This final rule implements provisions of the Patient Protection and Affordable Care Act and the Health Care and Education Reconciliation Act of 2010 (collectively referred to as the Affordable Care Act). Specifically, this final rule outlines financial integrity and oversight standards with respect to Affordable Insurance Exchanges, qualified health plan (QHP) issuers in Federally-facilitated Exchanges (FFEs), and States with regard to the operation of risk adjustment and reinsurance programs. It also establishes additional standards for special enrollment periods, survey vendors that may conduct enrollee satisfaction surveys on behalf of QHP issuers, and issuer participation in an FFE, and makes certain amendments to definitions and standards related to the market reform rules. These standards, which include financial integrity provisions and protections against fraud and abuse, are consistent with Title I of the Affordable Care Act. This final rule also amends and adopts as final interim provisions set forth in the Amendments to the HHS Notice of Benefit and Payment Parameters for 2014 interim final rule, published in the Federal Register on March 11, 2013, related to risk corridors and cost-sharing reduction reconciliation.

A system, method, and computer readable-media for creating a stable synthetic neural system. The method includes training an intellectual choice-driven synthetic neural system (SNS), training an emotional rule-driven SNS by generating emotions from rules, incorporating the rule-driven SNS into the choice-driven SNS through an evolvable interface, and balancing the emotional SNS and the intellectual SNS to achieve stability in a nontrivial autonomous environment with a Stability Algorithm for Neural Entities (SANE). Generating emotions from rules can include coding the rules into the rule-driven SNS in a self-consistent way. Training the emotional rule-driven SNS can occur during a training stage in parallel with training the choice-driven SNS. The training stage can include a self assessment loop which measures performance characteristics of the rule-driven SNS against core genetic code. The method uses a stability threshold to measure stability of the incorporated rule-driven SNS and choice-driven SNS using SANE.

The Food and Drug Administration (FDA) is classifying the resorbable calcium salt bone void filler device intended to fill bony voids or gaps of the extremities, spine, and pelvis that are caused by trauma or surgery and are not intrinsic to the stability of the bony structure into class II (special controls). Elsewhere in this issue of the Federal Register, FDA is announcing the availability of a class II special controls guidance entitled "Class II Special Controls Guidance Document: Resorbable Calcium Salt Bone Void Filler Device; Guidance for Industry and FDA." This action is being undertaken based on new information submitted in a classification proposal from Wright Medical Technology under the Federal Food, Drug, and Cosmetic Act as amended by the Medical Device Amendments of 1976, the Safe Medical Devices Act of 1990, and the Food and Drug Administration Modernization Act of 1997.

Bredt's rule holds a special place in the realm of physical organic chemistry, but its application to natural products chemistry—the field in which the rule was originally formulated—is not well defined. Herein, the use of olefin strain (OS) energy as a readily calculated predictor of the stability of natural products containing a bridgehead alkene is introduced. Schleyer first used OS energies to classify parent bridgehead alkenes into "isolable", "observable", and "unstable" classes. OS calculations on natural products, using contemporary forcefield methods, unequivocally predict all structurally verified bridgehead alkene natural products to be "isolable". Thus, when one assigns the structure of a putative bridgehead alkene natural product, an OS in the "observable" or "unstable" ranges is a red flag for error. © 2015 WILEY-VCH Verlag GmbH & Co. KGaA, Weinheim.

An Integrated Children Disease Prediction Tool within a Special Social Network.

This paper proposes a social network with an integrated children disease prediction system developed by the use of the specially designed Children General Disease Ontology (CGDO). This ontology consists of children diseases and their relationship with symptoms and Semantic Web Rule Language (SWRL rules) that are specially designed for predicting diseases. The prediction process starts by filling data about the appeared signs and symptoms by the user which are after that mapped with the CGDO ontology. Once the data are mapped, the prediction results are presented. The phase of prediction executes the rules which extract the predicted disease details based on the SWRL rule specified. The motivation behind the development of this system is to spread knowledge about the children diseases and their symptoms in a very simple way using the specialized social networking website www.emama.mk.

Effect of Ionic Soil Stabilizers on Soil-Water Characteristic of Special Clay

The engineering properties of special clay are conventionally improved through the use of chemical additive such as ionic soil stabilizer (ISS). Such special clays are often referred to as stabilized or treated clays. The soil-water characteristic curves (SWCC) of special clays from Henan province and Hubei province were measured both in natural and stabilized conditions using the pressure plate apparatus in the suction range of 0-500 kPa. The SWCC results are used to interpret the special clays behavior due to stabilizer treatment. In addition, relationships were developed between the basic clay and stabilized properties such as specific surface area and pore size distribution. The analysis showed that specific surface area decreases, cumulative pore volume and average pore size diameter decrease, dehydration rate slows and the thickness of water film thins after treatment with Ionic Soil Stabilizer. The research data and interpretation analysis presented here can be extended to understand the water film change behaviors influencing the mechanical and physical properties of stabilized special clay soils. Modern ideas of thermomechanics are used to develop families of models describing the elastic/plastic behaviour of cohesionless soils deforming under triaxial conditions. Once the form of the free energy and dissipation potential functions have been specified, the corresponding yield loci, flow rules, isotropic and kinematic hardening rules as well as the elasticity law are deduced in a systematic manner. The families contain the classical linear frictional (Coulomb type) models and the classical critical state models as special cases. The generalized models discussed here include non-associated flow rules, shear as well as volumetric hardening, anisotropic responses and rotational yield loci. The various parameters needed to describe the models can be interpreted in terms of ratio of the plastic work, which is dissipated, to that which is stored. Non-associated behaviour is found to occur whenever this division between dissipated and stored work is not equal. Micro-level interpretations of stored plastic work are discussed. The models automatically satisfy the laws of thermodynamics, and there is no need to invoke any stability postulates. Some classical forms of the peak-strength/dilatancy relationship are established theoretically. Some representative drained and undrained paths are computed.

In order to overcome the problems of poor understandability of the pattern recognition-based transient stability assessment (PRTSA) methods, a new rule extraction method based on extreme learning machine (ELM) and an improved Ant-miner (IAM) algorithm is presented in this paper. First, the basic principles of ELM and Ant-miner algorithm are respectively introduced. Then, based on the selected optimal feature subset, an example sample set is generated by the trained ELM-based PRTSA model. And finally, a set of classification rules are obtained by IAM algorithm to replace the original ELM network. The novelty of this proposal is that transient stability rules are extracted from an example sample set generated by the trained ELM-based transient stability assessment model by using IAM algorithm. The effectiveness of the proposed method is shown by the application results on the New England 39-bus power system and a practical power system--the southern power system of Hebei province.

The presence of unstable dead-time systems in process plants often leads to a daunting challenge in the design of standard PID controllers, which are not only intended to provide close-loop stability but also to give good performance-robustness overall. In this paper, we conduct stability analysis on a double-loop control scheme based on the Routh-Hurwitz stability criteria. We propose to use this unstable double-loop control scheme which employs two P/PID controllers to control first-order or second-order unstable dead-time processes typically found in process industries. Based on the Routh-Hurwitz stability necessary and sufficient criteria, we establish several stability regions which enclose within them the P/PID parameter values that guarantee close-loop stability of the double-loop control scheme. A systematic tuning rule is developed for the purpose of obtaining the optimal P/PID parameter values within the established regions. The effectiveness of the proposed tuning rule is demonstrated using several numerical examples and the result are compared with some well-established tuning methods reported in the literature.

We present an approach and architecture for implementing scalable and maintainable clinical decision support at the Partners HealthCare System. The architecture integrates a business rules engine that executes declarative if-then rules stored in a rule-base referencing objects and methods in a business object model. The rules engine executes object methods by invoking services implemented on the clinical data repository. Specialized inferences that support classification of data and instances into classes are identified and an approach to implement these inferences using an OWL based ontology engine is presented. Alternative representations of these specialized inferences as if-then rules or OWL axioms are explored and their impact on the scalability and maintenance of the system is presented. The impact of loss sensitivity on a mobile phone supply chain system stability based on the chaos theory

This paper, based on the China's communications and the current situation of the mobile phone industry, focuses on the stability of a supply chain system that consists of one supplier and one bounded rational retailer. We explore the influence of the decision makers' loss sensitivity and decision adjustment speed on the stability of the supply chain. It is found that when the retailer is not sensitive to the loss or adjusts decisions cautiously, the system can be stable. The single-retailer model is extended to a multi-retailer one to study the influence of competition on the system stability. The results show that the market share of each retailer does not affect the system stability when it is fixed. The decision of each retailer does not affect that of any other retailer and the system stability. We present two decision adjustment rules (;bounded rationality expectation (BRE); and "adaptive exponential smoothing (AES)") and compare their performances on the system stability, and find that the AES rule does not affect the system stability, while the BRE rule will make the system stability be sensitive to the retailers' loss sensitivity and the decision adjustment speed. We also reveal the unstable system's negative impact on the retailers' decisions and profits, to emphasize the importance to maintain the system stability.

The basis of evidence for hygiene rules implemented in hospitals is traditionally small. This is not only because there is little theoretical knowledge on the reciprocal influence between a single hygienic mistake/a single microbial input and the manifestation of a nosocomial infection. There are also not enough clinical studies, especially on complex hygiene questions, to determine whether special measures (e.g., septic rooms)can compensate for deficits in hygiene practice. Furthermore, it would be necessary to designate security buffers distinctly. In-house traditions are able to stabilize hygienic behavior in an excellent manner. They should be fostered and not disparaged as myths. Discussions of experts should not be conducted in public; that is disastrous for the everyday work of physicians in hospitals.
