We present an approach and architecture for implementing scalable and maintainable clinical decision support at the Partners HealthCare System. The architecture integrates a business rules engine that executes declarative if-then rules stored in a rule-base referencing objects and methods in a business object model. The rules engine executes object methods by invoking services implemented on the clinical data repository. Specialized inferences that support classification of data and instances into classes are identified and an approach to implement these inferences using an OWL based ontology engine is presented. Alternative representations of these specialized inferences as if-then rules or OWL axioms are explored and their impact on the scalability and maintenance of the system is presented. Architectural alternatives for integration of clinical decision support functionality with the invoking application and the underlying clinical data repository; and their associated trade-offs are discussed and presented.

We derive general expressions for the sum rules of the eigenvalues of drums of arbitrary shape and arbitrary density, obeying different boundary conditions. The formulas that we present are a generalization of the analogous formulas for one dimensional inhomogeneous systems that we have obtained in a previous paper. We also discuss the extension of these formulas to higher dimensions. We show that in the special case of a density depending only on one variable the sum rules of any integer order can be expressed in terms of a single series. As an application of our result we derive exact summore » rules for the homogeneous circular annulus with different boundary conditions, for a homogeneous circular sector and for a radially inhomogeneous circular annulus with Dirichlet boundary conditions. -- Highlights: •We derive an explicit expression for the sum rules of inhomogeneous drums. •We discuss the extension to higher dimensions. •We discuss the special case of an inhomogeneity only along one direction.« less

Stable isotope ratio variations are regulated by physical and chemical laws. These rules depend on a relation with mass differences between isotopes. New classes of isotope variation effects that deviate from mass dependent laws, termed mass independent isotope effects, were discovered in 1983 and have a wide range of applications in basic chemistry and nature. In this special edition, new applications of these effects to physical chemistry, solar system origin models, terrestrial atmospheric and biogenic evolution, polar paleo climatology, snowball earth geology, and present day atmospheric sciences are presented.

New classes of isotope variation effects that deviate from mass dependent laws, termed mass independent isotope effects, were discovered in 1983 and have a wide range of applications in basic chemistry and nature. We examine some surprising results that have been obtained for pre- and postselected quantum systems. We show that these results depend on a counterfactual interpretation of the rule of Aharonov, Bergmann, and Lebowitz [Phys. Rev. 134, B1410 (1964)] (the ABL rule) and that this interpretation is not valid in general. We then argue, with the help of the consistent histories interpretation of quantum mechanics, that there is a special class of situations where application of the counterfactual interpretation of the ABL rule can be justified. We then reexamine the aforementioned surprising results.

In this paper we present PhotoRApToR (Photometric Research Application To Redshift): a Java/C ++ based desktop application capable to solve non-linear regression and multi-variate classification problems, in particular specialized for photo-z estimation. It embeds a machine learning algorithm, namely a multi-layer neural network trained by the Quasi Newton learning rule, and special tools dedicated to pre- and post-processing data. PhotoRApToR has been successfully tested on several scientific cases. The application is available for free download from the DAME Program web site.

Medicare has established medical necessity rules that define the medical conditions that make beneficiaries eligible for particular services. These rules are codified in local medical review policies (LMRPs) that are established by Medicare claims payment contractors. If a beneficiary's provider does not inform the patient that a service may not be covered, the provider cannot subsequently bill the beneficiary for the service if it is denied. This article discusses the application of these policies. It illustrates the circumstances in which advance beneficiary notices (ABN) are required to ensure that patients have been notified that services rendered will not be covered by Medicare and will become their financial responsibility. The author also presents special applications of the ABN regulations as they apply to the EMTALA rules, anti-kickback, and other statutes. Samples of the official ABN forms are illustrated.
