Driving forces of biomedical science education and research in state-of-the arts academic medical centres: the United States as example.

Basic science departments in academic medical centres are influenced by changes that are commonly directed at medical education and financial gain. Some of such changes may have been detrimental to or may have enhanced basic science education. They may have determined basic science research focus or basic science research methods. However, there is lack of research on the educational process in the basic sciences including training of PhD's while there is ample research on medical education pertaining to training of medical doctors. The author here identifies, from university websites and available literature, some forces that have driven teaching and research focus and methods in state-of-the-arts academic medical centres in recent times with a view of seeing through their possible influences on basic science education and research, using the United States of America as an example. The "forces" are: Changes in medical schools; Medical educational philosophies: problem based learning, evidence based medicine, cyberlearning and self-directed learning; Shifting impressions of the value of basic sciences in medical schools; Research trends in Basic Sciences: role of antivivisectionists, alternative experimentations, explosion of molecular and cell biology; Technological advancements; Commercialization of research; and Funding agencies. The author encourages African leaders in academia to pay attention to such forces as the leadership seeks to raise African Universities as centres of knowledge that have a major role in acquiring, preserving, imparting, and utilizing knowledge.

The production and consumption of industrial minerals in the United States traditionally have played important roles in mining and in the supply of the bulk of basic raw materials to the economy. This diverse group of minerals extracted and consumed by a variety of industries accounts, on a weight and volume basis, for most mineral-based products consumed in our economy. Industrial minerals form the bulk of the basic raw-materials feedstock for most of the construction, agricultural, and inorganic-chemical-manufacturing sectors, and a good portion of the transportation, manufacturing, organic chemical, and service sectors of the U.S. economy. In this presentation, I discuss current estimates of the amount and value of industrial minerals mined compared with all mining in 1997. I then present a summary of 1997 production of industrial minerals by several end-use industries. This is followed by a historical perspective of the consumption of industrial minerals as a subset of all basic materials, with special emphasis on the aggregates industry. The presentation concludes with a look at the geographic distribution of industrial operations in the United States and some thoughts about the future.

We employ two surveys to identify similarities and differences in the risk of abuse among poor urban Mexican-origin women in the United States and Mexico. While the two surveys reveal basic structural similarity in the predictors of partner violence, the rate of violence among Mexican women is far lower than among either foreign-born or native-born Mexican origin women in the United States. While these differences may reflect reality, we argue that survey data must be interpreted cautiously and with an understanding of the cultural, economic, and political context in which the information is collected as well as methodological differences between the surveys.

ABSTRACT OBJECTIVE To describe the utilization of basic health units according to coverage by discount card or private health insurance. METHODS Household survey in the area covered by Family Health Strategy in Pelotas, state of Rio Grande do Sul, Brazil, from December 2007 to February 2008, with persons of all age groups. The frequency of (medical or non-medical) healthcare seeking at the basic health units in the last six months and the prevalence of basic health unit utilization for the last medical consultation (in case it had been performed up to six months before, for a non-routine reason) were analyzed by Poisson regression adjusted for the sampling design. RESULTS Of the 1,423 persons, 75.6% had no discount card or private health insurance. The average frequency of (medical or non-medical) healthcare seeking was 1.6 times in six months (95%CI 1.3-2.0); this frequency was 55.8% lower (p < 0.001) among privately insured persons compared to those with no discount card or private health insurance. Among the last medical consultations, 35.8% (95%CI 25.4-47.7) had been performed at the basic health units; this prevalence was 36.4% lower (p = 0.003) among persons covered by discount card and 87.7% lower (p = 0.007) among privately insured persons compared to those without both coverages. CONCLUSIONS Private health insurance and, to a lesser degree, discount card coverage, are related to lower utilization of basic health units. This can be used to size the population under the accountability of each Family Health Strategy team, to the extent that community health workers are able to differentiate discount card from PHI during family registration. This study aimed to compare information provided on student counseling center websites of universities and colleges in Japan, the United States, the United Kingdom, and Taiwan. A survey was conducted on websites of 315 centers in Japan, 282 centers in the United States, 70 centers in the United Kingdom and 61 centers in Taiwan. Trends in the provision of information on websites in each country were analyzed and compared for the rate and quantity of information published. Results of multiple correspondence analyses indicated two basic dimensions of information that could effectively distinguish information provided in the four countries. These were provision of necessary information and provision of information for use of individual counseling or support of community. Finally, issues related to websites in student counseling centers of Japanese universities and colleges are discussed.

To characterize the state of ophthalmology medical student education in the United States and Canada. Survey of United States and Canadian medical schools. One hundred thirty-five Association of University Professors of Ophthalmology (AUPO) member institutions were surveyed, along with 30 osteopathic medical schools in the United States and 40 non-AUPO-affiliated allopathic medical schools in the United States. A survey characterizing preclinical, clinical, and extracurricular exposures to ophthalmology was used. Response rate, presence of, and types of preclinical and clinical exposures. Response rates to the survey were lower from non-AUPO institutions. Preclinical exposures largely consisted of basic lectures and examination skills, and most responding institutions had some sort of required preclinical ophthalmology experience. Clinical exposures were more variable, with an overall rate of required clinical rotations diminishing. There continues to be a gradual erosion of the role of ophthalmic medical education in the standard medical school curriculum. Building a human rights framework for workers' compensation in the United States: opening the debate on first principles.

This article introduces the idea of human rights to the topic of workers' compensation in the United States. It discusses what constitutes a human rights approach and explains how this approach conflicts with those policy ideas that have provided the foundation historically for workers' compensation in the United States. Using legal and historical research, key international labor and human rights standards on employment injury benefits and influential writings in the development of the U.S. workers' compensation system are cited. Workers' injury and illness compensation in the United States does not conform to basic international human rights norms. A comprehensive review of the U.S. workers' compensation system under international human rights standards is needed. Examples of policy changes are highlighted that would begin the process of moving workers' compensation into conformity with human rights standards. Copyright © 2012 Wiley Periodicals, Inc.

We report on scalable solid-state neutron detector system that is specifically designed to yield high thermal neutron detection sensitivity. The basic detector unit in this system is made of a (6)Li foil coupled to two crystalline silicon diodes. The theoretical intrinsic efficiency of a detector-unit is 23.8% and that of detector element comprising a stack of five detector-units is 60%. Based on the measured performance of this detector-unit, the performance of a detector system comprising a planar array of detector elements, scaled to encompass effective area of 0.43 m(2), is estimated to yield the minimum absolute efficiency required of radiological portal monitors used in homeland security.

Rapid changes in the healthcare environment and public dissatisfaction with the cost and quality of medical care have prompted a critical analysis of how physicians are trained in the United States. Accrediting agencies have catalyzed a transformation from a process based to a competency-based curriculum, both at the undergraduate and the graduate levels. The objective of this overview is to determine how these changes are likely to alter the role of basic science in medical education. Policy statements related to basic science education from the National Board of Medical Examiners (NBME), the Accreditation Council for Graduate Medical Education (ACGME), American Board of Medical Specialties (ABMS), and the Federation of State Medical Boards (FSMB) were reviewed and assessed for common themes. Three primary roles for the basic sciences in medical education are proposed: (1) basic science to support the development of clinical reasoning skills; (2) basic science to support a critical analysis of medical and surgical interventions ("evidence-based medicine"); and (3) basic and translational science to support analysis of processes to improve healthcare ("science of healthcare delivery"). With these roles in mind, several methods to incorporate basic sciences into the curriculum are suggested.

The U.S. health care system has major problems with respect to patient access and cost control. Trimming excess hospital expenses and expanding public health activities are cost effective. By budgeting well, with global budgets set for the high cost sectors, the United States might emerge with lower tax hikes, a healthier population, better facilities, and enhanced access to service. Nations with global budgets have better health statistics, and lower costs, compared to the United States. With global budgets, these countries employ 75 to 85 percent fewer employees in administration and regulation, but patient satisfaction is almost double the rate in the United States. Implement a global budget for health care, or substantially raise taxes, is the basic choice faced in this country. Key words: global budget control cost containment.

Racial Formation in Theory and Practice: The Case of Mexicans in the United States.

Mechanisms of social stratification require the categorical definition of an out-group to that can be excluded and exploited. Historically, in the United States, African Americans have been the subject of a systematic process of racial formation to define socially in this fashion. Beginning in the 1970s, however, and accelerating in the 1980s and 1990s, Mexicans were increasingly subject to processes of racialization that have rendered them more exploitable and excludable than ever before. Over the past decade, Mexican Americans moved steadily away from their middle position in the socioeconomic hierarchy and gravitated toward the bottom. This paper describes the basic mechanisms of stratification in the United States and how Mexicans have steadily been racialized to label them socially as a dehumanized and vulnerable out-group.

The "four principles plus scope" approach provides a simple, accessible, and culturally neutral approach to thinking about ethical issues in health care. The approach, developed in the United States, is based on four common, basic prima facie moral commitments--respect for autonomy, beneficence, nonmaleficence, and justice--plus concern for their scope of application. It offers a common, basic moral analytical framework and a common, basic moral language. Although they do not provide ordered rules, these principles can help doctors and other health care workers to make decisions when reflecting on moral issues that arise at work.

This report is the third in a series presenting, annually, basic data on surface-water records by States. Through September 30, 1960, the records of discharge and stage of streams and contents and stage of lakes or reservoirs were published in an annual series of U. S. Geological Survey water-supply papers entitled "Surface Water Supply of the United States". Since 1951 there have been 20 volumes in the series; each volume covered an area whose boundaries coincided with those of certain natural drainage areas. The records in New Mexico were contained in Parts 7, 8, and 9 of that series. Beginning with the 1961 water year, streamflow records and related data will be released by the Geological Survey in annual reports on a State-boundary basis. Distribution of these basic-data reports will be limited and primarily for local needs. The records later will be published in Geological Survey water-supply papers at 5- year intervals. These 5-year water-supply papers will show daily discharge and will be compiled on the same geographical areas previously used for the annual series; however, some of the 14 parts of conterminous United States will be further subdivided.

The establishment of Universitas Pendidikan Indonesia (later to be referred as UPI) Statute as a State-Owned State University (PTN-BH) has implications for UPI requirements. One of them is the need for UPI to generate an Income Generating Unit (IGU) of at least IDR 100 Billion (one hundred billion rupiah). This requirement is considered difficult since UPI is one of the universities whose focus is on the world of education and not the business and industry. Surely this becomes the thinking of the entire academic community to make a breakthrough by optimizing their potential. This study aims to find the pattern of learning practice that produces economic value products as one indicator of IGU value achievement as an effort to support UPI as PTN-BH. Learning strategy is done by designing and implementing the production base learning (PBL) approach as the basis strategy for the development of production units capable of becoming IGU in UPI. The research method used refers to research and development methods with adjustments taking into account the effectiveness in validating and conducting field model trials. The result of this research is the basic design of PBL model as the development strategy of production unit in the achievement of IGU UPI PTN-BH.

Divergent projections of future land use in the United States arising from different models and scenarios

A variety of land-use and land-cover (LULC) models operating at scales from local to global have been developed in recent years, including a number of models that provide spatially explicit, multi-class LULC projections for the conterminous United States. This diversity of modeling approaches raises the question: how consistent are their projections of future land use? We compared projections from six LULC modeling applications for the United States and assessed quantitative, spatial, and conceptual inconsistencies. Each set of projections provided multiple scenarios covering a period from roughly 2000 to 2050. Given the unique spatial, thematic, and temporal characteristics of each set of projections, individual projections were aggregated to a common set of basic, generalized LULC classes (i.e., cropland, pasture, forest, range, and urban) and summarized at the county level across the conterminous United States. We found very little agreement in projected future LULC trends and patterns among the different models. Variability among scenarios for a given model was generally lower than variability among different models, in terms of both trends in the amounts of basic LULC classes and their projected spatial patterns. Even when different models assessed the same purported scenario, model projections varied substantially. Projections of agricultural trends were often far above the maximum historical amounts, raising concerns about the realism of the projections. Comparisons among models were hindered by major discrepancies in categorical definitions, and suggest a need for standardization of historical LULC data sources. To capture a broader range of uncertainties, ensemble modeling approaches are also recommended. However, the vast inconsistencies among LULC models raise questions about the theoretical and conceptual underpinnings of current modeling approaches. Given the substantial effects that land-use change can have on ecological and societal processes, there is a need for improvement in LULC theory and modeling capabilities to improve acceptance and use of regional- to national-scale LULC projections for the United States and elsewhere.

"Choosing Healthplans All Together" (CHAT) is a small group decision exercise designed to give the public a voice in priority setting in the face of unsustainable health care costs. It has been used for research, policy, and teaching purposes. Departments of insurance in various states in the United States have used CHAT to determine public opinion about what should be included in basic health insurance packages for the uninsured. Some municipalities have used it to assess public priorities for direct service delivery to the uninsured. Setting up the exercise requires substantial preparation, but the public finds it simple to use and understand.

An economic analysis of payment for health care services: the United States and Switzerland compared.

This article seeks to assess whether physician payment reforms in the United States and Switzerland were likely to attain their objectives. We first introduce basic contract theory, with the organizing principle being the degree of information asymmetry between the patient and the health care provider. Depending on the degree of information asymmetry, different forms of payment induce "appropriate" behavior. These theoretical results are then pitted against the RBRVS of the United States to find that a number of its aspects are not optimal. We then turn to Switzerland's Tarmed and find that it fails to conform with the prescriptions of economic contract theory as well. The article closes with a review of possible reforms that could do away with uniform fee schedules to improve the performance of the health care system.

Quantum Computation Using Optically Coupled Quantum Dot Arrays

A solid state model for quantum computation has potential advantages in terms of the ease of fabrication, characterization, and integration. The fundamental requirements for a quantum computer involve the realization of basic processing units (qubits), and a scheme for controlled switching and coupling among the qubits, which enables one to perform controlled operations on qubits. We propose a model for quantum computation based on optically coupled quantum dot arrays, which is computationally similar to the atomic model proposed by Cirac and Zoller. In this model, individual qubits are comprised of two coupled quantum dots, and an array of these basic units is placed in an optical cavity. Switching among the states of the individual units is done by controlled laser pulses via near field interaction using the NSOM technology. Controlled rotations involving two or more qubits are performed via common cavity mode photon. We have calculated critical times, including the spontaneous emission and switching times, and show that they are comparable to the best times projected for other proposed models of quantum computation. We have also shown the feasibility of accessing individual quantum dots using the NSOM technology by calculating the photon density at the tip, and estimating the power necessary to perform the basic controlled operations. We are currently in the process of estimating the decoherence times for this system; however, we have formulated initial arguments which seem to indicate that the decoherence times will be comparable, if not longer, than many other proposed models.

The ongoing, dramatic increase in seismicity in the central United States that began in 2009 is believed to be the result of injection‐induced seismicity (Ellsworth, 2013). Although the basic mechanism for activation of slip on a fault by subsurface fluid injection is well established (Healy et al., 1968; Raleighet al., 1976; Nicholson and Wesson, 1992; McGarr et al., 2002; Ellsworth, 2013), the occurrence of damaging M≥5 earthquakes and the dramatic increase in seismicity in the central United States has brought heightened attention to this issue. The elevated seismicity is confined to a limited number of areas, and accumulating evidence indicates that the seismicity in these locations is directly linked to nearby industrial operations. This Seismological Research Letters (SRL) focus section presents a selected set of seven technical papers that cover various aspects of this topic, including basic seismological and ground‐motion observations, case studies, numerical simulation of fault activation, and risk mitigation.

Operational integration in primary health care: patient encounters and workflows.

Despite several countrywide attempts to strengthen and standardise the primary healthcare (PHC) system, Greece is still lacking a sustainable, policy-based model of integrated services. The aim of our study was to identify operational integration levels through existing patient care pathways and to recommend an alternative PHC model for optimum integration. The study was part of a large state-funded project, which included 22 randomly selected PHC units located across two health regions of Greece. Dimensions of operational integration in PHC were selected based on the work of Kringos and colleagues. A five-point Likert-type scale, coupled with an algorithm, was used to capture and transform theoretical framework features into measurable attributes. PHC services were grouped under the main categories of chronic care, urgent/acute care, preventive care, and home care. A web-based platform was used to assess patient pathways, evaluate integration levels and propose improvement actions. Analysis relied on a comparison of actual pathways versus optimal, the latter ones having been identified through literature review. Overall integration varied among units. The majority (57%) of units corresponded to a basic level. Integration by type of PHC service ranged as follows: basic (86%) or poor (14%) for chronic care units, poor (78%) or basic (22%) for urgent/acute care units, basic (50%) for preventive care units, and partial or basic (50%) for home care units. The actual pathways across all four categories of PHC services differed from those captured in the optimum integration model. Certain similarities were observed in the operational flows between chronic care management and urgent/acute care management. Such similarities were present at the highest level of abstraction, but also in common steps along the operational flows. Existing patient care pathways were mapped and analysed, and recommendations for an optimum integration PHC model were made. The developed web platform, based on a strong theoretical framework, can serve as a robust integration evaluation tool. This could be a first step towards restructuring and improving PHC services within a financially restrained environment.

The United States Air Traffic Management (ATM) system provides services to enable safe, orderly and efficient aircraft operations within the airspace over the continental United States and over large portions of the Pacific and Atlantic Oceans, and the Gulf of Mexico. It consists of two components, Air Traffic Control (ATC) and Traffic Flow Management (TFM). The ATC function ensures that the aircraft within the airspace are separated at all times while the TFM function organizes the aircraft into a flow pattern to ensure their safe and efficient movement. In order to accomplish the ATC and TFM functions, the airspace over United States is organized into 22 Air Route Traffic Control Centers (ARTCCs). The Center airspace is stratified into low-altitude, high-altitude and super-high altitude groups of Sectors. Each vertical layer is further partitioned into several horizontal Sectors. A typical ARTCC airspace is partitioned into 20 to 80 Sectors. These Sectors are the basic control units within the ATM system.

This study investigated the association between contraceptive sterilization and socio-economic status (measured by educational attainment) in ten countries, using data from the 2006-10 National Survey of Family Growth and the 2004-10 Generations and Gender Surveys. The findings confirm that a long-standing association between socio-economic status and sterilization persists in the contemporary United States: female sterilization is associated with economic disadvantage, whereas male sterilization is associated with economic advantage. The latter association is found to be unique to the United States, but female sterilization is associated with disadvantage in most of the other countries studied. While basic demographic background factors such as early childbearing and parity can explain the observed associations in most of the countries, a strong gendered association between sterilization and socio-economic status remains in the United States and Belgium even after adjusting for these factors.

Despite the media attention focused on the Islamic community after the terrorist attacks on the World Trade Center on September 11, 2001, Muslims remain one of the most misunderstood populations in the United States. Few articles have appeared in the social work literature orienting practitioners to the Islamic community, and much of the mainstream media coverage misrepresents the population. This article reviews the basic beliefs, practices, and values that commonly characterize, or inform, the House of Islam in the United States. The organizations that embody and sustain the Muslim communities that constitute the House of Islam are profiled, and areas of possible value conflicts are examined. The article concludes by offering suggestions for integrating the article's themes into practice settings. Particular attention is given to enhancing cultural competence and to suggestions for spiritual assessment and interventions.

A significant number of empirical studies have reported that parental support for basic psychological needs is a robust correlate of adolescent happiness. Yet, less is known about the mechanisms responsible for this link. The present study proposed a model suggesting that personal sense of uniqueness explains why satisfaction of basic psychological needs in parent-child relationships is related to happiness. This mediational model was tested among late adolescents in Turkey and the United States. Analyses relying on structural equation modeling and bootstrapping supported the model in both cultures. Implications of the findings for theory and cross-cultural research are discussed. Directions for future research that could improve our understanding of the dynamic interplay between basic needs, sense of uniqueness and well-being are provided.

In recent decades, great strides have been made globally in decreasing child mortality. However, given that many countries still do not have basic healthcare, additional emphasis is being placed on health promotion activities among industrialized nations. As cultural differences of individual countries impact these health promotion practices, the cultural characteristics influencing children and families in two countries, South Korea and the United States, were compared. Major child health risk factors were examined, and health retention strategies tailored to the cultural characteristics and needs of the populations of each country are proposed, using the Neuman Systems Model as a guideline.

The average annual streamflow--simplified measure of the total available water supply--is approximately 1,200 bgd in the conterminous United States. Total water withdrawn in 1970 for off-channel uses (withdrawals other than for hydroelectric power) amounted to about 30 percent of the average annual streamflow: 7 percent of the 1,200 bgd basic supply was consumed. However, comparisons of Water Resources Council regions indicate that the rate of withdrawal was higher than the locally dependable supply in the Middle Atlantic, Texas-Gulf, Rio Grande, Lower Colorado, and California-South Pacific regions. Consumption amounted to nearly 25 percent of withdrawals in the conterminous United States; however, fresh-water consumption amounted to only 14 percent of off-channel withdrawals in the 31 Eastern States and ranged from 30 percent to nearly 70 percent of off-channel withdrawals in the Water Resources Council regions in the West. In the Rio Grande and Lower Colorado regions, fresh-water consumption in 1970 exceeded the estimated dependable supply of fresh water.

In 1958, the United Nations (UN) formally recognized a new potential for international cooperation by establishing an ad hoc Committee on the Peaceful Uses of Outer Space (COPUOS). A year later the Committee became a permanent body, and by 1983 membership had expanded to 53 states, with more than half of the members coming from the developing world. In 1970, COPUOS established the UN Program on Space Applications in order to strengthen cooperation in space science and technology between non-industrialized and industrialized countries. In the last few years, the UN and its COPUOS have paid increasing attention to education and research in space science and technology, including basic space science. In 1991 the UN, in cooperation with ESA, initiated the organization of annual Workshops in Basic Space Science for developing countries. These Workshops are designed to be held in one of the following major regions: Asia and the Pacific, Latin America and the Caribbean, Africa, Western Asia, and Europe. Accordingly, Basic Space Science Workshops have already been held in India (1991), Costa Rica andColombia (1992), and Nigeria (1993). The fourth Workshop was held from 27 June to 1 July 1994 at the Cairo University, in Egypt, for Western Asia.

The family is the basic unit of any society and culture. The concept, structure, and function of the family unit vary considerably across different cultures; however, its role continues to be imperative to the development of individuals and their psychological make-up. All societies have a concept of 'family', its relative importance, structure, and functions; however, this varies according to the particular culture. In the Arabic culture, as well as other collectivistic cultures, the extended family is often regarded as the basic unit. The family is the foundational and basic social unit that fosters the stability, well-being and sustainability of society. The quality of family relationships shapes and influences the social, psychological, and biological development and functioning of its members. This may be especially relevant to individuals with mental health problems. The people of ancient Egypt valued family life highly, and this is the case even now. They treasured children and regarded them as a great blessing. If a couple had no children, they would pray to the gods and goddesses for help. They would also place letters at the tombs of dead relatives asking them to use their influence with the gods. The importance of family has not changed dramatically even though the structures are beginning to. In this paper we highlight changes in family set-up and the state of family therapy in Egypt.

Influence of curriculum type on student performance in the United States Medical Licensing Examination Step 1 and Step 2 exams: problem-based learning vs. lecture-based curriculum.

The results of the United States Medical Licensing Examination Step 1 and 2 examinations are reported for students enrolled in a problem-based and traditional lecture-based curricula over a seven-year period at a single institution. There were no statistically significant differences in mean scores on either examination over the seven year period as a whole. There were statistically significant main effects noted by cohort year and curricular track for both the Step 1 and 2 examinations. These results support the general, long-term effectiveness of problem-based learning with respect to basic and clinical science knowledge acquisition. This paper reports the United States Medical Licensing Examination Step 1 and Step 2 results for students enrolled in a problem-based and traditional lecture-based learning curricula over the seven-year period (1992-98) in order to evaluate the adequacy of each curriculum in supporting students learning of the basic and clinical sciences. Six hundred and eighty-nine students who took the United States Medical Licensing Examination Step 1 and 540 students who took Step 2 for the first time over the seven-year period were included in the analyses. T-test analyses were utilized to compare students' Step 1 and Step 2 performance by curriculum groups. United States Medical Licensing Examination Step 1 scores over the seven-year period were 214 for Traditional Curriculum students and 208 for Parallel Curriculum students (t-value = 1.32, P=0.21). Mean Step 2 scores over the seven-year period were 208 for Traditional Curriculum students and 206 for Parallel Curriculum students (t-value=1.08, P=0.30). Statistically significant main effects were noted by cohort year and curricular track for both the Step 1 and Step 2 examinations. The totality of experience in both groups, although differing by curricular type, may be similar enough that the comparable scores are what should be expected. These results should be reassuring to curricular planners and faculty that problem-based learning can provide students with the knowledge needed for the subsequent phases of their medical education.

This article overviews the training of nuclear medicine technologists in chosen European countries, the United States and Canada. There are basically two types of training: at medical schools following secondary school, without any university degree, usually on a 2- or 3-year basis, or else as a university course, leading to a BSc degree after 3 years, and in some countries to an MSc degree after an additional 2 years. In the United States both systems coexist, while in Europe the picture varies from country to country. The number of hours devoted to nuclear medicine also varies between curricula. Some efforts are being made to unify this system by transition to the university model of education in many European countries.

As gray wolves (Canis lupus) are removed from the federal Endangered Species List, management reverts to the states. Eventually most states will probably allow public wolf harvesting. Open seasons between about 1 November and 1 March accord more with basic wolf biology than during other times. Managers who consider wolf biology and public sensitivities, adapt public-taking regulations accordingly, and adjust harvest regulations as they learn will be best able to maximize the recreational value of wolf harvesting, minimize public animosity toward it, and meet their harvest objectives.

The annual meeting of the American Academy of Neurology (AAN) is a major venue for presentation of the latest disease-related clinical and basic neurologic research and is attended by a large number of neurologists from countries outside the United States and Canada. One-third of annual meeting attendees and abstract submissions are international in origin, with wide variations between countries and world regions, and this proportion has remained stable for the past 5 years. By comparison, international neurologists constitute 12% (n = 2,485) of AAN membership, and international membership has declined slightly over the past 5 years compared to a 15% increase from the United States and Canada. The scientific topics covered by international abstracts are similar to those from the United States and Canada. Abstract acceptance rates are 15% lower for international submissions than for those from the United States and Canada although variations between countries are seen. Three times more European neurologists attend the annual meeting than are AAN members whereas Asian neurologists are more likely to be AAN members than to attend the annual meeting. The AAN is working to understand and address the issues that affect international physicians' decisions to participate in the annual meeting.

Management of hepatitis C virus infection: the basics.

Chronic hepatitis C virus (HCV) infection affects some 170 million people worldwide, including 3 to 4 million in the United States who are largely unaware of their infection status. HCV has 6 genotypes; genotype 1 is the most common in the United States and genotypes 1 and 4 are less responsive to interferon alfa-based therapy than the other genotypes. Treatment with available direct-acting antiviral (DAA) drugs has increased sustained virologic response (SVR) rates in genotype 1 infection and shortened duration of therapy in many patients, but at this time these agents must still be administered with peginterferon alfa and ribavirin to prevent rapid emergence of resistance. Baseline predictors of response to therapy continue to play a role with triple-drug combination therapy including the pharmacogenetic IL28B genotype, which differs in prevalence throughout the world. The stopping/futility rules are different for triple-drug combination therapy, allowing for earlier decision-making. Ultimately, SVR is the goal of HCV treatment because it dramatically reduces likelihood of poor long-term outcome, even among patients with histologically advanced disease. This article summarizes a basic review presented by Susanna Naggie, MD, at the IAS-USA live management of HCV continuing medical education program held in Atlanta in October 2012. This article is intended for practitioners who are new to HCV management or who are interested in reviewing the basics of HCV treatments.

Although patient rights is a concept that all nurse managers need to be aware of, this concept often becomes confusing when applied to patients undergoing psychiatric treatment. It is important for the nurse manager to understand the basic rights that psychiatric patients are entitled to, to best be able to help staff nurses under his/her supervision to protect these rights. The nurse manager on a psychiatric unit often serves as a reference for staff nurses, and even for physicians, when questions regarding patient rights present themselves. The nurse manager should be certain to discuss these issues with the facility's legal and risk management team to be aware of particulars of the law of the state in which the facility is located, as state laws may differ somewhat in their treatment of psychiatric patients.

Drosophila suzukii Matsumura (spotted wing drosophila) has recently become a serious pest of a wide variety of fruit crops in the United States as well as in Europe, leading to substantial yearly crop losses. To enable basic and applied research of this important pest, we sequenced the D. suzukii genome to obtain a high-quality reference sequence. Here, we discuss the basic properties of the genome and transcriptome and describe patterns of genome evolution in D. suzukii and its close relatives. The National Elevation Dataset (NED) is the primary elevation data product produced and distributed by the USGS. The NED provides seamless raster elevation data of the conterminous United States, Alaska, Hawaii, and the island territories. The NED is derived from diverse source data sets that are processed to a specification with a consistent resolution, coordinate system, elevation units, and horizontal and vertical datums. The NED is the logical result of the maturation of the long-standing USGS elevation program, which for many years concentrated on production of topographic map quadrangle-based digital elevation models. The NED serves as the elevation layer of The National Map, and provides basic elevation information for earth science studies and mapping applications in the United States. The NED is a multi-resolution dataset that is updated bimonthly to integrate newly available, improved elevation source data. NED data are available nationally at grid spacings of 1 arc-second (approximately 30 meters) for the conterminous United States, and at 1/3 and 1/9 arc-seconds (approximately 10 and 3 meters, respectively) for parts of the United States. Most of the NED for Alaska is available at 2-arc-second (about 60 meters) grid spacing, where only lower resolution source data exist. Part of Alaska is available at the 1/3-arc-second resolution, and plans are in development for a significant upgrade in elevation data coverage of the State over the next 5 years. Specifications for the NED include the following: *Coordinate system: Geographic (decimal degrees of latitude and longitude), *Horizontal datum: North American Datum of 1983 (NAD 83), *Vertical datum: North American Vertical Datum of 1988 (NAVD 88) over the conterminous United States and varies in other areas, and *Elevation units: Decimal meters.

Basic Life Support Access to Injectable Epinephrine across the United States.

 Aggressive epinephrine administration has growing support in the treatment of anaphylaxis, a life-threatening allergic reaction. Emergency Medical Services (EMS) providers are frequently in a position to provide the first care to someone experiencing an anaphylactic reaction. Intramuscular injection of epinephrine is the definitive pharmacologic treatment for many associated symptoms. While easy to use, epinephrine autoinjectors (EAI) are prohibitively expensive, having increased in price ten-fold in ten years. Some states and EMS departments have begun expanding the scope of practice to allow Basic Life Support (BLS) providers, previously restricted to noninvasive therapies, to administer epinephrine by syringe. To compile a current and comprehensive list of how epinephrine is carried and used by EMS across the USA. An online survey focusing on anaphylaxis protocols and epinephrine administration was sent to state EMS medical directors and officials in all 50 states. Follow-up telephone calls were made to ensure compliance. Data were analyzed with descriptive statistics. Forty-nine of the 50 states in the USA provided a survey response. Texas responded but declined to participate in the survey because of practice variability across the state. In the other states, the form of epinephrine allowed or required on BLS ambulances was consistent with the scope of practice of their Basic Emergency Medical Technician (EMT). Thirteen states had training programs to allow BLS providers to inject epinephrine; 7 were considering it; 29 were not. Twenty-seven states specified EAI as the only form of epinephrine required or allowed on their BLS ambulances. No states reported allowing any level of EMS provider below EMT to use alternatives to EAI. This study confirms that many states have expanded the training of BLS providers to include the use of syringe injectable epinephrine. Even so, the majority of states relied on EAI in BLS ambulances.

The concept of negativity font, a basic unit of multipartite entanglement, is introduced. Transformation properties of determinants of negativity fonts under local unitary (LU) transformations are exploited to obtain relevant N-qubit polynomial invariants and construct entanglement monotones from first principles. It is shown that entanglement monotones that detect the entanglement of specific parts of the composite system may be constructed to distinguish between states with distinct types of entanglement. Perceptions of D.M.D. student readiness for basic science courses in the United States: can online review modules help?

There can be a disconnect between the level of content covered in undergraduate coursework and the expectations of professional-level faculty of their incoming students. Some basic science faculty members may assume that students have a good knowledge base in the material and neglect to appropriately review, whilst others may spend too much class time reviewing basic material. It was hypothesised that the replacement of introductory didactic physiology lectures with interactive online modules could improve student preparedness prior to lectures. These modules would also allow faculty members to analyse incoming student abilities and save valuable face-to-face class time for alternative teaching strategies. Results indicated that the performance levels of incoming U.S. students were poor (57% average on a pre-test), and students often under-predicted their abilities (by 13% on average). Faculty expectations varied greatly between the different content areas and did not appear to correlate with the actual student performance. Three review modules were created which produced a statistically significant increase in post-test scores (46% increase, P < 0.0001, n = 114-115). The positive results of this study suggest a need to incorporate online review units in the basic science dental school courses and revise introductory material tailored to students' strengths and needs.

The child and adult victims of human trafficking are denied their basic human rights and subjected to unspeakable physical and emotional harm. Traffickers exert complete control over their victims and are proficient at hiding their condition from authorities. Healthcare practitioners may be the only professionals who come into contact with victims if they present for medical care. This article will describe human trafficking and its potential victims, as well as guide medical management and access to services that will ensure their safety and restore their freedom.

To assess the effect of neurology residency education as trainees advance into independent practice, the American Academy of Neurology (AAN) elected to survey all graduating neurology residents at time of graduation and in 3-year cycles thereafter. A 22-question survey was sent to all neurology residents completing residency training in the United States in 2007. Of 523 eligible residents, 285 (54.5%) responded. Of these, 92% reported good to excellent quality teaching of basic neurology from their faculty; however, 47% noted less than ideal training in basic neuroscience. Two-thirds indicated that the Residency In-service Training Examination was used only as a self-assessment tool, but reports of misuse were made by some residents. After residency, 78% entered fellowships (with 61% choosing a fellowship based on interactions with a mentor at their institution), whereas 20% entered practice directly. After adjustment for the proportion of residents who worked before the duty hour rules were implemented and after their implementation, more than half reported improvement in quality of life (87%), education (60%), and patient care (62%). The majority of international medical graduates reported wanting to stay in the United States to practice rather than return to their country of residence. Neurology residents are generally satisfied with training, and most entered a fellowship. Duty hour implementation may have improved resident quality of life, but reciprocal concerns were raised about impact on patient care and education. Despite the majority of international trainees wishing to stay in the United States, stricter immigration laws may limit their entry into the future neurology workforce.

Using an iPad for Basic Communication Between Spanish-Speaking Families and Nurses in Pediatric Acute Care: A Feasibility Pilot Study.

The growing number of Spanish speakers in the United States poses communication challenges for healthcare providers. Language barriers in pediatric acute care have been associated with an increased risk for adverse events, longer hospital stays, and decreased quality of care. In addition, clinicians' usage of interpreter services is inconsistent. In fact, nurses often lack interpreter support during daily bedside care. Nursing staff at a pediatric children's hospital in the southeastern United States identified bedside communication with Spanish-speaking patients and families as a clinical challenge. To address this challenge, a basic communication interface, UTalk version 1.0 (the author is the owner and proprietor), supported by an Apple iPad, was developed by the researcher with input from nursing staff, a certified medical interpreter, and Spanish-speaking community members. A feasibility pilot study of the interface's usability and engagement was conducted on the hospital's pediatric medical-surgical unit through qualitative interviews with nurse-family dyads. Three themes emerged from the data: UTalk-facilitated communication, UTalk needs improvement, and interpreter miscommunication. These findings indicate that a mobile digital device interface is a feasible method for augmenting bedside communication with Spanish-speaking patients and families. These results also may serve as a reference for the development of similar mobile device interfaces. Further research with a larger sample size is needed.

In addition, the speech made no note of the Administration's recent denial of a controversial application for the Keystone XL pipeline to transport crude oil from Canada to the United States and made just an elliptical reference regarding the bankrupt Solyndra Corporation, which the administration had touted as a clean energy company. Innovation "demands basic research," Obama said, adding that Congress should not "gut these investments in our budget." Noting that one promise for innovation is American-made energy, Obama said he is directing the administration to "open more than 75% of our potential offshore oil and gas resources."

The complex structure of Ta2O5 led to the development of various structural models. Among them, superstructures represent the most stable configurations. However, their formation requires kinetic activity and long-range ordering processes, which are hardly present during physical vapor deposition. Based on nano-beam X-ray diffraction and concomitant ab initio studies, a new metastable orthorhombic basic structure is introduced for Ta2O5 with lattice parameters a = 6.425 Å, b = 3.769 Å and c = 7.706 Å. The unit cell containing only 14 atoms, i.e. two formula unit blocks in the c direction, is characterized by periodically alternating the occupied oxygen site between two possible positions in succeeding 002-planes. This structure can be described by the space group 53 (Pncm) with four Wyckoff positions, and exhibits an energy of formation of −3.209 eV atom−1. Among all the reported basic structures, its energy of formation is closest to those of superstructures. Furthermore, this model exhibits a 2.5 eV band gap, which is closer to experimental data than the band gap of any other basic-structure model. The sputtered Ta2O5 films develop only a superstructure if annealed at temperatures >800 °C in air or vacuum. Based on these results and the conveniently small unit cell size, it is proposed that the basic-structure model described here is an ideal candidate for both structure and electronic state descriptions of orthorhombic Ta2O5 materials. This article examines the similarities and differences in the education and training of gerontologists and others who work with older people in Sweden and the United States. It outlines the aging trends in both countries and assesses the level of training for those who provide care in a variety of fields. Both countries are aging, but the programs for gerontological training are quite different in the two countries, reflecting underlying cultural values. Sweden's education is generally more oriented toward the integration of some aging education in more disciplinary fields, such as nursing and social work and thus could benefit from more specialized, aging-specific courses. The United States is highly specialized, with multiple programs in various subfields of aging (e.g., geropsychology; aging services administration) and could benefit from integrating more aging knowledge into courses in other disciplines. The authors challenge professionals to consider if there is a basic but global curriculum and/or set of competencies in gerontology that could be agreed upon. As an increasingly global village, the ability to share and learn is more easily achievable. Sweden and the United States have much to learn from each other in terms of appropriately educating and training those who support our older people.

This assessment of recent data compression and coding research outside the United States examines fundamental and applied work in the basic areas of signal decomposition, quantization, lossless compression, and error control, as well as application development efforts in image/video compression and speech/audio compression. Seven computer scientists and engineers who are active in development of these technologies in US academia, government, and industry carried out the assessment. Strong industrial and academic research groups in Western Europe, Israel, and the Pacific Rim are active in the worldwide search for compression algorithms that provide good tradeoffs among fidelity, bit rate, and computational complexity,more » though the theoretical roots and virtually all of the classical compression algorithms were developed in the United States. Certain areas, such as segmentation coding, model-based coding, and trellis-coded modulation, have developed earlier or in more depth outside the United States, though the United States has maintained its early lead in most areas of theory and algorithm development. Researchers abroad are active in other currently popular areas, such as quantizer design techniques based on neural networks and signal decompositions based on fractals and wavelets, but, in most cases, either similar research is or has been going on in the United States, or the work has not led to useful improvements in compression performance. Because there is a high degree of international cooperation and interaction in this field, good ideas spread rapidly across borders (both ways) through international conferences, journals, and technical exchanges. Though there have been no fundamental data compression breakthroughs in the past five years--outside or inside the United State--there have been an enormous number of significant improvements in both places in the tradeoffs among fidelity, bit rate, and computational complexity.« less

Trends and patterns in research and development expenditures in the United States

This paper is a review of recent trends in United States expenditures on research and development (R&D). Real expenditures by both the government and the private sector increased rapidly between the mid-1970s and the mid-1980s, and have since leveled off. This is true of both overall expenditures and expenditures on basic research, as well as funding of academic research. Preliminary estimates indicate that about $170 billion was spent on R&D in the United States in 1995, with ≈60% of that funding coming from the private sector and about 35% from the federal government. In comparison to other countries, we have historically spent more on R&D relative to our economy than other advanced economies, but this advantage appears to be disappearing. If defense-related R&D is excluded, our expenditures relative to the size of the economy are considerably smaller than those of other similar economies. PMID:8917476

The occurrence of the Bauru Group in Sao Paulo State was studied, with emphasis on the western plateau. Regional geological mapping was carried out on a 1:250.000 scale with the help of MSS/LANDSAT images. The visual interpretation of images consisted basically of identifying different spectral characteristics of the geological units using channels 5 and 7. Complementary studies were made for treatment of data with an Interative Image (I-100) analyser in order to facilitate the extraction of information, particularly for areas where visual interpretation proved to be difficult. Regional characteristics provided by MSS/LANDSAT images, coupled with lithostratigraphic studies carried out in the areas of occurrence of Bauru Group sediments, enabled the homogenization of criteria for the subdivision of this group. A spatial distribution of the mapped units was obtained for the entire State of Sao Paulo and results were correlated with proposed stratigraphic divisions.

Toward a United States Arctic research policy

Of all countries bordering on the Arctic, the United States is the only one without a national institute, laboratory, or any other organization devoted to the sustained planning and support of Arctic research. Up to now, the responsibility for planning, implementing, and funding Arctic research has been divided between several federal agencies, the state of Alaska, and private groups whose mandates or objectives are often unconnected.The result of this pluralistic approach to U.S. science in the Arctic is that basic research has been conducted in piecemeal fashion. Individual studies are proposed and supported separately, and their costly logistic requirements must be funded in competition with research carried out under less-demanding environmental conditions in the rest of the country. Fundamental data-gathering and interpretation of information has been the responsibility of public agencies whose missions are separate and whose budgets may not reflect the priorities of Arctic issues.
