Determining Primary Care Physician Information Needs to Inform Ambulatory Visit Note Display

Summary Background With the increase in the adoption of electronic health records (EHR) across the US, primary care physicians are experiencing information overload. The purpose of this pilot study was to determine the information needs of primary care physicians (PCPs) as they review clinic visit notes to inform EHR display. Method Data collection was conducted with 15 primary care physicians during semi-structured interviews, including a third party observer to control bias. Physicians reviewed major sections of an artificial but typical acute and chronic care visit note to identify the note sections that were relevant to their information needs. Statistical methods used were McNemar-Mosteller’s and Cochran Q. Results Physicians identified History of Present Illness (HPI), Assessment, and Plan (A&P) as the most important sections of a visit note. In contrast, they largely judged the Review of Systems (ROS) to be superfluous. There was also a statistical difference in physicians’ highlighting among all seven major note sections in acute (p = 0.00) and chronic (p = 0.00) care visit notes. Conclusion A&P and HPI sections were most frequently identified as important which suggests that physicians may have to identify a few key sections out of a long, unnecessarily verbose visit note. ROS is viewed by doctors as mostly “not needed,” but can have relevant information. The ROS can contain information needed for patient care when other sections of the Visit note, such as the HPI, lack the relevant information. Future studies should include producing a display that provides only relevant information to increase physician efficiency at the point of care. Also, research on moving A&P to the top of visit notes instead of having A&P at the bottom of the page is needed, since those are usually the first sections physicians refer to and reviewing from top to bottom may cause cognitive load. This slide presentation reviews several topics related to operating unmanned aircraft in particular sharing aspects of unmanned aircraft from the perspective of a pilot. There is a section on the Global Hawk project which contains information about the first Global Hawk science mission, (i.e., Global Hawk Pacific (GloPac). Included in this information is GloPac science highlights, a listing of the GloPac Instruments. The second Global Hawk science mission was Genesis and Rapid Intensification Process (GRIP), for the NASA Hurricane Science Research Team. Information includes the instrumentation and the flights that were undertaken during the program. A section on Ikhana is next. This section includes views of the Ground Control Station (GCS), and a discussion of how the piloting of UAS is different from piloting in a manned aircraft. There is also discussion about displays and controls of aircraft. There is also discussion about what makes a pilot. The last section relates the use of Ikhana in the western states fire mission.

Volume 3 assists Earth Observing System (EOS) investigators in locating required non-EOS data products by identifying their non-EOS input requirements and providing the information on data sets available at various Distributed Active Archive Centers (DAAC's), including those from Pathfinder Activities and Earth Probes. Volume 3 is intended to complement, not to duplicate, the the EOSDIS Science Data Plan (SDP) by providing detailed data set information which was not presented in the SDP. Section 9 of this volume discusses the algorithm summary tables containing information on retrieval algorithms, expected outputs and required input data. Section 10 describes the non-EOS input requirements of instrument teams and IDS investigators. Also described are the current and future data holdings of the original seven DAACS and data products planned from the future missions and projects including Earth Probes and Pathfinder Activities. Section 11 describes source of information used in compiling data set information presented in this volume. A list of data set attributes used to describe various data sets is presented in section 12 along with their descriptions. Finally, Section 13 presents the SPSO's future plan to improve this report .

NREL's first annual Information Resources Catalogue is intended to inform anyone interested in energy efficiency and renewable energy technologies of NREL's outreach activities, including publications and services. For ease of use, all entries are categorized by subject. The catalogue is separated into six main sections. The first section lists and describes services that are available through NREL and how they may be accessed. The second section contains a list of documents that are published by NREL on a regular or periodic basis. The third section highlights NREL's series publications written for specific audiences and presenting a wide range of subjects. NREL's General Interest Publications constitute the fourth section of the catalogue and are written for nontechnical audiences. Descriptions are provided for these publications. Objective To perform an evaluation of the information given to patients seeking emergency dental services during out-of-hours periods in Greater Manchester.Method This is a cross-sectional study. A list of all NHS dental practices in Greater Manchester was obtained from the NHS website. The investigators then accessed websites and voicemails of all practices to assess the information given to patients.Results The study shows that most practices have voicemail, which includes varying information on how to access emergency dental services during out-of-hours periods. Few of those included advice on the management of common dental emergencies. On the other hand, the majority of practices did not have websites. From those who included advice on how to access emergency dental services during out-of-hours periods on how to manage common dental emergencies on their websites, the information was easy to find.Conclusion The current information could be improved by ensuring the inclusion of essential details. Websites would help by including more details. In turn, this could help to reduce the number of inappropriate presentations to A&E departments.

This document contains final regulations providing guidance toemployers that are subject to the information reporting requirements under section 6056 of the Internal Revenue Code (Code), enacted by the Affordable Care Act (generally employers with at least 50 full-time employees, including full-time equivalent employees). Section 6056 requires those employers to report to the IRS information about the health care coverage, if any, they offered to full-time employees, in order to administer the employer shared responsibility provisions of section 4980H of the Code. Section 6056 also requires those employers to furnish related statements to employees that employees may use to determine whether, for each month of the calendar year, they may claim on their individual tax returns a premium tax credit under section 36B (premium tax credit). The regulations provide for a general reporting method and alternative reporting methods designed to simplify and reduce the cost of reporting for employers subject to the information reporting requirements under section 6056. The regulations affect those employers, employees and other individuals.

An overload of health-related information is available for patients on numerous websites, guidelines, and information leaflets. However, the increasing need for personalized health-related information is currently unmet. This study evaluates an educational e-tool for patients with non-Hodgkin's lymphoma (NHL) designed to meet patient needs with respect to personalized and complete health-related information provision. The e-tool aims to help NHL patients manage and understand their personal care pathway, by providing them with insight into their own care pathway, the possibility to keep a diary, and structured health-related information. Together with a multidisciplinary NHL expert panel, we developed an e-tool consisting of two sections: (1) a personal section for patients' own care pathway and their experiences, and (2) an informative section including information on NHL. We developed an ideal NHL care pathway based on the available (inter)national guidelines. The ideal care pathway, including date of first consultation, diagnosis, and therapy start, was used to set up the personal care pathway. The informative section was developed in collaboration with the patient association, Hematon. Regarding participants, 14 patients and 6 laymen were asked to evaluate the e-tool. The 24-item questionnaire used discussed issues concerning layout (6 questions), user convenience (3 questions), menu clarity (3 questions), information clarity (5 questions), and general impression (7 questions). In addition, the panel members were asked to give their feedback by email. A comprehensive overview of diagnostics, treatments, and aftercare can be established by patients completing the questions from the personal section. The informative section consisted of NHL information regarding NHL in general, diagnostics, therapy, aftercare, and waiting times. Regarding participants, 6 patients and 6 laymen completed the questionnaire. Overall, the feedback was positive, with at least 75% satisfaction on each feedback item. Important strengths mentioned were the use of a low health-literacy level, the opportunity to document the personal care pathway and experiences, and the clear overview of the information provided. The added value of the e-tool in general was pointed out as very useful for preparing the consultation with one's doctor and for providing all information on one website, including the opportunity for a personalized care pathway and diary. The majority of the revisions concerned wording and clarity. In addition, more explicit information on immunotherapy, experimental therapy, and psychosocial support was added. We have developed a personal care management e-tool for NHL patients. This tool contains a unique way to help patients manage their personal care pathway and give them insight into their NHL by providing health-related information and a personal diary. This evaluation showed that our e-tool meets patients' needs concerning personalized health-related information, which might serve as a good example for other oncologic diseases. Future research should focus on the possible impact of the e-tool on doctor-patient communication during consultations.

The goal of this special section is to encourage greater awareness of evidence-based assessment (EBA) in the development of a scientifically supported clinical psychology. In this introductory article, the authors describe the elements that authors in this special section were asked to consider in their focused reviews (including the scope of available psychometric evidence, advancements in psychopathology research, and evidence of attention to factors such as gender, age, and ethnicity in measure validation). The authors then present central issues evident in the articles that deal with anxiety, depression, personality disorders, and couple distress and in the accompanying commentaries. The authors conclude by presenting key themes emerging from the articles in this special section, including gaps in psychometric information, limited information about the utility of assessment, the discrepancy between recommended EBAs and current training and practice, and the need for further data on the process of clinical assessment.

Microarrays are a very powerful tool for quantifying the amount of RNA in samples; however, their ability to query essentially every gene in a genome, which can number in the tens of thousands, presents analytical and interpretative problems. As a result, a variety of software and web-based tools have been developed to help with these issues. This article highlights and reviews some of the tools for the first steps in the analysis of a microarray study. We have tried for a balance between free and commercial systems. We have organized the tools by topics including image processing tools (Section 2), power analysis tools (Section 3), image analysis tools (Section 4), database tools (Section 5), databases of functional information (Section 6), annotation tools (Section 7), statistical and data mining tools (Section 8), and dissemination tools (Section 9).

It is proposed to establish a Photon Data Section (PDS) of the BNL National Nuclear Data Center (NNDC). This would be a total program encompassing both photon-atom and photon-nucleus interactions. By utilizing the existing NNDC data base management expertise and on-line access capabilities, the implementation of photon interaction data activities within the existing NNDC nuclear structure and nuclear-reaction activities can reestablish a viable photon interaction data program at minimum cost. By taking advantage of the on-line capabilities, the x-ray users' community will have access to a dynamic, state-of-the-art data base of interaction information. The proposed information base would include datamore » that presently are scattered throughout the literature usually in tabulated form. Uncertainty-driven nuclear data evaluation including thermal (n,α) applied to 59Ni

This paper presents a novel approach to the evaluation of nuclear data (ND), combining experimental data for thermal cross sections with resonance parameters and nuclear reaction modeling. The method involves sampling of various uncertain parameters, in particular uncertain components in experimental setups, and provides extensive covariance information, including consistent cross-channel correlations over the whole energy spectrum. The method is developed for, and applied to, 59Ni, but may be used as a whole, or in part, for other nuclides. 59Ni is particularly interesting since a substantial amount of 59Ni is produced in thermal nuclear reactors by neutron capture in 58Ni and since it has a non-threshold (n,α) cross section. Therefore, 59Ni gives a very important contribution to the helium production in stainless steel in a thermal reactor. However, current evaluated ND libraries contain old information for 59Ni, without any uncertainty information. The work includes a study of thermal cross section experiments and a novel combination of this experimental information, giving the full multivariate distribution of the thermal cross sections. In particular, the thermal (n,α) cross section is found to be 12.7 ± . 7 b. This is consistent with, but yet different from, current established values. Further, the distribution of thermal cross sections is combined with reported resonance parameters, and with TENDL-2015 data, to provide full random ENDF files; all of this is done in a novel way, keeping uncertainties and correlations in mind. The random files are also condensed into one single ENDF file with covariance information, which is now part of a beta version of JEFF 3.3. Finally, the random ENDF files have been processed and used in an MCNP model to study the helium production in stainless steel. The increase in the (n,α) rate due to 59Ni compared to fresh stainless steel is found to be a factor of 5.2 at a certain time in the reactor vessel, with a relative uncertainty due to the 59Ni data of 5.4%.

Involving women in personalised decision-making on mode of delivery after caesarean section: the development and pilot testing of a patient decision aid.

To develop a patient decision aid (PtDA) for mode of delivery after caesarean section that integrates personalised prediction of vaginal birth after caesarean (VBAC) with the elicitation of patient preferences and evidence-based information. A PtDA was developed and pilot tested using the International Patients Decision Aid Standards (IPDAS) criteria. Obstetric health care in the Netherlands. A multidisciplinary steering group, an expert panel, and 25 future users of the PtDA, i.e. women with a previous caesarean section. The development consisted of a construction phase (definition of scope and purpose, and selection of content, framework, and format) and a pilot testing phase by interview. The process was supervised by a multidisciplinary steering group. Usability, clarity, and relevance. The construction phase resulted in a booklet including unbiased balanced information on mode of birth after caesarean section, a preference elicitation exercise, and tailored risk information, including a prediction model for successful VBAC. During pilot testing, visualisation of risks and clarity formed the main basis for revisions. Pilot testing showed the availability of tailored structured information to be the main factor involving women in decision-making. The PtDA meets 39 out of 50 IPDAS criteria (78%): 23 out of 23 criteria for content (100%) and 16 out of 20 criteria for the development process (80%). Criteria for effectiveness (n = 7) were not evaluated. An evidence-based PtDA was developed, with the probability of successful VBAC and the availability of structured information as key items. It is likely that the PtDA enhances the quality of decision-making on mode of birth after caesarean section. © 2013 Royal College of Obstetricians and Gynaecologists.

2011 Radioactive Materials Usage Survey for Unmonitored Point Sources

This report provides the results of the 2011 Radioactive Materials Usage Survey for Unmonitored Point Sources (RMUS), which was updated by the Environmental Protection (ENV) Division's Environmental Stewardship (ES) at Los Alamos National Laboratory (LANL). ES classifies LANL emission sources into one of four Tiers, based on the potential effective dose equivalent (PEDE) calculated for each point source. Detailed descriptions of these tiers are provided in Section 3. The usage survey is conducted annually; in odd-numbered years the survey addresses all monitored and unmonitored point sources and in even-numbered years it addresses all Tier III and various selected other sources.more » This graded approach was designed to ensure that the appropriate emphasis is placed on point sources that have higher potential emissions to the environment. For calendar year (CY) 2011, ES has divided the usage survey into two distinct reports, one covering the monitored point sources (to be completed later this year) and this report covering all unmonitored point sources. This usage survey includes the following release points: (1) all unmonitored sources identified in the 2010 usage survey, (2) any new release points identified through the new project review (NPR) process, and (3) other release points as designated by the Rad-NESHAP Team Leader. Data for all unmonitored point sources at LANL is stored in the survey files at ES. LANL uses this survey data to help demonstrate compliance with Clean Air Act radioactive air emissions regulations (40 CFR 61, Subpart H). The remainder of this introduction provides a brief description of the information contained in each section. Section 2 of this report describes the methods that were employed for gathering usage survey data and for calculating usage, emissions, and dose for these point sources. It also references the appropriate ES procedures for further information. Section 3 describes the RMUS and explains how the survey results are organized. The RMUS Interview Form with the attached RMUS Process Form(s) provides the radioactive materials survey data by technical area (TA) and building number. The survey data for each release point includes information such as: exhaust stack identification number, room number, radioactive material source type (i.e., potential source or future potential source of air emissions), radionuclide, usage (in curies) and usage basis, physical state (gas, liquid, particulate, solid, or custom), release fraction (from Appendix D to 40 CFR 61, Subpart H), and process descriptions. In addition, the interview form also calculates emissions (in curies), lists mrem/Ci factors, calculates PEDEs, and states the location of the critical receptor for that release point. [The critical receptor is the maximum exposed off-site member of the public, specific to each individual facility.] Each of these data fields is described in this section. The Tier classification of release points, which was first introduced with the 1999 usage survey, is also described in detail in this section. Section 4 includes a brief discussion of the dose estimate methodology, and includes a discussion of several release points of particular interest in the CY 2011 usage survey report. It also includes a table of the calculated PEDEs for each release point at its critical receptor. Section 5 describes ES's approach to Quality Assurance (QA) for the usage survey. Satisfactory completion of the survey requires that team members responsible for Rad-NESHAP (National Emissions Standard for Hazardous Air Pollutants) compliance accurately collect and process several types of information, including radioactive materials usage data, process information, and supporting information. They must also perform and document the QA reviews outlined in Section 5.2.6 (Process Verification and Peer Review) of ES-RN, 'Quality Assurance Project Plan for the Rad-NESHAP Compliance Project' to verify that all information is complete and correct.« less

Extracted arrival time distributions of negative ion CID-derived fragments produced prior to traveling-wave ion mobility separation were evaluated for their ability to provide structural information on N-linked glycans. Fragmentation of high-mannose glycans released from several glycoproteins, including those from viral sources, provided over 50 fragments, many of which gave unique collisional cross-sections and provided additional information used to assign structural isomers. For example, cross-ring fragments arising from cleavage of the reducing terminal GlcNAc residue on Man 8 GlcNAc 2 isomers have unique collision cross-sections enabling isomers to be differentiated in mixtures. Specific fragment collision cross-sections enabled identification of glycans, the antennae of which terminated in the antigenic α-galactose residue, and ions defining the composition of the 6-antenna of several of the glycans were also found to have different cross-sections from isomeric ions produced in the same spectra. Potential mechanisms for the formation of the various ions are discussed and the estimated collisional cross-sections are tabulated. Graphical Abstract ᅟ.

Materials management information systems.

The hospital materials management function--ensuring that goods and services get from a source to an end user--encompasses many areas of the hospital and can significantly affect hospital costs. Performing this function in a manner that will keep costs down and ensure adequate cash flow requires effective management of a large amount of information from a variety of sources. To effectively coordinate such information, most hospitals have implemented some form of materials management information system (MMIS). These systems can be used to automate or facilitate functions such as purchasing, accounting, inventory management, and patient supply charges. In this study, we evaluated seven MMISs from seven vendors, focusing on the functional capabilities of each system and the quality of the service and support provided by the vendor. This Evaluation is intended to (1) assist hospitals purchasing an MMIS by educating materials managers about the capabilities, benefits, and limitations of MMISs and (2) educate clinical engineers and information system managers about the scope of materials management within a healthcare facility. Because software products cannot be evaluated in the same manner as most devices typically included in Health Devices Evaluations, our standard Evaluation protocol was not applicable for this technology. Instead, we based our ratings on our observations (e.g., during site visits), interviews we conducted with current users of each system, and information provided by the vendor (e.g., in response to a request for information [RFI]). We divided the Evaluation into the following sections: Section 1. Responsibilities and Information Requirements of Materials Management: Provides an overview of typical materials management functions and describes the capabilities, benefits, and limitations of MMISs. Also includes the supplementary article, "Inventory Cost and Reimbursement Issues" and the glossary, "Materials Management Terminology." Section 2. The MMIS Selection Process: Outlines steps to follow and describes factors to consider when selecting an MMIS. Also includes our Materials Management Process Evaluation and Needs Assessment Worksheet (which is also available online through ECRInet(TM)) and a list of suggested interview questions to be used when gathering user experience information for systems under consideration. Section 3A. MMIS Vendor Profiles: Presents information for the evaluated systems in a standardized, easy-to-compare format. Profiles include an Executive Summary describing our findings, a discussion of user comments, a listing of MMIS specifications, and information on the vendor's business background. Section 3B. Discussion of Vendor Profile Conclusions and Ratings: Presents our ratings and summarizes our rationale for all evaluated systems. Also includes a blank Vendor Profile Template to be used when gathering information on other vendors and systems. We found that, in general, all of the evaluated systems are able to meet most of the functional needs of a materials management department. However, we did uncover significant differences in the quality of service and support provided by each vendor, and our ratings reflect these differences: we rated two of the systems Acceptable--Preferred and four of the systems Acceptable. We have not yet rated the seventh system because our user experience information may not reflect the vendor's new ownership and management. When this vendor provides the references we requested, we will interview users and supply a rating. We caution readers against basing purchasing decisions solely on our ratings. Each hospital must consider the unique needs of its users and its overall strategic plans--a process that can be aided by using our Process Evaluation and Needs Assessment Worksheet. Primary care physicians face cognitive overload daily, perhaps exacerbated by the form of electronic health record documentation. We examined physician information needs to prepare for clinic visits, focusing on past clinic progress notes. This study used cognitive task analysis with 16 primary care physicians in the scenario of preparing for office visits. Physicians reviewed simulated acute and chronic care visit notes. We collected field notes and document highlighting and review, and we audio-recorded cognitive interview while on task, with subsequent thematic qualitative analysis. Member checks included the presentation of findings to the interviewed physicians and their faculty peers. The Assessment and Plan section was most important and usually reviewed first. The History of the Present Illness section could provide supporting information, especially if in narrative form. Physicians expressed frustration with the Review of Systems section, lamenting that the forces driving note construction did not match their information needs. Repetition of information contained in other parts of the chart (eg, medication lists) was identified as a source of note clutter. A workflow that included a patient summary dashboard made some elements of past notes redundant and therefore a source of clutter. Current ambulatory progress notes present more information to the physician than necessary and in an antiquated format. It is time to reengineer the clinic progress note to match the workflow and information needs of its primary consumer. © Copyright 2015 by the American Board of Family Medicine.

To assess the quality and comprehensiveness of the information on caesarean section provided in Brazilian women's magazines. Review of articles published during 1988-2008 in top selling women's magazines. Brazil, one of the countries with the highest caesarean section rates in the world. Women's magazines with the largest distribution during the study period, identified through the official national media indexing organisations. Articles with objective scientific information or advice, comments, opinions, or the experience of ordinary women or celebrities on delivery by caesarean section. Sources of information mentioned by the author of the article, the accuracy and completeness of data presented on caesarean section, and alleged reasons why women would prefer to deliver though caesarean section. 118 articles were included. The main cited sources of information were health professionals (78% (n=92) of the articles). 71% (n=84) of the articles reported at least one benefit of caesarean section, and 82% (n=97) reported at least one short term maternal risk of caesarean section. The benefits most often attributed to delivery by caesarean section were reduction of pain and convenience for family or health professionals. The most frequently reported short term maternal risks of caesarean section were increased time to recover and that it is a less natural way of giving birth. Only one third of the articles mentioned any long term maternal risks or perinatal complications associated with caesarean section. Fear of pain was the main reported reason why women would prefer to deliver by caesarean section. Most of the articles published in Brazilian women's magazines do not use optimal sources of information. The portrayal of caesarean section is mostly balanced, not explicitly in favour of one or another route of delivery, but incomplete and may be leading women to underestimate the maternal/perinatal risks associated with this route of delivery.

Bridge-scour analysis using the water surface profile (WSPRO) model

A program was developed to extract hydraulic information required for bridge-scour computations, from the Water-Surface Profile computation model (WSPRO). The program is written in compiled BASIC and is menu driven. Using only ground points, the program can compute average ground elevation, cross-sectional area below a specified datum, or create a Drawing Exchange Format (DXF) fie of cross section. Using both ground points ad hydraulic information form the equal-conveyance tubes computed by WSPRO, the program can compute hydraulic parameters at a user-specified station or in a user-specified subsection of the cross section. The program can identify the maximum velocity in a cross section and the velocity and depth at a user-specified station. The program also can identify the maximum velocity in the cross section and the average velocity, average depth, average ground elevation, width perpendicular to the flow, cross-sectional area of flow, and discharge in a subsection of the cross section. This program does not include any help or suggestions as to what data should be extracted; therefore, the used must understand the scour equations and associated variables to the able to extract the proper information from the WSPRO output.

Isomer Information from Ion Mobility Separation of High-Mannose Glycan Fragments

Extracted arrival time distributions of negative ion CID-derived fragments produced prior to traveling-wave ion mobility separation were evaluated for their ability to provide structural information on N-linked glycans. Fragmentation of high-mannose glycans released from several glycoproteins, including those from viral sources, provided over 50 fragments, many of which gave unique collisional cross-sections and provided additional information used to assign structural isomers. For example, cross-ring fragments arising from cleavage of the reducing terminal GlcNAc residue on Man8GlcNAc2 isomers have unique collision cross-sections enabling isomers to be differentiated in mixtures. Specific fragment collision cross-sections enabled identification of glycans, the antennae of which terminated in the antigenic α-galactose residue, and ions defining the composition of the 6-antenna of several of the glycans were also found to have different cross-sections from isomeric ions produced in the same spectra. Potential mechanisms for the formation of the various ions are discussed and the estimated collisional cross-sections are tabulated. [Figure not available: see fulltext.

The Annual Energy Review (AER) presents the Energy Information Administration’s historical energy statistics. For many series, statistics are given for every year from 1949 through 2002. The statistics, expressed in either physical units or British thermal units, cover all major energy activities, including consumption, production, trade, stocks, and prices, for all major energy commodities, including fossil fuels, electricity, and renewable energy sources. Publication of this report is required under Public Law 95–91 (Department of Energy Organization Act), Section 205(c), and is in keeping with responsibilities given to the Energy Information Administration (EIA) under Section 205(a)(2), which states: “The Administrator shallmore » be responsible for carrying out a central, comprehensive, and unified energy data and information program which will collect, evaluate, assemble, analyze, and disseminate data and information....” The AER is intended for use by Members of Congress, Federal and State agencies, energy analysts, and the general public.
