A Survey of Registered Dietitians’ Concern and Actions Regarding Climate Change in the United States

Dietary choices are a tool to reduce greenhouse gas emissions. While registered dietitians are on the front lines of food and nutrition recommendations, it is unclear how many are concerned with climate change and take action in practice in the United States. We explored concern about climate change among registered dietitians, and identified factors that may influence practice-related behaviors. Our study population included a random sample of all registered dietitians credentialed in the United States. Primary data were gathered using a cross-sectional survey. Of the 570 survey responses, 75% strongly agreed or agreed that climate change is an important issue while 34% strongly agreed or agreed that dietitians should play a major role in climate change mitigation strategies. Thirty-eight percent engaged in activities that promoted diet as a climate change mitigation strategy. Vegetarian (p = 0.002) and vegan dietitians (p = 0.007) were significantly more likely than non-vegetarian and non-vegan dietitians to engage in activities that promoted diet as a climate change mitigation strategy. Overall, concern for climate change among dietitians varied significantly by the region of the country in which the dietitian resided, and awareness that animal products are implicated in climate change. Registered dietitians in the United States are concerned with climate change. However, there is a discrepancy between concern and practice-based actions. These results suggest the need for educational and experiential opportunities connecting climate change mitigation to dietetics practice. PMID:26217666

As has been the case with other types of medical tourism, the phenomenon of cross border fertility care ("CBFC") has sparked concern about the lack of global or even national harmonization in the regulation of the fertility industry. The diversity of laws around the globe leads would-be parents to forum shop for a welcoming place to make babies. Focusing specifically on the phenomenon of travel to the United States, this Article takes up the question of whether there should be any legal barriers to those who come to the United States seeking CBFC. In part, CBFC suffers from the same general concerns raised about the use of fertility treatment in general, but it is possible to imagine a subset of arguments that would lead to forbidding or at least discouraging people from coming to the United States for CBFC, either as a matter of law or policy. This paper stands in opposition to any such effort and contemplates the moral and ethical concerns about CBFC and how, and if, those concerns warrant expression in law. Part I describes the conditions that lead some couples and individuals to leave their home countries to access fertility treatments abroad and details why the United States, with its comparatively liberal regulation of ART, has become a popular CBFC destination for travelers from around the world. Part II offers and refutes arguments supporting greater domestic control over those who seek to satisfy their desires for CBFC in the United States by reasserting the importance of the right of procreation while also noting appropriate concerns about justice and equality in the market for babies. Part III continues the exploration of justice by investigating the question of international cooperation in legislating against perceived wrongs. This Part concludes that consistent legislation across borders is appropriate where there is consensus about the wrong of an act, but it is unnecessary and inappropriate where there remain cultural conflicts about certain practices—in this case assisted reproduction.

Several factors have generated interest in proposals to offer incentives in exchange for kidneys from living donors, including the growing shortage of available organs, the apparent asymptote of traditional means of organ procurement, and the intimate link between the inadequacies of organ procurement policies in developed countries with the flourishing of underground organ trafficking in developing countries. Herein, we review the scope and dimensions of the growing shortage of organs in the United States, with attention to how each of the proposed solutions to same has proven insufficient. With special attention to the concerns leveled by Gabriel Danovitch in his 'Open Letter,' we conclude that each of his concerns are unfounded, and offer a prospectus on how a trial of such systems might be pursued in the United States. The failure of current approaches to organ procurement in the United States and other developed countries has led to unnecessary suffering and death, with morally unacceptable consequences for developing countries. For these reasons, a structured trial of incentives for organ procurement in the United States is a moral imperative.

The boundary between the United States and Mexico was created for convenient expediency through political debate and agreements (fig. 1–1). With the exception of the eastern segment of the border, which follows the course of the Rio Grande (known as the Rio Bravo in Mexico), the defining of this border was based on political decisions that had little concern for ecosystems, geologic features, or water—all of which span that imaginary line. However, the location of the border has had a remarkable effect on the biologic and physical systems in the border region and, in turn, has had a growing influence on what we now see as 21st century socioeconomic and environmental priorities. Because of the complex interactions of the human, ecological, political, and economic exigencies associated with this area, the status of the United States–Mexican border region, known as the Borderlands, has become an ever-present concern for most American citizens and for Mexican and United States Federal, State, and local governments.

Two disparate statistics often cited for the Western United States raise concern about risks for developmental disabilities in Native American children. First, 13 of the states with the highest percentage of Native American population are located in the Western United States (U.S. Census Bureau, 2012 ). Second, more than 161,000 abandoned hard-rock mines are located in 12 Western states (General Accounting Office, 2014 ). Moreover, numerous studies have linked low-level metals exposure with birth defects and developmental delays. Concern has emerged among tribal populations that metals exposure from abandoned mines might threaten development of future generations.

To assess the attitudes of medical students in India about participating in graduate medical education in the United States and other countries and in subsequent clinical practice in those countries. A total of 240 students who were attending their final year at two medical schools in Bangalore, India, were surveyed during 2004. Surveys were completed by 166 (69%) of the students. Among the responding students, 98 (59%) thought of leaving India for further training abroad. Of those who wished to leave, 41 (42%) preferred the United States, 42 (43%) preferred the United Kingdom, and 9 (9%) preferred Canada, Australia or New Zealand. Only two students preferred the Middle East. Most who favored training in the United States indicated that they intended to remain after training, whereas fewer than 20% of those who favored training in the United Kingdom had such intentions. While more than 60% perceived greater professional opportunities in the United States than in India, approximately 75% were concerned that the United States had become less welcoming after the terrorist attacks of 9/11, and similar numbers were concerned about the examination administered by the Educational Commission on Foreign Medical Graduates. Conversely, the majority of respondents felt that opportunities for physicians in India were improving. While optimism about future medical careers in India is increasing, the interest of Indian medical students in training and subsequently practicing in the United States remains high.

Objectives—To determine public attitudes in the United States concerning gun carrying. Setting—In the past 15 years, many state legislatures have passed laws making it easier for United States citizens to carry concealed firearms, not only on the street but into various locations, including churches and government buildings. Methods—National random digit dial telephone surveys conducted in 1996 and 1999 asked questions concerning the public's feelings of safety as more people in their community carry firearms, and whether, in the language of the question, respondents believe "regular" citizens should be allowed to carry guns into public or government buildings. Results—Americans feel less safe rather than more safe as more people in their community begin to carry guns. By margins of at least nine to one, Americans do not believe that "regular" citizens should be allowed to bring their guns into restaurants, college campuses, sports stadiums, bars, hospitals, or government buildings. Conclusions—The public believes that increased gun carrying by others reduces rather than increases their safety. Overwhelmingly, the public believes that in many venues gun carrying should be prohibited. PMID:11770652

Muddling through the Health System: Experiences of Three Groups of Black Women in Three Regions

Health care disparities are a well-documented concern among patients and providers who care for minority groups in the United States. In this study, focus groups were created from an original sample of 606 Black women representing three regions in the United States: the South, the Midwest, and the Virgin Islands. Composed of 10 randomly selected members each (n = 30), the focus groups provided insights into the nature of these disparities, with some suggestions for viable solutions. Participants voiced concerns about cultural taboos about discussing menopause, financial concerns, and negative experiences with health care leading to distrust in medical systems. The primary solution proposed was an increase in Black health care professionals who would have increased rapport with, empathy for, and understanding of the concerns of Black women. PMID:26371357

Precipitation and the occurrence of lyme disease in the Northeastern United States

The occurrence of Lyme disease is a growing concern in the United States, and various studies have been performed to understand the factors related to Lyme disease occurrence. In the United States, Lyme disease has occurred most frequently in the northeastern United States. Positive correlations between the number of cases of Lyme disease reported in the northeastern United States during the 1992-2002 period indicate that late spring/early summer precipitation was a significant climate factor affecting the occurrence of Lyme disease. When late spring/early summer precipitation was greater than average, the occurrence of Lyme disease was above average, possibly due to increased tick activity and survival rate during wet conditions. Temperature did not seem to explain the variability in Lyme disease reports for the northeastern United States. ?? Mary Ann Liebert, Inc.

Kissing Bugs in the United States: Risk for Vector-Borne Disease in Humans

Eleven species of kissing bugs are found in the United States. Their home ranges may be expanding northward, perhaps as a consequence of climate change. At least eight of the species, perhaps all, are reported to harbor Trypanosoma cruzi, the parasite that causes Chagas disease. Because humans are encroaching on kissing bug habitat, there is concern for vector-transmitted Chagas disease in the United States. To date, documented autochthonous cases of Chagas in humans in the United States are rare. Kissing bugs are capable of adapting to new habitats such as human domiciles; however, they do not colonize homes in the United States as in Central and South America. We review the biology, behavior, and medical importance of kissing bugs and the risk they pose for transmission of Chagas disease in the United States. Where possible, descriptions of US species are compared to the epidemiologically important Latin American species. PMID:25574143

A Comparative Analysis of Domestic Violence Shelter Staff Perceptions Regarding Barriers to Services in Bosnia and Herzegovina and the United States.

Service provision for domestic violence (DV) survivors has been a long-standing staple of shelters in the United States. Although shelter services provide numerous benefits for survivors, barriers tied to acquisition remain a pressing concern when combatting DV. Nevertheless, there has been minimal research exploring barriers to service acquisition on a cross-national level. As such, the current research cross-nationally examines perceptions of shelter staff regarding acquisition barriers as well as the effectiveness of local agencies to meet survivor needs and differences in populations served in the United States (specifically Texas) as well as in Bosnia and Herzegovina. Data collection stemmed from self-report surveys originally constructed in English and translated into Bosnian/Serbian/Croatian. Results underscored differences between populations served, perceptions of local agencies assisting survivors of DV, and barriers tied to cultural and financial concerns. Implications, limitations, and future directions are also discussed.

The introduction and spread of West Nile virus and the recent introduction of chikungunya and Zika viruses into the Americas have raised concern about the potential for various tropical pathogens to become established in North America. A historical analysis of yellow fever and malaria incidences in the United States suggests that it is not merely a temperate climate that keeps these pathogens from becoming established. Instead, socioeconomic changes are the most likely explanation for why these pathogens essentially disappeared from the United States yet remain a problem in tropical areas. In contrast to these anthroponotic pathogens that require humans in their transmission cycle, zoonotic pathogens are only slightly affected by socioeconomic factors, which is why West Nile virus became established in North America. In light of increasing globalization, we need to be concerned about the introduction of pathogens such as Rift Valley fever, Japanese encephalitis, and Venezuelan equine encephalitis viruses.

Bats and wind energy: a literature synthesis and annotated bibliography

Turbines have been used to harness energy from wind for hundreds of years. However, with growing concerns about climate change, wind energy has only recently entered the mainstream of global electricity production. Since early on in the development of wind-energy production, concerns have arisen about the potential impacts of turbines to wildlife; these concerns have especially focused on the mortality of birds. Despite recent improvements to turbines that have resulted in reduced mortality of birds, there is clear evidence that bat mortality at wind turbines is of far greater conservation concern. Bats of certain species are dying by the thousands at turbines across North America, and the species consistently affected tend to be those that rely on trees as roosts and most migrate long distances. Turbine-related bat mortalities are now affecting nearly a quarter of all bat species occurring in the United States and Canada. Most documented bat mortality at wind-energy facilities has occurred in late summer and early fall and has involved tree bats, with hoary bats (Lasiurus cinereus) being the most prevalent among fatalities. This literature synthesis and annotated bibliography focuses on refereed journal publications and theses about bats and wind-energy development in North America (United States and Canada). Thirty-six publications and eight theses were found, and their key findings were summarized. These publications date from 1996 through 2011, with the bulk of publications appearing from 2007 to present, reflecting the relatively recent conservation concerns about bats and wind energy. The idea for this Open-File Report formed while organizing a joint U.S. Fish and Wildlife Service/U.S. Geological Survey "Bats and Wind Energy Workshop," on January 25-26, 2012. The purposes of the workshop were to develop a list of research priorities to support decision making concerning bats with respect to siting and operations of wind-energy facilities across the United States. This document was intended to provide background information for the workshop participants on what has been published on bats and wind-energy issues in North America (United States and Canada).

Source water quality is of major concern to all drinking water utilities. The accidental introduction of contaminants to their source water is a constant threat to utilities withdrawing water from navigable or industrialized rivers. Since 1996, the emergence of Asian-origin highly pathogenic avian influenza subtype H5N1 has spurred great concern for the global poultry industry. In the United States, there is concern over the potential of a foreign avian disease incursion into the country. Noncommercial poultry operations, such as upland game bird facilities in the United States, may serve as a potential source of avian disease introduction to other bird populations including the commercial poultry industry, backyard flocks, or wildlife. In order to evaluate how to prevent disease transmission from these facilities to other populations, we examined biosecurity practices and bird movement within the upland game bird industry in the United States. Persons that held a current permit to keep, breed, or release upland game birds were surveyed for information on biosecurity practices, flock and release environments, and bird movement parameters. Biosecurity practices vary greatly among permit holders. Many facilities allow for interaction between wild birds and pen-reared birds, and there is regular long-distance movement of live adult birds among facilities. Results suggest that upland game bird facilities should be targeted for biosecurity education and disease surveillance efforts.

Rising health care costs are an international concern, particularly in the United States, where spending on health care outpaces that of other industrialized countries. Consequently, there is growing desire in the United States and Europe to take a more value-based approach to health care, particularly with respect to the adoption and use of new health technology. This article examines medical device reimbursement and pricing policies in the United States and Europe, with a particular focus on value. Compared to the United States, Europe more formally and consistently considers value to determine which technologies to cover and at what price, especially for complex, costly devices. Both the United States and Europe have introduced policies to provide temporary coverage and reimbursement for promising technologies while additional evidence of value is generated. But additional actions are needed in both the United States and Europe to ensure wise value-based reimbursement and pricing policies for all devices, including the generation of better pre- and postmarket evidence and the development of new methods to evaluate value and link evidence of value to reimbursement.

Status and management of moose in the northeastern United States

Moose (Alces alces) populations have recolonized much of their historic range in the northeastern United States in the past 30 years, with their southern range edge extending to southern New England and northern New York. This southerly expansion occurred when certain other populations in the United States were in decline along the southern range edge, with climate change often cited as a probable cause. The areas that moose have recently occupied in the northeastern United States are some of the most densely human populated in moose range, which has raised concern about human safety and moose-vehicle collisions (MVC). We conducted a literature search about moose in the northeastern United States, and distributed a questionnaire and conducted phone interviews with regional biologists responsible for moose management to determine the status of moose, management activity, and research deficiencies and needs. Moose numbers appear stable throughout much of the region, with slow population growth in northern New York. Management activity ranges from regulated harvest of moose in Maine, New Hampshire, and Vermont, to no active management in southern New England and New York. The combined annual harvest in Maine, New Hampshire, and Vermont is >3,000. MVCs are a widespread regional concern with >1,000 occurring annually involving several human fatalities. Research should address impacts of parasitism by winter tick (Dermacentor albipictus) and brain-worm (Parelaphostrongylus tenuis) on productivity and mortality of moose, influence of climate change on population dynamics and range, and conflicts in areas with high human population density.

How patients understand physicians' solicitations of additional concerns: implications for up-front agenda setting in primary care.

In the more than 1 billion primary-care visits each year in the United States, the majority of patients bring more than one distinct concern, yet many leave with "unmet" concerns (i.e., ones not addressed during visits). Unmet concerns have potentially negative consequences for patients' health, and may pose utilization-based financial burdens to health care systems if patients return to deal with such concerns. One solution to the problem of unmet concerns is the communication skill known as up-front agenda setting, where physicians (after soliciting patients' chief concerns) continue to solicit patients' concerns to "exhaustion" with questions such as "Are there some other issues you'd like to address?" Although this skill is trainable and efficacious, it is not yet a panacea. This article uses conversation analysis to demonstrate that patients understand up-front agenda-setting questions in ways that hamper their effectiveness. Specifically, we demonstrate that up-front agenda-setting questions are understood as making relevant "new problems" (i.e., concerns that are either totally new or "new since last visit," and in need of diagnosis), and consequently bias answers away from "non-new problems" (i.e., issues related to previously diagnosed concerns, including much of chronic care). Suggestions are made for why this might be so, and for improving up-front agenda setting. Data are 144 videotapes of community-based, acute, primary-care, outpatient visits collected in the United States between adult patients and 20 family-practice physicians.

We evaluated the commercial and recreational fishery landings over the past 22 years, first at the national level, then for populations of concern (those that are overfished or experiencing overfishing), and finally by region. Recreational landings in 2002 account for 4% of total marine fish landed in the United States. With large industrial fisheries excluded (e.g., menhaden and pollock), the recreational component rises to 10%. Among populations of concern, recreational landings in 2002 account for 23% of the total nationwide, rising to 38% in the South Atlantic and 64% in the Gulf of Mexico. Moreover, it affects many of the most-valued overfished species-including red drum, bocaccio, and red snapper-all of which are taken primarily in the recreational fishery.

Childhood immunization involves a balance between parents’ autonomy in deciding whether to immunize their children and the benefits to public health from mandating vaccines. Ethical concerns about pediatric vaccination span several public health domains, including those of policymakers, clinicians, and other professionals. In light of ongoing developments and debates, we discuss several key ethical issues concerning childhood immunization in the United States and describe how they affect policy development and clinical practice. We focus on ethical considerations pertaining to herd immunity as a community good, vaccine communication, dismissal of vaccine-refusing families from practice, and vaccine mandates. Clinicians and policymakers need to consider the nature and timing of vaccine-related discussions and invoke deliberative approaches to policymaking. PMID:26691123

Although these changes have been beneficial for increasing agricultural production, some of them have resulted in environmental concerns. The agriculture timeline is divided into four categories (1) crop and animal changes, (2) mechanical changes, (3) biological and chemical changes, and (4) regulatory and societal changes. The timeline attempts to compile events that have had a lasting impact on agriculture in the United States. Concerns have been voiced about an impending oversupply of physicians in the United States. Do these concerns also apply to California, a state with many unique demographic characteristics? We examined trends in physician supply and medical education in California and the United States between 1980 and 1995 to better inform the formulation of workforce policies appropriate to the state's requirements for physicians. We found that similar to the United States, California has more than an ample supply of physicians in the aggregate, but too many specialists, too few underrepresented racial/ethnic minority physicians, and poor distribution of physicians across the state. However, recent growth in the supply of practicing physicians and resident physicians per capita in California has been much less dramatic than in the country overall. The state's unusually high rate of population growth has enabled California, unlike the United States as a whole, to absorb large increases in the number of practicing physicians and residents during 1980 to 1995 without substantially increasing the physician-to-population ratio. Due to a projected slowing of the state's rate of population growth, the supply of physicians per capita in the state will begin to rise steeply in coming years unless the state implements prompt reductions in the production of specialists. An immediate 25% reduction in specialist residency positions would be necessary to bring the state's supply of practicing specialists in line with projected physician requirements for the state by 2020. We conclude that major changes will be required if the state's residency programs and medical schools are to produce the number and mix of physicians the state requires. California's medical schools and residency programs will need to act in concert with federal and state government to develop effective policies to address the imbalance between physician supply and state requirements. Condoned or condemned: the situational affordance of anger and shame in the United States and Japan.

Two studies tested the idea that the situations that people encounter frequently and the situations that they associate most strongly with an emotion differ across cultures in ways that can be understood from what a culture condones or condemns. In a questionnaire study, N = 163 students from the United States and Japan perceived situations as more frequent to the extent that they elicited condoned emotions (anger in the United States, shame in Japan), and they perceived situations as less frequent to the extent that they elicited condemned emotions (shame in the United States, anger in Japan). In a second study, N = 160 students from the United States and Japan free-sorted the same situations. For each emotion, the situations could be organized along two cross-culturally common dimensions. Those situations that touched upon central cultural concerns were perceived to elicit stronger emotions. The largest cultural differences were found for shame; smaller, yet meaningful, differences were found for anger.

This designation resulted from extremely high incidences of mesothelioma discovered in three small villages from the Cappadocia region of Turkey, where the disease was linked to environmental exposures to fibrous forms of erionite. Natural deposits of erionite, including fibrous forms, have been identified in the past in the western United States. Until recently, these occurrences have generally been overlooked as a potential hazard. In the last several years, concerns have emerged regarding the potential for environmental and occupational exposures to erionite in the United States, such as erionite-bearing gravels in western North Dakota mined and used to surface unpaved roads. As a result, there has been much interest in identifying locations and geologic environments across the United States where erionite occurs naturally. A 1996 U.S. Geological Survey report describing erionite occurrences in the United States has been widely cited as a compilation of all US erionite deposits; however, this compilation only focused on one of several geologic environments in which erionite can form. Also, new occurrences of erionite have been identified in recent years. Using a detailed literature survey, this paper updates and expands the erionite occurrences database, provided in a supplemental file (US_erionite.xls). Epidemiology, public health, and natural hazard studies can incorporate this information on known erionite occurrences and their characteristics. Childhood Ideology in the United States: A Comparative Cultural View

Childhood ideology functions in each nation as a complex of ideas about what children are like and how best to teach and socialise them. One important domain of childhood ideology concerns ideas and practices related to children's development and behaviour management. Drawing from an analysis of popular childrearing magazines and early childhood education materials in the United States, this cultural study describes contemporary American mainstream beliefs concerning children's early emotional and behavioural development. In particular, the paper explores themes of emotional expression, autonomy, individuality, power, and consumerism. Some comparisons with Japanese views on child development and emotional/behavioural socialisation are also made. The paper suggests that popular ideas and techniques of emotional and behavioural management in the United States in both families and early childcare environments reflect a dominant ideology of children that has potentially negative consequences for children's welfare. Furthermore, childhood ideologies, while retaining culturally specific values and ideas neither remain static nor exist in isolation from one another. The paper questions the global diffusion of a Western-style professionalised discourse of child psychology that may not be applicable to all nations and their children.

Breast cancer is the number one cause of cancer death among women in the United States Virgin Islands. Consequently, the Bureau of Health has identified breast cancer as a priority health concern. Within the medical community, increasing emphasis is being placed on the importance of hereditary, familial, environmental, and behavioral risk factors to breast cancer control. Little research has been conducted regarding these factors, however, to explore their influence on breast cancer detection and breast cancer risk management. This report highlights the outcomes of a study undertaken to explore the associations between breast cancer risk, risk assessment, risk communication, screening, and receptivity to the management of breast cancer risk among women from the United States Virgin Islands. Results of this study suggest a need within the territory to expand the systems that are responsible for monitoring and reporting breast cancer trends; forums to discuss concerns of women relative to breast health; forums to discuss communication with health-care providers; and, research efforts that address breast cancer detection and control among women in the United States Virgin Islands.

Effects of demographic factors and information sources on United States consumer perceptions of animal welfare.

As consumers have become more interested in understanding how their food is produced, scrutiny and criticism have increased regarding intensified food animal production methods. Resolution of public concerns about animal agricultural practices depends on understanding the myriad factors that provide the basis for concerns. An online survey of 798 U.S. households was conducted to investigate relationships between household characteristics (demographics, geographic location, and experiences) and level of concern for animal welfare as well as sources used to obtain information on the subject. Because recent media attention has focused on animal care practices used in the U.S. swine industry, respondents were also asked specific questions pertaining to their perceptions of pig management practices and welfare issues and their corresponding pork purchasing behavior. Respondents reporting higher levels of concern about animal welfare were more frequently female, younger, and self-reported members of the Democratic Party. Fourteen percent of respondents reported reduction in pork consumption because of animal welfare concerns with an average reduction of 56%. Over half of the respondents (56%) did not have a primary source for animal welfare information; those who identified a primary information source most commonly used information provided by animal protection organizations, the Humane Society of the United States (HSUS), and People for the Ethical Treatment of Animals (PETA). Midwest participants were significantly, at the 5% significance level, less concerned about domestic livestock animal welfare and more frequently reported not having a source for animal welfare information than those from other regions of the United States. Overall, the U.S. livestock and poultry industries and other organizations affiliated with animal agriculture appear to be less used public sources of information on animal welfare than popular animal protection organizations. Improved understanding of the factors that contribute to consumers' evolving perceptions of the care and welfare of farm animals is an essential step toward enhanced sustainability and social responsibility in contemporary food production systems.

The impulsivity of suicide provides opportunities to reduce the risk of suicide by restricting access to lethal means. In the United States, firearms, particularly handguns, are the most common means of suicide. Despite strong empirical evidence that restriction of access to firearms reduces suicides, access to firearms in the United States is generally subject to few restrictions. Implementation and evaluation of measures such as waiting periods and permit requirements that restrict access to handguns should be a top priority for reducing deaths from impulsive suicide in the United States.

The study objective was to identify facilitators and barriers of parental attitudes and beliefs toward school-located influenza vaccination in the United States. In 2009, the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention expanded their recommendations for influenza vaccination to include school-aged children. We conducted a systematic review of studies focused on facilitators and barriers of parental attitudes toward school-located influenza vaccination in the United States from 1990 to 2016. We reviewed 11 articles by use of the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) framework. Facilitators were free/low cost vaccination; having belief in vaccine efficacy, influenza severity, and susceptibility; belief that vaccination is beneficial, important, and a social norm; perception of school setting advantages; trust; and parental presence. Barriers were cost; concerns regarding vaccine safety, efficacy, equipment sterility, and adverse effects; perception of school setting barriers; negative physician advice of contraindications; distrust in vaccines and school-located vaccination programs; and health information privacy concerns. We identified the facilitators and barriers of parental attitudes and beliefs toward school-located influenza vaccination to assist in the evidence-based design and implementation of influenza vaccination programs targeted for children in the United States and to improve influenza vaccination coverage for population-wide health benefits. The emphasis on rabies control and prevention in the United States seems to be a function of our perception of proximity of the threat. Wildlife rabies epizootics within a state may be of little concern to the uninformed urban dweller. Additionally, many parts of the western United States are free of terrestrial rabies; were it not for the presence of bat rabies, people in those areas would likely interpret rabies control as a minor public health concern. It is essential that federal, state, and local public health programs emphasize the importance of rabies control through activities that include rabies education, sponsorship of legislated requirements for domestic animal vaccination, support for local animal control programs, and the promotion of recommendations that encourage the appropriate use of PEP. We are almost guaranteed that rabies is going to remain a major public health issue well into the next century because of expanding wildlife rabies epizootics, identification of new rabies viral variants with increased public health concern, emotional and legal concerns associated with rabies exposure, and increasing national cost associated with rabies control and prevention. Nevertheless, the development of new laboratory technology that allows an understanding of the epidemiologic nature of the rabies virus based on an evolving genetic history and the interrelationship with wildlife reservoirs should allow access to valuable tools for rabies control. When combined with programs using new developments in oral rabies vaccine that can immunize whole populations of wildlife reservoirs, that technology offers encouragement in our effort to control one of the diseases of antiquity.

This article briefly recounts the 21st July 2005, Blood Products Advisory Committee (BPAC) meeting concerning recommendations for management of donors and units testing positive for hepatitis B virus (HBV) DNA. The author attended the meeting. The United States Food and Drug Administration (FDA) web site was used for meeting materials, and handouts were collected at the meeting to provide narrative information. Two European experts assisted with HBV subject matter. The proceedings of the advisory committee, the issue briefing materials, and testing algorithms are presented. BPAC voted concurrence with the FDA algorithm for Management of Donors and Units Testing Positive for Hepatitis B Virus DNA.

The disposal of wastes associated with the processing of cotton is posing increasing problems for cotton gin operators in the western United States. Traditional disposal methods, such as open-air incineration and landfilling are no longer adequate due to increasing environmental concerns. This paper evaluates the technical, economic and environmental feasibility for cotton gin trash to serve as an energy resource. Cotton gin trash has been quantified, by county, in the five cotton-growing states of the western United States. Ultimately, the choice of dataset depends on the particular application, the catchment size, climate and the time period of study. In a region with a long record and a dense rain gauge network, the elevation-modified ground-based precipitation product, PRISM, has been found to work well. However, in poorly gauged regions the use of remotely sensed precipitation products is an absolute necessity. The Tropical Rainfall Measuring Mission (TRMM) has provided valuable precipitation datasets for hydrometeorological studies over the past two decades (1998-2015). One concern regarding the usage of TRMM data is the accuracy of the precipitation estimates, when compared to those obtained using PRISM. The reason for this concern is that TRMM and PRISM do not always agree and, typically, TRMM underestimates PRISM over the mountainous regions of the United States. In this study, we develop a correction function to improve the accuracy of the TRMM monthly product (TRMM-3B43) by estimating and removing the bias in the satellite data using the ground-based precipitation product, PRISM. We observe a strong relationship between the bias and land surface elevation; TRMM-3B43 tends to underestimate the PRISM product at altitudes greater than 1500 m above mean sea level (m.amsl) in the contiguous United States. A relationship is developed between TRMM-PRISM bias and elevation. The correction function is used to adjust the TRMM monthly precipitation using PRISM and elevation data. The model is calibrated using 25% of the available time period and the remaining 75% of the time period is used for validation. The corrected TRMM-3B43 product is verified for the high elevations over the contiguous United States and two local regions in the mountainous areas of the western United States. The results show a significant improvement in the accuracy of the TRMM product in the high elevations of the contiguous United States.

Snowy Plovers (Charadrius alexandrinus) are small, partially migrant shorebirds that are broadly distributed across North America. Snowy Plover distribution west of the Rocky Mountains has been well described. However, distribution and abundance east of the Rocky Mountains has not received much attention despite current status and ESA listing concerns for Snowy Plovers in the southeastern United States and the Caribbean. Thus, a first step in developing a monitoring program for Snowy Plovers is to understand the species' distribution. We summarize information on distribution and abundance of Snowy Plovers in the eastern United States, Caribbean, and Bahamas. Breeding and winter distribution maps for the continental United States were generated from a database of 3563 records from 388 sites in continental North America constructed from International Shorebird Survey (ISS), Christmas Bird Count (CBC), unpublished field data, and published accounts. Comparison of maximum counts per site (1980–present) indicated the number of breeding Snowy Plovers was greatest in Kansas and Oklahoma, while the greatest number of wintering birds occurred in the Laguna Madre of Texas and Mexico. Snowy Plovers concentrate at sites in Oklahoma and Texas during migration, with higher concentrations on the upper Texas coast in spring compared to fall migration. Data regarding historic abundance and trends are limited but suggest that Snowy Plovers in the eastern United States may have experienced regional population declines and may have suffered a range contraction in Texas. Serious concerns about the conservation status of Snowy Plovers in the eastern United States, the Caribbean, and the Bahamas indicate an immediate need for systematic surveys and up-to-date population estimates.

By combining digital humanities text-mining tools and a qualitative approach, we examine changing concepts in forestry journals in Sweden and the United States (US) in the early twentieth and early twenty-first centuries. Our first hypothesis is that foresters at the beginning of the twentieth century were more concerned with production and less concerned with ecology than foresters at the beginning of the twenty-first century. Our second hypothesis is that US foresters in the early twentieth century were less concerned with local site conditions than Swedish foresters. We find that early foresters in both countries had broader-and often ecologically focused-concerns than hypothesized. Ecological concerns in the forestry literature have increased, but in the Nordic countries, production concerns have increased as well. In both regions and both time periods, timber management is closely connected to concerns about governance and state power, but the forms that governance takes have changed.

A major concern of land managers in the United States is the response of watersheds to weather after a wildfire. With an ever-expanding wildland-urban interface (WUI), land managers must be cognizant of potential damage to private property and other values at risk. In the United States, land-management agencies from the U.S. Department of Agriculture (USDA) and the U.S. Department of the Interior (DOI) deploy Burned Area Emergency Response (BAER) teams to address these concerns and to “prescribe and implement emergency treatments to minimize threats to life or property or to stabilize and prevent unacceptable degradation to natural and cultural resources resulting from the effects of a fire” (USDA Forest Service 2004, p. 17). BAER teams’ objective is emergency stabilization of burned areas, rather than long-term restoration of the landscape after a fire.

Abstract Chihuahua is Mexico’s largest state, and its physiographic complexity affects the distribution of its herpetofauna. We list amphibians and reptiles for the state of Chihuahua, with their conservation status. We also compare this list to those of six adjoining states in the United States and Mexico (New Mexico, Texas, Coahuila, Durango, Sinaloa, and Sonora). A total of 175 species of amphibians and reptiles is found in Chihuahua. Thirty-eight are amphibians, and 137 reptiles. Chihuahuan amphibians and reptiles represent just over 37% of such species from Chihuahua and neighboring states. Chihuahua shares the highest proportion of its herpetofauna with Sonora and Durango. Most of the herpetofauna of Chihuahua falls in IUCNs least concern category and is not listed by SEMARNAT. However, turtles in Chihuahua are a group of particular conservation concern. PMID:28435388

The costs of health care continue to increase rapidly and steeply in the United States. One area of great expense is that of intensive care units (ICUs). The causes of inflation have not been addressed effectively. ICU resources could become stretched such that they may no longer be available. This paper discusses some of the ethics and concerns behind decision making when providing ICU services in the United States. In particular, the use of electronic records with decision making tools, risk-analysis methods, and documentation of patient wishes for extraordinary care may help with better utilization of resources in the future. Income Inequality and Intergenerational Income Mobility in the United States

Is there a relationship between family income inequality and income mobility across generations in the United States? As family income inequality rose in the United States, parental resources available for improving children’s health, education, and care diverged. The amount and rate of divergence also varied across US states. Researchers and policy analysts have expressed concern that relatively high inequality might be accompanied by relatively low mobility, tightening the connection between individuals’ incomes during childhood and adulthood. Using data from the Panel Study of Income Dynamics, the National Longitudinal Survey of Youth, and various government sources, this paper exploits state and cohort variation to estimate the relationship between inequality and mobility. Results provide very little support for the hypothesis that inequality shapes mobility in the United States. The inequality children experienced during youth had no robust association with their economic mobility as adults. Formal analysis reveals that offsetting effects could underlie this result. In theory, mobility-enhancing forces may counterbalance mobility-reducing effects. In practice, the results suggest that in the US context, the intergenerational transmission of income may not be very responsive to changes in inequality. PMID:26388653

The major issues and challenges confronting the dental professions in the United States and Norway were studied through speeches of and interviews with the presidents of the American Dental Association (ADA) and the Norwegian Dental Association (NDA) in the period 1980-86. The issues most frequently dealt with related to public authorities, particularly legislation and government involvement in dental practice. Anxiety concerning "busyness," the future dental market, and a drop in the quality of applicants to dental schools were also major subjects. The spokespersons of both associations were engaged in increasing the demand for dental services by marketing, but they were ambivalent regarding advertising, especially individual advertising. Both were concerned with protecting dentistry's autonomy. While the Norwegian presidents apparently feared the medical profession's influence upon dentistry, the Americans were concerned with the hygienists and denturists, and with the insurance companies which they suspected of intruding into the dentist-patient relationship. The presidents' statements, which frequently varied, were apparently influenced by the current domestic political climate, the basic socio-political principles of the two countries, and the different socio-demographic make-up of their memberships.

Legitimacy of concerns about caffeine and energy drink consumption.

Whether caffeine and energy drink consumption presents a critical emerging health problem is not currently known. Available evidence suggests that energy drink consumption represents a change in the ways in which individuals in the United States consume caffeine but that the amount of caffeine consumed daily has not appreciably increased. In the present review, the question of whether Americans are sleep deprived (a potential reason for using caffeine) is briefly explored. Reported rates of daily caffeine consumption (based on beverage formulation) and data obtained from both civilian and military populations in the United States are examined, the efficacy of ingredients other than caffeine in energy drinks is discussed, and the safety and side effects of caffeine are addressed, including whether evidence supports the contention that excessive caffeine/energy drink consumption induces risky behavior. The available evidence suggests that the main legitimate concern regarding caffeine and energy drink use is the potential negative impact on sleep but that, otherwise, there is no cause for concern regarding caffeine use in the general population. This study is concerned with the performance modeling of a fault tolerant system consisting of operating units supported by a combination of warm and cold spares. The on-line as well as warm standby units are subject to failures and are send for the repair to a repair facility having single repairman which is prone to failure. If the failed unit is not detected, the system enters into an unsafe state from which it is cleared by the reboot and recovery action. The server is allowed to go for vacation if there is no failed unit present in the system. Markov model is developed to obtain the transient probabilities associated with the system states. Runge-Kutta method is used to evaluate the system state probabilities and queueing measures. To explore the sensitivity and cost associated with the system, numerical simulation is conducted.

HIV and Mexican migrant workers in the United States: a review applying the vulnerable populations conceptual model.

Mexican migrant workers residing in the United States are a vulnerable population at high risk for HIV infection. This article critically appraises the published data surrounding HIV prevalence in this vulnerable group, as seen through the lens of the Vulnerable Populations Conceptual Model. This model demonstrates how exposure to risk and resource availability affect health status. The health status of Mexican migrants in the United States is compromised by a number of factors that increase risk of HIV: limited access to health services, multiple sexual partners, low rates of condom use, men having sex with men, and lay injection practices. Migration from Mexico to the United States has increased the prevalence of HIV in rural Mexico, making this an issue of urgent binational concern. The growing number of cancer survivors worldwide has led to of the emergence of diverse survivorship movements in the United States and Europe. Understanding the evolution of cancer survivorship within the context of different political and health care systems is important for identifying the future steps that need to be taken and collaborations needed to promote research among and enhance the care of those living after cancer. The authors first review the history of survivorship internationally and important related events in both the United States and Europe. Lessons learned from survivorship research are then broadly discussed, followed by examination of the infrastructure needed to sustain and advance this work, including platforms for research, assessment tools, and vehicles for the dissemination of findings. Future perspectives concern the identification of collaborative opportunities for investigators in Europe and the United States to accelerate the pace of survivorship science going forward. Food colors are added to different types of commodities to increase their visual attractiveness or to compensate for natural color variations. The use of these additives is strictly regulated in the European Union, the United States, and many other countries worldwide. There is a growing concern about the safety of some commonly used legal food colorants and there is a trend to replace the synthetic forms with natural products. Additionally, a number of dyes with known or suspected genotoxic or carcinogenic properties have been shown to be added illegally to foods. Robust monitoring programs based on reliable detection methods are required to assure the food is free from harmful colors. The aim of this review is to present an up to date status of the various concerns arising from use of color additives in food. The most important food safety concerns in the field of food colors are lack of uniform regulation concerning legal food colors worldwide, possible link of artificial colors to hyperactive behavior, replacement of synthetic colors with natural ones, and the presence of harmful illegal dyes-both known but also new, emerging ones in food. The legal status of food color additives in the EU, United States, and worldwide is summarized. The reported negative health effects of both legal and illegal colors are presented. The European Rapid Alert System for Food and Feed notifications and US import alerts concerning food colors are analyzed and trends in fraudulent use of color additives identified. The detection methods for synthetic colors are also reviewed.

This handbook provides information to residents of the Central United States about the threat of earthquakes in that area, particularly along the New Madrid seismic zone, and explains how to prepare for, survive, and recover from such events. It explains the need for concern about earthquakes for those residents and describes what one can expect during and after an earthquake. Much is known about the threat of earthquakes in the Central United States, including where they are likely to occur and what can be done to reduce losses from future earthquakes, but not enough has been done to prepare for future earthquakes. The handbook describes such preparations that can be taken by individual residents before an earthquake to be safe and protect property.
