To identify strengths and weaknesses of nutrition education via telephone calls as part of a larger stage-of-change tailored intervention with mailed materials. Evaluative feedback was elicited from educators who placed the calls and respondents who received the calls. An internet and telephone survey of 10 states in the midwestern United States. 21 educators in 10 states reached via the internet and 50 young adults reached via telephone. VARIABLES MEASURED AND ANALYSIS: Rankings of intervention components, ratings of key aspects of educational calls, and cost data (as provided by a lead researcher in each state) were summarized via descriptive statistics. RESULTS, CONCLUSIONS, AND IMPLICATIONS: Educational calls used 6 to 17 minutes of preparation time, required 8 to 15 minutes of contact time, and had a mean estimated cost of $5.82 per call. Low-income young adults favored print materials over educational calls. However, the calls were reported to have positive effects on motivating participants to set goals. Educators who use educational telephone calls to reach young adults, a highly mobile target audience, may require a robust and flexible contact plan.

This article examines rates of nonmedical use and diversion of extended-release amphetamine and extended-release oral methylphenidate in the United States. Prescription dispensing data were sourced from retail pharmacies. Nonmedical use data were collected from the Researched Abuse, Diversion and Addiction-Related Surveillance (RADARS) System Drug Diversion Program and Poison Center Program. Drug diversion trends nearly overlapped for extended-release amphetamine and extended-release oral methylphenidate. Calls to poison centers were generally similar; however, calls regarding extended-release amphetamine trended slightly lower than those for extended-release oral methylphenidate. Data suggest similar diversion and poison center call rates for extended-release amphetamine and extended-release oral methylphenidate. PMID:23480245

In 2001, the intentional release of anthrax spores in the eastern United States increased concern about exposure to anthrax nationwide, and residents of Idaho sought assistance. Response from state and local agencies was required, increasing the strain on epidemiologists, laboratorians, and communications personnel. In late 2001, Idaho’s public health communications system handled 133 calls about suspicious powders. For each call, a multiagency bridge call was established, and participants (public health officials, epidemiologists, police, Federal Bureau of Investigation personnel, hazardous materials officials, and others) determined which samples would be tested by the state public health laboratory. A triage system for calls helped relieve the burden on public safety and health systems. PMID:12396922

Ciguatera Fish Poisoning and Climate Change: Analysis of National Poison Center Data in the United States, 2001–2011

Background: Warm sea surface temperatures (SSTs) are positively related to incidence of ciguatera fish poisoning (CFP). Increased severe storm frequency may create more habitat for ciguatoxic organisms. Although climate change could expand the endemic range of CFP, the relationship between CFP incidence and specific environmental conditions is unknown. Objectives: We estimated associations between monthly CFP incidence in the contiguous United States and SST and storm frequency in the Caribbean basin. Methods: We obtained information on 1,102 CFP-related calls to U.S. poison control centers during 2001–2011 from the National Poison Data System. We performed a time-series analysis using Poisson regression to relate monthly CFP call incidence to SST and tropical storms. We investigated associations across a range of plausible lag structures. Results: Results showed associations between monthly CFP calls and both warmer SSTs and increased tropical storm frequency. The SST variable with the strongest association linked current monthly CFP calls to the peak August SST of the previous year. The lag period with the strongest association for storms was 18 months. If climate change increases SST in the Caribbean 2.5–3.5°C over the coming century as projected, this model implies that CFP incidence in the United States is likely to increase 200–400%. Conclusions: Using CFP calls as a marker of CFP incidence, these results clarify associations between climate variability and CFP incidence and suggest that, all other things equal, climate change could increase the burden of CFP. Women's rights to legal abortion in the United States are now facing their greatest social and legislative challenges since its 1973 legalization. Legislation restricting rights and access to abortion care has been passed at state and federal levels at an unprecedented rate. Given the renewed vigor of anti-abortion movements, we call on anthropologists to engage with this shifting landscape of reproductive politics. This article examines recent legislation that has severely limited abortion access and maps possible directions for future anthropological analysis. We argue that anthropology can provide unique contributions to broader abortion research. The study of abortion politics in the United States today is not only a rich opportunity for applied and policy-oriented ethnographic research. It also provides a sharply focused lens onto broader theoretical concerns in anthropology and new social formations across moral, medical, political, and scientific fields in 21st-century America. © 2016 by the American Anthropological Association.

Assessing the need for communication training for specialists in poison information.

Effective communication has been shown to be essential to physician-patient communication and may be even more critical for poison control center (PCC) calls because of the absence of visual cues, the need for quick and accurate information exchange, and possible suboptimal conditions such as call surges. Professionals who answer poison control calls typically receive extensive training in toxicology but very little formal training in communication. An instrument was developed to assess the perceived need for communication training for specialists in poison information (SPIs) with input from focus groups and a panel of experts. Requests to respond to an online questionnaire were made to PCCs throughout the United States and Canada. The 537 respondents were 70% SPIs or poison information providers (PIPs), primarily educated in nursing or pharmacy, working across the United States and Canada, and employed by their current centers an average of 10 years. SPIs rated communication skills as extremely important to securing positive outcomes for PCC calls even though they reported that their own training was not strongly focused on communication and existing training in communication was perceived as only moderately useful. Ratings of the usefulness of 21 specific training units were consistently high, especially for new SPIs but also for experienced SPIs. Directors rated the usefulness of training for experienced SPIs higher for 5 of the 21 challenges compared to the ratings of SPIs. Findings support the need for communication training for SPIs and provide an empirical basis for setting priorities in developing training units.

In the television series Korea the Unknown War produced jointly by Thames Television (London) and WGBH (Boston) in 1990, General Matthew Ridgway, Commander in Chief of United Nations forces during the Korean War, called the accusations that the United States waged bacteriological warfare "black propaganda." The charges discredited the United States and, despite denials and many international discussions, have not been completely refuted until new. Following studies in archives previously not available for research and after uncovering new sources, many specific examples of black propaganda were discovered that contained false information and lies discrediting the United States. The mechanism of lies, which convinced the Korean population that bacteriological warfare was going on and that the only way not to become victims of the United States' inhuman cruelty was to fight, are shown in this paper.

Characterization of edible marijuana product exposures reported to United States poison centers.

Edible marijuana products are sold as brownies, cookies, and candies, which may be indistinguishable from counterparts without marijuana and are palatable to children and adults. The consumption of an entire product containing multiple dose-units may result in overdose. To characterize edible marijuana exposures reported to US poison centers with subgroup analysis by age. We analyzed single substance, human exposure calls coded to marijuana brownies, candies, cookies, beverages, or other foods reported to the National Poison Data System from January 2013 to December 2015. Calls were analyzed by state, age, gender, exposure route, clinical effect, therapies, and level of healthcare facility utilization. Four-hundred and thirty calls were reported: Colorado (N = 166, 1.05/100,000 population/year) and Washington (96, 0.46) yielded the highest number of exposures. Three hundred and eighty-one (91%) calls occurred in states with decriminalized medical/recreational marijuana. The number of calls increased every year of the study. The most common age groups were: ≤5 years (N = 109, 0.15/100,000 population/year) and 13-19 (78, 0.09). The most frequent clinical effects were drowsiness/lethargy (N = 118, percentage = 43%), tachycardia (84, 31%), agitated/irritable (37, 14%), and confusion (37, 14%). Children ≤5 years have more drowsiness/lethargy, ataxia, and red eye/conjunctivitis. No deaths were reported. The most common therapies administered were intravenous fluids (85, 20%), dilute/irrigate/wash (48, 11 %), and benzodiazepines (47, 11%). Three patients (ages 4, 10, and 57 years) received intubation. 97 (23%), 217 (50%), and 12 (3%) calls were managed at home, treated/released, admitted to a critical care unit, respectively. Although most clinical effects are minor, ventilatory support may be necessary for children and adults. We speculate the increasing exposures may be related to a combination of delayed absorption kinetics of Δ9-tetrahydrocannablnol, lagging packaging regulations, increased accessibility in decriminalized states, and increased familiarity of poison center specialists with edible product codes. Edible marijuana exposures are increasing and may lead to severe respiratory depression.

Colonias, which are unincorporated border settlements in the United States, have emerged in rural areas without the governance and services normally provided by local government. The expansion of colonias in the United States-Mexico border region can be traced to the rapid growth associated with the Mexican Border Industrial Program during the 1960s. This rapid population growth created a lack of affordable housing, causing new migrants in the United States to purchase rural homestead lots through a contract-for-deed program from land developers. Because of the need to keep prices affordable and the absence of effective land-use controls, these homesteads expanded into rural subdivisions, commonly called colonias, without proper infrastructure. Colonias have been identified in the four U.S. border states, with Texas having designated the majority, which numbered over 1,400 colonias in 2001. Because the region is binationally interconnected economically, politically, and socially, the phenomenon of colonias in the United States is a transborder issue.

Perhaps to policymakers the decision to hang on to miles, pounds, and gallons is one of tradition. However, as a physics teacher I have seen firsthand how growing up with U.S. Customary Units (commonly called by the pseudonym "British Units") has negatively impacted my students. Sure, they can convert between "British" and SI units; and they can effectively toggle between SI prefixes. However, they typically lack intuition regarding the values they express in SI. This is a major problem, but is by no means a criticism against them. Their disadvantage is the result of a unique learning gap that exists in the United States. I call it the SI(sī)-gap. The SI-gap not only impedes our ability as teachers to inspire students when it comes to science, technology, engineering, and mathematics (STEM); it also prevents our students from properly assessing the validity of their own results. As we shall soon see, the SI-gap carries consequences beyond the classroom. If we are to solve the problems that relate to the SI-gap, we need to reform the way we introduce SI units to the current generation of students.

Strontium isotope values (87Sr/86Sr) in bone and tooth enamel have been used increasingly to identify non-local individuals within prehistoric human populations worldwide. Archaeological research in the Midwestern United States has increasingly highlighted the role of population movement in affecting interregional cultural change. However, the comparatively low level of geologic variation in the Midwestern United States might suggest a corresponding low level of strontium variation, and calls into question the sensitivity of strontium isotopes to identify non-local individuals in this region. Using strontium isotopes of archaeological fauna, we explore the degree of variability in strontium ratios across this region. Our results demonstrate measurable variation in strontium ratios and indicate the potential of strontium analysis for addressing questions of origin and population movement in the Midwestern United States.

Despite decades of experience treating heroin or prescription opioid dependence with methadone or buprenorphine--two forms of opioid substitution therapy--gaps remain between current practices and evidence-based standards in both Canada and the United States. This is largely because of regulatory constraints and pervasive suboptimal clinical practices. Fewer than 10 percent of all people dependent on opioids in the United States are receiving substitution treatment, although the proportion may increase with expanded health insurance coverage as a result of the Affordable Care Act. In light of the accumulated evidence, we recommend eliminating restrictions on office-based methadone prescribing in the United States; reducing financial barriers to treatment, such as varying levels of copayment in Canada and the United States; reducing reliance on less effective and potentially unsafe opioid detoxification; and evaluating and creating mechanisms to integrate emerging treatments. Taking these steps can greatly reduce the harms of opioid dependence by maximizing the individual and public health benefits of treatment.

Comparative Study on Subjective Experience of Elder Abuse Between Older Korean Immigrants in the United States and Older Koreans in Korea.

This study examines the prevalence of elder abuse and the relationship between sociodemographic factors and elder abuse among older Koreans in the United States and Korea. Survey data from older Koreans aged between 60 and 79 years from the two countries ( n = 480) were analyzed descriptively and in binary logistic regressions. This study found a similar prevalence of elder abuse in the two samples, with 26% of older Korean immigrants in the United States reporting abuse and 23% of older Koreans in Korea reporting abuse. However, there were significant differences in the types of emotional abuse experienced by older Koreans in both countries. Reports of some types of emotional abuse, such as 'name calling' and 'silent treatment,' were significantly higher in the United States than in Korea. These findings expand our knowledge of the experience of elder abuse among older Koreans in both countries.

The absence of national health care reform and the growing number of uninsured individuals in the United States have prompted states to develop plans to provide medical care for the low income and the indigent. Many local health departments are not only responsible for the core public health functions; but they are increasingly called upon to provide person health care services for those who cannot afford it. Religion, sense of calling, and the practice of medicine: findings from a national survey of primary care physicians and psychiatrists.

A sense of calling is a concept with religious and theological roots; however, it is unclear whether contemporary physicians in the United States still embrace this concept in their practice of medicine. This study assesses the association between religious characteristics and endorsing a sense of calling among practicing primary care physicians (PCPs) and psychiatrists. In 2009, we surveyed a stratified random sample of 2016 PCPs and psychiatrists in the United States. Physicians were asked whether they agreed with the statement, "For me, the practice of medicine is a calling." Primary predictors included demographic and self-reported religious characteristics, (eg, attendance, affiliation, importance of religion, intrinsic religiosity) and spirituality. Among eligible respondents, the response rate was 63% (896/1427) for PCPs and 64% (312/487) for psychiatrists. A total of 40% of PCPs and 42% of psychiatrists endorsed a strong sense of calling. PCPs and psychiatrists who were more spiritual and/or religious as assessed by all four measures were more likely to report a strong sense of calling in the practice of medicine. Nearly half of Muslim (46%) and Catholic (45%) PCPs and the majority of evangelical Protestant PCPs (60%) report a strong sense of calling in their practice, and PCPs with these affiliations were more likely to endorse a strong sense of calling than those with no affiliation (26%, bivariate P < 0.001). We found similar trends for psychiatrists. In this national study of PCPs and psychiatrists, we found that PCPs who considered themselves religious were more likely to report a strong sense of calling in the practice of medicine. Although this cross-sectional study cannot be used to make definitive causal inferences between religion and developing a strong sense of calling, PCPs who considered themselves religious are more likely to embrace the concept of calling in their practice of medicine.

The United States faces an addiction health crisis. Presidential election cycles in the United States are cause for creation of political party platforms. These platforms provide general stances and specific policies on a variety of issues. We undertook a review of the addiction policies of the 2016 Republican and Democratic platforms. Despite differences in focus, we found more similarities than differences between the two. We call upon those in political power to use every evidence-based policy at their disposal to promote addiction treatment and prevention.

The United States has the highest drug costs in the world. Consumers complain about large price differences at pharmacies on generic drugs. To evaluate variation in cash prices of generic medications most prescribed in dermatology across different drugstores and states in United States. The 11 generic drugs most prescribed by dermatologists according to National Ambulatory Medical Care Survey were assessed. By using Google, the most common used pharmacies in United States were listed, which are located at a random selection of six states. By calling the first available number of each pharmacy in the six states and asking about the generic cash price of the smallest stock size and the most prescribed type, the data were collected. Drug prices varied; the median cumulative price of the 11 medications was highest at Rite Aid ($1226) and lowest at Walmart ($795.34) with 35% difference. The prices at CVS differed by 20% across different states; however, the prices at Walmart, Rite Aid and Walgreens were consistent. New York has the highest and Iowa the lowest prices, especially at CVS, ($1160.79) versus ($931.32). There are varieties in the prices for the generic medications in different pharmacies and States.

We present the first regional trends in anuran occupancy for eight states of the southeastern United States, based on 13 y (2001–2013) of North American Amphibian Monitoring Program (NAAMP) data. The NAAMP is a longterm monitoring program in which observers collect anuran calling observation data at fixed locations along random roadside routes. We assessed occupancy trends for 14 species. We found weak evidence for a general regional pattern of decline in calling anurans within breeding habitats along roads in the southeastern USA over the last 13 y. Two species had positive regional trends with 95% posterior intervals that did not include zero (Hyla cinerea and Pseudacris crucifer). Five other species also showed an increasing trend, while eight species showed a declining trend, although 95% posterior intervals included zero. We also assessed state level trends for 107 species/state combinations. Of these, 14 showed a significant decline and 12 showed a significant increase in occupancy (i.e., credible intervals did not include zero for these 26 trends).

Mourning dove population trend estimates from Call-Count and North American Breeding Bird Surveys

The mourning dove (Zenaida macroura) Callcount Survey and the North American Breeding Bird Survey provide information on population trends of mourning doves throughout the continental United States. Because surveys are an integral part of the development of hunting regulations, a need exists to determine which survey provides precise information. We estimated population trends from 1966 to 1988 by state and dove management unit, and assessed the relative efficiency of each survey. Estimates of population trend differ (P lt 0.05) between surveys in 11 of 48 states; 9 of 11 states with divergent results occur in the Eastern Management Unit. Differences were probably a consequence of smaller sample sizes in the Callcount Survey. The Breeding Bird Survey generally provided trend estimates with smaller variances than did the Callcount Survey. Although the Callcount Survey probably provides more withinroute accuracy because of survey methods and timing, the Breeding Bird Survey has a larger sample size of survey routes and greater consistency of coverage in the Eastern Unit.

S94-47810 (2 Dec. 1994) --- Lockheed Space Operations Company workers in the Extended Duration Orbiter (EDO) Facility, located inside the Vehicle Assembly Building (VAB), carefully hoist the Orbiter Docking System (ODS) from its shipping container into a test stand. The ODS was shipped in a horizontal position to the Kennedy Space Center (KSC) from contractor Rockwell Aerospace's Downey plant. Once the ODS is upright, work can continue to prepare the hardware for the first docking of the United States Space Shuttle and Russian Space Station MIR in 1995. The ODS contains both United States-made and Russian-made hardware. The black band is Russian-made thermal insulation protecting part of the docking mechanism, also Russian-made, called the Androgynous Peripheral Docking System (APDS). A red protective cap covers the APDS itself. Other elements of the ODS, most of it protected by white United States-made thermal insulation, were developed by Rockwell, which also integrated and checked out the assembled Russian-United States system.

Monitoring Sensitive Bat Species at Los Alamos National Laboratory

Bats play a critical role in ecosystems and are vulnerable to disturbance and disruption by human activities. In recent decades, bat populations in the United States and elsewhere have decreased tremendously. There are 47 different species of bat in the United States and 28 of these occur in New Mexico with 15 different species documented at the Los Alamos National Laboratory (LANL) and surrounding areas. Euderma maculatum(the spotted bat) is listed as “threatened” by the state of New Mexico and is known to occur at LANL. Four other species of bats are listed as “sensitive” and also occur here. Inmore » 1995, a four year study was initiated at LANL to assess the status of bat species of concern, elucidate distribution and relative abundance, and obtain information on roosting sites. There have been no definitive studies since then. Biologists in the Environmental Protection Division at LANL initiated a multi-year monitoring program for bats in May 2013 to implement the Biological Resources Management Plan. The objective of this ongoing study is to monitor bat species diversity and seasonal activity over time at LANL. Bat species diversity and seasonal activity were measured using an acoustic bat detector, the Pettersson D500X. This ultrasound recording unit is intended for long-term, unattended recording of bat and other high frequency animal calls. During 2013, the detector was deployed at two locations around LANL. Study sites were selected based on proximity to water where bats may be foraging. Recorded bat calls were analyzed using Sonobat, software that can help determine specific species of bat through their calls. A list of bat species at the two sites was developed and compared to lists from previous studies. Species diversity and seasonal activity, measured as the number of call sequences recorded each month, were compared between sites and among months. A total of 17,923 bat calls were recorded representing 15 species. Results indicate that there is a statistically significant relationship between bat diversity and month of the year. Conducting a study covering more than two decades of United States defense policy and the development of doctrine towards the Third World is an ambitious project. Nevertheless, there is a continuum from President Kennedy to President Reagan in the decision-making process and application of resources towards United States involvement in the less developed world. The incumbent president is perhaps closer to Kennedy in his world view of the root causes of instability and unrest in the Third World, specifically communist inspired and supported subversive insurgency. This calls for examination in view of the significant variations in the emphasis given tomore » US security interests in the Third World from Kennedy to Reagan. Each president since Kennedy has been faced with the problem of subversive insurgencies in the less developed world, inspired and supported by the communists both internally and externally. The Soviet Union is on record in vowing to support so called Wars of National Liberation in the Third World. Each president has had to deal with such threats to US security interests in the Third World. How each president from Kennedy to Reagan has dealt with such threats, and the conclusion that Reagan has resurrected much of the Kennedy US counterinsurgency doctrine, is the crux of this examination.« less

Ciguatera and scombroid fish poisoning in the United States.

Ciguatera and scombroid fish poisonings are common causes of fish-related foodborne illness in the United States; however, existing surveillance systems underestimate the overall human health impact. This study aimed to describe existing data on ciguatera and scombroid fish poisonings from outbreak and poison control center reports and to estimate the overall number of ciguatera and scombroid fish-poisoning illnesses, hospitalizations, and deaths in the United States. We analyzed outbreak data from the Foodborne Disease Outbreak Surveillance Systems (FDOSS) from 2000 to 2007 and poison control center call data from the National Poison Data System (NPDS) from 2005 to 2009 for reports of ciguatera and scombroid fish poisonings. Using a statistical model with many inputs, we adjusted the outbreak data for undercounting due to underreporting and underdiagnosis to generate estimates. Underreporting and underdiagnosis multipliers were derived from the poison control call data and the published literature. Annually, an average of 15 ciguatera and 28 scombroid fish-poisoning outbreaks, involving a total of 60 and 108 ill persons, respectively, were reported to FDOSS (2000-2007). NPDS reported an average of 173 exposure calls for ciguatoxin and 200 exposure calls for scombroid fish poisoning annually (2005-2009). After adjusting for undercounting, we estimated 15,910 (90% credible interval [CrI] 4140-37,408) ciguatera fish-poisoning illnesses annually, resulting in 343 (90% CrI 69-851) hospitalizations and three deaths (90% CrI 1-7). We estimated 35,142 (90% CrI: 10,496-78,128) scombroid fish-poisoning illnesses, resulting in 162 (90% CrI 0-558) hospitalizations and 0 deaths. Ciguatera and scombroid fish poisonings affect more Americans than reported in surveillance systems. Although additional data can improve these assessments, the estimated number of illnesses caused by seafood intoxication illuminates this public health problem. Efforts, including education, can reduce ciguatera and scombroid fish poisonings.

Computer-aided boundary delineation of agricultural lands

The National Agricultural Statistics Service of the United States Department of Agriculture (USDA) presently uses labor-intensive aerial photographic interpretation techniques to divide large geographical areas into manageable-sized units for estimating domestic crop and livestock production. Prototype software, the computer-aided stratification (CAS) system, was developed to automate the procedure, and currently runs on a Sun-based image processing system. With a background display of LANDSAT Thematic Mapper and United States Geological Survey Digital Line Graph data, the operator uses a cursor to delineate agricultural areas, called sampling units, which are assigned to strata of land-use and land-cover types. The resultant stratified sampling units are used as input into subsequent USDA sampling procedures. As a test, three counties in Missouri were chosen for application of the CAS procedures. Subsequent analysis indicates that CAS was five times faster in creating sampling units than the manual techniques were.

Problems of poverty, poor health, and incarceration are unevenly distributed among racial and ethnic minorities in the United States. We argue that this is due, in part, to the ascendance of United States-style neoliberalism, a prevailing political and economic doctrine that shapes social policy, including public health and anti-poverty intervention strategies. Public health research most often associates inequalities in health outcomes, poverty, and incarceration with individual and cultural risk factors. Contextual links to structural inequality and the neoliberal doctrine animating state-sanctioned interventions are given less attention. The interrelationships among these are not clear in the extant literature. Less is known about public health and incarceration. Thus, the authors describe the linkages between neoliberalism, public health, and criminal justice outcomes. We suggest that neoliberalism exacerbates racial disparities in health, poverty, and incarceration in the United States. We conclude by calling for a new direction in public health research that advances a pro-poor public health agenda to improve the general well-being of disadvantaged groups.

Implications of National Anesthesia Workload on the Staffing of a Call Center: The Malignant Hyperthermia Consultant Hotline.

Recently, we analyzed data from the American Society of Anesthesiologist's (ASA) Anesthesia Quality Institute (AQI) to report the United States (U.S.) anesthesia workload by time of day and day of the week. The AQI data were reported using the Central Time zone. Times for the N = 613 calls to the Malignant Hyperthermia Association of the United States (MHAUS) Malignant Hyperthermia (MH) Hotline from August 1, 2012, through March 7, 2014, were adjusted similarly. The MH Hotline effectively provides at all times to each anesthesia group an additional board-certified anesthesiologist who has expertise in managing, diagnosing, and/or preventing MH crises. We compared the timing of calls with the MH Hotline consultants relative to times of most anesthesia workload nationally. The interval 6:30 AM to 6:30 PM Central Time on regular workdays accounted for most (P < 0.0001) calls to the MH Hotline (62.5% ± 2.0% [mean ± standard error]). However, the interval accounted for significantly less than the 82.2% of anesthesia minutes and 84.5% of general anesthesia minutes during that interval nationally (both P < 0.0001). Thus, most calls to the MH Hotline occurred when anesthesia groups nationwide were the busiest. Weekends accounted for 15.3% ± 1.5% of MH Hotline calls, significantly greater than the rates of 5.2% of anesthesia minutes and 4.3% of general anesthesia minutes during weekends nationally (both P < 0.0001). Thus, the MH Hotline was used proportionately more often when anesthesia providers have fewer colleagues present and available for consultation (all P < 0.0001). These findings may be expected of other (future) national support centers for anesthesia.

Some people may have thought that the problem of acid deposition, commonly referred to as acid rain, had been solved in the United States with the passage of the Acid Deposition Control Program under Title IV of the 1990 Clean Air Act Amendments (CAAA).Although that legislation has helped to dramatically limit emissions of sulfur dioxide and nitrogen oxide—gases that can react in the atmosphere and form acidic compounds, including fine particles of sulfates and nitrates— much steeper cuts are needed for a quicker recovery from acid rain in the north-eastern United States, according to a new scientific appraisal of the effectiveness of measures called for in that law. The appraisal was issued on March 26 and is entitled “Acidic Deposition in the Northeastern United States: Sources and Inputs, Ecosystem Effects, and Management Strategies.”

NHDPlusHR: A national geospatial framework for surface-water information

The U.S. Geological Survey is developing a new geospatial hydrographic framework for the United States, called the National Hydrography Dataset Plus High Resolution (NHDPlusHR), that integrates a diversity of the best-available information, robustly supports ongoing dataset improvements, enables hydrographic generalization to derive alternate representations of the network while maintaining feature identity, and supports modern scientific computing and Internet accessibility needs. This framework is based on the High Resolution National Hydrography Dataset, the Watershed Boundaries Dataset, and elevation from the 3-D Elevation Program, and will provide an authoritative, high precision, and attribute-rich geospatial framework for surface-water information for the United States. Using this common geospatial framework will provide a consistent basis for indexing water information in the United States, eliminate redundancy, and harmonize access to, and exchange of water information.

Weather and emotional state: a search for associations between weather and calls to telephone counseling services

Previous research has revealed that an emotional response to weather might be indicated by calls to telephone counseling services. We analyzed call frequency from such "hotlines", each serving communities in a major metropolitan area of the United States (Detroit, Washington DC, Dallas and Seattle). The periods examined were all, or parts of, the years 1997 and 1998. Associations with subjectively derived synoptic weather types for all cities except Seattle, as well as with individual weather elements [cloudiness (sky cover), precipitation, windspeed, and interdiurnal temperature change] for all four cities, were investigated. Analysis of variance and t-tests (significance of means) were applied to test the statistical significance of differences. Although statistically significant results were obtained in scattered instances, the total number was within that expected by chance, and there was little in the way of consistency to these associations. One clear exception was the increased call frequency during destructive (severe) weather, when there is obvious concern about the damage done by it.

Woodcock singing-ground counts and habitat changes in the northeastern United States

Aerial photography from the late 1960's and the late 1970's was used to study habitat changes along 78 American woodcock (Scolopax minor) singing-ground routes in 9 northeastern states. The most noticeable changes were declines in the amount of abandoned field, cropland, shrubland, and field/pasture. The amount of land in the urban/industrial type increased 33.4% from the late 1960's to the late 1970's. We examined relationships between the woodcock call-count index and habitat variables using multiple-regression techniques. The abundance of calling male woodcock was positively correlated with the amount of abandoned field and alder (Alnus sp.) and negatively correlated with the amount of urban/industrial type. However, only the change in the urban/industrial type was significantly (P < 0.05) related to the change in the call-count index. Urban/industrial area increased, whereas the call-count index declined on average in our sample of routes by 1.4 birds/route (40.5%).

Electronic nicotine delivery devices such as electronic cigarettes (e-cigarettes) are battery-powered devices that deliver nicotine, flavorings (e.g., fruit, mint, and chocolate), and other chemicals via an inhaled aerosol. E-cigarettes that are marketed without a therapeutic claim by the product manufacturer are currently not regulated by the Food and Drug Administration (FDA). In many states, there are no restrictions on the sale of e-cigarettes to minors. Although e-cigarette use is increasing among U.S. adolescents and adults, its overall impact on public health remains unclear. One area of concern is the potential of e-cigarettes to cause acute nicotine toxicity. To assess the frequency of exposures to e-cigarettes and characterize the reported adverse health effects associated with e-cigarettes, CDC analyzed data on calls to U.S. poison centers (PCs) about human exposures to e-cigarettes (exposure calls) for the period September 2010 (when new, unique codes were added specifically for capturing e-cigarette calls) through February 2014. To provide a comparison to a conventional product with known toxicity, the number and characteristics of e-cigarette exposure calls were compared with those of conventional tobacco cigarette exposure calls.

Although turnover is an issue of global concern, paradoxically there have been few studies of turnover across cultures. We investigated the cross-cultural generalizability of the job embeddedness model (Mitchell & Lee, 2001) by examining turnover in an individualistic country (United States) and a collectivistic country (India). Using cross-cultural data from call centers (N = 797), we demonstrated that although organization job embeddedness predicted turnover in both countries, different dimensions of job embeddedness predicted turnover in the United States and India. As hypothesized, on the basis of individualism-collectivism theory, person-job fit was a significant predictor of lower turnover in the United States, whereas person-organization fit, organization links, and community links were significant predictors of lower turnover in India. We also explored whether a newly developed construct of embeddedness-family embeddedness-predicts turnover above and beyond job embeddedness and found initial support for its utility in both the United States and India. A short CV of Britton Chance, an outstanding American biophysicist and biochemists, who has passed away in November 2010, is presented. Chance invented and applied in biochemical research fast recording of difference spectra during chemical reactions. Due to this technique he discovered and studied formation of enzyme-substrate complexes as exemplified by the peroxidase reaction. He also described spectral changes of electron carriers of mitochondrial respiratory chain that accompany transition from the resting to the active (phosphorylating) states. Applying vibrating platinum electrode, he recorded changes in the rate of oxygen uptake by isolated mitochondria in resting and phosphorylating states and defined the so-called respiratory states, called after his name. He was the first to describe formation of reactive oxygen species by mitochondria. Britton Chance was a devoted yachtsman and won a gold medal for the United States in sailing at the 1952 Olympic Games in Helsinki.

Despite a continuous increase of the limited English proficiency (LEP) population in the United States, disparities in the quality of care received in health care systems persist. This qualitative study explores the perceptions of hospitalized LEP patients on their call light use, as well as their perceptions of a prototype of a new multilingual call light system, Eloquence™. Individual interviews were conducted with 10 Spanish-speaking patient participants. Using a constant comparative method, the following themes emerged: (1) reasons for call light use, (2) challenges with communication, (3) patients' adaptation to language barriers, (4) perceived staff responses to call light, and (5) responses to the Eloquence™ demonstration. Data from this study shed light on the patient care experience of LEP patients and can help guide clinicians and administrators in providing culturally and linguistically competent care. © The Author(s) 2016.

Assessing the quality of water in every location of the Nation would not be practical. Therefore, NAWQA investigations are conducted within 59 selected areas called study units (fig. 1). These study units encompass important river and aquifer systems in the United States and represent the diverse geographic, waterresource, land-use, and water-use characteristics of the Nation. The Delaware River Basin is one of 15 study units in which work began in 1996. Water-quality sampling in the study unit will begin in 1999. In recent years, unaccompanied minors have been journeying to the United States (U.S.)-Mexico border in great numbers in order to escape violence, poverty and exploitation in their home countries. Yet, unaccompanied children attempting to cross the United States border face treatment at the hands of government representatives which violates their inherent rights as children. The result is a human rights crisis that has severe health consequences for the children. Their rights as children are clearly delineated in various, international human rights documents which merit increased understanding of and recognition by the U.S. government. This paper calls for the improvement of policies and procedures for addressing the rights of unaccompanied immigrant children; it provides specific, rights-based recommendations which work together to safeguard the rights of the child at the U.S. southwestern border.

A medical-surgical unit in a southwestern United States hospital examined the results of adding wireless communication technology to assist nurses in identifying patient bed status changes and enhancing team communication. Following the addition of wireless communication, response time to patient calls and the number of nurse-initiated communications were compared to pre-wireless calls and response time sampling period. In the baseline study, nurse-initiated communications and response time to patient calls were investigated for a team nursing model (Guarascio-Howard & Malloch, 2007). At this time, technology consisted of a nurse call system and telephones located at each decentralized nurse station and health unit coordinator (HUC) station. For this follow-up study, a wireless device was given to nurses and their team members following training on device use and privacy issues. Four registered nurses (RNs) were shadowed for 8 hours (32 hours total) before and after the introduction of the wireless devices. Data were collected regarding patient room visits, number of patient calls, bed status calls, response time to calls, and the initiator of the communication episodes. Follow-up study response time to calls significantly decreased (t-test p = .03). RNs and licensed practical nurses responded to bed status calls in less than 1 minute-62% of the 37 calls. Communication results indicated a significant shift (One Proportion Z Test) in RN-initiated communications, suggesting an enhanced ability to communicate with team members and to assist in monitoring patient status. Patient falls trended downward, although not significantly (p > .05), for a 6-month period of wireless technology use compared to the same period the previous year. The addition of a wireless device has advantages in team nursing, namely increasing communication with staff members and decreasing response time to patient and bed status calls. Limitations of the study included a change in caregiver team members and issues regarding wireless device and locator badge compliance. Administrative issues that arose during this field study included bed and cable maintenance, device battery charging, and the training of new and floating team members.

Affordable housing development authorities throughout the United States continually struggle to find the most cost-effective pathway to provide quality, durable, and sustainable housing. The challenge for these authorities is to achieve the mission of delivering affordable housing at the lowest cost per square foot in environments that may be rural, urban, suburban, or withinmore » a designated redevelopment district. With the challenges the U.S. faces regarding energy, the environmental impacts of consumer use of fossil fuels and the increased focus on reducing greenhouse gas emissions, housing authorities are pursuing the goal of constructing affordable, energy efficient and sustainable housing at the lowest life-cycle cost of ownership. UNAIDS executive director Peter Piot called for the United States to do more to support HIV prevention efforts internationally. Speaking at the United States Conference on AIDS, he said that U.S. groups will need to offer more technical assistance and advice to community-based organizations around the world if they ever hope to manage the epidemic. UNAIDS will issue model legislation which other countries can use to pattern their own anti-discrimination laws. Health Canada recently published a guide in which ways that community-based organizations in industrialized countries can assist their counterparts in the developing world are outlined. The service will soon be provided on a commercial basis by a consortium of eight U.S. companies called the American Mobile Satellite Consortium (AMSC). AMSC will build a three-satellite MSS system that will offer superior performance, reliability and cost effectiveness for organizations requiring mobile communications across the U.S. The development and operation of MSS in North America is being coordinated with Telesat Canada and Mexico. AMSC expects NASA to provide launch services in exchange for capacity on the first AMSC satellite for MSAT-X activities and for government demonstrations.

Canada is, in many respects, culturally and economically similar to the United States, and until relatively recently, the two countries had similar health systems. However, since passage of the Canada Health Act in the 1970s, that nation's health statistics have become increasingly superior. Although the costs of Canada's health system are high by international standards, they are much lower than U.S. costs. This paper describes several factors likely to be responsible for Canada's better health at lower cost: universal financial coverage through a so-called single payer; features conducive to a strong primary care infrastructure; and provincial autonomy under general principles set by national law.

In England during the late nineteenth century, intellectuals, especially Francis Galton, called for a variety of eugenic policies aimed at ensuring the health of the human species. In the United States, members of the Progressive movement embraced eugenic ideas, especially immigration restriction and sterilization. Indiana enacted the first eugenic sterilization law in 1907, and the US Supreme Court upheld such laws in 1927. State programs targeted institutionalized, mentally disabled women. Beginning in the late 1930s, proponents rationalized involuntary sterilization as protecting vulnerable women from unwanted pregnancy. By World War II, programs in the United States had sterilized approximately 60,000 persons. After the horrific revelations concerning Nazi eugenics (German Hereditary Health Courts approved at least 400,000 sterilization operations in less than a decade), eugenic sterilization programs in the United States declined rapidly. Simplistic eugenic thinking has faded, but coerced sterilization remains widespread, especially in China and India. In many parts of the world, involuntary sterilization is still intermittently used against minority groups.

Effect of January vacations and prior night call status on resident ABSITE performance.

To determine if vacations in January or on-call status have an effect on American Board of Surgery In-Training Examination (ABSITE) scores. Retrospective review of the performance of general surgery residents on ABSITE. Data collected included ABSITE scores, United States Medical Licensing Examination Step 2 scores, January vacation schedules, and call schedules. ABSITE performance was examined for correlation with vacation or call schedules. Student t test was used for statistical analysis, with a p value of less than 0.05 considered significant. General surgery residency program at the Louisiana State University Health Sciences Center-Shreveport, a university hospital-based program with 5 categorical residents per year. Postgraduate year (PGY) 1 through 5 general surgery categorical residents from 2006 to 2012. A total of 170 ABSITE scores from 55 residents were reviewed. The mean score when vacation was taken was 48.6 as compared with 36.3 when no vacation was taken (p = 0.02). Residents who took a January vacation at least once in their residency had a mean score of 42.8 as compared with 37.7 of those who did not (p = 0.43). The mean United States Medical Licensing Examination Step 2 score of residents who took a January vacation at least once in their residency was 218 as compared with 217 for their peers (p = 0.78). Among residents who took January vacations, the mean score in the years they took vacation was 49.4 as compared with 35.4 in the years they did not (p = 0.02). Prior night call status had no effect on the examination scores (44.2 vs 38.6, p = 0.30). Mean ABSITE scores were higher for residents who took a January vacation before the examination, despite no apparent difference in baseline test-taking ability. Among residents who took January vacations, mean scores were higher in the years they took vacation than in other years. On-call status did not have an effect on ABSITE performance. We offer a UK-based commentary on the recent "Perspective" published in IJHPM by Thakkar and Sullivan. We are sympathetic to the authors' call for increased funding for health service and policy research (HSPR). However, we point out that increasing that investment - in any of the three countries they compare: Canada, the United States and the United Kingdom- will ipso facto not necessarily lead to any better use of research by health system decision-makers in these settings. We cite previous authors' descriptions of the many factors that tend to make the worlds of researchers and decision-makers into "two solitudes." And we call for changes in the structure and funding of HSPR, particularly the incentives now in place for purely academic publishing, to tackle a widespread reality: most published research in HSPR, as in other applied fields of science, is never read or used by the vast majority of decision-makers, working out in the "real world. © 2018 The Author(s); Published by Kerman University of Medical Sciences. After the Second World War, a renaissance in field primatology took place in the United States under the aegis of the 'new physical anthropology'. Its leader, Sherwood Washburn, envisioned a science uniting studies of hominid fossils with Darwinian population genetics, experimental functional anatomy, and field observation of non-human primates and human hunter-gatherers. Thanks to Washburn's stimulus, his colleague at Berkeley, the bird ethologist Peter Marler, took up the study of the natural communicative behaviour of apes and monkeys. When Marler's first primatological student, Thomas Struhsaker, reported in the mid-1960s that the vervet monkeys of Amboseli, Kenya, give acoustically distinct alarm calls to different predators, and respond to alarm calls as if to the sight of those predators, a debate broke out over whether the vervet calls thus function as names, translating as 'leopard', 'eagle' and 'python'. Washburn and his students argued that no matter what the behavioural evidence, vervet calls could not be predator names, since monkeys had been shown to lack the neuroanatomical basis of naming. This controversy thus reveals, first, the persistence of older patterns of disciplinary allegiance within the new, synthetic physical anthropology; and second, the impotence of adaptationist Darwinism--common to both sides of the debate--as a force for unity.

Divergence among barking frogs (Eleutherodactylus augusti) in the southwestern United States

Barking frogs (Eleutherodactylus augusti) are distributed from southern Mexico along the Sierra Madre Occidental into Arizona and the Sierra Madre Oriental into Texas and New Mexico. Barking frogs in Arizona and most of Texas live in rocky areas in oak woodland, while those in New Mexico and far western Texas live in rodent burrows in desertscrub. Barking frogs in each of the three states have distinct coloration and differ in sexually dimorphic characters, female vocalization, and skin toxicity. We analyzed advertisement call variation and conducted a phylogenetic analysis using mitochondrial DNA sequences (ND2 and tRNA regions) for barking frogs from these three states. Advertisement calls of frogs from Arizona were significantly longer in duration, higher in frequency, and had longer duration pulses than those of frogs from either New Mexico or Texas; frogs from these latter two sites were indistinguishable in these call variables. Phylogenetic analysis showed deep divisions among barking frogs from the three states. Differences in call structure, coloration, and mitochondrial DNA sequences strongly suggest that barking frogs in Arizona are reproductively isolated from those in New Mexico and Texas. Our results indicate that either northern populations are connected via gene flow through southern Mexico (i.e., they are subspecies as currently recognized), or represent independent lineages as originally described (i.e., western barking frogs, E. cactorum in AZ, and the eastern barking frogs, E. latrans in NM, TX).

Bottom Lines • THE KEY STRATEGIC NUCLEAR CHOICE. Whether to attempt to preserve its damage-limitation capability against China is the key strategic nuclear choice facing the United States. The answer is much less clear-cut than when the United States faced the Soviet Union during the Cold War. • FEASIBILITY OF DAMAGE LIMITATION. Although technology has advanced significantly over the past three decades, future military competition between the U.S. and Chinese forces will favor large-scale nuclear retaliation over significant damage limitation. • BENEFITS AND RISKS OF A DAMAGE-LIMITATION CAPABILITY. The benefits provided by a modest damage-limitation capability would be small, because the United States can meet its most important regional deterrent requirements without one. In comparison, the risks, which include an increased probability of accidental and unauthorized Chinese attacks, as well as strained U.S.—China relations, would be large. • FOREGO DAMAGE LIMITATION. These twin findings—the poor prospects for prevailing in the military competition, and the small benefits and likely overall decrease in U.S. security—call for a U.S. policy that foregoes efforts to preserve or enhance its damage-limitation capability.

Pairing call-response surveys and distance sampling for a mammalian carnivore

Density estimates accounting for differential animal detectability are difficult to acquire for wide-ranging and elusive species such as mammalian carnivores. Pairing distance sampling with call-response surveys may provide an efficient means of tracking changes in populations of coyotes (Canis latrans), a species of particular interest in the eastern United States. Blind field trials in rural New York State indicated 119-m linear error for triangulated coyote calls, and a 1.8-km distance threshold for call detectability, which was sufficient to estimate a detection function with precision using distance sampling. We conducted statewide road-based surveys with sampling locations spaced ≥6 km apart from June to August 2010. Each detected call (be it a single or group) counted as a single object, representing 1 territorial pair, because of uncertainty in the number of vocalizing animals. From 524 survey points and 75 detections, we estimated the probability of detecting a calling coyote to be 0.17 ± 0.02 SE, yielding a detection-corrected index of 0.75 pairs/10 km2 (95% CI: 0.52–1.1, 18.5% CV) for a minimum of 8,133 pairs across rural New York State. Importantly, we consider this an index rather than true estimate of abundance given the unknown probability of coyote availability for detection during our surveys. Even so, pairing distance sampling with call-response surveys provided a novel, efficient, and noninvasive means of monitoring populations of wide-ranging and elusive, albeit reliably vocal, mammalian carnivores. Our approach offers an effective new means of tracking species like coyotes, one that is readily extendable to other species and geographic extents, provided key assumptions of distance sampling are met.

Funding for graduate medical education is at risk despite the services provided by residents. We quantified the potential monetary value of services provided by on-call orthopedic surgery residents. We conducted a prospective, cross-sectional, multicenter cohort study design. Over a 90-day period in 2014, we collected data on consults by on-call orthopedic surgery residents at 4 tertiary academic medical centers in the United States. All inpatient and emergency department consults evaluated by first-call residents during the study period were eligible for inclusion. Based on their current procedural terminology codes, procedures and evaluations for each consult were assigned a relative value unit and converted into a monetary value to determine the value of services provided by residents. The primary outcome measures were the total dollar value of each consult and the percentage of resident salaries that could be funded by the generated value of the resident consult services. In total, 2644 consults seen by 33 residents from the 4 institutions were included for analysis. These yielded an average value of $81,868 per center for the 90-day study period, that is, $327,471 annually. With a median resident stipend of $53,992, the extrapolated average percentage of resident stipends that could be funded by these consult revenues was 73% of the stipends of the residents who took call or 36% of the stipends of the overall resident cohort. The potential monetary value generated by on-call orthopedic surgery residents is substantial.

By means of computer modelling, the self-organization of dense planar brushes of macromolecules with amphiphilic monomer units was addressed and their state diagram was constructed. The diagram of states includes the following regions: disordered position of monomer units with respect to each other, strands composed of a few polymer chains and lamellae with different domain spacing. The transformation of lamellae structures with different domain spacing occurred within the intermediate region and could proceed through the formation of so-called parking garage structures. The parking garage structure joins the lamellae with large (on the top of the brushes) and small (close to the grafted surface) domain spacing, which appears like a system of inclined locally parallel layers connected with each other by bridges. The parking garage structures were observed for incompatible A and B groups in selective solvents, which result in aggregation of the side B groups and dense packing of amphiphilic macromolecules in the restricted volume of the planar brushes.

Trends in rates of acetaminophen-related adverse events in the United States

Purpose The goal of this study is to summarize trends in rates of adverse events attributable to acetaminophen use, including hepatotoxicity and mortality. Methods A comprehensive analysis of data from three national surveillance systems estimated rates of acetaminophen-related events identified in different settings, including calls to poison centers (2008–2012), emergency department visits (2004–2012), and inpatient hospitalizations (1998–2011). Rates of acetaminophen-related events were calculated per setting, census population, and distributed drug units. Results Rates of poison center calls with acetaminophen-related exposures decreased from 49.5/1000 calls in 2009 to 43.5/1000 calls in 2012. Rates of emergency department visits for unintentional acetaminophen-related adverse events decreased from 58.0/1000 emergency department visits for adverse drug events in 2009 to 50.2/1000 emergency department visits in 2012. Rates of hospital inpatient discharges with acetaminophen-related poisoning decreased from 119.8/100 000 hospitalizations in 2009 to 108.6/100 000 hospitalizations in 2011. After 2009, population rates of acetaminophen-related events per 1million census population decreased for poison center calls and hospitalizations, while emergency department visit rates remained stable. However, when accounting for drug sales, the rate of acetaminophen-related events (per 1 million distributed drug units) increased after 2009. Prior to 2009, the rates of acetaminophen-related hospitalizations had been slowly increasing (p-trend = 0.001). Conclusions Acetaminophen-related adverse events continue to be a public health burden. Future studies with additional time points are necessary to confirm trends and determine whether recent risk mitigation efforts had a beneficial impact on acetaminophen-related adverse events.
