To determine the prevalence of transportation related safety behaviors, such as seatbelt and helmet use, in primetime US television programs and commercials. Cross sectional study. Top rated television programs and associated commercials from four major US television networks were reviewed for the prevalence of transportation safety related behaviors during a one month period in 2005. Programs were categorized according to the time and network of airing, program type, program rating, and--for commercials--type of product being advertised Occupants of automobiles, motorcycles, or bicycles in 507 instances in which a transportation scene was aired. Seatbelt use was depicted in 62% and 86% of individuals in television program and commercial automobile scenes, respectively. The prevalence of motorcycle helmet use was 47% in television programs and 100% in commercials. Bicycle helmets were used in 9% of television programs and 84% of commercials. The frequency of seatbelt use in programs and commercials varied by television rating and genre but did not differ by network, time of airing, or age of character portrayed. The prevalence of safety related behaviors aired on major US networks during primetime slots is higher than previous reports but still much lower than national averages. Commercials, in contrast, portray transportation safety measures with a frequency that exceeds that of US television programs or most national surveys.

The DOT classification for transportation, the military classification for quantity distance, and hazard compatibility grouping used to regulate the transportation and storage of explosives are presented along with a discussion of tests used in determining sensitivity of propellants to an impact/shock environment in the absence of a large explosive donor. The safety procedures and requirements of a Scout launch vehicle, Western and Eastern Test Range, and the Minuteman, Delta, and Poseidon programs are reviewed and summarized. Requirements of the space transportation system safety program include safety reviews from the subsystem level to the completed payload. The Scout safety procedures will satisfy a portion of these requirements but additional procedures need to be implemented to comply with the safety requirements for Shuttle operation from the Eastern Test Range.

This report provides findings, conclusions and recommendations regarding the National Space Transportation System (NSTS), the Space Station Freedom Program (SSFP), aeronautical projects and other areas of NASA activities. The main focus of the Aerospace Safety Advisory Panel (ASAP) during 1988 has been monitoring and advising NASA and its contractors on the Space Transportation System (STS) recovery program. NASA efforts have restored the flight program with a much better management organization, safety and quality assurance organizations, and management communication system. The NASA National Space Transportation System (NSTS) organization in conjunction with its prime contractors should be encouraged to continue development and incorporation of appropriate design and operational improvements which will further reduce risk. The data from each Shuttle flight should be used to determine if affordable design and/or operational improvements could further increase safety. Safe-transport is important to well-being in later life but balancing safety and independence for older drivers can be challenging. While self-regulation is a promising tool to promote road safety, more research is required to optimise programs. Qualitative research was used to inform the choice and adaptation of a safe-transport education program for older drivers. Three focus groups were conducted with older drivers living in northwest Sydney to explore four key areas related to driving in later life including aged-based licensing, stopping or limiting driving, barriers to driving cessation and alternative modes of transportation. Data were analysed using content analysis. Four categories emerged from the data; bad press for older drivers, COMPETENCE not age, call for fairness in licensing regulations, and hanging up the keys: It's complicated! Two key issues being (1) older drivers wanted to drive for as long as possible but (2) were not prepared for driving cessation; guided the choice and adaption of the Knowledge Enhances Your Safety (KEYS) program. This program was adapted for the Australian context and focus group findings raised the need for practical solutions, including transport alternatives, to be added. The Commercial Crew Program (CCP) is leading NASA's efforts to develop the next U.S. capability for crew transportation and rescue services to and from the International Space Station (ISS) by the middecade timeframe. The outcome of this capability is expected to stimulate and expand the U.S. space transportation industry. NASA is relying on its decades of human space flight experience to certify U.S. crewed vehicles to the ISS and is doing so in a two phase certification approach. NASA certification will cover all aspects of a crew transportation system, including: Development, test, evaluation, and verification. Program management and control. Flight readiness certification. Launch, landing, recovery, and mission operations. Sustaining engineering and maintenance/upgrades. To ensure NASA crew safety, NASA certification will validate technical and performance requirements, verify compliance with NASA requirements, validate that the crew transportation system operates in the appropriate environments, and quantify residual risks. The Commercial Crew Program will present progress to date and how it manages safety and reduces risk.

The investigation of vehicle safety needs for older drivers and passengers is integral for their safe transportation. A program of research on safe transportation for seniors was launched through AUTO21, a Canadian Network of Centres of Excellence. This national research network focuses on a wide range of automotive issues, from materials and design to safety and societal issues. An inductive qualitative inquiry of seniors' driving experiences, safety feature use, and strategies to prevent injury and manage risks was a first step in this program. We conducted interviews and focus groups with 58 seniors without disabilities and 9 seniors with disabilities. We identified a lack of congruity between the vehicle and safety feature design and seniors' needs. Seniors described strategies to manage their safety and that of others. Specific aspects of vehicle design, safety features, and action strategies that support safer use and operation of a vehicle by seniors are outlined.

Background There are concerns over safety of older drivers due to increased crash involvement and vulnerability to injury. However, loss of driving privileges can dramatically reduce independence and quality of life for older members of the community. The aim of this trial is to examine the effectiveness of a safe transport program for drivers aged 75 years and older at reducing driving exposure but maintaining mobility. Methods and design A randomised trial will be conducted, involving 380 drivers aged 75 years and older, resident in urban and semi-rural areas of North-West Sydney. The intervention is an education program based on the Knowledge Enhances Your Safety (KEYS) program, adapted for the Australian context. Driving experience will be measured objectively using an in-vehicle monitoring device which includes a global positioning system (GPS) to assess driving exposure and an accelerometer to detect rapid deceleration events. Participation will be assessed using the Keele Assessment of Participation (KAP). Data will be analysed on an intention-to-treat basis; the primary outcomes include driving exposure, rapid deceleration events and scores for KAP. Secondary outcomes include self-reported measures of driving, socialisation, uptake of alternative forms of transport, depressive symptoms and mood. A detailed process evaluation will be conducted, including examination of the delivery of the program and uptake of alternative forms of transport. A subgroup analysis is planned for drivers with reduced function as characterized by established cut-off scores on the Drivesafe assessment tool. Discussion This randomised trial is powered to provide an objective assessment of the efficacy of an individually tailored education and alternative transportation program to promote safety of older drivers but maintain mobility. NASA's Aviation Safety Program (AvSP) develops and advances methodologies and technologies to improve air transportation safety. The Safety Analysis and Integration Team (SAIT) conducts a safety technology portfolio assessment (PA) to analyze the program content, to examine the benefits and risks of products with respect to program goals, and to support programmatic decision making. The PA process includes systematic identification of current and future safety risks as well as tracking several quantitative and qualitative metrics to ensure the program goals are addressing prominent safety risks accurately and effectively. One of the metrics within the PA process involves using quantitative aviation safety models to gauge the impact of the safety products. This paper demonstrates the role of aviation safety modeling by providing model outputs and evaluating a sample of portfolio elements using the Flightdeck Automation Problems (FLAP) model. The model enables not only ranking of the quantitative relative risk reduction impact of all portfolio elements, but also highlighting the areas with high potential impact via sensitivity and gap analyses in support of the program office. Although the model outputs are preliminary and products are notional, the process shown in this paper is essential to a comprehensive PA of NASA's safety products in the current program and future programs/projects.

The level of activity of the Aerospace Safety Advisory Panel was increased smewhat during 1985 in concert with the increased mission rate of the National Space Transportation System, the evolutionary changes in management and operation of that program, and the preparation of the Vandenberg Launch Site; the implementation of the Program Definition Phase of the Space Station Program; and the actual flight testing of the X-29 research aircraft. Impending payload STS missions and NASA's overall aircraft operations are reviewed. The safety aspects of the LEASAT salvage mission were assessed. The findings and recommendation of the committee are summerized.

NASA's Integrated Space Transportation Plan (ISTP) calls for investments in Space Shuttle safety upgrades, second generation Reusable Launch Vehicle (RLV) advanced development and third generation RLV and in-space research and technology. NASA's third generation launch systems are to be fully reusable and operation by 2025. The goals for third generation launch systems are to reduce cost by a factor of 100 and improve safety by a factor of 10,000 over current systems. The Advanced Space Transportation Program Office (ASTP) at NASA's Marshall Space Flight Center in Huntsville, AL has the agency lead to develop third generation space transportation technologies. The Hypersonics Investment Area, part of ASTP, is developing the third generation launch vehicle technologies in two main areas, propulsion and airframes. The program's major investment is in hypersonic airbreathing propulsion since it offers the greatest potential for meeting the third generation launch vehicles. The program will mature the technologies in three key propulsion areas, scramjets, rocket-based combined cycle and turbine-based combination cycle. Ground and flight propulsion tests are being planned for the propulsion technologies. Airframe technologies will be matured primarily through ground testing. This paper describes NASA's activities in hypersonics. Current programs, accomplishments, future plans and technologies that are being pursued by the Hypersonics Investment Area under the Advanced Space Transportation Program Office will be discussed.

NASA's Office of Safety and Mission Assurance sponsors an Agency-wide NDE Program that supports Aeronautics and Space Transportation Technology, Human Exploration and Development of Space, Earth Science, and Space Science Enterprises. For each of these Enterprises, safety is the number one priority. Development of the next generation aero-space launch and transportation vehicles, satellites, and deep space probes have highlighted the enabling role that NDE plays in these advanced technology systems. Specific areas of advanced component development, component integrity, and structural heath management are critically supported by NDE technologies. The simultaneous goals of assuring safety, maintaining overall operational efficiency, and developing and utilizing revolutionary technologies to expand human activity and space-based commerce in the frontiers of air and space places increasing demands on the Agencies NDE infrastructure and resources. In this presentation, an overview of NASA's NDE Program will be presented, that includes a background and status of current Enterprise NDE issues, and the NDE investment areas being developed to meet Enterprise safety and mission assurance needs through the year 2009 and beyond.

Walking or cycling to school represents an opportunity for children to engage in physical activity. The study objectives were to: 1) describe active transportation policies, programs, and built environments of Canadian schools and their surrounding neighbourhoods, and 2) document variations based on urban-rural location and school type (primary vs. secondary vs. mixed primary/secondary schools). 397 schools from across Canada were studied. A school administrator completed a questionnaire and responses were used to assess schools' policies and programs related to active transportation and the safety and aesthetics of their respective neighbourhoods. Built environment features in a 1 km-radius circular buffer around each school were measured using geographic information systems. Greater than 70% of schools had passive policies (e.g., skateboards permitted on school grounds) and facilities (e.g., bicycle racks in secure area to avoid theft) to encourage bicycle and small-wheeled vehicle use. Less than 40% of schools had active programs designed to encourage active transportation, such as organized 'walk to school' days. Garbage in the streets, crime and substance abuse were barriers in most school neighbourhoods. Approximately 42% of schools were located on high-speed roads not amenable to active transportation and 14% did not have a sidewalk leading to the school. Secondary schools had less favourable active transportation policies/programs and neighbourhood safety/aesthetics compared to primary schools. Rural schools had less favourable built environments than urban schools. Canadian children, particularly those from rural areas, face a number of impediments to active transportation as a method of travelling to school.

Hazardous materials transportation is an important and hot issue of public safety. Based on the shortest path model, this paper presents a fuzzy multi-objective programming model that minimizes the transportation risk to life, travel time and fuel consumption. First, we present the risk model, travel time model and fuel consumption model. Furthermore, we formulate a chance-constrained programming model within the framework of credibility theory, in which the lengths of arcs in the transportation network are assumed to be fuzzy variables. A hybrid intelligent algorithm integrating fuzzy simulation and genetic algorithm is designed for finding a satisfactory solution. Finally, some numerical examples are given to demonstrate the efficiency of the proposed model and algorithm.

The level of safety of the transportation system in Akure, Nigeria was assessed by identifying the associated road safety problems and developing the corresponding safety performance indicators. These indicators were analysed with respect to accidents that occurred within the city from the year 2005 to 2009 based on the corresponding attributable risk measures. The results of the analysis showed the state of existing safety programs in Akure town. Six safety performance indicators were identified namely alcohol and drug use, excessive speeds, protection system (use of seat belts and helmets), use of day time running lights, state of vehicles (passive safety) and road condition. These indicators were used to determine the percentage of injury accidents as follows: 83.33% and 86.36% for years 2005 and 2006 respectively, 81.46% for year 2007 while years 2008 and 2009 had 82.86% and 78.12% injury accidents respectively.

The programs within NASA's Aeronautics Research Mission Directorate (ARMD) conduct research and development to improve the national air transportation system so that Americans can travel as safely as possible. NASA aviation safety systems analysis personnel support various levels of ARMD management in their fulfillment of system analysis and technology prioritization as defined in the agency's program and project requirements. This paper provides a framework for the assessment of aviation safety research and technology portfolios that includes metrics such as projected impact on current and future safety, technical development risk and implementation risk. The paper also contains methods for presenting portfolio analysis and aviation safety Bayesian Belief Network (BBN) output results to management using bubble charts and quantitative decision analysis techniques.

The U.S. Department of Energy (DOE) chartered the Phosphoric Acid Fuel-Cell (PAFC) Bus Program to demonstrate the feasibility of fuel cells in heavy-duty transportation systems. As part of this program, PAFC- powered buses are being built to meet transit industry design and performance standards. Test-bed bus-1 (TBB-1) was designed in 1993 and integrated in March 1994. TBB-2 and TBB-3 are under construction and should be integrated in early 1995. In 1987 Phase I of the program began with the development and testing of two conceptual system designs- liquid- and air-cooled systems. The liquid-cooled PAFC system was chosen to continue, throughmore » a competitive award, into Phase H, beginning in 1991. Three hybrid buses, which combine fuel-cell and battery technologies, were designed during Phase III. After completing Phase II, DOE plans a comprehensive performance testing program (Phase HI) to verify that the buses meet stringent transit industry requirements. The Phase III study will evaluate the PAFC bus and compare it to a conventional diesel bus. This NREL study assesses the environmental, health, and safety (EH&S) issues that may affect the commercialization of the PAFC bus. There has been a long history of considering Safety, Security, and Safeguards (3S) as three functions of nuclear security design and operations that need to be properly and collectively integrated with operations. This paper specifically considers how safety programmes can be extended directly to benefit security as part of an integrated facility management programme. The discussion will draw on experiences implementing such a programme at Sandia National Laboratories’ Annular Research Reactor Facility. While the paper focuses on nuclear facilities, similar ideas could be used to support security programmes at other types of high-consequence facilities and transportation activities.

Preharvest food safety is essential for the protection of our food supply. The production and transport of livestock and poultry play an integral part in the safety of these food products. The goals of this safety assurance include freedom from pathogenic microorganisms, disease, and parasites, and from potentially harmful residues and physical hazards. Its functions should be based on hazard analysis and critical control points from producer to slaughter plant with emphasis on prevention of identifiable hazards rather than on removal of contaminated products. The production goal is to minimize infection and insure freedom from potentially harmful residues and physical hazards. The marketing goal is control of exposure to pathogens and stress. Both groups should have functional hazard analysis and critical control points management programs which include personnel training and certification of producers. These programs must cover production procedures, chemical usage, feeding, treatment practices, drug usage, assembly and transportation, and animal identification. Plans must use risk assessment principles, and the procedures must be defined. Other elements would include preslaughter certification, environmental protection, control of chemical hazards, live-animal drug-testing procedures, and identification of physical hazards.

The Commercial Crew Program (CCP) is leading NASA's efforts to develop the next U.S. capability for crew transportation and rescue services to and from the International Space Station (ISS) by the mid-decade timeframe. The outcome of this capability is expected to stimulate and expand the U.S. space transportation industry. NASA is relying on its decades of human space flight experience to certify U.S. crewed vehicles to the ISS and is doing so in a two phase certification approach. NASA Certification will cover all aspects of a crew transportation system, including development, test, evaluation, and verification; program management and control; flight readiness certification; launch, landing, recovery, and mission operations; sustaining engineering and maintenance/upgrades. To ensure NASA crew safety, NASA Certification will validate technical and performance requirements, verify compliance with NASA requirements, validate the crew transportation system operates in appropriate environments, and quantify residual risks.

A three-month study (February to April 2010) of the NASA Aviation Safety (AvSafe) program was conducted. This study comprised three components: (1) a statistical analysis of currently available civilian subsonic aircraft data from the National Transportation Safety Board (NTSB), the Federal Aviation Administration (FAA), and the Aviation Safety Information Analysis and Sharing (ASIAS) system to identify any significant or overlooked aviation safety issues; (2) a high-level qualitative identification of future safety risks, with an assessment of the potential impact of the NASA AvSafe research on the National Airspace System (NAS) based on these risks; and (3) a detailed, top-down analysis of the NASA AvSafe program using an established and peer-reviewed systems analysis methodology. The statistical analysis identified the top aviation "tall poles" based on NTSB accident and FAA incident data from 1997 to 2006. A separate examination of medical helicopter accidents in the United States was also conducted. Multiple external sources were used to develop a compilation of ten "tall poles" in future safety issues/risks. The top-down analysis of the AvSafe was conducted by using a modification of the Gibson methodology. Of the 17 challenging safety issues that were identified, 11 were directly addressed by the AvSafe program research portfolio.

Aviation is one of the critical modes of our national transportation system. As such, it is essential that new technologies be continually developed to ensure that a safe mode of transportation becomes even safer in the future. The NASA Aviation Safety Program (AvSP) is managing the development of new technologies and interventions aimed at reducing the fatal aviation accident rate by a factor of 5 by year 2007 and by a factor of 10 by year 2022. A portfolio assessment is currently being conducted to determine the projected impact that the new technologies and/or interventions may have on reducing aviation safety system risk. This paper reports on advanced risk analytics that combine the use of a human error taxonomy, probabilistic Bayesian Belief Networks, and case-based scenarios to assess a relative risk intensity metric. A sample case is used for illustrative purposes.

Aviation safety is challenged by the practical necessity of compromising inherent factors of design, environment, and operation. If accidents are to be avoided these factors must be controlled to a degree not often required by other transport modes. The operational problems which challenge safety seem to occur most often in the interfaces within and between the design, the environment, and operations where mismatches occur due to ignorance or lack of sufficient understanding of these interactions. The results of a study to identify potential hazards arising from nuclear reactor power systems for use on the lunar and Martian surfaces, related safety issues, and resolutions of such issues by system design changes, operating procedures, and other means are presented. All safety aspects of nuclear reactor power systems from prelaunch ground handling to eventual disposal were examined consistent with the level of detail for SP-100 reactor design at the 1988 System Design Review and for launch vehicle and space transport vehicle designs and mission descriptions as defined in the 90-day Space Exploration Initiative (SEI) study. Information from previous aerospace nuclear safety studies was used where appropriate. Safety requirements for the SP-100 space nuclear reactor system were compiled. Mission profiles were defined with emphasis on activities after low earth orbit insertion. Accident scenarios were then qualitatively defined for each mission phase. Safety issues were identified for all mission phases with the aid of simplified event trees. Safety issue resolution approaches of the SP-100 program were compiled. Resolution approaches for those safety issues not covered by the SP-100 program were identified. Additionally, the resolution approaches of the SP-100 program were examined in light of the moon and Mars missions.

The United States National Aeronautics and Space Administration (NASA) is in the midst of a 10-year Second Generation Reusable Launch Vehicle (RLV) program to improve its space transportation capabilities for both cargo and crewed missions. The objectives of the program are to: significantly increase safety and reliability, reduce the cost of accessing low-earth orbit, attempt to leverage commercial launch capabilities, and provide a growth path for manned space exploration. The safety, reliability and life cycle cost of the next generation vehicles are major concerns, and NASA aims to achieve orders of magnitude improvement in these areas. To get these significant improvements, requires a rigorous process that addresses Reliability, Maintainability and Supportability (RMS) and safety through all the phases of the life cycle of the program. This paper discusses the RMS process being implemented for the Second Generation RLV program.

Guidelines and requirements are presented for the shuttle, nuclear payloads (reactor, isotope-Brayton and small isotope sources), ground support systems and facilities. Cross indices and references are provided which relate guidelines to each other, and to substantiating data in other volumes. The guidelines are intended for the implementation of nuclear safety related design and operational considerations in future space programs.

The Safety, Tritium and Applied Research facility at the Idaho National Laboratory is a US Department of Energy National User Facility engaged in various aspects of materials research for nuclear applications related to fusion and advanced fission systems. Research activities are mainly focused on the interaction of tritium with materials, in particular plasma facing components, liquid breeders, high temperature coolants, fuel cladding, cooling and blanket structures and heat exchangers. Other activities include validation and verification experiments in support of the Fusion Safety Program, such as beryllium dust reactivity and dust transport in vacuum vessels, and support of Advanced Test Reactor irradiation experiments. This paper presents an overview of the programs engaged in the activities, which include the US-Japan TITAN collaboration, the US ITER program, the Next Generation Power Plant program and the tritium production program, and a presentation of ongoing experiments as well as a summary of recent results with emphasis on fusion relevant materials.

A particular challenge to NASA has been how to determine the commercial providers transportation system complies with Programmatic safety requirements. The process used in this determination is the Safety Technical Review Board which reviews and approves provider submitted Hazard Reports. One significant product of the review is a set of hazard control verifications. In past NASA programs, 100 percent of these safety critical verifications were typically confirmed by NASA. The traditional Safety and Mission Assurance (SMA) model does not support the nature of the Commercial Crew Program. To that end, NASA SMA is implementing a Risk Based Assurance (RBA) process to determine which hazard control verifications require NASA authentication. Additionally, a Shared Assurance Model is also being developed to efficiently use the available resources to execute the verifications. This paper will describe the evolution of the CCP Mission Assurance process from the beginning of the Program to its current incarnation. Topics to be covered include a short history of the CCP; the development of the Programmatic mission assurance requirements; the current safety review process; a description of the RBA process and its products and ending with a description of the Shared Assurance Model.

Probabilistic Risk Assessment (PRA) is a comprehensive, structured, and logical analysis method aimed at identifying and assessing risks in complex technological systems for the purpose of cost-effectively improving their safety and performance. NASA's objective is to better understand and effectively manage risk, and thus more effectively ensure mission and programmatic success, and to achieve and maintain high safety standards at NASA. NASA intends to use risk assessment in its programs and projects to support optimal management decision making for the improvement of safety and program performance. In addition to using quantitative/probabilistic risk assessment to improve safety and enhance the safety decision process, NASA has incorporated quantitative risk assessment into its system safety assessment process, which until now has relied primarily on a qualitative representation of risk. Also, NASA has recently adopted the Risk-Informed Decision Making (RIDM) process [1-1] as a valuable addition to supplement existing deterministic and experience-based engineering methods and tools. Over the years, NASA has been a leader in most of the technologies it has employed in its programs. One would think that PRA should be no exception. In fact, it would be natural for NASA to be a leader in PRA because, as a technology pioneer, NASA uses risk assessment and management implicitly or explicitly on a daily basis. NASA has probabilistic safety requirements (thresholds and goals) for crew transportation system missions to the International Space Station (ISS) [1-2]. NASA intends to have probabilistic requirements for any new human spaceflight transportation system acquisition. Methods to perform risk and reliability assessment in the early 1960s originated in U.S. aerospace and missile programs. Fault tree analysis (FTA) is an example. It would have been a reasonable extrapolation to expect that NASA would also become the world leader in the application of PRA. That was, however, not to happen. Early in the Apollo program, estimates of the probability for a successful roundtrip human mission to the moon yielded disappointingly low (and suspect) values and NASA became discouraged from further performing quantitative risk analyses until some two decades later when the methods were more refined, rigorous, and repeatable. Instead, NASA decided to rely primarily on the Hazard Analysis (HA) and Failure Modes and Effects Analysis (FMEA) methods for system safety assessment.

In 2010, the National Aeronautics and Space Administration (NASA) established the Commercial Crew Program (CCP) in order to provide human access to the International Space Station and low Earth orbit via the commercial (non-governmental) sector. A particular challenge to NASA has been how to determine that the Commercial Provider's transportation system complies with programmatic safety requirements. The process used in this determination is the Safety Technical Review Board which reviews and approves provider submitted hazard reports. One significant product of the review is a set of hazard control verifications. In past NASA programs, 100% of these safety critical verifications were typically confirmed by NASA. The traditional Safety and Mission Assurance (S&MA) model does not support the nature of the CCP. To that end, NASA S&MA is implementing a Risk Based Assurance process to determine which hazard control verifications require NASA authentication. Additionally, a Shared Assurance Model is also being developed to efficiently use the available resources to execute the verifications.

The Commercial Crew Program (CCP) is unique to any other program office at NASA. After the agency suffered devastating budget cuts and the Shuttle Program retired, the U.S. gave up its human spaceflight capabilities. Since 2011 the U.S. has been dependent on Russia to transport American astronauts and cargo to the International Space Station (ISS) and back. NASA adapted and formed CCP, which gives private, domestic, aerospace companies unprecedented reign over America's next ride to space. The program began back in 2010 with 5 companies and is now in the final phase of certification with 2 commercial partners. The Commercial Crew Program is made up of 7 divisions, each working rigorously with the commercial providers to complete the certification phase. One of these 7 divisions is Systems Engineering and Integration (SE&I) which is partly comprised of the Safety Technical Review Board (STRB). The STRB is primarily concerned with mitigating improbable, but catastrophic hazards. It does this by identifying, managing, and tracking these hazards in reports. With the STRB being in SE&I, it significantly contributes to the overall certification of the partners' vehicles. After the partners receive agency certification approval, they will have the capability to provide the U.S. with a reliable, safe, and cost-effective means of human spaceflight and cargo transport to the ISS and back.

Work-related travel and transport by road is fundamental for industry, government and organisations. Traditionally, road safety interventions at societal level have focussed on improving road and vehicle engineering and changing road-user behaviour through transport laws and safety campaigns. Crash data indicate that significant numbers of road-user fatalities occur while driving to or for work. Therefore, workplace initiatives can improve both road and worker safety. This paper reviews regulatory approaches to work-related road safety (WRRS) in Australia, the United Kingdom and United States, identifying significant and consistent gaps in policy, management and research. In all three countries, responsibility for managing and regulating WRRS is spread across government agencies, without a single coordinating body. This paper makes the case that integrating management of WRRS into regulatory and non-regulatory occupational health and safety (OHS) initiatives would foster and support collaboration between research and practice communities, ensuring a comprehensive evidence base for future programs. NASA's has established long term goals for access-to-space. NASA's third generation launch systems are to be fully reusable and operational in approximately 25 years. The goals for third generation launch systems are to reduce cost by a factor of 100 and improve safety by a factor of 10,000 over current conditions. The Advanced Space Transportation Program Office (ASTP) at NASA's Marshall Space Flight Center in Huntsville, AL has the agency lead to develop third generation space transportation technologies. The Hypersonics Investment Area, part of ASTP, is developing the third generation launch vehicle technologies in two main areas, propulsion and airframes. The program's major investment is in hypersonic airbreathing propulsion since it offers the greatest potential for meeting the third generation launch vehicles. The program will mature the technologies in three key propulsion areas, scramjets, rocket-based combined cycle and turbine-based combination cycle. Ground and flight propulsion tests are being planned for the propulsion technologies. Airframe technologies will be matured primarily through ground testing. This paper describes NASA's activities in hypersonics. Current programs, accomplishments, future plans and technologies that are being pursued by the Hypersonics Investment Area under the Advanced Space Transportation Program Office will be discussed.

This report presents three-year accomplishments from the national program on Commercial Remote Sensing and Geospatial Technology (CRSGT) application to transportation, administered by the U.S. Department of Transportation (U.S. DOT) in collaboration with the National Aeronautics and Space Administration (NASA). The joint program was authorized under Section 5113 of the Transportation Equity Act for the 21st Century (TEA-21). This is the first national program of its type focusing on transportation applications of emerging commercial remote sensing technologies. U.S. DOT's Research and Special Programs Administration manages the program in coordination with NASA's Earth Science Enterprise's application programs. The program focuses on applications of CRSGT products and systems for providing smarter and more efficient transportation operations and services. The program is performed in partnership with four major National Consortia for Remote Sensing in Transportation (NCRST). Each consortium focuses on research and development of products in one of the four priority areas for transportation application, and includes technical application and demonstration projects carried out in partnership with industries and service providers in their respective areas. The report identifies products and accomplishments from each of the four consortia in meeting the goal of providing smarter and more efficient transportation services. The products and results emerging from the program are being implemented in transportation operations and services through state and local agencies. The Environmental Assessment and Application Consortium (NCRST-E) provides leadership for developing and deploying cost effective environmental and transportation planning services, and integrates CRSGT advances for achieving smarter and cost effective corridor planning. The Infrastructure Management Consortium (NCRST-I) provides leadership in technologies that achieve smarter and cheaper ways of managing transportation infrastructure assets, operation, and inspection, and integrates CRSGT advances for achieving infrastructure security. The Traffic Flow Consortium (NCRST-F) provides leadership to develop new tools for regional traffic flow management including heavy vehicles and intermodal flow of freight, and integrates CRSGT advances for complementing and extending the reach of ITS user services. The Safety, Hazards and Disasters (NCRST-H) provides leadership for deploying remote sensing technology to locate transportation hazards and improve disaster recovery, and integrates CRSGT advances for application to protect transportation systems from terrorism. The DOT-NASA team is proud to present this report of accomplishments on products and results emerging from the joint program for application to transportation practice.

This report examines the extent of human factors effects on the safety of transporting radioactive waste materials. It is seen principally as a scoping effort, to establish whether there is a need for DOE to undertake a more formal approach to studying human factors in radioactive waste transport, and if so, logical directions for that program to follow. Human factors effects are evaluated on driving and loading/transfer operations only. Particular emphasis is placed on the driving function, examining the relationship between human error and safety as it relates to the impairment of driver performance. A preliminary shielding analysis on the transport of the Chinese helium cooled ceramic breeder test blanket module (HCCB TBM) from France back to China after being irradiated in ITER is presented in this contribution. Emphasis was placed on irradiation safety during transport. The dose rate calculated by MCNP/4C for the conceptual package design satisfies the relevant dose limits from IAEA that the dose rate 3 m away from the surface of the package containing low specific activity III materials should be less than 10 mSv/h. The change with location and the time evolution of dose rates after shutdown have also been studied. An assessment of NASA's safety performance for 1983 affirms that NASA Headquarters and Center management teams continue to hold the safety of manned flight to be their prime concern, and that essential effort and resources are allocated for maintaining safety in all of the development and operational programs. Those conclusions most worthy of NASA management concentration are given along with recommendations for action concerning; product quality and utility; space shuttle main engine; landing gear; logistics and management; orbiter structural loads, landing speed, and pitch control; the shuttle processing contractor; and the safety of flight operations. It appears that much needs to be done before the Space Transportation System can achieve the reliability necessary for safe, high rate, low cost operations.
