To assess the total medical costs and productivity losses associated with the 1993 waterborne outbreak of cryptosporidiosis in Milwaukee, Wisconsin, including the average cost per person with mild, moderate, and severe illness, we conducted a retrospective cost-of-illness analysis using data from 11 hospitals in the greater Milwaukee area and epidemiologic data collected during the outbreak. The total cost of outbreak-associated illness was $96.2 million: $31.7 million in medical costs and $64.6 million in productivity losses. The average total costs for persons with mild, moderate, and severe illness were $116, $475, and $7,808, respectively. The potentially high cost of waterborne disease outbreaks should be considered in economic decisions regarding the safety of public drinking water supplies. PMID:12702221

Organic waste compounds (OWCs) are ingredients and by-products of common agricultural, industrial, and household substances that can contaminate our streams through sources like urban runoff, sewage overflows, and leaking septic systems. To better understand how OWCs are affecting Milwaukee-area streams, the U.S. Geological Survey, in cooperation with the Milwaukee Metropolitan Sewerage District, conducted a three-year study to investigate the presence and potential toxicity of 69 OWCs in base flow, stormflow, pore water, and sediment at 14 stream sites and 3 Milwaukee harbor locations. This fact sheet summarizes the major findings of this study, including detection frequencies and concentrations, potential toxicity, the prevalence of polycyclic aromatic hydrocarbons (PAHs), and the influence of urbanization.

According to the National Highway Traffic Safety Administration (NHTSA), while fatalities from traffic crashes have decreased, the proportion of pedestrian fatalities has steadily increased from 11% to 14% over the past decade. This study aims at identifying two zonal levels factors. The first is to identify hot zones at which pedestrian crashes occurs, while the second are zones where crash-involved pedestrians came from. Bayesian Poisson lognormal simultaneous equation spatial error model (BPLSESEM) was estimated and revealed significant factors for the two target variables. Then, PSIs (potential for safety improvements) were computed using the model. Subsequently, a novel hot zone identification method was suggested to combine both hot zones from where vulnerable pedestrians originated with hot zones where many pedestrian crashes occur. For the former zones, targeted safety education and awareness campaigns can be provided as countermeasures whereas area-wide engineering treatments and enforcement may be effective safety treatments for the latter ones. Thus, it is expected that practitioners are able to suggest appropriate safety treatments for pedestrian crashes using the method and results from this study. Copyright © 2015 Elsevier Ltd. Domestic violence is a prevalent problem with significant health consequences. Early recognition and appropriate intervention with referral to local domestic violence agencies can be life-saving. Little is known, however, about the current level of training, knowledge base and attitudes of physicians in this area. A survey was sent to 1300 physicians practicing in Milwaukee County in the following specialties: Family Practice, Internal Medicine, OB/GYN, Psychiatry. Demographic information was obtained. Questions were designed to explore attitudes towards domestic violence, frequency of encounters with victims or abusers, and knowledge of resources and appropriate intervention. Of the 192 respondents, 74% reported having some training in domestic violence. Thirty percent reported seeing victims in their practice on a daily or weekly basis. Seventy percent feel able to identify a victim of domestic violence. Less than a third of respondents screened at least half of the patients they see for the possibility of abuse. Less than half always refer victims to a hotline or shelter, and less than a quarter of the respondents discuss safety plans with victims. A potentially dangerous response is telling a victim not to go back to an abuser without providing referrals and safety supports. In spite of this, almost a quarter of respondents always tell a victim to not go back to the abuser. Family practitioners and psychiatrists were more likely to discuss abuse with patients than were internists. Significant numbers of physicians, in Milwaukee County, practicing certain specialties that potentially have a high rate of contact with domestic violence victims have had insufficient training in domestic violence assessment and intervention. Physicians should be familiar with the domestic violence hotlines and shelters in their communities and need to incorporate screen questions for domestic violence into their regular practice.

To address this deficit, this article aims to provide a comprehensive review of the existing extensive research efforts focused on work zone crash-related analysis and modeling, in the hopes of providing researchers and practitioners with a complete overview. Relevant literature published in the last 5 decades was retrieved from the National Work Zone Crash Information Clearinghouse and the Transport Research International Documentation database and other public digital libraries and search engines. Both peer-reviewed publications and research reports were obtained. Each study was carefully reviewed, and those that focused on either work zone crash data analysis or work zone safety modeling were identified. The most relevant studies are specifically examined and discussed in the article. The identified studies were carefully synthesized to understand the state of knowledge on work zone safety. Agreement and inconsistency regarding the characteristics of the work zone crashes discussed in the descriptive studies were summarized. Progress and issues about the current practices on work zone crash frequency and severity modeling are also explored and discussed. The challenges facing work zone safety research are then presented. The synthesis of the literature suggests that the presence of a work zone is likely to increase the crash rate. Crashes are not uniformly distributed within work zones and rear-end crashes are the most prevalent type of crashes in work zones. Every year numerous occupational fatalities result from pedestrians being struck by motor vehicles intruding into work zones. Attributes of retroreflective personal safety garments on pedestrian conspicuity at night were assessed in a field study. Using instrumented vehicles on a closed track, participants drove through simulated work zones attempting to detect pedestrians located in the work zones. Configuration of the retroreflective trim, trim color, placement in the work zone, and driver age significantly affected pedestrian conspicuity. Intensity and the amount of retroreflective trim did not. Personal safety garments incorporating retroreflective trim significantly improve pedestrian conspicuity in work zones. The results emphasize the importance of retroreflective trim on personal safety garments, particularly if the trim is located on garment sleeves. We examine the design attributes that contribute to making a personal safety garment conspicuous. The results have implications regarding preferred garment designs, industry standards, and service life of personal safety garments.

School Safety Zone is a location or region on particular roads that are time-based speed zone to set the speed of the vehicle in the school environment. The maximum speed limit permits entering a School Safety Zone, especially in Pekanbaru City is 25 km / h and an outline of the speed limit permit vehicles that pass through the School Safety Zone in Indonesia is generally 20-30 km / h. However, the vehicles speeds that pass School Safety Zone are higher than permit speeds.To ensure the level of vehicle offense across the territory of the School Safety Zone so it is necessary a primary data which is taken randomly based on field survey for 3 days at schools that has that facility ie SDN 3 Jalan Kesehatan Pekanbaru City, SDN 68 Jalan Balam Ujung Kota Pekanbaru and SDN 143 Jalan Taskurun Kota Pekanbaru. Furthermore, the data were taken in good condition that is at 6:30 to 7:30 am and at 12:00 to 13:00 pm. In addition, the data obtained is mileage and travel time of the vehicle. Both of these data can generate good speed value that passes through the area of School Safety Zone. Based on the research findings, the vehicle speed passing through the area of School Safety Zone is incompatible with speed permit at 35 km / h with a maximum average percentage of the rate of offense in the area of the school zone is 91.7%. This indicates that the vehicle passes School Safety Zone not following the rules of the maximum limit area and can be potentially harmful to elementary school students.

Extreme heat waves elevate the population's risk for heat-related morbidity and mortality, specifically for vulnerable groups such as older adults and young children. In this context, we developed 2 Heat Vulnerability Indices (HVIs), one for the state of Wisconsin and one for the Milwaukee metropolitan area. Through the creation of an HVI, state and local agencies will be able to use the indices as a planning tool for extreme heat events. Data used for the HVIs were grouped into 4 categories: (1) population density; (2) health factors; (3) demographic and socioeconomic factors; and (4) natural and built environment factors. These categories were mapped at the Census block group level. Unweighted z-score data were used to determine index scores, which were then mapped by quantiles ranging from "high" to "low" vulnerability. Statewide, Menominee County exhibited the highest vulnerability to extreme heat. Milwaukee HVI findings indicated high vulnerability in the city's inner core versus low vulnerability along the lakeshore. Visualization of vulnerability could help local public health agencies prepare for future extreme heat events.

A national study found that infants born in low socioeconomic areas had the worst infant mortality rates (IMRs) and the highest racial disparity. Racial disparities in birth outcomes are also evident in the city of Milwaukee, with African American infants at 3 times greater the risk than white infants. This study was conducted to examine the influence of socioeconomic status (SES) and race on birth outcomes in the city of Milwaukee. Milwaukee ZIP codes were stratified into lower, middle, and upper SES groups. IMR, low birth weight, and preterm birth rates by race were analyzed by SES group for the years 2003 to 2007. The overall IMR for the lower, middle, and upper SES groups were 12.4, 10.7, and 7.7, respectively. The largest racial disparity in IMR (3.1) was in the middle SES group, versus lower (1.6) and upper (1.8) SES groups. The overall percent of low birth weight infants for the lower, middle, and upper SES groups was 10.9%, 9.5%, and 7.5%, respectively. Racial disparity ratios in low birth weight were 2.0, 1.9, and 1.9 for lower, middle and upper SES groups. The overall percent of preterm birth was 15.4%, 13.2%, and 10.6% of births within the lower, middle, and upper SES groups, respectively, with a disparity ratio of 1.6 across all SES groups. For all outcomes, African American infants born in the upper SES group fared the same or worse than white infants born in the lower SES group. Although higher SES appeared to have a protective effect for whites in Milwaukee, it did not have the same protective effect for African Americans.
