the charts depicted the role of chance, that is, by including control lines or error bars. Thirty board papers, containing a total of 1488 charts, were sampled. Only 88 (6%) of these charts depicted the role of chance, and only 17 of the 30 board papers included any charts depicting the role of chance. Of the 88 charts that attempted to represent the role of chance, 16 included error bars and 72 included control lines. Only 6 (8%) of the 72 control charts indicated where the control lines had been set (eg, 2 vs 3 SDs). Hospital board members are expected to consider large amounts of information. Control charts can help board members distinguish signals from noise, but often boards are not using them. We discuss demand-side and supply-side barriers that could be overcome to increase use of control charts in healthcare. Published by the BMJ Publishing Group Limited. ...FRA is amending its hours of service recordkeeping regulations, to add substantive hours of service regulations, including maximum on-duty periods, minimum off-duty periods, and other limitations, for train employees (e.g., locomotive engineers and conductors) providing commuter and intercity rail passenger transportation. The new substantive regulations require that railroads employing such train employees analyze and mitigate the risks for fatigue in the schedules worked by these train employees, and that the railroads submit to FRA for its approval the relevant schedules and fatigue mitigation plans. This final rule also makes corresponding changes to FRA's hours of service recordkeeping regulation, to require railroads to keep hours of service records and report excess service to FRA in a manner consistent with the new substantive requirements. This regulation is authorized by the Rail Safety Improvement Act of 2008.

Reliable and safe urban system conditions have to be a crucial goal of ordinary planning activities. Among planning goals, priority must be given to indications relating to the safety levels to be achieved and to the amount of resources to be directed towards reducing the vulnerability of urban systems and therefore of the measures to be taken. Uban vulnerability cannot in fact be reduced to the sum of the vulnerability of single buildings or to the physical vulnerability of its various components. This research work consists of identifying those urban sub-areas that are important for safety in relation to natural risks, ambits that should be highlighted by means of permanent emergency notice boards/billboards. What are the hazard notices relating to all natural hazards and related risks? Where are they located? Are they clear and straightforward so that all residents and visitors are able to understand them, as it is already the case for road signs (or at least it should be)? What urban sub-areas are worth highlighting in relation to natural risks, acting for example as escape routes or meeting points? How is information for the public managed in order that people are immediately, easily and regularly notified? What is the relation of such signals to ordinary traffic signals? Research into the state of the art of permanent notice boards/billboards of this type, currently distinguished only by sporadic and local initiatives, aims at carrying out a census of and recognizing urban elements already considered as important for reducing the vulnerability of the urban system to different natural calamities and at providing new highlights as regards the identification of new ones. The next step is to work out a decision and common-language strategy for planning these elements and for their adequate signposting, so as to be able to live in the urban environment with awareness, safety and confidence, including with respect to more remote and therefore often neglected

Contributing factors of crash injury severity at public highway-railroad grade crossings in the U.S.

The Moving Ahead for Progress in the 21st Century (MAP-21) includes a separate program that supports safety improvements to reduce the number of fatalities and injuries at public highway-railroad grade crossings (HRGCs). This study identifies the significant factors affecting crash injury severity at public HRGCs in the United States. Crashes from 2009 through 2013 on 5,528 public HRGCs, extracted from the Federal Railroad Administration database, were used in the analysis. A comprehensive list of risk factors was explored. Examples include predictors related to geographic region of crash, geometry (e.g., area type and pavement marking type), railroad (e.g., warning device type and railroad class), traffic (e.g., train speed and vehicles annual average daily traffic "AADT"), highway user (e.g., driver age and gender), and environment (e.g., lighting and weather conditions). The study used the mixed logit model to better capture the complex highway user behavior at HRGCs. Female highway users were at higher risk of involvement in injuries and fatalities compared to males. Higher train speeds, very old drivers, open areas, concrete road surface types, and railroad equipment striking highway users before crash, were all found to increase the injury likelihood. On the other hand, young and middle-age drivers, non-passing of standing vehicles at HRGCs, industrial areas, and presence of warning bells were found to reduce injuries and fatalities. The mixed logit model succeeded in identifying contributing factors of crash severity at public HRGCs and potential countermeasures to reduce both fatalities and injuries are suggested. It is important to install warning bells at public HRGCs, especially at those with high number of injury and fatality crashes. A case-control study of lung cancer mortality in U.S. railroad workers in jobs with and without diesel exhaust exposure is reanalyzed using a new threshold regression methodology. The study included 1256 workers who died of lung cancer and 2385 controls who died primarily of circulatory system diseases. Diesel exhaust exposure was assessed using railroad job history from the US Railroad Retirement Board and an industrial hygiene survey. Smoking habits were available from next-of-kin and potential asbestos exposure was assessed by job history review. The new analysis reassesses lung cancer mortality and examines circulatory system disease mortality. Jobs with regular exposure to diesel exhaust had a survival pattern characterized by an initial delay in mortality, followed by a rapid deterioration of health prior to death. The pattern is seen in subjects dying of lung cancer, circulatory system diseases, and other causes. The unique pattern is illustrated using a new type of Kaplan-Meier survival plot in which the time scale represents a measure of disease progression rather than calendar time. The disease progression scale accounts for a healthy-worker effect when describing the effects of cumulative exposures on mortality.

Applying an extended theory of planned behavior to predicting violations at automated railroad crossings.

Based on an extended Theory of Planned Behavior (TPB, Ajzen, 1985, 1991), we conducted surveys in order to explain and predict violations at a railroad crossing, among pedestrians (n=153) and car drivers (n=151). Measures were made with respect to three chronologically related railroad crossing situations that varied in risk level. The situations were described in scenarios and depicted on photographs. The participants were recruited in the suburbs of Paris, at two automated railroad crossings with four half-barriers. We found that the pedestrians had stronger crossing intentions than did car drivers, especially at the more congested crossing of the two under study. For both categories of road users, intentions and the amount of intention variance explained by the extended TPB factors decreased significantly with risk level. In the most dangerous situations, risk-taking was the most unlikely and the least predictable Self-reported past frequency of crossing against safety warning devices was the main predictor of the intention to commit this violation again, especially among males, followed by the attitude and the injunctive norm in favor the violation. Moreover, car drivers were influenced in their crossing intentions by the descriptive norm. The presence of another vehicle on the tracks when the safety warning devices were activated was perceived not as facilitating, but as an additional risk factor. The discussion addresses the importance of taking into account these determinants of violations in conceiving countermeasures. Our findings could be especially useful for conceiving risk-communication campaigns. Copyright © 2016 Elsevier Ltd. All rights reserved.
