The effect of federal health policy on occupational medicine.

All three branches of the federal government affect occupational medicine. Notable examples include: 1) the Department of Transportation ruling (1988) requiring drug testing in diverse areas of the transportation industry (executive branch); 2) the Workplace Drug Act (1988) calling for organizations to have a policy towards drug and alcohol abuse (legislative branch); and 3) the Supreme Court ruling on the constitutionality of drug testing in the transportation industry (1989) and that infectious diseases are a handicap in accordance with the 1973 Federal Rehabilitation Act (1987). The executive branch plays a major role in occupational medicine primarily through the Occupational Safety and Health Administration (OSHA), which issues standards based on a rule making process; the executive branch can also affect occupational medicine indirectly, as evidenced by President Reagan's Executive Order 12291 calling for Office of Management and Budget oversight of regulatory initiatives. The legislative branch enacts laws, conducts hearings, and requests reports on the operations of federal agencies. The judicial branch addresses occupational health issues when people affected by an executive ruling want to challenge the ruling; or in the case of the Supreme Court, when deliberating an issue over which two circuit courts of appeal have come to divergent opinions. The Occupational Medicine profession can participate in the political process through awareness of proposed legislation and by responding accordingly with letters, resolutions, or testimony. Similar options exist within the executive branch by participating in the rule-making process. A representative of the Governmental Affairs Committee, through periodic visits with key Washington representatives, can keep members of the American College of Occupational Medicine informed about federal legislative and regulatory activities. In appropriate cases, the organization can then take a formal position on governmental activities that affect the speciality.

Using the 2005 National Association of County and City Health Officers Profile of Local Health Departments data set, bivariate probit and Heckman selection models were used to test the hypothesis that the level of federal funding received for bioterrorism preparedness is related to the preparedness activities undertaken by local health departments. Overall budget, leadership, and crisis experience are found to be the most important determinants of local preparedness activity, but Centers for Disease Control and Prevention preparedness funding plays a mediating role by building capacity through the hiring of one key leadership position, the emergency preparedness coordinator. Additional research is needed to determine the potential impact of these funds on other aspects of the local public health system, such as the scope of services delivered, to determine secondary effects of the program.

This article examines local emergency manager's beliefs regarding control over tasks during various stages of the hazard cycle since federal policies went into effect following the September 11 attacks. To do so, hypothesis testing investigates the jurisdiction's use of comprehensive emergency management (CEM) practices, the perceived "clarity" of the federal policy demands, and if the local actors feel coerced to comply with federal policy demands so that grant funding is not compromised. Using a model developed from "third-generation" policy implementation research, the results show that the odds of local officials citing federal control over these actions have very limited statistical significance. This signals that the perceived lack of local input into the development of these federal policies and the policies' limited use of traditional CEM measures may not be in concert with what local actors perform in the field. Simply put, the respondents claim to understand the federal policy demands, support the concept of federal control as the policies describe, yet follow their own plans or traditional CEM principles, even if such actions do not support the federal policy demands. These results align with pre-existing research in the emergency management field that show issues with efforts to centralize policies under the Department of Homeland Security and Federal Emergency Management Agency.
Getting the Biggest Bang for Your Buck: Wildlife Monitoring on Shrublands of the Nevada Test Site

The Nevada Test Site (NTS) covers 3,561 km2 and extends over portions of both the Mojave and Great Basin Deserts. The resulting diverse and complex flora and fauna exhibit elements of both deserts. There are 20 vegetation associations, composed primarily of shrubs, nested within 10 vegetation alliances. Of the more than 1,200 invertebrate and 339 vertebrate species found in these shrubland habitats, 267 are considered sensitive or protected/regulated by federal or state laws. Wildlife and wildlife habitat monitoring ensures NTS activities comply with all federal and state laws enacted for the protection of these valuable biological resources and provides ecologicalmore » information that can be used to predict and evaluate the potential impacts of proposed projects and current activities on these resources. This paper describes the monitoring approach used at this large site. Monitoring strategies include conducting preactivity surveys, proactively monitoring sensitive species, monitoring long-term population trends, and collaborating with other agencies and biologists. The activities and accomplishments of the Photovoltaic Tests and Applications Project during the period April 1976 through June 1977 are summarized. Results of efforts to identify potential near-term photovoltaic applications and users are discussed, including the outcome of an extensive survey of Federal government agencies. The status of application experiments is presented. Various general engineering efforts are reported, including the design and construction of a photovoltaic Systems Test Facility. Efforts to develop a high efficiency 10 kVA self-commutated inverter and controller specifically designed for photovoltaic systems are also discussed. The results of a wide variety of activities in the area of photovoltaic measurements and standards are related. It is now undeniable that laboratory testing is vital for the diagnosis, prognostication and therapeutic monitoring of human disease. Despite the many advances made for achieving a high degree of quality and safety in the analytical part of diagnostic testing, many hurdles in the total testing process remain, especially in the preanalytical phase ranging from test ordering to obtaining and managing the biological specimens. The Working Group for the Preanalytical Phase (WG-PRE) of the European Federation of Clinical Chemistry and Laboratory Medicine (EFLM) has planned many activities aimed at mitigating the vulnerability of the preanalytical phase, including the organization of three European meetings in the past 7 years. Hence, this collective article follows the previous three opinion papers that were published by the EFLM WGPRE on the same topic, and brings together the summaries of the presentations that will be given at the 4th EFLM-BD meeting "Improving quality in the preanalytical phase through innovation" in Amsterdam, 24-25 March, 2017.

An exercise program was initiated in a federal agency to assess the feasibility of such a program, and to identify the factors that influenced joining, adherence to, and effectiveness of the program. The program was utilized by 237 of the 998 eligible federal employees; mean attendance rate was 1.3 days/week. Those who volunteered perceived a need for increased physical activity, believed they had sufficient time to participate and derived subjective as well as objective benefits. Significant improvements were found in heart rate response to the standard exercise test, body weight, skinfold measurements and triglyceride levels.

In recent years, changes in the Brazilian economic and social scenario have generated a growing demand for higher education in the country. In response to this new context, the federal government launched in 2007 a programme aiming at expansion of the enrolment to public higher education. In such environment of changes, several proposals have emerged to adapt the Brazilian federal universities to a new reality. Taking this context into account, the focus of our study is on proposals from the Mathematics Department of the Federal University of Minas Gerais. Their aim is to create a new model of teaching practices for the freshman lessons of the Exact Sciences area, which at first were being experimented in special classrooms of students attending their first course on differential and integral calculus. The data were collected from interviews with students and professors from the mathematics department. They were analyzed and systematized using an activity theory approach. We became instigated by a model developed by Engeström in his study on the changes in the Finnish public health system, considering it as a test (testbench) for activity theory in its application to a particular case. Following Engeström's footsteps when developing his research, we arrived at our own model showing the internal tensions in the activity of reformulation of the courses offered by the Department of Mathematics and in the conceptions of teachers that promote - and constraint - the proposals for change.

A one-year prospective examination of injury rates and injury risk factors was conducted in Federal Bureau of Investigation (FBI) new agent training. Injury incidents were obtained from medical records and injury compensation forms. Potential injury risk factors were acquired from a lifestyle questionnaire and existing data at the FBI Academy. A total of 426 men and 105 women participated in the project. Thirty-five percent of men and 42% of women experienced one or more injuries during training. The injury incidence rate was 2.5 and 3.2 injuries/1,000 person-days for men and women, respectively (risk ratio (women/men) = 1.3, 95% confidence interval = 0.9-1.7). The activities most commonly associated with injuries (% of total) were defensive tactics training (58%), physical fitness training (20%), physical fitness testing (5%), and firearms training (3%). Among the men, higher injury risk was associated with older age, slower 300-meter sprint time, slower 1.5-mile run time, lower total points on the physical fitness test (PFT), lower self-rated physical activity, lower frequency of aerobic exercise, a prior upper or lower limb injury, and prior foot or knee pain that limited activity. Among the women higher injury risk was associated with slower 300-meter sprint time, slower 1.5-mile run time, lower total points on the PFT, and prior back pain that limited activity. The results of this investigation supported those of a previous retrospective investigation emphasizing that lower fitness and self-reported pain limiting activity were associated with higher injury risk among FBI new agents.

The Earth System Grid Federation (ESGF) Peer-to-Peer (P2P) is a software infrastructure for the management, dissemination, and analysis of model output and observational data. The ESGF grid is composed with several types of nodes which have different roles. About 40 data nodes host model outputs and datasets using thredds catalogs. About 25 compute nodes offer remote visualization and analysis tools. About 15 index nodes crawl data nodes catalogs and implement faceted and federated search in a web interface. About 15 Identity providers nodes manage accounts, authentication and authorization. Here we will present an actual size test federation spread across different institutes in different countries and a python test suite that were started in December 2013. The first objective of the test suite is to provide a simple tool that helps to test and validate a single data node and its closest index, compute and identity provider peer. The next objective will be to run this test suite on every data node of the federation and therefore test and validate every single node of the whole federation. The suite already implements nosetests, requests, myproxy-logon, subprocess, selenium and fabric python libraries in order to test both web front ends, back ends and security services. The goal of this project is to improve the quality of deliverable in a small developers team context. Developers are widely spread around the world working collaboratively and without hierarchy. This kind of working organization context en-lighted the need of a federated integration test and validation process.

The Federal Motor Carrier Safety Administration, in coordination with the Federal Highway Administration, sponsored the Heavy and Overweight Vehicle Brake Testing (HOVBT) program in order to provide information about the effect of gross vehicle weight (GVW) on braking performance. Because the Federal Motor Carrier Safety Regulations limit the number of braking system defects that may exist for a vehicle to be allowed to operate on the roadways, the examination of the effect of brake defects on brake performance for increased loads is also relevant. The HOVBT program seeks to provide relevant information to policy makers responsible for establishing load limits,more » beginning with providing test data for a combination tractor/trailer. This testing was conducted on a five-axle combination vehicle with tractor brakes meeting the Reduced Stopping Distance requirement rulemaking. This report provides a summary of the testing activities, the results of various analyses of the data, and recommendations for future research. Following a complete brake rebuild, instrumentation, and brake burnish, stopping tests were performed from 20 and 40 mph with various brake application pressures (15 psi, 25 psi, 35 psi, 45 psi, 55 psi, and full system pressure). These tests were conducted for various brake conditions at the following GVWs: 60,000, 80,000, 91,000, 97,000, 106,000, and 116,000 lb. The 80,000-lb GVWs included both balanced and unbalanced loads. The condition of the braking system was also varied. To introduce these defects, brakes (none, forward drive axle, or rear trailer axle) were made inoperative. In addition to the stopping tests, performance-based brake tests were conducted for the various loading and brake conditions. Analysis of the stopping test data showed the stopping distance to increase with load (as expected) and also showed that more braking force was generated by the drive axle brakes than the trailer axle brakes. The constant-pressure stopping test data revealed a linear relationship between brake application pressure and was used to develop an algorithm to normalize stopping data for weight and initial speed.« less

My project at Kennedy Space Center (KSC) during the spring 2013 Project Management and Systems Engineering Internship was to functionalJy test and deploy the SharePoint Extranet system and ensure successful completion of the project's various lifecycle milestones as described by NASA Procedural Requirement (NPR) 7 120.7. I worked alongside NASA Project Managers, Systems Integration Engineers, and Information Technology (IT) Professionals to pilot this collaboration capability between NASA and its External Partners. The use of identity federation allows NASA to leverage externally-issued credentials of other federal agencies and private aerospace and defense companies, versus the traditional process of granting and maintaining full NASA identities for these individuals. This is the first system of its kind at NASA and it will serve as a pilot for the Federal Government. Recognizing the novelty of this service, NASA's initial approach for deployment included a pilot period where nearby employees of Patrick Air Force Base would assist in testing and deployment. By utilizing a credential registration process, Air Force users mapped their Air Force-issued Common Access Cards (CAC) to a NASA identity for access to the External SharePoint. Once the Air Force stands up an Active Directory Federation Services (ADFS) instance within their Data Center and establishes a direct trust with NASA, true identity federation can be established. The next partner NASA is targeting for collaboration is Lockheed Martin (LMCO), since they collaborate frequently for the ORION Program. Through the use of Exostar as an identity hub, LMCO employees will be able to access NASA data on a need to know basis, with NASA ultimately managing access. In a time when every dollar and resource is being scrutinized, this capability is an exciting new way for NASA to continue its collaboration efforts in a cost and resource effective manner.

On December 30, 1994, the Internal Revenue Service (IRS) published proposed regulations (Proposed Regulations) that if enacted would significantly change the climate and rules of federal income tax law controlling the issuance and maintenance of tax-exempt bonds for governmental and 501(c)(3) health care borrowers. This article (1) summarizes the aspects of the Proposed Regulations dealing with private activity tests, management contracts, allocation and accounting rules, change in use of financed facilities, and antiabuse rules, and (2) summarizes the possible interrelationship of the IRS's audit program for tax-exempt bonds and the Proposed Regulations. The article reviews features of the Proposed Regulations that will affect either the costs or administrative burdens of managing the federal tax compliance of future tax-exempt health care borrowings.

The Nevada Test Site (NTS) covers 1,375 square miles and extends over portions of both the Mojave and Great Basin Deserts. The resulting diverse and complex flora and fauna exhibit elements of both deserts. There are 20 vegetation associations, composed primarily of shrubs, nested within 10 vegetation alliances. Few interdisciplinary research groups include basic scientists, pharmacists, therapists, nutritionists, lab technicians, as well as trauma patients and families, in addition to clinicians. Increasing interprofessional diversity within scientific teams working to improve trauma care is a goal of national organizations and federal funding agencies like the National Institutes of Health (NIH). This paper describes the design, implementation, and outcomes of a Trauma Interdisciplinary Group for Research (TIGR) at a Level 1 trauma center as it relates to increasing research productivity, with specific examples excerpted from an on-going NIH-funded study. We utilized a pre-test/post-test design with objectives aimed at measuring increases in research productivity following a targeted intervention. A SWOT (strengths, weaknesses, opportunities, threats) analysis was used to develop the intervention which included research skill-building activities, accomplished by adding multidisciplinary investigators to an existing NIH-funded project. The NIH project aimed to test the hypothesis that accelerated biologic aging from chronic stress increases baseline inflammation and reduces inflammatory response to trauma (projected N=150). Pre/Post-TIGR data related to participant screening, recruitment, consent, and research processes were compared. Research productivity was measured through abstracts, publications, and investigator-initiated projects. Research products increased from N =12 to N=42; (~ 400%). Research proposals for federal funding increased from N=0 to N=3, with success rate of 66%. Participant screenings for the NIH-funded study increased from N=40 to N=313. Consents increased from N=14 to N=70. Lab service fees were reduced from $300/participant to $5/participant. Adding diversity to our scientific team via TIGR was exponentially successful in 1) improving research productivity, 2) reducing research costs, and 3) increasing research products and mentoring activities that the team prior to TIGR had not entertained. The team is now well-positioned to apply for more federally funded projects and more trauma clinicians are considering research careers than before.

Injury rates and injury risk factors among federal bureau of investigation new agent trainees

Background A one-year prospective examination of injury rates and injury risk factors was conducted in Federal Bureau of Investigation (FBI) new agent training. Methods Injury incidents were obtained from medical records and injury compensation forms. Potential injury risk factors were acquired from a lifestyle questionnaire and existing data at the FBI Academy. Results A total of 426 men and 105 women participated in the project. Thirty-five percent of men and 42% of women experienced one or more injuries during training. The injury incidence rate was 2.5 and 3.2 injuries/1,000 person-days for men and women, respectively (risk ratio (women/men) = 1.3, 95% confidence interval = 0.9-1.7). The activities most commonly associated with injuries (% of total) were defensive tactics training (58%), physical fitness training (20%), physical fitness testing (5%), and firearms training (3%). Among the men, higher injury risk was associated with older age, slower 300-meter sprint time, slower 1.5-mile run time, lower total points on the physical fitness test (PFT), lower self-rated physical activity, lower frequency of aerobic exercise, a prior upper or lower limb injury, and prior foot or knee pain that limited activity. Among the women higher injury risk was associated with slower 300-meter sprint time, slower 1.5-mile run time, lower total points on the PFT, and prior back pain that limited activity. Conclusion The results of this investigation supported those of a previous retrospective investigation emphasizing that lower fitness and self-reported pain limiting activity were associated with higher injury risk among FBI new agents. According to the International Diabetes Federation (IDF), a quarter of the world's population has Metabolic Syndrome (MS). To develop (and assess the users' degree of satisfaction of) an online social network for patients who suffer from Metabolic Syndrome, based on the recommendations and requirements of the Human-Centered Design. Following the recommendations of the ISO 9241-210 for Human-Centered Design (HCD), an online social network was designed to promote physical activity and healthy nutrition. In order to guarantee the active participation of the users during the development of the social network, a survey, an in-depth interview, a focal group, and usability tests were carried out with people suffering from MS. The study demonstrated how the different activities, recommendations, and requirements of the ISO 9241-210 are integrated into a traditional software development process. Early usability tests demonstrated that the user's acceptance and the effectiveness and efficiency of the social network are satisfactory.

The State AIDS Policy Center at the Inter-governmental Health Policy Project (IHPP) at George Washington University surveyed all 50 states to determine state AIDS (acquired immunodeficiency syndrome) expenditures, without Medicaid or federal funds, for fiscal 1984-88. During this period, state-only expenditures increased 15-fold, to $156.3 million. Between fiscal 1986-1988, the distribution of state funding for AIDS patient care and support services doubled from 16 to 35 per cent and the number of states supplementing federal funds for testing and counseling increased from eight to 20. Five states continue to account for the largest AIDS appropriations. Of these, California leads in funding research; New York, Florida, and New Jersey have directed funds to provide care and services to IV (intravenous) drug users, prisoners, and children. The average state expenditure per diagnosed AIDS case is $3,323 and an increasing number of states with relatively low case loads are appropriating funds beyond this level. Across states, AIDS expenditures per person average $.65 and $.21 for education, testing and counseling--below the level recommended by the Institute of Medicine for AIDS prevention activities. Some jurisdictions support AIDS activities indirectly by shifting resources, often from their STD (sexually transmitted disease) programs--this trend deserves continuing review given the rise in STD cases and their relationship to diagnosed AIDS. With impending federal action, and the likelihood of "copycat" legislation at the state and local levels if a federal statue is adopted, nonprofit hospitals need to ensure that they will be in compliance if a mandatory charity care bill were to become law. Such facilities should take the initiative now, by clearly articulating their charity care policies and by describing their other undercompensated or uncompensated activities that are maintained in order to benefit their communities. They should also ensure that all of their activities that could be counted as charity care or community benefit are fully documented and quantified. Financial consultants and counsel may be helpful in this process. Exempt facilities also should, of course, closely monitor the progress of the bills discussed above. Most importantly, they should continue to test their charity care and community benefit statistics on a regular basis to determine if their facilities meet the thresholds specified in the bills as they proceed through Congress.

In the U.S., colorectal cancer (CRC) incidence and mortality have declined due to screening and improvements in early detection; however, racial/ethnic disparities in screening and mortality persist. Patient navigation has been shown to be effective in increasing CRC screening prevalence. This systematic review answered three questions about navigation in federally qualified community health centers (FQHCs): 1) Which navigation activities increased CRC screening prevalence? 2) What were the challenges to implementing these programs in FQHCs? 3) Which clinic protocols supported screening completion? Findings suggest that navigation services must be tailored to the specific screening test provided. Federally qualified community health centers report difficulty maintaining a current electronic medical records system and sustaining funding; they should establish excellent patient tracking systems (for follow-up and annual rescreening) and establish multiple protocols to facilitate screening completion. With the movement toward patient-centered care models, patient navigation will be integral to FQHCs and their clients. PMID:28238992

Carroll Island was used for open-air testing of chemical warfare agents from the late 1940's until 1971. Testing and disposal activities weresuspected of causing environmental contamination at 16 sites on the island. The hydrogeology and chemical quality of ground water, surface water, and soil at these sites were investigated with borehole logs, environmental samples, water-level measurements, and hydrologic tests. A surficial aquifer, upper confining unit, and upper confined aquifer were defined. Ground water in the surficial aquifer generally flows from the east-central part of the island toward the surface-water bodies, butgradient reversals caused by evapotranspiration can occur during dry seasons. In the confined aquifer, hydraulic gradients are low, and hydraulic head is affected by tidal loading and by seasonal pumpage from the west. Inorganic chemistry in the aquifers is affected by brackish-water intrusion from gradient reversals and by dissolution ofcarboniferous shell material in the confining unit.The concentrations of most inorganic constituents probably resulted from natural processes, but some concentrations exceeded Federal water-quality regulations and criteria. Organic compounds were detected in water and soil samples at maximum concentrations of 138 micrograms per liter (thiodiglycol in surface water) and 12 micrograms per gram (octadecanoic acid in soil).Concentrations of organic compounds in ground water exceeded Federal drinking-water regulations at two sites. The organic compounds that weredetected in environmental samples were variously attributed to natural processes, laboratory or field- sampling contamination, fallout from industrial air pollution, and historical military activities.

To explore relationships between local health department policy behaviors, levels of government activity, policy focus areas, and selected health department characteristics. Cross-sectional analysis of secondary data from the 2013 National Association of County & City Health Officials (NACCHO) Profile Survey. Local health departments throughout the United States. A total of 2000 local health departments responding to the 2013 Profile Survey of Local Health Departments. Survey data were gathered by the NACCHO. Secondary analysis of reported policy behaviors for the 2013 NACCHO Profile Survey. A structural equation model tested effects on and between state population size, rurality, census region and policy focus, and the latent variables of policy behavior formed from a confirmatory factor analysis. Policy behaviors, levels of government activity (local, state, and federal), policy focus areas, and selected local health department characteristics. The majority (85.1%) of health departments reported at least one of the possible policy behaviors. State population size increased the probability of local policy behavior, and local behavior increased the probability of state policy behavior. State size increased the likelihood of federal policy behavior and the focus on tobacco, emergency preparedness, and obesity/chronic disease. However, the more rural a state was, the more likely policy behavior was at the state and federal levels and not at local levels. Specific policy behaviors mattered less than the level of government activity. Size of state and rurality of health departments influence the government level of policy behavior.

Recently, the United States has increased its activities related to aircraft icing in numerous fields: ice phobics, revised characterization of icing conditions, instrument development/evaluation, de-ice/anti-ice devices, simulated supercooled clouds, computer simulation and flight tests. The Federal Coordinator for Meteorology is involved in two efforts, one a National Plan on Aircraft Icing and the other a plan for Improved Aircraft Icing Forecasts and Associated Warning Services. These two plans will provide an approved structure for future U.S. activities related to aircraft icing. The recommended activities will significantly improve the position of government agencies to perform mandated activities and to enable U.S. manufacturers to be competitive in the world market.

Tests were conducted in 2008 on 16 late-model conventional vehicles (1999-2007) to determine short-term effects of mid-level ethanol blends on performance and emissions. Vehicle odometer readings ranged from 10,000 to 100,000 miles, and all vehicles conformed to federal emissions requirements for their federal certification level. The LA92 drive cycle, also known as the Unified Cycle, was used for testing because it more accurately represents real-world acceleration rates and speeds than the Federal Test Procedure. Test fuels were splash-blends of up to 20 volume percent ethanol with federal certification gasoline. Both regulated and unregulated air-toxic emissions were measured. As the commercial space sector grew, there emerged a corresponding growth in demand for launch range services. However, the expense and complexity of activities has thus far deterred a rapid rise in the establishment of purely commercial launch sites. In this context, purely commercial is defined as "without benefit of capabilities provided by the federal government." Consistent with the Commercial Space Launch Act, in recent years NASA and the Air Force have supported commercial launches from government launch ranges on a cost-reimbursable, non-interference basis. In this mode the commercial launch service providers contract with the government to provide services including use of facilities, tracking and data services, and range safety. As the commercial market projections began to show significant opportunities for economic development, several states established spaceports to provide the services necessary to meet these projected commercial needs. In 1997, NASA agreed to the establishment of the Virginia Space Flight Center (VSFC) at the Wallops Flight Facility. Under this arrangement, NASA agreed to allow Virginia Commercial Space Flight Authority (VCSFA) to construct facilities on NASA property and agreed to provide services in accordance with the Space Act of 1958 and the Commercial Space Launch Act of 1984 (as amended) to support VSFC launch customers. The relationship between NASA and VCSFA, however, has evolved beyond a customer supplier relationship. A partnership relationship has emerged which pairs the strengths of the established NASA test range and the state-sponsored, commercial launch facility provider, in an attempt to satisfy the needs for flexible, low-cost access to space. Furthermore, the future of the NASA/Wallops Test Range is closely linked with the success of VCSFA in promoting commercial launches from Wallops. This paper will describe the changing paradigm of the federal launch range and the unique aspects of the NASA/Wallops Facility relationship with VCSFA. Discussion will include institutional cost-sharing, business development and marketing, joint educational programs, and strategic planning.

Local public health departments have a wide array of responsibilities, including coordinating childhood lead poisoning prevention activities. This study was conducted in an effort to understand how local public health officers prioritized lead poisoning prevention activities and the barriers to the delivery of childhood lead poisoning prevention services delivered through local health departments. A telephone survey was conducted of health officers in Michigan, a state with a high burden of environmental lead. Analysis included Spearman rank correlation and Fisher's exact test. No association was found between the local risk of lead poisoning and the priority placed by local health departments on lead poisoning prevention activities. Similarly, there was no association between the local risk of lead poisoning and the availability of services. Only 60% of local health departments offered blood lead testing, environmental investigation, and case management. Most (74%) believed that lead poisoning is inadequately addressed within the area served by their local health department. New strategies of providing lead poisoning prevention activities are needed to achieve the federal and state goals of eliminating childhood lead poisoning over the next decade.

Recently, educators in public K-12 schools have added testing of science knowledge to the measures of Adequate Yearly Progress required by the federal No Child Left Behind (NCLB) legislation. Research of the impact of NCLB policy on general teaching practices had credited the policy with improving instruction; however, negative impacts noted included the concern that teachers "teach to the test," narrowing the curriculum. Testing as an assessment strategy was not advocated by the professional educators and scientists responsible for the National Science Education Standards (NSES). Results from previous studies pointed to a potential conflict between the NCLB reforms and the National Science Education Standards science standards, in which teachers might reduce or eliminate hands-on activities and other constructivist practices in order to focus class time on other topics and tasks. Most research on NCLB policy, however, had not evaluated instructional practices regarding science education. This study examined the relationship among teacher beliefs, specifically the strength of their constructivist versus traditional beliefs, teachers' responses to NCLB policy, and teachers' use of constructivist practices in the form of manipulatives. This study showed that national policy did have an impact on teachers; however, that impact was not specific to the hands-on practices in science education. Teachers who responded to this survey had found many benefits in student learning using manipulatives and those positive impacts on their students justified the increased use of manipulatives in the classroom. The strength of teachers' constructivist beliefs showed a weak positive correlation to choices related to curriculum priorities, learning goals and advantages in using manipulatives. However, a relationship to beliefs was not found in the changes teachers made to various instructional practices, or in how they viewed certain manipulative materials, or in how they viewed federal policies. The impact of the federal policies was related more to school climate and teacher motivation than instructional choices. Most teachers reported positive impacts on student learning using manipulatives and these results, more than the pressure they felt to "cover the curriculum" or prepare students for standardized tests, provided reasons to continue using the practice.

To examine how institutional constraints, comprising federal actions and states' substance abuse policy environments, influence states' decisions to activate Medicaid reimbursement codes for screening and brief intervention for risky substance use in the United States. A discrete-time duration model was used to estimate the effect of institutional constraints on the likelihood of activating the Medicaid reimbursement codes. Primary constraints included federal Screening, Brief Intervention and Referral to Treatment (SBIRT) grant funding, substance abuse priority, economic climate, political climate and interstate diffusion. Study data came from publicly available secondary data sources. Federal SBIRT grant funding did not affect significantly the likelihood of activation (P = 0.628). A $1 increase in per-capita block grant funding was associated with a 10-percentage point reduction in the likelihood of activation (P = 0.003) and a $1 increase in per-capita state substance use disorder expenditures was associated with a 2-percentage point increase in the likelihood of activation (P = 0.004). States with enacted parity laws (P = 0.016) and a Democratic-controlled state government were also more likely to activate the codes. In the United States, the determinants of state activation of Medicaid Screening, Brief Intervention and Referral to Treatment (SBIRT) reimbursement codes are complex, and include more than financial considerations. Federal block grant funding is a strong disincentive to activating the SBIRT reimbursement codes, while more direct federal SBIRT grant funding has no detectable effects. © 2017 Society for the Study of Addiction.

Forty-three ongoing technology transfer programs in Federal agencies other than NASA were selected from over 200 current Federal technology transfer activities. Selection was made and specific technology transfer mechanisms utilized. Detailed information was obtained on the selected programs by reviewing published literature, and conducting telephone interviews with each program manager. Specific information collected on each program includes technology areas; user groups, mechanisms employed, duration of program, and level of effort. Twenty-four distinct mechanisms are currently employed in Federal technology transfer activities totaling $260 million per year. Typical applications of each mechanism were reviewed, and caveats on evaluating program effectiveness were discussed. A review of recent federally funded research in technology transfer to state and local governments was made utilizing the Smithsonian Science Information Exchange, and abstracts of interest to NASA were selected for further reference.

In the article there are reported the methodological approaches to the calculation of actual and avoided as a result of control and supervisory activities of economic losses caused by mortality, morbidity and disability in the population, associated with the negative impact of environmental factors. There is suggested a consistent solution of the chain of problems: the establishment of cause-and-effect relationships between indices of the health status and indices of the quality of the environment; indices of the quality of the environment and indices of the control and supervision activity of organs and institutions of Federal Service for Surveillance on Consumer Rights Protection and Human Wellbeing calculation of the cases of violations of health prevented as a result of the activity ofthe service; evaluation of their economic equivalents. Approbation of approaches on the example of the Russian Federation allowed to establish that as a result of the activity of organs and institutions of Federal Service for Surveillance on Consumer Rights Protection and Human Wellbeing in 2013, a positive trend was observed on 51 indices of quality of the habitat environment, there were prevented about 160 thousands of deaths and more than 2 million cases of disease that would be held in conditions of the lack of adequate control and surveillance measures in the field of sanitary and epidemiological welfare of the population. In the annals of criminology and the sociology of law, the phenomenon of corporate illegalities has been seriously neglected. Moreover, the relatively few social scientific studies of illegal corporate behavior have often approached the subject from limited theoretical perspectives. These studies have generally overlooked a critical element in criminological understanding; the role of law and its administration in generating violation rates that may be said to be characteristic of the prevailing social structure. This dissertation develops an integrated theoretical approach to a type of corporate illegality, environmental pollution. In this approach, law is seen as integral to the understanding ofmore » crime, and both are situated within the broad socio-economic structure of the United States in the latter half of the 20th century. This research first reviews and analyzes the history of the federal government's efforts to control industrial water pollution, seeking to identify the social structural limits on regulatory activity. Statistical analyses of data on the federal processing of industrial pollution violations are then presented, with a view toward analyzing the correlates of both business behavior and the federal response. The analyses test a proposed model of industrial regulation, and provide important support for the perspective linking law and crime.« less

The Impact of the Affordable Care Act on Funding for Newborn Screening Services.

The Affordable Care Act requires most health plans to cover the federal Recommended Uniform Screening Panel of newborn screening (NBS) tests with no cost sharing. However, state NBS programs vary widely in both the number of mandated tests and their funding mechanisms, including a combination of state laboratory fees, third-party billing, and other federal and state funding. We assessed the potential impact of the Affordable Care Act coverage mandate on states' NBS funding. We performed an extensive review of the refereed literature, federal and state agency reports, relevant organizations' websites, and applicable state laws and regulations; interviewed 28 state and federal officials from August to December 2014; and then assessed the interview findings manually. Although a majority of states had well-established systems for including laboratory-based NBS tests in bundled charges for newborn care, billing practices for critical congenital heart disease and newborn hearing tests were less uniform. Most commonly, birthing facilities either prepaid the costs of laboratory-based tests when acquiring the filter paper kits, or the facilities paid for the tests when the kits were submitted. Some states had separate arrangements for billing Medicaid, and smaller facilities sometimes contracted with hearing test vendors that billed families separately. Although the Affordable Care Act coverage mandate may offset some state NBS funding for the screenings themselves, federal support is still required to assure access to the full range of NBS program services. Limiting reimbursement to the costs of screening tests alone would undermine the common practice of using screening charges to fund follow-up services counseling, and medical food or formula, particularly for low-income families.
Using fee-for-service testing to generate revenue for the 21st century public health laboratory.

The decrease in appropriations for state public health laboratories (SPHLs) has become a major concern as tax revenues and, subsequently, state and federal funding, have decreased. These reductions have forced SPHLs to pursue revenue-generating opportunities to support their work. We describe the current state of funding in a sampling of SPHLs and the challenges these laboratories face as they implement or expand fee-for-service testing. We conducted surveys of SPHLs to collect data concerning laboratory funding sources, test menus, fee-for-service testing, and challenges to implementing fee-for-service testing. Most SPHLS receive funding through three revenue sources: state appropriation, federal funding, and fee-for-service testing (cash funds). Among SPHLs, state appropriations ranged from $0 to more than $6 per capita, federal funding ranged from $0.10 to $5 per capita, and revenue from fee-for-service testing ranged from $0 to $4 per capita. The tests commonly performed on a fee-for-service basis included assays for sexually transmitted diseases, mycobacterial cultures, newborn screening, and water testing. We found that restrictive legislation, staffing shortages, inadequate software for billing fee-for-service testing, and regulations on how SPHLs use their generated revenue are impediments to implementing fee-for-service testing. Some SPHLs are considering implementing or expanding fee-for-service testing as a way to recapture funds lost as a result of state and federal budget cuts. This analysis revealed many of the obstacles to implementing fee-for-service testing in SPHLs and the potential impact on SPHLs of continued decreases in funding.

The Earth System Grid Federation (ESGF) Peer-to-Peer (P2P) enterprise system is a collaboration that develops, deploys and maintains software infrastructure for the management, dissemination, and analysis of model output and observational data. ESGF's primary goal is to facilitate advancements in Earth System Science. It is an interagency and international effort led by the US Department of Energy (DOE), and co-funded by National Aeronautics and Space Administration (NASA), National Oceanic and Atmospheric Administration (NOAA), National Science Foundation (NSF), Infrastructure for the European Network of Earth System Modelling (IS-ENES) and international laboratories such as the Max Planck Institute for Meteorology (MPI-M) german Climate Computing Centre (DKRZ), the Australian National University (ANU) National Computational Infrastructure (NCI), Institut Pierre-Simon Laplace (IPSL), and the British Atmospheric Data Center (BADC). Its main mission is to support current CMIP5 activities and prepare for future assesments. The ESGF architecture is based on a system of autonomous and distributed nodes, which interoperate through common acceptance of federation protocols and trust agreements. Data is stored at multiple nodes around the world, and served through local data and metadata services. Nodes exchange information about their data holdings and services, trust each other for registering users and establishing access control decisions. The net result is that a user can use a web browser, connect to any node, and seamlessly find and access data throughout the federation. This type of collaborative working organization and distributed architecture context en-lighted the need of integration and testing processes definition to ensure the quality of software releases and interoperability. This presentation will introduce the ESGF project and demonstrate the range of tools and processes that have been set up to support release management activities.

The NSF-funded EarthCube Integration and Test Environment (ECITE) prototype was proposed as a 2015 Integrated Activities project and resulted in the prototyping of an EarthCube federated cloud environment and the Integration and Testing Framework. The ECITE team has worked with EarthCube science and technology governance committees to define the types of integration, testing and evaluation necessary to achieve and demonstrate interoperability and functionality that benefit and support the objectives of the EarthCube cyber-infrastructure. The scope of ECITE also includes reaching beyond NSF and EarthCube to work with the broader Earth science community, such as the Earth Science Information Partners (ESIP) to incorporate lessons learned from other testbed activities, and ultimately provide broader community benefits. This presentation will discuss evolving ECITE ideas for a science-driven workbench that will start with documented science use cases, map the use cases to solution scenarios that identify the available technology and data resources that match the use case, the generation of solution workflows and test plans, the testing and evaluation of the solutions in a cloud environment, and finally the documentation of identified technology and data gaps that will assist with driving the development of additional EarthCube resources.

A comprehensive study and development of measuring instruments and techniques for measuring all components of flow in a storm-sewer drainage system was undertaken by the U.S. Geological Survey under the sponsorship of the Federal Highway Administration. The study involved laboratory and field calibration and testing of measuring flumes, pipe insert meters, weirs, electromagnetic velocity meters as well as the development and calibration of pneumatic-bubbler pressure transducer head measuring systems. Tracer-dilution and acoustic flow meter measurements were used in field verification tests. A single micrologger was used to record data from all the above instruments as well as from a tipping-bucket rain gage and also to activate on command the electromagnetic velocity meter and tracer-dilution systems. Unconventional drilling (the combination of hydraulic fracturing and horizontal drilling) to extract oil and natural gas is expanding rapidly around the world. The rate of expansion challenges scientists and regulators to assess the risks of the new technologies on drinking water resources. One concern is the potential for subsurface drinking water resource contamination by naturally occurring radioactive materials co-extracted during unconventional drilling activities. Given the rate of expansion, opportunities to test drinking water resources in the pre- and post-fracturing setting are rare. This pilot study investigated the levels of natural uranium, lead-210, and polonium-210 in private drinking wells within 2000 m of a large-volume hydraulic fracturing operation--before and approximately one-year following the fracturing activities. Observed radionuclide concentrations in well waters tested did not exceed maximum contaminant levels recommended by state and federal agencies. No statistically-significant differences in radionuclide concentrations were observed in well-water samples collected before and after the hydraulic fracturing activities. Expanded monitoring of private drinking wells before and after hydraulic fracturing activities is needed to develop understanding of the potential for drinking water resource contamination from unconventional drilling and gas extraction activities. Epidemiological Characteristics of Brucellosis in Federation of Bosnia and Herzegovina

Aim To analyze the frequency and distribution of human brucellosis in the Federation of Bosnia and Herzegovina in the period 2001-2008, and measures and activities undertaken for prevention and control of the disease. Method In this descriptive, retrospective study, we used official reports on infectious diseases from public health institutes at the federal and cantonal level, as well as epidemiological surveys. For comparison with animal brucellosis cases, we used the distribution data from veterinary surveillance. Results Since 2001, the number of infected people has rapidly increased and brucellosis has become a very important public health problem. In the period 2001-2008, there were 1639 human brucellosis cases and the number of cases increased every year. The morbidity rate over the study period ranged from 3.8 to 33.4 per 100 000 inhabitants. According to epidemiological surveys, in villages human brucellosis was transmitted mostly by contact with infected animals and their products, and in cities by consumption of dairy products made from contaminated, unpasteurized milk. When test-and-slaughter control approach was used, the prevalence of seropositive livestock was 4.6% and approximately 70 000 animals were slaughtered after testing between 2001 and 2008. From 1 June 2009, this approach was replaced with mass vaccination of sheep and goats. Conclusion The large number of human brucellosis cases and seropositive livestock poses a very serious problem for Bosnia and Herzegovina. The solution may be the introduction of mass vaccination. The past decade has witnessed both a proliferation of state oral health plans that include very specific proposals for action and an emergence of federal laws that include support for oral health. This paper provides an overview of state oral health priorities for action as reflected in 40 oral health plans that were developed independently by states. It examines four federal laws - the 2002 Safety Net Improvement Act, the 2009 CHIP Reauthorization Act, the 2009 economic stimulus law, and the 2010 health reform law - to identify opportunities for alignment with action steps proposed in state plans. This analysis identifies 23 categories of activity proposed by states in their action plans and determines that all but six of these activities are now supported by one or more of these four federal laws. State activities undertaken through grants provided under the 2002 Safety Net Improvement Act are analyzed as an example of how states can leverage federal legislation to advance their oral health plans. The paper concludes with consideration of the steps needed for states to promote their oral health plans by leveraging the full capacity of federal legislation. © 2012 American Association of Public Health Dentistry.

Climate change will likely have adverse human health effects that require federal agency involvement in adaptation activities. In 2009, President Obama issued Executive Order 13514, Federal Leadership in Environmental, Energy, and Economic Performance. The order required federal agencies to develop and implement climate change adaptation plans. The Centers for Disease Control and Prevention (CDC), as part of a larger Department of Health and Human Services response to climate change, is developing such plans. We provide background on Executive Orders, outline tenets of climate change adaptation, discuss public health adaptation planning at both the Department of Health and Human Services and the CDC, and outline possible future CDC efforts.
