The Special Care Dentistry Association (SCDA) has acted on a proposal regarding the status of training in the care of patients with special needs. Two phases of action were undertaken. Phase 1: (a) examination of the literature on existing training and curricula in the care of patients with special needs and (b) a survey of existing postdoctoral programs in special needs. Phase 2: establish a group of experts who: (a) submitted to the Commission on Dental Accreditation a request to approve a postdoctoral general dentistry residency program in Special Care Dentistry and (b) created suggested accreditation standards for such postdoctoral programs. This article describes efforts by the SCDA to evaluate: The status of existing training of dental students in the care of patients with special needs. The number and characteristics of postdoctoral general dentistry programs offering formal training in the care of patients with special needs. Whether additional training in the care of patients with special needs is needed for dental students and -dentists. Possible actions by SCDA to impact the numbers of dentists trained each year in the care of patients with -special needs. © 2016 Special Care Dentistry Association and Wiley Periodicals, Inc.

This report discusses the background and design of a study on special programs of education for medical librarianship in the United States. Between 1957 and 1971, 238 persons completed special programs in medical librarianship. Since little information has been available concerning the occupational success of these individuals, the study was undertaken (1) to determine the nature and distinguishing characteristics of the special programs of education for medical librarianship in the United States and (2) to provide some quantitative evidence of the value of those programs by studying a group of professional medical librarians who have had specialized training and a group of practicing medical librarians who have not had specialized training and comparing and correlating the data with regard to selected characteristics. The findings of the study will be presented in three future reports: (1) a descriptive analysis of the programs; (2) a description of the characteristics of all former trainees in medical librarianship who responded to a questionnaire and a discussion of their reactions to their training programs; and (3) a comparison, based on selected characteristics, of a group of former trainees who are currently employed in medical libraries with a group of medical librarians who did not go through special training programs. To evaluate the impact of a tertiary care center special needs program that partners with families and primary care physicians to ensure seamless inpatient and outpatient care and assist in providing medical homes. Up to 3 years of preenrollment and postenrollment data were compared for patients in the special needs program from July 1, 2002, through June 30, 2005. A tertiary care center pediatric hospital and medical school serving urban and rural patients. A total of 227 of 230 medically complex and fragile children and youth with special needs who had a wide range of chronic disorders and were enrolled in the special needs program. Care coordination provided by a special needs program pediatric nurse case manager with or without a special needs program physician. Preenrollment and postenrollment tertiary care center resource utilization, charges, and payments. A statistically significant decrease was found in the number of hospitalizations, number of hospital days, and tertiary care center charges and payments, and an increase was found in the use of outpatient services. Aggregate data revealed a decrease in hospital days from 7926 to 3831, an increase in clinic visits from 3150 to 5420, and a decrease in tertiary care center payments of $10.7 million. The special needs program budget for fiscal year 2005 had a deficit of $400,000. This tertiary care-primary care partnership model improved health care and reduced costs with relatively modest institutional support.

While there are outreach programs for the public and for children, there are few programs for special needs children. Here I describe two NASA-IDEAS/STSc funded outreach programs I created for special needs children using telescope observations (including remote and robotic observations) and hands-on astronomy activities. The target audience is seriously ill children and their families who are staying at the Ronald McDonald House of Long Island in conjunction the children's surgery and medical treatments or children hospitalized at the Hagedorn Pediatric Inpatient Center at Winthrop University Hospital. These educational activities help children and their families learn about astronomy while providing a diversion to take their minds off their illness during a stressful time. I have also conducted a similar program in camps for special needs children. These programs should be expanded so that special needs children and their families part of the IYA 2009 activities.

The U.S. Forest Service (USFS) Special Uses-Lands Program is in jeopardy. Although this program, authorized in Title 36, Part 251, of the U.S. Code of Federal Regulations (36 CFR Part 251), ranks among the top four revenue-generating programs for use of National Forest System (NFS) lands, along with the Timber, Minerals, and Special Uses-Recreation Programs, the Special Uses-Lands Program is in a state of neglect. Repeated cuts in funding (a decrease of 26% from fiscal years 2010 to 2014) are adversely affecting staffing and training, which in turn is affecting timely permit processing and ultimately the public’s ability to usemore » and benefit from NFS lands. In addition, highly experienced staff with valuable institutional knowledge of the program have begun to retire. The ability of the program to function under these dire circumstances can be attributed to the dedication of Special Uses staff to the program and their commitment to the public. The initial focus of this report was to identify opportunities for improving performance of permitting and review for large energy infrastructure-related projects. However, it became clear during this analysis that these projects are generally adequately staffed and managed. This is due in large part to the availability of cost-recovery dollars and the high-profile nature of these projects. However, it also became apparent that larger issues affecting the bulk of the work of the Special Uses-Lands Program need to be addressed immediately. This report is a preliminary examination of the state of the Special Uses-Lands Program and focuses on a few key items requiring immediate attention. Further investigation through case studies is recommended to dig deeper into the Special Uses-Lands Program business process to determine the most costeffective strategies for streamlining the overall process and the metrics by which performance can be evaluated, including for the permitting and tracking of energy infrastructure projects.« less

Respite care services for children with special healthcare needs: Parental perceptions.
Whitmore, Kim E; Snethen, Julia
2018-04-26
Parents of children with special healthcare needs may become overwhelmed with the ongoing caregiving needs of their children. Caring for a child with special healthcare needs is often challenging, requiring specialized training in many cases. As a result, parents can struggle to find qualified caregivers capable of providing them a break from the 24/7 care of their child. Respite care programs are designed to provide caregivers with a much-needed temporary break. The purpose of this study was to examine parental perceptions of utilizing a respite care program. Twenty-two parents who had a child with special healthcare needs who attended a Midwestern respite care program completed a Participant Characteristic Form addressing their experiences caring for their child with special healthcare needs and using respite care services. Parents participated in a focus group (N = 4) to explore their perceptions and experiences of respite care participation. Multistage thematic analysis and descriptive statistics were used to analyze the data. Themes emerging from the data included: Constant care demands; It is just so stressful; Respite is a gift, we get a break; Respite program "fit"; and Respite is their special time too. Parents emphasized the benefits of respite care for their marital relationship, as well as the benefits to the children with special healthcare needs and their siblings. Parents also described the importance of tailoring respite care to the unique needs of their family. Nurses and other healthcare professionals play a critical role in addressing the unmet respite care needs of parents of children with special healthcare needs by identifying unmet needs and making appropriate referrals to services that will meet the unique needs of the family. Healthcare professionals can also volunteer with existing programs to help expand access to respite care services and increase the availability of adequately trained respite care providers whom parents can trust to provide for the complex healthcare needs of their children. © 2018 Wiley Periodicals, Inc.

Four schools cover at least half, and four schools cover less than one-third of this knowledge content; There is no systematic difference in the job-relatedness of the different types of specialized educational programs, A.S. degree, B.S. degree, and coursework; and Traditional B.S. degree programs in nuclear engineering cover as much job-related knowledge (about one-half of this knowledge content) as most of the specialized educational programs.« less
[Medical specialization in Chile. A centralized vision].
Clouet-Huerta, Diego E; González, Bárbara; Correa, Katherine
2017-11-01
Medical graduates face different postgraduate training options, but their priority is to obtain a primary medical specialty, defined as a specialty that does not derive from other. There are different specialty training programs in Chile, which can be dependent or independent of the Ministry of Health. The information about these programs is available in different Internet sites. However a centralized information service that groups and synthetize these programs is lacking, hampering graduate choice decisions. This article aims to review all specialization program modalities, providing a general vision of the institutional structure and implications that govern the specialization process in Chile.

Referral decisions of teachers and school psychologists for twice-exceptional students
NASA Astrophysics Data System (ADS)
Hoffman, Jennifer Marie
The accurate and timely referral and identification of twice-exceptional students remains a challenge. In a statewide study, the referral decisions for both special education and gifted programming evaluations made by four participant groups (i.e., general education teachers, special education teachers, gifted education teachers, and school psychologists) were compared. Participants were randomly assigned to read one of three identically described students in a vignette that differed only in the presence of a diagnostic label--- autism spectrum disorder (ASD), specific learning disability (SLD), or no diagnostic label. In all, special education teachers made the most special education referrals, while gifted education teachers made the most gifted programming referrals, both regardless of the diagnostic label present. The students with diagnostic labels were recommended for special education referrals significantly more than for gifted programming, while this difference was not evident in the no diagnostic label condition. Moreover, the student with the ASD label was the most likely to be referred for evaluations for both special education and gifted programming out of all three vignette conditions. Overall findings indicated the importance of considering the referral source as well as how the presence of a diagnostic label might influence educational referral decisions, particularly in how this might influence overall multidisciplinary team decisions for these unique learners.

Special-effect edit detection using VideoTrails: a comparison with existing techniques

Video segmentation plays an integral role in many multimedia applications, such as digital libraries, content management systems, and various other video browsing, indexing, and retrieval systems. Many algorithms for segmentation of video have appeared within the past few years. Most of these algorithms perform well on cuts, but yield poor performance on gradual transitions or special effects edits. A complete video segmentation system must also achieve good performance on special effect edit detection. In this paper, we discuss the performance of our Video Trails-based algorithms, with other existing special effect edit-detection algorithms within the literature. Results from experiments testing for the ability to detect edits from TV programs, ranging from commercials to news magazine programs, including diverse special effect edits, which we have introduced.

New graduate RN retention in the first year of employment is a challenge for hospitals, ranging from a low of 25% to a high of 64%. In 2005, hospitals in New York state spent 11.7% of their nursing budgets on temporary nursing staffing. The objectives of this study were to determine the retention and costs associated with the employment of new graduate RNs before and after the initiation of specialized year-long pediatric critical care, emergency department, and hematology/oncology orientation programs. The major study findings were improved retention of 84% to 94%, significant retention between the two groups at 9 months, and an annual financial savings related to decreased nursing turnover in the specialized orientation group. Specialized orientation programs that support new graduate RNs have documented increased RN retention and decreased RN turnover. In concert with the increased retention and decreased turnover, health care finances were positively impacted by specialized orientation programs.

Theory and empirical work suggest inclusion preschool improves the school readiness of young children with special needs, but only 2 studies of the model have used rigorous designs that could identify causality. The present study examined the impacts of the Boston Public prekindergarten program-which combined proven language, literacy, and mathematics curricula with coaching-on the language, literacy, mathematics, executive function, and emotional skills of young children with special needs (N = 242). Children with special needs benefitted from the program in all examined domains. Effects were on par with or surpassed those of their typically developing peers. Results are discussed in the context of their relevance for policy, practice, and theory. (PsycINFO Database Record (c) 2016 APA, all rights reserved).

[Trends among medical students towards general practice or specialization].
Breinbauer K, Hayo; Fromm R, Germán; Fleck L, Daniela; Araya C, Luis
2009-07-01
A 60/40 ratio has been estimated as a country's ideal proportion between general practitioners and specialists. In Chile this proportion was 36/ 64 in 2004, exactly the opposite of the ideal. Trends towards specialization or general practice among medical students have not been thoughtfully studied. To assess trends among medical students towards becoming general practitioners or specialists, exploring associated factors. Descriptive survey of 822 first to seventh year medical students at the University of Chile, School of Medicine. Desired activity to pursue (general practice or specialization) after graduation and general orientations within clinical practice were explored. Fifty three percent of students desired to enter a specialization program. Only 20% would work as a general practitioner (27% were still indecisive). Furthermore, a trend in early years of medical training towards an integral medicine is gradually reversed within later years. Seventh year students give significantly more importance to specialization than to integral medicine (p <0.01). Ten percent of this opinion change is related to the emphasis given to specialized medicine in the teaching environment. Most students prefer to enter a specialization program immediately after finishing medical school. Moreover, there is a social trend, at least within the teacher-attending environment, promoting not only the desire to specialize, but a pro-specialist culture.
