Geographic, racial, and ethnic variations in quality of care and outcomes have been well documented among the Medicare population. Few data exist on beneficiaries living in Puerto Rico, three-quarters of whom enroll in Medicare Advantage (MA). To determine the quality of care provided to white and Hispanic MA enrollees in the United States and Puerto Rico. A cross-sectional study of MA enrollees in 2011 was conducted, including white enrollees in the United States (n = 6 289 374), Hispanic enrollees in the United States (n = 795 039), and Hispanic enrollees in Puerto Rico (n = 267 016). The study was conducted from January 1, 2011, to December 31, 2011; data analysis took place from January 19, 2015, to January 2, 2016. Seventeen performance measures related to diabetes mellitus (including hemoglobin A1c control, retinal eye examination, low-density lipoprotein cholesterol control, nephropathy screening, and blood pressure control), cardiovascular disease (including low-density lipoprotein cholesterol control, blood pressure control, and use of a β-blocker after myocardial infarction), cancer screening (colorectal and breast), and appropriate medications (including systemic corticosteroids and bronchodilators for chronic obstructive pulmonary disease [COPD] and disease-modifying antirheumatic drugs). Of the 7.35 million MA enrollees in the United States and Puerto Rico in our study, 1.06 million (14.4%) were Hispanic. Approximately 25.1% of all Hispanic MA enrollees resided in Puerto Rico, which was more than those residing in any state. For 15 of the 17 measures assessed, Hispanic MA enrollees in Puerto Rico received worse care compared with Hispanics in the United States, with absolute differences in performance rates ranging from 2.2 percentage points for blood pressure control in diabetes mellitus (P = .03) to 31.3 percentage points for use of disease-modifying antirheumatic drug therapy (P < .01). Adjusted performance differences between Hispanic MA enrollees in Puerto Rico and Hispanic MA enrollees in the United States exceeded 20 percentage points for 3 measures: use of disease-modifying antirheumatic drug therapy (-23.8 percentage points [95% CI, -30.9 to -16.8]), use of systemic corticosteroid in COPD exacerbation (-21.3 percentage points [95% CI, -27.5 to -15.1]), and use of bronchodilator therapy in COPD exacerbation (-22.7 percentage points [95% CI, -27.7 to -17.6]). We found modest differences in care between white and Hispanic MA enrollees in the United States but substantially worse care for enrollees in Puerto Rico compared with their US counterparts. Major efforts are needed to improve care delivery on the island to a level equivalent to the United States.

IMPORTANCE Geographic, racial, and ethnic variations in quality of care and outcomes have been well documented among the Medicare population. Few data exist on beneficiaries living in Puerto Rico, three-quarters of whom enroll in Medicare Advantage (MA). OBJECTIVE To determine the quality of care provided to white and Hispanic MA enrollees in the United States and Puerto Rico. DESIGN, SETTING, AND PARTICIPANTS A cross-sectional study of MA enrollees in 2011 was conducted, including white enrollees in the United States (n = 6 289 374), Hispanic enrollees in the United States (n = 795 039), and Hispanic enrollees in Puerto Rico (n = 267 016). The study was conducted from January 1, 2011, to December 31, 2011; data analysis took place from January 19, 2015, to January 2, 2016. MAIN OUTCOMES AND MEASURES Seventeen performance measures related to diabetes mellitus (including hemoglobin A1c control, retinal eye examination, low-density lipoprotein cholesterol control, nephropathy screening, and blood pressure control), cardiovascular disease (including low-density lipoprotein cholesterol control, blood pressure control, and use of a β-blocker after myocardial infarction), cancer screening (colorectal and breast), and appropriate medications (including systemic corticosteroids and bronchodilators for chronic obstructive pulmonary disease [COPD] and disease-modifying antirheumatic drugs). RESULTS Of the 7.35 million MA enrollees in the United States and Puerto Rico in our study, 1.06 million (14.4%) were Hispanic. Adjusted performance differences between Hispanic MA enrollees in Puerto Rico and Hispanic MA enrollees in the United States exceeded 20 percentage points for 3 measures: use of disease-modifying antirheumatic drug therapy (−23.8 percentage points [95% CI, −30.9 to −16.8]), use of systemic corticosteroid in COPD exacerbation (−21.3 percentage points [95% CI, −27.5 to −15.1]), and use of bronchodilator therapy in COPD exacerbation (−22.7 percentage points [95% CI, −27.7 to −17.6]). CONCLUSIONS AND RELEVANCE We found modest differences in care between white and Hispanic MA enrollees in the United States but substantially worse care for enrollees in Puerto Rico compared with their US counterparts. Major efforts are needed to improve care delivery on the island to a level equivalent to the United States. PMID:27111865

Nonpoint and Point Sources of Nitrogen in Major Watersheds of the United States
Puckett, Larry J.
1994-01-01
Estimates of nonpoint and point sources of nitrogen were made for 107 watersheds located in the U.S. Geological Survey's National Water-Quality Assessment Program study units throughout the conterminous United States. The proportions of nitrogen originating from fertilizer, manure, atmospheric deposition, sewage, and industrial sources were found to vary with climate, hydrologic conditions, land use, population, and physiography. Fertilizer sources of nitrogen are proportionally greater in agricultural areas of the West and the Midwest than in other parts of the Nation. Animal manure contributes large proportions of nitrogen in the South and parts of the Northeast. Atmospheric deposition of nitrogen is generally greatest in areas of greatest precipitation, such as the Northeast. Point sources (sewage and industrial) generally are predominant in watersheds near cities, where they may account for large proportions of the nitrogen in streams. The transport of nitrogen in streams increases as amounts of precipitation and runoff increase and is greatest in the Northeastern United States. Because no single nonpoint nitrogen source is dominant everywhere, approaches to control nitrogen must vary throughout the Nation. Watershed-based approaches to understanding nonpoint and point sources of contamination, as used by the National Water-Quality Assessment Program, will aid water-quality and environmental managers to devise methods to reduce nitrogen pollution.

To perform a bibliometric analysis of publications rates in orthopedics in the top 15 orthopaedic journals. Based on their 2015 impact factor, the fifteen highest ranked orthopaedic journals between January 2010 and December 2014 were used to establish the total number of publications; cumulative impact factor points (IF) per country were determined, and normalized to population size, GDP, and GDP/capita, comparison to the median country output and the global leader. Twenty-three thousand and twenty-one orthopaedic articles were published, with 66 countries publishing. The United States had 8149 publications, followed by the United Kingdom (1644) and Japan (1467). The highest IF was achieved by the United States (24744), United Kingdom (4776), and Japan (4053). Normalized by population size Switzerland lead. Normalized by GDP, Croatia was the top achiever. Adjusting GDP/capita, for publications and IF, China, India, and the United States were the leaders. Adjusting for population size and GDP, 28 countries achieved numbers of publications to be considered at least equivalent with the median academic output. Adjusting GDP/capita only China and India reached the number of publications to be considered equivalent to the current global leader, the United States. Five countries were responsible for 60% of the orthopaedic research output over this 5-year period. After correcting for GDP/capita, only 28 of 66 countries achieved a publication rate equivalent to the median country. The United States, United Kingdom, South Korea, Japan, and Germany were the top five countries for both publication totals and cumulative impact factor points.

This paper details development of a Model Predictive Control (MPC) algorithm for a boiler-turbine unit, which is a nonlinear multiple-input multiple-output process. The control objective is to follow set-point changes imposed on two state (output) variables and to satisfy constraints imposed on three inputs and one output. In order to obtain a computationally efficient control scheme, the state-space model is successively linearised on-line for the current operating point and used for prediction. In consequence, the future control policy is easily calculated from a quadratic optimisation problem. For state estimation the extended Kalman filter is used. It is demonstrated that the MPC strategy based on constant linear models does not work satisfactorily for the boiler-turbine unit whereas the discussed algorithm with on-line successive model linearisation gives practically the same trajectories as the truly nonlinear MPC controller with nonlinear optimisation repeated at each sampling instant. This is a supplement to the report of the International Board of Inquiry relative to the preservation and development of the Great Lakes fisheries as provided by an exchange of notes between Canada and the United States on February 29, 1940. The report aimed to present in a very brief manner the background of the establishment of the Board, its program, and the fishery problems that confronted it with particular reference to the international point of view. (See Contribution #100.) The Board, however, recognized the significance and import of those problems and data more or less peculiar or applicable largely to each country. It was, therefore, agreed that the representatives of each country should, if they deem it desirable, submit to their respective governments a supplementary report to cover the basis of action in greater detail than it was possible in a joint report. The United States members believed it advisable not only to publish detailed statistics of catch, but also other information deemed pertinent for a full understanding of the Great Lakes fishery problems from the United States point of view.

AIM To perform a bibliometric analysis of publications rates in orthopedics in the top 15 orthopaedic journals. METHODS Based on their 2015 impact factor, the fifteen highest ranked orthopaedic journals between January 2010 and December 2014 were used to establish the total number of publications; cumulative impact factor points (IF) per country were determined, and normalized to population size, GDP, and GDP/capita, comparison to the median country output and the global leader. RESULTS Twenty-three thousand and twenty-one orthopaedic articles were published, with 66 countries publishing. CONCLUSION Five countries were responsible for 60% of the orthopaedic research output over this 5-year period. After correcting for GDP/capita, only 28 of 66 countries achieved a publication rate equivalent to the median country. The United States, United Kingdom, South Korea, Japan, and Germany were the top five countries for both publication totals and cumulative impact factor points. PMID:28660144

Introduction Understanding the reasons for fluctuations in teenage driver crashes over time in the United States is clouded by the lack of information on licensure rates and driving exposure. Methods We examined results from the Monitoring the Future survey to estimate the proportion of high school seniors who possessed a driver’s license and the proportion of seniors who did not drive “during an average week” during the 15-year period of 1996–2010. Results During 1996–2010, the proportion of high school seniors in United States who reported having a driver’s license declined by 12 percentage points (14%) from 85% to 73%. Two-thirds of the decline (8 percentage points) occurred during 2006–2010. During the same 15-year period, the proportion of high school seniors who did not drive during an average week increased by 7 percentage points (47%) from 15% in 1996 to 22% in 2010, with essentially all of the increase occurring during 2006–2009. Discussion Findings in this report suggest that the economic recession in recent years has reduced rates of licensure and driving among high school seniors. PMID:23932698

Optical Interface States Protected by Synthetic Weyl Points

Weyl fermions have not been found in nature as elementary particles, but they emerge as nodal points in the band structure of electronic and classical wave crystals. Novel phenomena such as Fermi arcs and chiral anomaly have fueled the interest in these topological points which are frequently perceived as monopoles in momentum space. Here, we report the experimental observation of generalized optical Weyl points inside the parameter space of a photonic crystal with a specially designed four-layer unit cell. The reflection at the surface of a truncated photonic crystal exhibits phase vortexes due to the synthetic Weyl points, which in turn guarantees the existence of interface states between photonic crystals and any reflecting substrates. The reflection phase vortexes have been confirmed for the first time in our experiments, which serve as an experimental signature of the generalized Weyl points. The existence of these interface states is protected by the topological properties of the Weyl points, and the trajectories of these states in the parameter space resembles those of Weyl semimetal "Fermi arc surface states" in momentum space. Tracing the origin of interface states to the topological character of the parameter space paves the way for a rational design of strongly localized states with enhanced local field.

Uncertainty in gridded CO 2 emissions estimates

We are interested in the spatial distribution of fossil-fuel-related emissions of CO 2 for both geochemical and geopolitical reasons, but it is important to understand the uncertainty that exists in spatially explicit emissions estimates. Working from one of the widely used gridded data sets of CO 2 emissions, we examine the elements of uncertainty, focusing on gridded data for the United States at the scale of 1° latitude by 1° longitude. Uncertainty is introduced in the magnitude of total United States emissions, the magnitude and location of large point sources, the magnitude and distribution of non-point sources, and from themore » use of proxy data to characterize emissions. For the United States, we develop estimates of the contribution of each component of uncertainty. At 1° resolution, in most grid cells, the largest contribution to uncertainty comes from how well the distribution of the proxy (in this case population density) represents the distribution of emissions. In other grid cells, the magnitude and location of large point sources make the major contribution to uncertainty. Uncertainty in population density can be important where a large gradient in population density occurs near a grid cell boundary. Uncertainty is strongly scale-dependent with uncertainty increasing as grid size decreases. In conclusion, uncertainty for our data set with 1° grid cells for the United States is typically on the order of ±150%, but this is perhaps not excessive in a data set where emissions per grid cell vary over 8 orders of magnitude.« less

Utility of phosphodiesterase inhibitors (PDEi's) for the treatment of erectile dysfunction (ED) has been the focus of experimental and clinical studies. However, public preferences, attitudes, and experiences with PDEi's are rarely addressed from a population/epidemiology viewpoint. The Global Online Sexuality Survey (GOSS) is a worldwide epidemiologic study of sexuality and sexual disorders, first launched in the Middle East in 2010, followed by the United States in 2011. To describe the utilization rates, trends, and attitudes toward PDEi's in the United States in the year 2011. GOSS was randomly deployed to English-speaking male Web surfers in the United States via paid advertising on Facebook®, comprising 146 questions. Utilization rates and preferences for PDEi's by brand. Six hundred three subjects participated; mean age 53.43 years ± 13.9. Twenty-three point seven percent used PDEi's on more consistent basis, 37.5% of those with ED vs. 15.6% of those without ED (recreational users). Unrealistic safety concerns including habituation were pronounced. Seventy-nine point six percent of utilization was on prescription basis. PDEi's were purchased through pharmacies (5.3% without prescription) and in 16.5% over the Internet (68% without prescription). Nine point six percent nonprescription users suffered coronary heart disease. Prescription use was inclined toward sildenafil, generally, and particularly in severe cases, and shifted toward tadalafil in moderate ED and for recreational use, followed by vardenafil. Nonprescription utilization trends were similar, except in recreational use where sildenafil came first. In the United States unrealistic safety concerns over PDEi's utility exist and should be addressed. Preference for particular PDEi's over the others is primarily dictated by health-care providers, despite lack of guidelines that govern physician choice. Online and over-the-counter sales of PDEi's are common, and can expose a subset of users to health risks. Recreational use of PDEi's is common, and could be driven by undiagnosed premature ejaculation. © 2012 International Society for Sexual Medicine.

How can we best internationalize undergraduate psychology education in the United States and elsewhere? This question is more timely than ever, for at least 2 reasons: Within the United States, educators and students seek greater contact with psychology programs abroad, and outside the United States, psychology is growing apace, with educators and students in other nations often looking to U.S. curricula and practices as models. In this article, we outline international developments in undergraduate psychology education both in the United States and abroad, and analyze the dramatic rise of online courses and Internet-based technologies from an instructional and international point of view. Building on the recommendations of the 2005 APA Working Group on Internationalizing the Undergraduate Psychology Curriculum, we then advance 14 recommendations on internationalizing undergraduate psychology education--for students, faculty, and institutions. This study examined the impact of point-of-sale (POS) tobacco marketing restrictions in Australia and Canada, in relation to the United Kingdom and the United States where there were no such restrictions during the study period (2006–10). The data came from the International Tobacco Control Four Country Survey, a prospective multi-country cohort survey of adult smokers. In jurisdictions where POS display bans were implemented, smokers’ reported exposure to tobacco marketing declined markedly. From 2006 to 2010, in Canada, the percentages noticing POS tobacco displays declined from 74.1 to 6.1% [adjusted odds ratio (OR) = 0.26, P < 0.001]; and reported exposure to POS tobacco advertising decreased from 40.3 to 14.1% (adjusted OR = 0.61, P < 0.001). Similarly, in Australia, noticing of POS displays decreased from 73.9 to 42.9%. In contrast, exposure to POS marketing in the United States and United Kingdom remained high during this period. In parallel, there were declines in reported exposures to other forms of advertising/promotion in Canada and Australia, but again, not in the United States or United Kingdom. Impulse purchasing of cigarettes was lower in places that enacted POS display bans. These findings indicate that implementing POS tobacco display bans does result in lower exposure to tobacco marketing and less frequent impulse purchasing of cigarettes. PMID:23640986

Hand hygiene at the point of care is recognized as a best practice for promoting compliance at the moments when hand hygiene is most critical. The objective of this study was to compare knowledge, attitudes, and practices of US and Canadian frontline health care personnel regarding hand hygiene at the point of care. Physicians and nurses in US and Canadian hospitals were invited to complete a 32-question online survey based on evidence supporting point of care hand hygiene. Eligible health care personnel were in direct clinical practice at least 50% of the time. Three hundred fifty frontline caregivers completed the survey. Among respondents, 57.1% were from the United States and 42.9% were from Canada. Respondents were evenly distributed between physician and nurses. The US and Canadian respondents gave identical ranking to their perceived barriers to hand hygiene compliance. More than half of the respondents from both the United States and Canada agreed or strongly agreed that they would be more likely to clean their hands when recommended if alcohol-based handrub was closer to the patient. This survey demonstrates that similarities between Canada and the United States were more common than not, and the survey raises, or suggests, potential knowledge gaps that require further illumination. This brief is a compilation of data points and market insights that reflect the state of the project finance market for solar photovoltaic (PV) assets in the United States as of the third quarter of 2016. This information can generally be used as a simplified benchmark of the costs associated with securing financing for solar PV as well as the cost of the financing itself (i.e., the cost of capital). Three sources of capital are considered -- tax equity, sponsor equity, and debt -- across three segments of the PV marketplace.

Internationally, occupational therapy education has gone through several paradigm shifts during the last few decades, moving from certificate to diploma to bachelors to masters and now in some instances to clinical doctorate as the entry-level professional credential to practice. In the United States there is a recommendation under consideration by the American Occupational Therapy Association (AOTA) that by 2025, all occupational therapy university programs will move to the clinical doctorate level. It should be noted, however, that the AOTA Board can only make recommendations and it is the Accreditation Council for Occupational Therapy Education (ACOTE) who has regulatory authority to approve such a change. What are the potential implications for the profession, our clients, and funders of occupational therapy services? What are the primary drivers for the move towards the clinical doctorate being the educational entry point? Is the next step in the evolution of occupational therapy education globally a shift to the entry-level clinical doctorate? This article reviews current literature and discusses issues about the occupational therapy entry-level clinical doctorate. The published evidence available about the occupational therapy entry-level clinical doctorate is summarized and the perceived or frequently cited pros and cons of moving to the clinical doctorate as the singular entry point to occupational therapy practice are considered. The potential impacts of the introduction of the clinical doctorate as the entry-to-practice qualification across the United States on the occupational therapy community internationally will be briefly discussed. If the United States moves toward the entry-level clinical doctorate as the only educational starting point for the profession, will other jurisdictions follow suit? Further discourse and investigation of this issue both inside and outside of the United States is needed so that informed decisions can be made.

Mapping extent and change in surface mines within the United States for 2001 to 2006
Soulard, Christopher E.; Acevedo, William; Stehman, Stephen V.; Parker, Owen P.
2016-01-01
A complete, spatially explicit dataset illustrating the 21st century mining footprint for the conterminous United States does not exist. To address this need, we developed a semi-automated procedure to map the country's mining footprint (30-m pixel) and establish a baseline to monitor changes in mine extent over time. The process uses mine seed points derived from the U.S. Energy Information Administration (EIA), U.S. Geological Survey (USGS) Mineral Resources Data System (MRDS), and USGS National Land Cover Dataset (NLCD) and recodes patches of barren land that meet a “distance to seed” requirement and a patch area requirement before mapping a pixel as mining. Seed points derived from EIA coal points, an edited MRDS point file, and 1992 NLCD mine points were used in three separate efforts using different distance and patch area parameters for each. The three products were then merged to create a 2001 map of moderate-to-large mines in the United States, which was subsequently manually edited to reduce omission and commission errors. This process was replicated using NLCD 2006 barren pixels as a base layer to create a 2006 mine map and a 2001–2006 mine change map focusing on areas with surface mine expansion. In 2001, 8,324 km2 of surface mines were mapped. The footprint increased to 9,181 km2 in 2006, representing a 10·3% increase over 5 years. These methods exhibit merit as a timely approach to generate wall-to-wall, spatially explicit maps representing the recent extent of a wide range of surface mining activities across the country.

The second United States Microgravity Payload (USMP-2), flown in March 1994, carried four major microgravity experiments plus a sophisticated accelerometer system. The USMP program is designed to accommodate experiments requiring extensive resources short of a full Spacelab mission. The four USMP-2 experiments dealt with understanding fundamental aspects of materials behavior, three with the formation of crystals from melts and one with the critical point of a noble gas. This successful, scientifically rich mission also demonstrated telescience operations.

The photoinduced intramolecular excited-state energy-transfer (EET) process in conjugated polymers has received a great deal of research interest because of its important role in the light harvesting and energy transport of organic photovoltaic materials in photoelectric devices. In this work, the silylene-bridged biphenyl and stilbene (SBS) system was chosen as a simplified model system to obtain physical insight into the photoinduced intramolecular energy transfer between the different building units of the SBS copolymer. In the SBS system, the vinylbiphenyl and vinylstilbene moieties serve as the donor (D) unit and the acceptor (A) unit, respectively. The ultrafast excited-state dynamics of the SBS system was investigated from the point of view of nonadiabatic dynamics with the surface-hopping method at the TDDFT level. The first two excited states (S1 and S2) are characterized by local excitations at the acceptor (vinylstilbene) and donor (vinylbiphenyl) units, respectively. Ultrafast S2-S1 decay is responsible for the intramolecular D-A excitonic energy transfer. The geometric distortion of the D moiety play an essential role in this EET process, whereas the A moiety remains unchanged during the nonadiabatic dynamics simulation. The present work provides a direct dynamical approach to understand the ultrafast intramolecular energy-transfer dynamics in SBS copolymers and other similar organic photovoltaic copolymers.

1993-01-01
The first phase of the Modern Technology Rotor Program, the Modern Rotor Aerodynamic Limits Survey, was a flight test conducted by the United States Army Aviation Engineering Flight Activity for NASA Ames Research Center. The test was performed using a United States Army UH-60A Black Hawk aircraft and the United States Air Force HH-60A Night Hawk instrumented main-rotor blade. The primary purpose of this test was to gather high-speed, steady-state, and maneuvering data suitable for correlation purposes with analytical prediction tools. All aspects of the data base, flight-test instrumentation, and test procedures are presented and analyzed. Because of the high volume of data, only select data points are presented. However, access to the entire data set is available upon request.

BACKGROUND The growing prevalence of migrant children in diverse contexts requires a reconsideration of the intergenerational consequences of migration. To understand how migration and duration of residence are associated with children’s schooling, we need more comparative work that can point to the similarities and differences in outcomes for children across contexts. OBJECTIVE This paper addresses the importance of nativity and duration of residence for children’s school enrollment on both sides of a binational migration system: The United States and Mexico. The analyses are designed to determine whether duration of residence has a similar association with school enrollment across these different settings. METHODS The analyses are based on nationally representative household data from the 2010 Mexican Census and the 2006–2010 American Community Survey. Logistic regression models compare school enrollment patterns of Mexican and U.S.-born children of Mexican origin in the United States and those of Mexican and U.S.-born children in Mexico. Interactions for nativity/duration of residence and age are also included. RESULTS The results demonstrate that, adjusting for household resources and household-level migration experience, Mexican-born children in the United States and U.S.-born children in Mexico, particularly those who arrived recently, lag behind in school enrollment. These differences are most pronounced at older ages. CONCLUSIONS The comparisons across migration contexts point to greater school attrition and non-enrollment among older, recent migrant youth, regardless of the context. The interactions suggest that recent migration is associated with lower schooling for youth who engage in migration at older ages in both the United States and Mexico. PMID:28077926

We determine in a closed form all scalar one-point functions of the defect CFT dual to the D3-D5 probe brane system with k units of flux which amounts to calculating the overlap between a Bethe eigenstate of the integrable SO(6) spin chain and a certain matrix product state of bond dimension k. In particular, we show that the matrix product state is annihilated by all the parity odd charges of the spin chain which has recently been suggested as the criterion for such a state to correspond to an integrable initial state. Finally, we discuss the properties of the analogous matrix product state for the SO(5) symmetric D3-D7 probe brane set-up.

2009-01-01
Ballet is one of the most popular youth activities in the United States. Many ballet students eventually train to dance "en pointe," the French words for "on pointe," or "on the tips of their toes." No research exists to define criteria for determining when a young dancer can transition from dancing in ballet slippers to dancing in pointe shoes. However, dancers can be evaluated for this progression based on a number of factors, including adequate foot and ankle plantarflexion, technique, training, proprioception, alignment, and strength.

State Gun Law Environment and Youth Gun Carrying in the United States.

2015-11-01
Gun violence and injuries pose a substantial threat to children and youth in the United States. Existing evidence points to the need for interventions and policies for keeping guns out of the hands of children and youth. (1) To examine the association between state gun law environment and youth gun carrying in the United States, and (2) to determine whether adult gun ownership mediates this association. This was a repeated cross-sectional observational study design with 3 years of data on youth gun carrying from US states. The Youth Risk Behavior Survey comprises data of representative samples of students in grades 9 to 12 from biennial years of 2007, 2009, and 2011. We hypothesized that states with more restrictive gun laws have lower rates of youth gun carrying, and this association is mediated by adult gun ownership. State gun law environment as measured by state gun law score. Youth gun carrying was defined as having carried a gun on at least 1 day during the 30 days before the survey. In the fully adjusted model, a 10-point increase in the state gun law score, which represented a more restrictive gun law environment, was associated with a 9% decrease in the odds of youth gun carrying (adjusted odds ratio [AOR], 0.91 [95% CI, 0.86-0.96]). Adult gun ownership mediated the association between state gun law score and youth gun carrying (AOR, 0.94 [ 95% CI, 0.86-1.01], with 29% attenuation of the regression coefficient from -0.09 to -0.07 based on bootstrap resampling). More restrictive overall gun control policies are associated with a reduced likelihood of youth gun carrying. These findings are relevant to gun policy debates about the critical importance of strengthening overall gun law environment to prevent youth gun carrying.

While most ecological studies have shown that higher levels of point-of-sale (POS) cigarette marketing are associated with larger proportions of residents from lower socioeconomic and minority backgrounds in neighborhoods, there are no studies that examine individual-level social disparities in exposure to POS cigarette marketing among smokers in the United States. Our aim was to examine these disparities in a Midwestern metropolitan area in the United States. We conducted a telephone survey to collect data on 999 smokers. Cigarette marketing was measured by asking respondents three questions about noticing advertisements, promotions, and displays of cigarettes within their respective neighborhoods. The questions were combined to create a summated scale. We estimated ordered logistic regression models to examine the association of sociodemographic variables with exposure to POS cigarette marketing. Adjusted results showed that having a lower income ( p < 0.003) and belonging to a race/ethnicity other than "non-Hispanic White" ( p = 0.011) were associated with higher levels of exposure to POS cigarette marketing. The results highlight social disparities in exposure to POS cigarette marketing in the United States, which can potentially be eliminated by banning all forms of cigarette marketing.

Remote sensing datasets are increasingly being used to provide spatially explicit large scale evapotranspiration (ET) estimates. Extensive evaluation of such large scale estimates is necessary before they can be used in various applications. In this study, two monthly MODIS 1 km ET products, MODIS global ET (MOD16) and Operational Simplified Surface Energy Balance (SSEBop) ET, are validated over the conterminous United States at both point and basin scales. Point scale validation was performed using eddy covariance FLUXNET ET (FLET) data (2001–2007) aggregated by year, land cover, elevation and climate zone. Basin scale validation was performed using annual gridded FLUXNET ET (GFET) and annual basin water balance ET (WBET) data aggregated by various hydrologic unit code (HUC) levels. Point scale validation using monthly data aggregated by years revealed that the MOD16 ET and SSEBop ET products showed overall comparable annual accuracies. For most land cover types, both ET products showed comparable results. However, SSEBop showed higher performance for Grassland and Forest classes; MOD16 showed improved performance in the Woody Savanna class. Accuracy of both the ET products was also found to be comparable over different climate zones. However, SSEBop data showed higher skill score across the climate zones covering the western United States. Validation results at different HUC levels over 2000–2011 using GFET as a reference indicate higher accuracies for MOD16 ET data. MOD16, SSEBop and GFET data were validated against WBET (2000–2009), and results indicate that both MOD16 and SSEBop ET matched the accuracies of the global GFET dataset at different HUC levels. Our results indicate that both MODIS ET products effectively reproduced basin scale ET response (up to 25% uncertainty) compared to CONUS-wide point-based ET response (up to 50–60% uncertainty) illustrating the reliability of MODIS ET products for basin-scale ET estimation. Results from this research would guide the additional parameter refinement required for the MOD16 and SSEBop algorithms in order to further improve their accuracy and performance for agro-hydrologic applications.
Mineral commodity profiles: Cadmium
Butterman, W.C.; Plachy, Jozef
2004-01-01
Overview -- Cadmium is a soft, low-melting-point metal that has many uses. It is similar in abundance to antimony and bismuth and is the 63d element in order of crustal abundance. Cadmium is associated in nature with zinc (and, less closely, with lead and copper) and is extracted mainly as a byproduct of the mining and processing of zinc. In 2000, it was refined in 27 countries, of which the 8 largest accounted for two-thirds of world production. The United States was the third largest refiner after Japan and China. World production in 2000 was 19,700 metric tons (t) and U.S. production was 1,890 t. In the United States, one company in Illinois and another in Tennessee refined primary cadmium. A Pennsylvania company recovered cadmium from scrap, mainly spent nickel-cadmium (NiCd) batteries. The supply of cadmium in the world and in the United States appears to be adequate to meet future industrial needs; the United States has about 23 percent of the world reserve base.

Thirty year normal (1981-2010) monthly latent heat fluxes (ET) over the conterminous United States were estimated by a modified Advection-Aridity model from North American Regional Reanalysis (NARR) radiation and wind as well as Parameter-Elevation Regressions on Independent Slopes Model (PRISM) air and dew-point temperature data. Mean annual ET values were calibrated with PRISM precipitation (P) and validated against United States Geological Survey runoff (Q) data. At the six-digit Hydrologic Unit Code level (sample size of 334) the estimated 30 year normal runoff (P - ET) had a bias of 18 mm yr-1, a root-mean-square error of 96 mm yr-1, and a linear correlation coefficient value of 0.95, making the estimates on par with the latest Land Surface Model results but without the need for soil and vegetation information or any soil moisture budgeting.
Tactics and Economics of Wildlife Oral Rabies Vaccination, Canada and the United States
Meltzer, Martin I.; Shwiff, Stephanie A.; Slate, Dennis
2009-01-01
Progressive elimination of rabies in wildlife has been a general strategy in Canada and the United States; common campaign tactics are trap–vaccinate–release (TVR), point infection control (PIC), and oral rabies vaccination (ORV). TVR and PIC are labor intensive and the most expensive tactics per unit area (≈$616/km2 [in 2008 Can$, converted from the reported $450/km2 in 1991 Can$] and ≈$612/km2 [$500/km2 in 1999 Can$], respectively), but these tactics have proven crucial to elimination of raccoon rabies in Canada and to maintenance of ORV zones for preventing the spread of raccoon rabies in the United States. Economic assessments have shown that during rabies epizootics, costs of human postexposure prophylaxis, pet vaccination, public health, and animal control spike. Modeling studies, involving diverse assumptions, have shown that ORV programs can be cost-efficient and yield benefit:cost ratios >1.0. PMID:19757549

A common approach for high accuracy sensor fusion based on 9D inertial measurement unit data is Kalman filtering. State of the art floating-point filter algorithms differ in their computational complexity nevertheless, real-time operation on a low-power microcontroller at high sampling rates is not possible. This work presents algorithmic modifications to reduce the computational demands of a two-step minimum order Kalman filter. Furthermore, the required bit-width of a fixed-point filter version is explored. For evaluation real-world data captured using an Xsens MTx inertial sensor is used. Changes in computational latency and orientation estimation accuracy due to the proposed algorithmic modifications and fixed-point number representation are evaluated in detail on a variety of processing platforms enabling on-board processing on wearable sensor platforms.

Background Geostatistical techniques that account for spatially varying population sizes and spatial patterns in the filtering of choropleth maps of cancer mortality were recently developed. Their implementation was facilitated by the initial assumption that all geographical units are the same size and shape, which allowed the use of geographic centroids in semivariogram estimation and kriging. Another implicit assumption was that the population at risk is uniformly distributed within each unit. This paper presents a generalization of Poisson kriging whereby the size and shape of administrative units, as well as the population density, is incorporated into the filtering of noisy mortality rates and the creation of isopleth risk maps. An innovative procedure to infer the point-support semivariogram of the risk from aggregated rates (i.e. areal data) is also proposed. Results The novel methodology is applied to age-adjusted lung and cervix cancer mortality rates recorded for white females in two contrasted county geographies: 1) state of Indiana that consists of 92 counties of fairly similar size and shape, and 2) four states in the Western US (Arizona, California, Nevada and Utah) forming a set of 118 counties that are vastly different geographical units. Area-to-point (ATP) Poisson kriging produces risk surfaces that are less smooth than the maps created by a naïve point kriging of empirical Bayesian smoothed rates. The coherence constraint of ATP kriging also ensures that the population-weighted average of risk estimates within each geographical unit equals the areal data for this unit. Simulation studies showed that the new approach yields more accurate predictions and confidence intervals than point kriging of areal data where all counties are simply collapsed into their respective polygon centroids. Its benefit over point kriging increases as the county geography becomes more heterogeneous. Conclusion A major limitation of choropleth maps is the common biased visual perception that larger rural and sparsely populated areas are of greater importance. The approach presented in this paper allows the continuous mapping of mortality risk, while accounting locally for population density and areal data through the coherence constraint. This form of Poisson kriging will facilitate the analysis of relationships between health data and putative covariates that are typically measured over different spatial supports. PMID:17137504

The primary comparator was with "definite positive" and "definite negative" patients defined as a concordance of combined results of the UBT and the HM-CAP IgG EIA. Overall, 188 eligible patients were enrolled (61 men, age range: 18-73 years, including 84 Hispanics, 73 Asian-Pacific Americans, 22 Black African-Americans, 6 non-Hispanic Caucasians, and 3 of "other" ethnicity). Compared with "definite positive" and "definite negative" results, the sensitivity and specificity of the urine antibody test were 0.9 and 1.0, respectively. The urine antibody test proved suitable for point-of-care rapid diagnosis of anti-H. pylori antibodies indicative of active or past H. pylori infection.

We calculate planar tree level one-point functions of non-protected operators in the defect conformal field theory dual to the D3-D5 brane system with k units of the world volume flux. Working in the operator basis of Bethe eigenstates of the Heisenberg XXX 1/2 spin chain we express the one-point functions as overlaps of these eigenstates with a matrix product state. For k = 2 we obtain a closed expression of determinant form for any number of excitations, and in the case of half-filling we find a relation with the Néel state. In addition, we present a number of results for the limiting case k → ∞.

Temporal trends in United States dew point temperatures
NASA Astrophysics Data System (ADS)
Robinson, Peter J.
2000-07-01
In this study, hourly data for the 1951-1990 period for 178 stations in the coterminous United States were used to establish temporal trends in dew point temperature. Although the data had been quality controlled previously (Robinson, 1998. Monthly variations of dew point temperatures in the coterminous United States. International Journal of Climatology 18: 1539-1556), comparisons of values between nearby stations suggested that instrumental changes, combined with locational changes, may have modified the results by as much as 1°C during the 40-year period. Nevertheless, seasonally averaged results indicated an increase over much of the area, of slightly over 1°C/100 years in spring and autumn, slightly less than this in summer. Winter displayed a drying of over 1°C/100 years. When only the 1961-1990 period was considered, the patterns were similar and trends increased by approximately 1-2°C/100 years, except in autumn, which displayed a slight drying. Analyses for specific stations indicated periods of both increasing and decreasing Td, the change between them varying with observation hour. No single change point was common over a wide area, although January commonly indicated maximum values early in the period in the east and west, and much later in the north-central portion. Rates of increase were generally higher in daytime than at night, especially in summer. Investigation of the inter-decadal differences in dew point, as a function of wind conditions, indicated that changes during calm conditions were commonly similar in magnitude to that of the overall average changes, suggesting an important role for the local hydrologic cycle in driving changes. Other inter-decadal changes could be attributed to the changes in the frequency and moisture content of invading air-streams. This was particularly clear for the changes in north-south flow in the interior.

The study aimed to describe the age distribution of breast cancer diagnosis among Chinese females for comparison with the United States and the European Union, and provide evidence for the screening target population in China. Median age was estimated from hospital databases from 7 tertiary hospitals in China. Population-based data in China, United States and European Union was extracted from the National Central Cancer Registry, SEER program and GLOBOCAN 2008, respectively. Age-standardized distribution of breast cancer at diagnosis in the 3 areas was estimated based on the World Standard Population 2000. The median age of breast cancer at diagnosis was around 50 in China, nearly 10 years earlier than United States and European Union. The diagnosis age in China did not vary between subgroups of calendar year, region and pathological characteristics. With adjustment for population structure, median age of breast cancer at diagnosis was 50~54 in China, but 55~59 in United States and European Union. The median diagnosis age of female breast cancer is much earlier in China than in the United States and the European Union pointing to racial differences in genetics and lifestyle. Screening programs should start at an earlier age for Chinese women and age disparities between Chinese and Western women warrant further studies.

In the year 1900, Galveston, Texas, was a bustling community of approximately 40,000 people. The former capital of the Republic of Texas remained a trade center for the state and was one of the largest cotton ports in the United States. On September 8 of that year, however, a powerful hurricane struck Galveston island, tearing the Weather Bureau wind gauge away as the winds exceeded 100 mph and bringing a storm surge that flooded the entire city. The worst natural disaster in United States history even today the hurricane caused the deaths of between 6000 and 8000 people. Critical in the events that led to such a terrible loss of life was the lack of precise knowledge of the strength of the storm before it hit. In 2008, Hurricane Ike, the third costliest hurricane ever to hit the United States coast, traveled through the Gulf of Mexico. Ike was gigantic, and the devastation in its path included the Turk and Caicos Islands, Haiti, and huge swaths of the coast of the Gulf of Mexico. Once again, Galveston, now a city of nearly 60,000, took the direct hit as Ike came ashore. Almost 200 people in the Caribbean and the United States lost their lives; a tragedy to be sure, but far less deadly than the 1900 storm. This time, people were prepared, having received excellent warning from the GOES satellite network. The Geostationary Operational Environmental Satellites have been a continuous monitor of the world's weather since 1975, and they have since been joined by other Earth-observing satellites. This weather surveillance to which so many now owe their lives is possible in part because of the ability to point accurately and steadily at the Earth below. The importance of accurately pointing spacecraft to our daily lives is pervasive, yet somehow escapes the notice of most people. But the example of the lives saved from Hurricane Ike as compared to the 1900 storm is something no one should ignore. In this section, we will summarize the processes and technologies used in designing and operating spacecraft pointing (i.e. attitude) systems.
Use of physical restraints in adult critical care: a bicultural study.
Martin, Beth; Mathisen, Lars
2005-03-01
Although controversial, physical restraints are commonly used in adult critical care units in the United States to prevent treatment interference and self-inflicted harm. Use of physical restraints in Norwegian hospitals is very limited. In the United States, an experimental design for research on use of restraints has not seemed feasible. However, international research provides an opportunity to compare and contrast practices. To describe the relationship between patients' characteristics, environment, and use of physical restraints in the United States and Norway. Observations of patients and chart data were collected from 2 intensive care units (n = 50 patients) in Norway and 3 (n = 50 patients) in the United States. Sedation was measured by using the Sedation-Agitation Scale. The Nine Equivalents of Nursing Manpower Use Score was used to indicate patients' acuity level. Restraints were in use in 39 of 100 observations in the United States and not at all in Norway (P = .001). Categories of patients were balanced. In the Norwegian sample, the median Nine Equivalents of Nursing Manpower Use Score was higher (37 vs 27 points, P < .001), patients were more sedated (P < .001), and nurse-to-patient ratios were higher (1.05:1 vs 0.65:1, P < .001). Seven incidents of unplanned device removal were reported in the US sample. Critical care units with similar technology and characteristics of patients vary between nations in restraint practices, levels of sedation, and nurse-to-patient ratios. Restraint-free care was, in this sample, safe in terms of treatment interference.

The chemical complexity of atmospheric organic aerosol (OA) has caused substantial uncertainties in understanding its origins and environmental impacts. Here, we provide constraints on OA origins through compositional characterization with molecular-level details. Our results suggest that secondary OA (SOA) from monoterpene oxidation accounts for approximately half of summertime fine OA in Centreville, AL, a forested area in the southeastern United States influenced by anthropogenic pollution. We find that different chemical processes involving nitrogen oxides, during days and nights, play a central role in determining the mass of monoterpene SOA produced. This study presents data on the factor structure of the Wechsler Preschool and Primary Scale of Intelligence (WPPSI) and sex and cultural differences in WPPSI test scores among 5- and 6-year-olds from China, Japan, and the United States. Results show the presence of a verbal and nonverbal factor structure across all three countries. Sex differences on the 10 subtests were generally consistent, with a male advantage on a subtest of spatial abilities (Mazes). Males in the Chinese sample obtained significantly higher Full Scale IQ scores than females and had lower variability in their test scores. These observations were not present in the Japan and United States samples. Mean Full Scale IQ score in the Chinese sample was 104.1, representing a 4-point increase from 1988 to 2004.

This article points to a sharp decline in children's sibling numbers (sibsize) that occurred in the United States since the 1970s and was large enough among children with lower socioeconomic status (SES) (particularly black children) to amount to a revolution in their family circumstances. It interprets sibsize decline as a source of social convergence in children's family contexts that ran counter to trends toward social divergence caused by the rise of lone parenthood. The article is based on new estimates of differences in children's sibsize and lone parenthood by race and maternal education generated from public-use samples from the Census of Population and Current Population Survey (CPS), focusing especially on the period 1940-2012. I discuss some methodological and substantive challenges for existing scholarship arising from the findings and point to questions for future research.
Point-of-purchase tobacco environments and variation by store type--United States, 1999.
2002-03-08
To promote its products, the tobacco industry spent $8.2 billion on marketing in 1999, an increase of $1.5 billion over the previous year. Tobacco advertising in various media increases tobacco consumption and adolescents are more susceptible than adults to being influenced by some forms of tobacco advertising. To describe the retail tobacco advertising and marketing environment, researchers from the Robert Wood Johnson Foundation-sponsored ImpacTeen Project collected and analyzed store observation data in 163 communities throughout the United States. This report summarizes the extent of point-of-purchase (POP) tobacco advertising and marketing found in various types of stores. The findings in this report indicate that certain retail environments frequented by teenagers heavily promote tobacco use. To reduce demand for tobacco products among adolescents, public health efforts should address POP environment exposure to tobacco advertising and marketing.

Successful decision making requires an ability to monitor contexts, actions, and outcomes. The anterior cingulate cortex (ACC) is thought to be critical for these functions, monitoring and guiding decisions especially in challenging situations involving conflict and errors. A number of different single-unit correlates have been observed in the ACC that reflect the diverse cognitive components involved. Yet how ACC neurons function as an integrated network is poorly understood. Here we show, using advanced population analysis of multiple single-unit recordings from the rat ACC during performance of an ecologically valid decision-making task, that ensembles of neurons move through different coherent and dissociable states as the cognitive requirements of the task change. This organization into distinct network patterns with respect to both firing-rate changes and correlations among units broke down during trials with numerous behavioral errors, especially at choice points of the task. These results point to an underlying functional organization into cell assemblies in the ACC that may monitor choices, outcomes, and task contexts, thus tracking the animal's progression through “task space.” PMID:18708525

Quaternary geologic map of the Wolf Point 1° × 2° quadrangle, Montana and North Dakota
Fullerton, David S.; Colton, Roger B.; Bush, Charles A.
2016-09-08
The Wolf Point quadrangle encompasses approximately 16,084 km2 (6,210 mi2). The northern boundary is the Montana/Saskatchewan (U.S.-Canada) boundary. The quadrangle is in the Northern Plains physiographic province and it includes the Peerless Plateau and Flaxville Plain. The primary river is the Missouri River. The map units are surficial deposits and materials, not landforms. Deposits that comprise some constructional landforms (for example, ground-moraine deposits, end-moraine deposits, and stagnation-moraine deposits, all composed of till) are distinguished for purposes of reconstruction of glacial history. Surficial deposits and materials are assigned to 23 map units on the basis of genesis, age, lithology or composition, texture or particle size, and other physical, chemical, and engineering characteristics. It is not a map of soils that are recognized in pedology or agronomy. Rather, it is a generalized map of soils recognized in engineering geology, or of substrata or parent materials in which pedologic or agronomic soils are formed. Glaciotectonic (ice-thrust) structures and deposits are mapped separately, represented by a symbol. The surficial deposits are glacial, ice-contact, glaciofluvial, alluvial, lacustrine, eolian, colluvial, and mass-movement deposits.Till of late Wisconsin age is represented by three map units. Till of Illinoian age also is mapped. Till deposited during pre-Illinoian glaciations is not mapped, but is widespread in the subsurface. Linear ice-molded landforms (primarily drumlins), shown by symbol, indicate directions of ice flow during late Wisconsin and Illinoian glaciations. The Quaternary geologic map of the Wolf Point quadrangle, northeastern Montana and North Dakota, was prepared to provide a database for compilation of a Quaternary geologic map of the Regina 4° × 6° quadrangle, United States and Canada, at scale 1:1,000,000, for the U.S. Geological Survey Quaternary Geologic Atlas of the United States map series. This map was compiled from data from many sources, at several different map scales. That information was generalized and simplified, and then transferred to a base map at 1:250,000 scale to serve as the base for final reduction to 1:1,000,000, the nominal reading scale of maps in the Quaternary Geologic Atlas of the United States map series. This map is the generalized and simplified 1:250,000 scale compilation. Letter symbols for the map units are those used for the same units in the Quaternary Geologic Atlas of the United States map series. The map summarizes new, and selected published and unpublished, geologic information for public use and for use by Federal, State, and local governmental agencies for land use planning, including assessment of natural resources, natural hazards, recreation potential, and land use management. It also is a base from which a variety of maps relating to earth surface processes and Quaternary geologic history can be derived.
Point-of-purchase alcohol marketing and promotion by store type--United States, 2000-2001.
2003-04-11
Alcohol consumption is the third leading preventable cause of death in the United States, accounting for approximately 100,000 deaths annually. Efforts to reduce the adverse health and social consequences from alcohol use include policies to restrict access to alcohol among underaged persons (i.e., persons aged <21 years) and to reduce alcohol-impaired driving among persons of all ages. Recent studies have focused on alcohol marketing as a potentially important contributor to alcohol consumption, particularly among underage drinkers. Point-of-purchase (POP) (i.e., on-site) marketing, including alcohol advertising and placement, can increase alcohol sales and consumption substantially, thereby increasing the risk for various alcohol-related health outcomes, including alcohol-impaired driving and interpersonal violence. To assess the type and frequency of POP alcohol marketing, researchers with the ImpacTeen Project collected and analyzed store observation data during 2000-2001 from 3,961 alcohol retailers in 329 communities throughout the United States. This report summarizes the results of the study, which indicate that POP alcohol marketing is extensive in certain store types frequented by teenagers and young adults. Public health agencies and policy makers should work with liquor control boards to reduce POP marketing that could promote risky or underage drinking.

Regions rely at least partially on the internal production of a qualified workforce in order to maintain their economic competitiveness. Increasingly, at least from a university or corporate point of view, a qualified workforce is viewed as one that is racially and ethnically diverse. However, the conceptualization and measurement of ethnic and racial diversity in higher education appears to be often based on normative values rather than solid benchmarks, making any regional comparisons or goals difficult to specify. Ideally, at least as a starting point, public state universities would, while attempting to increase overall student diversity, benchmark their progress against the state population composition. This paper combines enrollment data from the National Center for Education Statistics (NCES) with U.S. Census Bureau population estimates data to provide a point of comparison for state universities. The paper has two goals: first a university-level comparison of diversity scores, as measured by the interaction index and, second, an analysis of how university student population composition compares to that of the population the university was originally intended to serve – the state population. The paper has two goals: first a university-level comparison of diversity scores, as measured by the interaction index and, second, an analysis of how university student population composition compares to that of the population the university was originally intended to serve - the state population.

This brief is a compilation of data points and market insights that reflect the state of the project finance market for solar photovoltaic (PV) assets in the United States as of the third quarter of 2017. This information can generally be used as a simplified benchmark of the costs associated with securing financing for solar PV as well as the cost of the financing itself (i.e., the cost of capital). This work represents the second DOE sponsored effort to benchmark financing costs across the residential, commercial, and utility-scale PV markets, as part of its larger effort to benchmark the componentsmore » of PV system costs.« less
United States biodefense, international law, and the problem of intent.
Enemark, Christian
2005-01-01
Since the anthrax attacks of 2001 in the United States, annual U.S. government spending on biodefense programs has increased enormously. U.S. biodefense was once exclusively the domain of military agencies and was aimed principally at protecting battlefield troops against the products of state-run biological warfare programs. Today, it is engaged in and promoted by a variety of government agencies contemplating "bioterrorism," and it is aimed principally at protecting the American civilian population. I ask if certain U.S. biodefense policies, pointedly those funding "threat assessment" projects, make biological attacks paradoxically more likely by undermining international and transnational norms against deliberately causing disease. I conclude that they do and consider the ramifications of this answer.

In 2007, Illinois became the first state in the United States to mandate active surveillance of methicillin-resistant Staphylococcus aureus (MRSA). The Illinois law applies to intensive care unit (ICU) patients; contact precautions are required for patients found to be MRSA colonized. However, the effectiveness of a legislated "search and isolate" approach to reduce MRSA burden among critically ill patients is uncertain. We evaluated whether the prevalence of MRSA colonization declined in the 5 years after the start of mandatory active surveillance. All hospitals with an ICU having ≥10 beds in Chicago, Illinois, were eligible to participate in single-day serial point prevalence surveys. We assessed MRSA colonization among adult ICU patients present at time of survey using nasal and inguinal swab cultures. The primary outcome was region-wide MRSA colonization prevalence over time. All 25 eligible hospitals (51 ICUs) participated in serial point prevalence surveys over 8 survey periods (2008-2013). A total of 3909 adult ICU patients participated in the point prevalence surveys, with 432 (11.1%) found to be colonized with MRSA (95% confidence interval [CI], 10.1%-12.0%). The MRSA colonization prevalence among patients was unchanged during the study period; year-over-year relative risk for MRSA colonization was 0.97 (95% CI, .89-1.05; P = .48). MRSA colonization prevalence among critically ill adult patients did not decline during the time period following legislatively mandated MRSA active surveillance. Our findings highlight the limits of legislated MRSA active surveillance as a strategy to reduce MRSA colonization burden among ICU patients.

Highlights: Black-Right-Pointing-Pointer This study evaluates methane and carbon dioxide production after land burial of cattle carcasses. Black-Right-Pointing-Pointer Disposal of animal mortalities is often overlooked in evaluating the environmental impacts of animal production. Black-Right-Pointing-Pointer we quantify annual emissions from cattle carcass disposal in the United States as 1.6 Tg CO{sub 2} equivalents. - Abstract: Approximately 2.2 million cattle carcasses require disposal annually in the United States. Land burial is a convenient disposal method that has been widely used in animal production for disposal of both daily mortalities as well as during catastrophic mortality events. To date, greenhouse gas production after mortalitymore » burial has not been quantified, and this study represents the first attempt to quantify greenhouse gas emissions from land burial of animal carcasses. In this study, anaerobic decomposition of both homogenized and unhomogenized cattle carcass material was investigated using bench-scale reactors. Maximum yields of methane and carbon dioxide were 0.33 and 0.09 m{sup 3}/kg dry material, respectively, a higher methane yield than that previously reported for municipal solid waste. Variability in methane production rates were observed over time and between reactors. Based on our laboratory data, annual methane emissions from burial of cattle mortalities in the United States could total 1.6 Tg CO{sub 2} equivalents. Although this represents less than 1% of total emissions produced by the agricultural sector in 2009, greenhouse gas emissions from animal carcass burial may be significant if disposal of swine and poultry carcasses is also considered.« less

The genus Hepatozoon includes hundreds of species that infect birds, reptiles, amphibians and mammals, in all continents with tropical and subtropical climates. Two species have been described in domestic dogs: H. canis, reported in Europe, Asia, Africa, South America and the United States; and H. americanum, which so far has only been diagnosed in the United States. In Brazil, the only species found infecting dogs is H. canis. The objective of this review was to detail some aspects of canine hepatozoonosis, caused by H. canis, and the main points of its biology, transmission, pathogenicity, symptoms, epidemiology and diagnostic methods, with emphasis on research developed in Brazil.
Drug advertising: the right or wrong prescription for our ailments?
Brodie, Mollyann; Levitt, Larry
2002-11-01
As the cost of health care--particularly prescription drugs--is rising at a rate that has not been seen in a decade, the United States finds itself once again searching for new solutions to what has, until now, been an intractable problem. Drug advertising, which has almost become part of the social fabric in the United States, is an appealing and visible target. Critics point to pharmaceutical companies boosting profits by pitching pills, whereas advocates talk of empowering patients with information. With a topic that is so dominated by money and politics, it is sometimes hard to tell who is correct.
PL-VIO: Tightly-Coupled Monocular Visual–Inertial Odometry Using Point and Line Features
Zhao, Ji; Guo, Yue; He, Wenhao; Yuan, Kui
2018-01-01
To address the problem of estimating camera trajectory and to build a structural three-dimensional (3D) map based on inertial measurements and visual observations, this paper proposes point–line visual–inertial odometry (PL-VIO), a tightly-coupled monocular visual–inertial odometry system exploiting both point and line features. Compared with point features, lines provide significantly more geometrical structure information on the environment. To obtain both computation simplicity and representational compactness of a 3D spatial line, Plücker coordinates and orthonormal representation for the line are employed. To tightly and efficiently fuse the information from inertial measurement units (IMUs) and visual sensors, we optimize the states by minimizing a cost function which combines the pre-integrated IMU error term together with the point and line re-projection error terms in a sliding window optimization framework. The experiments evaluated on public datasets demonstrate that the PL-VIO method that combines point and line features outperforms several state-of-the-art VIO systems which use point features only. Intercultural-global competencies for the 21st century and beyond.
Esterhuizen, Philip; Kirkpatrick, Mary K
2015-05-01
Increased diversity exists in Anglo-Saxon countries, such as Australia, the United Kingdom, and the United States. By 2050, no single ethnic group is expected to be in a majority in the United States. Health care reform points to an urgent need for health care professionals, such as nursing, medicine, allied health, nutrition, and other interdisciplinary health care team members, to serve a multi-ethnic population by developing intercultural-global and 21st-century competencies. Nurse educators must acknowledge the need to familiarize themselves and integrate these competencies into university and continuing education programs by evaluating and reporting outcomes. All nurses can be expected to have these competencies as global citizens through local, intercultural, and global interactions and exchanges. Copyright 2015, SLACK Incorporated.
