The Southeastern Fishes Council Technical Advisory Committee reviewed the diversity, distribution, and status of all native freshwater and diadromous fishes across 51 major drainage units of the southern United States. The southern United States supports more native fishes than any area of comparable size on the North American continent north of Mexico, but also has a high proportion of its fishes in need of conservation action. The review included 662 native freshwater and diadromous fishes and 24 marine fishes that are significant components of freshwater ecosystems. Of this total, 560 described, freshwater fish species are documented, and 49 undescribed species are included provisionally pending formal description. Described subspecies (86) are recognized within 43 species, 6 fishes have undescribed subspecies, and 9 others are recognized as complexes of undescribed taxa. Extinct, endangered, threatened, or vulnerable status is recognized for 28% (187 taxa) of southern freshwater and diadromous fishes. To date, 3 southern fishes are known to be extinct throughout their ranges, 2 are extirpated from the study region, and 2 others may be extinct. Of the extant southern fishes, 41 (6%) are regarded as endangered, 46 (7%) are regarded as threatened, and 101 (15%) are regarded as vulnerable. Five marine fishes that frequent fresh water are regarded as vulnerable. Our assessment represents a 75% increase in jeopardized southern fishes since 1989 and a 125% increase in 20 years. The trend for fishes in the southern United States is clear; jeopardized fishes are successively being moved from the vulnerable category to that of imminent threat of extinction.

In the United States, all previously reported cases of Rickettsia parkeri rickettsiosis have been linked to transmission by the Gulf Coast tick (Amblyomma maculatum). Here we describe 1 confirmed and 1 probable case of R. parkeri rickettsiosis acquired in a mountainous region of southern Arizona, well beyond the recognized geographic range of A. maculatum ticks. The likely vector for these 2 infections was identified as the Amblyomma triste tick, a Neotropical species only recently recognized in the United States. Identification of R. parkeri rickettsiosis in southern Arizona demonstrates a need for local ecologic and epidemiologic assessments to better understand geographic distribution and define public health risk. Education and outreach aimed at persons recreating or working in this region of southern Arizona would improve awareness and promote prevention of tickborne rickettsioses.

Recognizing dangerous snakes in the United States and Canada: a novel 3-step identification method.

The rapid and accurate recognition of dangerously venomous snakes following bites is crucial to making appropriate decisions regarding first aid, evacuation, and treatment. Past recommendations for identification of dangerous North American pit vipers have often required subjective determinations of head shape or relied on traits shared with some nondangerous species (elliptical pupils and undivided subcaudal scales). Heat-sensitive facial pits are diagnostic but require close examination of the dangerous head, and cephalic traits are useless when working with a decapitated carcass. Exclusive of cephalic traits, pit vipers north of Mexico can be recognized by the combination of keeled middorsal scales and undivided subcaudal scales. The order of colored rings is usually suggested to identify coral snakes in the United States, yet extension of the colored rings across the ventral scales must be added as an essential identifying factor to ensure elimination of all harmless look-alikes. A novel 3-step flow chart is presented that allows dangerous snakes in the United States and Canada to be recognized quickly and dependably without relying on cephalic traits. This process cannot be used in other countries, however, due to greater variability of these characteristics in snakes from other parts of the world. Finally, close examination of potentially venomous snakes is extraordinarily dangerous and steps to safeguard those making such observations are discussed. This paper reviews the sentences dictated between 1993 and 2002 by the Supreme Courts of Canada and the Unites States, the House of Lords and Supreme Court of the United Kingdom and the European Human Rights Court, about the validity of the legal prohibition of assistance for suicide. These sentences constituted a judicial consensus about the right to die. This consensus recognized the legal right of patients to reject medical treatments but did not recognize the right to be assisted by a physician to commit suicide. This exclusion is changing in the recent case law of Canada and the United Kingdom, which accepts the fundamental right of terminal patients to medically assisted suicide.

Safe injection facilities (SIFs) have shown promise in reducing harms and social costs associated with injection drug use. Favorable evaluations elsewhere have raised the issue of their implementation in the United States. Recognizing that laws shape health interventions targeting drug users, we analyzed the legal environment for publicly authorized SIFs in the United States. Although states and some municipalities have the power to authorize SIFs under state law, federal authorities could still interfere with these facilities under the Controlled Substances Act. A state- or locally-authorized SIF could proceed free of legal uncertainty only if federal authorities explicitly authorized it or decided not to interfere. Given legal uncertainty, and the similar experience with syringe exchange programs, we recommend a process of sustained health research, strategic advocacy, and political deliberation.

Tickborne rickettsial diseases continue to cause severe illness and death in otherwise healthy adults and children, despite the availability of low-cost, effective antibacterial therapy. Recognition early in the clinical course is critical because this is the period when antibacterial therapy is most effective. Early signs and symptoms of these illnesses are nonspecific or mimic other illnesses, which can make diagnosis challenging. Previously undescribed tickborne rickettsial diseases continue to be recognized, and since 2004, three additional agents have been described as causes of human disease in the United States: Rickettsia parkeri, Ehrlichia muris-like agent, and Rickettsia species 364D. This report updates the 2006 CDC recommendations on the diagnosis and management of tickborne rickettsial diseases in the United States and includes information on the practical aspects of epidemiology, clinical assessment, treatment, laboratory diagnosis, and prevention of tickborne rickettsial diseases. The CDC Rickettsial Zoonoses Branch, in consultation with external clinical and academic specialists and public health professionals, developed this report to assist health care providers and public health professionals to 1) recognize key epidemiologic features and clinical manifestations of tickborne rickettsial diseases, 2) recognize that doxycycline is the treatment of choice for suspected tickborne rickettsial diseases in adults and children, 3) understand that early empiric antibacterial therapy can prevent severe disease and death, 4) request the appropriate confirmatory diagnostic tests and understand their usefulness and limitations, and 5) report probable and confirmed cases of tickborne rickettsial diseases to public health authorities.

From 1997 to 2001, sequence data from 55 clinical specimens were obtained from confirmed measles cases in the United States, representing 21 outbreaks and 34 sporadic cases. Sequence analysis indicated the presence of 11 of the recognized genotypes. The most common genotypes detected were genotype D6, usually identified from imported cases from Europe, and genotype D5, associated with importations from Japan. A number of viruses belonging to genotype D4 were imported from India and Pakistan. Overall, viral genotypes were determined for 13 chains of transmission with an unknown source of virus, and seven different genotypes were identified. Therefore, the diversity of Measles virus genotypes observed in the United States from 1997 to 2001 reflected multiple imported sources of virus and indicated that no strain of measles is endemic in the United States. The legal environment for lesbian, gay, bisexual, and transgender (LGBT) astronomers in the United States has changed dramatically in recent years. In 2013, the Supreme Court ruled that Section 3 of the Defense of Marriage Act (DOMA), which had barred the federal government from recognizing same-sex marriages, was unconstitutional. This decision particularly affects astronomers, since astronomers in the U.S. are more likely than the general population to be foreign nationals, to have a foreign-born spouse, or to work for the federal government. In 2014, the Attorney General directed the Department of Justice to take the position in litigation that the protection of Title VII of the Civil Rights Act of 1964 extends to claims of discrimination based on an individual’s gender identity, including transgender status. Title VII makes it unlawful for employers to discriminate in the employment of an individual “because of such individual’s... sex,” among other protected characteristics. As of March 2015, more than 70% of the population lives in states that recognize same-sex marriage, and the Supreme Court is expected to rule on the constitutionality of the remaining same-sex marriage bans during the current term. In this poster, we discuss these advances and their implications for the personal and professional lives of LGBT astronomers across the United States.

Ultraviolet radiation is recognized as a human carcinogen by the International Agency for Research on Cancer, the world's authority on cancer research. In particular, exposure to ultraviolet radiation can lead to melanoma of the skin, which is the deadliest form of skin cancer in the United States. Yet despite the significant public health burden that is associated with skin cancer in the United States, each year over a million Americans engage in indoor tanning where exposure to artificial ultraviolet radiation occurs. In this article, we argue for an immediate ban on the use of commercial indoor tanning by minors and, based on international precedents, the phasing out of all commercial tanning operations in the United States. We consider the use of indoor tanning devices in the United States, epidemiological data on indoor tanning devices and cancer, regulation of tanning devices, and scientific evidence for increased government intervention. © The Author(s) 2015.

These two systems are the primary source of national data describing the number of reported outbreaks; outbreak-associated illnesses, hospitalizations, and deaths; etiologic agents; water source or implicated foods; settings of exposure; and other factors associated with recognized foodborne and waterborne disease outbreaks in the United States.

Poison ivy, poison oak, and poison sumac are now classified in the genus Toxicodendron which is readily distinguished from Rhus. In the United States, there are two species of poison oak, Toxicodendron diversilobum (western poison oak) and Toxicodendron toxicarium (eastern poison oak). There are also two species of poison ivy, Toxicodendron rydbergii, a nonclimbing subshrub, and Toxicodendron radicans, which may be either a shrub or a climbing vine. There are nine subspecies of T. radicans, six of which are found in the United States. One species of poison sumac, Toxicodendron vernix, occurs in the United States. Distinguishing features of these plants and characteristics that separate Toxicodendron from Rhus are outlined in the text and illustrated in color plates.

Recently, Hawaii's mental health care system has been in the news because of its alleged infamy as one of the poorest systems in the United States of America today. Hawaii has not always been considered a state with a conspicuously poor commitment to mental health. The Department of Health (DOH), the oldest department of health in the United States of America, was formed in 1850 by King Kamehameha III. In fact, Kamehameha III, recognizing the importance of developing programs in mental health, was one of the earliest political leaders in history to support government-sponsored mental health services.

Although occupational injuries are among the leading causes of death and disability around the world, the burden due to occupational injuries has historically been under-recognized, obscuring the need to address a major public health problem. We established the Liberty Mutual Workplace Safety Index (LMWSI) to provide a reliable annual metric of the leading causes of the most serious workplace injuries in the United States based on direct workers compensation (WC) costs. More than $600 billion in direct WC costs were spent on the most disabling compensable non-fatal injuries and illnesses in the United States from 1998 to 2010. The burden in 2010 remained similar to the burden in 1998 in real terms. The categories of overexertion ($13.6B, 2010) and fall on same level ($8.6B, 2010) were consistently ranked 1st and 2nd. The LMWSI was created to establish the relative burdens of events leading to work-related injury so they could be better recognized and prioritized. Such a ranking might be used to develop research goals and interventions to reduce the burden of workplace injury in the United States. Copyright © 2015 The Authors. Published by Elsevier Ltd.. Exotic annual Bromus species are widely recognized for their potential to invade, dominate, and alter the structure and function of ecosystems. In this chapter, we summarize the invasion potential, ecosystem threats, and management strategies for different Bromus species within each of five ecoregions of the western United States. We characterize invasion potential and threats in terms of ecosystem resistance to Bromus invasion and ecosystem resilience to disturbance with an emphasis on the importance of fi re regimes. We also explain how soil temperature and moisture regimes can be linked to patterns of resistance and resilience and provide a conceptual framework that can be used to evaluate the relative potential for invasion and ecological impact of the dominant exotic annual Bromus species in the western United States.

Residency training is internationally recognized as the only way for the physicians to be qualified to practice independently. China has instituted a new residency training program for the specialty of plastic surgery. Meanwhile, plastic surgery residency training programs in the United States are presently in a transition because of restricted work hours. The purpose of this study is to compare the current characteristics of plastic surgery residency training in 2 countries. Flow path, structure, curriculum, operative experience, research, and evaluation of training in 2 countries were measured. The number of required cases was compared quantitatively whereas other aspects were compared qualitatively. Plastic surgery residency training programs in 2 countries differ regarding specific characteristics. Requirements to become a plastic surgery resident in the United States are more rigorous. Ownership structure of the regulatory agency for residency training in 2 countries is diverse. Training duration in the United States is more flexible. Clinical and research training is more practical and the method of evaluation of residency training is more reasonable in the United States. The job opportunities after residency differ substantially between 2 countries. Not every resident has a chance to be an independent surgeon and would require much more training time in China than it does in the United States. Plastic surgery residency training programs in the United States and China have their unique characteristics. The training programs in the United States are more standardized. Both the United States and China may complement each other to create training programs that will ultimately provide high-quality care for all people.

The United States Court of Appeals for the 4th Circuit has ruled that a jury considering charges of drug trafficking against a pain management physician should be instructed that the defendant's good faith is a defense to the charges. The court rejected a subjective standard of good faith, and instead ruled that the good faith of the defendant must be evaluated from an objective perspective. This objective standard requires that the jury determine whether the defendant was practicing in accordance with the standard generally recognized and accepted in the United States. General recognition and acceptance are determined on a case-by-case basis, within the context of a defendant's practice. Simply because a physician's practice is out of the norm for many physicians does not mean it can't be generally recognized and accepted within the standard of medical practice. Expert witness testimony of pain management physicians will assist juries in the application of this standard for good faith in prescribing.

The genetics of suicide is increasingly recognized and relevant for mental health literacy, but actual beliefs may lag behind current knowledge. We examined such beliefs in student samples (total N = 686) from Estonia, Malaysia, Romania, the United Kingdom, and the United States with the Beliefs in the Inheritance of Risk Factors for Suicide Scale. Cultural effects were small, those of key demographics nil. Several facets of construct validity were demonstrated. Marked differences in perceived plausibility of evidence about the genetics of suicide according to research design, observed in all samples, may be of general interest for investigating lay theories of abnormal behavior and communicating behavioral and psychiatric genetic research findings.

In many ICUs, admission and discharge hinge on the need for intubation and ventilatory support. As few as 5% to 10% of ICU patients require prolonged mechanical ventilation, and this patient group consumes > or = 50% of ICU patient days and ICU resources. Prolonged ventilatory support and chronic ventilator dependency, both in the ICU and non-ICU settings, have a significant and growing impact on healthcare economics. In the United States, the need for prolonged mechanical ventilation is increasingly recognized as separate and distinct from the initial diagnosis and/or procedure that leads to hospitalization. This distinction has led to improved reimbursement under the prospective diagnosis-related group (DRG) system, and demands more precise accounting from healthcare providers responsible for these patients. Using both published and theoretical examples, mechanical ventilation in the United States is discussed, with a focus on cost containment. Included in the discussion are ventilator teams, standards of care, management protocols, stepdown units, rehabilitation units, and home care. The expanding role of total quality management (TQM) is also presented.

Francisella tularensis has been recognized as a human pathogen for almost 100 years and is the etiological agent of the zoonotic disease tularemia. Soon after its discovery, it became recognized as an important pathogen in several parts of the world, for example, in the United States and Soviet Union. The number of tularemia cases in the two countries peaked in the 1940s and has thereafter steadily declined. Despite this decline, there was still much interest in the pathogen in the 1950s and 1960s since it is highly infectious and transmissible by aerosol, rendering it a potent biothreat agent. In fact, it was one of the agents that was given the highest priority in the offensive programs of the United States and Soviet Union. After termination of the offensive programs in the 1960s, the interest in F. tularensis diminished significantly and little research was carried out for several decades. Outbreaks of tularemia during the last decade in Europe, for example, in Kosovo, Spain, and Scandinavia, led to a renewed public interest in the disease. This, together with a massive increase in the research funding, in particular in the United States since 2001, has resulted in a significant increase in the number of active Francisella researchers. This article summarizes, predominantly with a historical perspective, the epidemiology and clinical manifestations of tularemia and the physiology of F. tularensis.

In 1932, J. N. 'Ding' Darling proposed a 3-year tripartite arrangement between the Iowa Fish and Game Commission, Iowa State University, and himself to establish the first Cooperative Wildlife Research Unit. Three years later, the Cooperative Wildlife Research Unit Program was broadened to include 9 land-grant colleges representing recognized ecoregions in the United States. In 1960, the Units were given statutory recognition by Public Law 86-686 that also included provision for establishing Cooperative Fishery Units. The Cooperative Research Unit idea has evolved to 39 Units in 2000. Today, the main cooperators of the Unit program are the land-grant university, the state fish and game or conservation agency, the U.S. Geological Survey, and the Wildlife Management Institute. The Cooperative Units mission, as stated in Public Law 86-686, remains: 'To facilitate cooperation between the Federal Government, colleges and universities, and private organizations for cooperative unit programs of research and education relating to fish and wildlife and for other purposes.' Graduate research and education continue to be the program's primary missions. In any given year >600 graduate and post-graduate students are involved. Post-graduate employment of Unit-afffiliated students is >90%. Perhaps the primary benefit to the education process is the Units' formal connection to the state cooperator and to their federal agency that might not otherwise be available to university faculty and students. Units are conduits to state and federal funding for research projects conducted by university faculty and students. The CRU program is well positioned to educate a multitalented, ethnically diverse cadre of graduate students who will be prepared not only for their first professional job but also for their career by having been instilled with a desire for life-long professional accomplishment.

Breast cancer is the most frequently diagnosed malignancy in women in the United States. It is the second most common malignancy to cause death, with approximately 39,000 women dying of breast cancer in the United States in 2013. Triple negative breast cancer is defined as the absence of estrogen, progesterone and human epidermal growth factor receptor 2 receptors. It has been associated with a higher incidence in African American women, a younger age and a more advanced stage at diagnosis, and an inferior overall survival. To recognize the differences of our West Virginia community population when compared to the national average, we conducted a retrospective review of all patients diagnosed with breast cancer from 2000-2012.

Wound botulism results from colonization of a contaminated wound by Clostridium botulinum and the anaerobic in situ production of a potent neurotoxin. Between 1943, when wound botulism was first recognized, and 1990, 47 laboratory-confirmed cases, mostly trauma-associated, were reported in the United States. Since 1990, wound botulism associated with injection drug use emerged as the leading cause of wound botulism in the United States; 210 of 217 cases reported to the Centers for Disease Control and Prevention between 1990 and 2002 were associated with drug injection. Despite the worldwide distribution of Clostridium botulinum spores, wound botulism has been reported only twice outside the United States, Europe, and Australia. However, wound botulism may go undiagnosed and untreated in many countries. We report two cases, both with type A toxin, from the Ecuadorian rain forest. Prompt clinical recognition, supportive care, and administration of trivalent equine botulinum antitoxin were life-saving.

To review the current information on medical complications, psychological implications, and legislative issues related to body piercing, a largely unregulated industry in the United States. We conducted a MEDLINE search of English language articles from 1966 until May 1998 using the search terms "body piercing" and "ear piercing." Bibliographies of these references were reviewed for additional citations. We also conducted an Internet search for "body piercing" on the World Wide Web. In this manuscript, we review the available body piercing literature. We conclude that body piercing is an increasingly common practice in the United States, that this practice carries substantial risk of morbidity, and that most body piercing in the United States is being performed by unlicensed, unregulated individuals. Primary care physicians are seeing growing numbers of patients with body pierces. Practitioners must be able to recognize, treat, and counsel patients on body piercing complications and be alert to associated psychological conditions in patients who undergo body piercing.

Historical Analysis and Charaterization of Ground Level Ozone for Canada and United State

Ground-level ozone has long been recognized as an important health and ecosystem-related air quality concern in Canada and the United States. In this work we seek to understand the characteristics of ground level ozone conditions for Canada and United States to support the Ozone Annex under the Canada-U.S. Air Quality Agreement. Our analyses are based upon the data collected by Canadian National Air Pollution Surveillance (NAPS, the NAPS database has also been expanded to include U.S. EPA ground level ozone data) network. Historical ozone data from 1974 to 2002 at a total of 538 stations (253 Canadian stations and 285 U.S. stations) were statistically analyzed using several methodologies including the Canada Wide Standard (CWS). A more detailed analysis including hourly, daily, monthly, seasonally and yearly ozone concentration distributions and trends was undertaken for 54 stations.

The Glasgow quadrangle encompasses approximately 16,084 km2 (6,210 mi2). The northern boundary is the Montana/Saskatchewan (U.S./Canada) boundary. The quadrangle is in the Northern Plains physiographic province and it includes the Boundary Plateau, Peerless Plateau, and Larb Hills. The primary river is the Milk River. The map units are surficial deposits and materials, not landforms. Deposits that comprise some constructional landforms (for example, ground-moraine deposits, end-moraine deposits, and stagnation-moraine deposits, all composed of till) are distinguished for purposes of reconstruction of glacial history. Surficial deposits and materials are assigned to 23 map units on the basis of genesis, age, lithology or composition, texture or particle size, and other physical, chemical, and engineering characteristics. It is not a map of soils that are recognized in pedology or agronomy. Rather, it is a generalized map of soils recognized in engineering geology, or of substrata or parent materials in which pedologic or agronomic soils are formed. Glaciotectonic (ice-thrust) structures and deposits are mapped separately, represented by a symbol. The surficial deposits are glacial, ice-contact, glaciofluvial, alluvial, lacustrine, eolian, colluvial, and mass-movement deposits. Residuum, a surficial material, also is mapped. Till of late Wisconsin age is represented by three map units. Till of Illinoian age is also represented locally but is widespread in the subsurface. This map was prepared to serve as a database for compilation of a Quaternary geologic map of the United States and Canada (scale 1:1,000,000). Letter symbols for the map units are those used for the same units in the Quaternary Geologic Atlas of the United States map series.

Swine influenza viruses (SIV) have been recognized as important pathogens for pigs and occasional human infections with swine origin influenza viruses (SOIV) have been reported. Between 1990 and 2010, a total of twenty seven human cases of SOIV infections have been identified in the United States. Six viruses isolated from 1990 to 1995 were recognized as classical SOIV (cSOIV) A(H1N1). After 1998, twenty-one SOIV recovered from human cases were characterized as triple reassortant (tr_SOIV) inheriting genes from classical swine, avian and human influenza viruses. Of those twenty-one tr_SOIV, thirteen were of A(H1N1), one of A(H1N2), and seven of A(H3N2) subtype. SOIV characterized were antigenically and genetically closely related to the subtypes of influenza viruses circulating in pigs but distinct from contemporary influenza viruses circulating in humans. The diversity of subtypes and genetic lineages in SOIV cases highlights the importance of continued surveillance at the animal-human interface. Copyright © 2011. Published by Elsevier Inc.

Tuberculosis along the United States-Mexico border, 1993-2001.

Tuberculosis (TB) is a leading public health problem and a recognized priority for the federal Governments of both Mexico and the United States of America. The objectives of this research, primarily for the four states in the United States that are along the border with Mexico, were to: (1) describe the epidemiological situation of TB, (2) identify TB risk factors, and (3) discuss tuberculosis program strategies. We analyzed tuberculosis case reports collected from 1993 through 2001 by the tuberculosis surveillance system of the United States. We used those data to compare TB cases mainly among three groups: (1) Mexican-born persons in the four United States border states (Arizona, California, New Mexico, and Texas), (2) persons in those four border states who had been born in the United States, and (3) Mexican-born persons in the 46 other states of the United States, which do not border Mexico. For the period from 1993 through 2001, of the 16 223 TB cases reported for Mexican-born persons in the United States, 12 450 of them (76.7%) were reported by Arizona, California, New Mexico, and Texas. In those four border states overall in 2001, tuberculosis case rates for Mexican-born persons were 5.0 times as high as the rates for persons born in the United States; those four states have 23 counties that directly border on Mexico, and the ratio in those counties was 5.8. HIV seropositivity, drug and alcohol use, unemployment, and incarceration were significantly less likely to be reported in Mexican-born TB patients from the four border states and the nonborder states than in patients born in the United States from the four border states (P < 0.001). Multivariate analysis revealed that among pulmonary tuberculosis patients who were 18-64 years of age and residing in the four border states, the Mexican-born patients were 3.6 times as likely as the United States-born patients were to have resistance to at least isoniazid and rifampin (i. e., to have multidrug-resistant TB) and twice as likely to have isoniazid resistance. Mexican-born TB patients from the four border states and the nonborder states were significantly more likely to have moved or to be lost to follow-up than were the TB patients born in the United States from the four border states (P < 0.001). Increased collaborative tuberculosis control efforts by the federal Governments of both Mexico and the United States along the border that they share are needed if tuberculosis is to be eliminated in the United States.

Out of 20 spirochete species from Borrelia burgdorferi sensu lato (s.l.) complex recognized to date some are considered to have a limited distribution, while others are worldwide dispersed. Among those are Borrelia burgdorferi sensu stricto (s.s.) and Borrelia bissettii which are distributed both in North America and in Europe. While B. burgdorferi s.s. is recognized as a cause of Lyme borreliosis worldwide, involvement of B. bissettii in human Lyme disease was not so definite yet. Multilocus sequence typing of spirochete isolates originating from residents of Georgia and Florida, USA, revealed the presence of two Borrelia burgdorferi sensu stricto strains highly similar to those from endemic Lyme borreliosis regions of the northeastern United States, and an unusual strain that differed from any previously described in Europe or North America. Based on phylogenetic analysis of eight chromosomally located housekeeping genes divergent strain clustered between Borrelia bissettii and Borrelia carolinensis, two species from the B.burgdorferi s.l. complex, widely distributed among the multiple hosts and vector ticks in the southeastern United States. The genetic distance analysis showed a close relationship of the diverged strain to B. bissettii. Here, we present the analysis of the first North American human originated live spirochete strain that revealed close relatedness to B. bissettii. The potential of B. bissettii to cause human disease, even if it is infrequent, is of importance for clinicians due to the extensive range of its geographic distribution.

Political Gender Inequality and Infant Mortality in the United States, 1990–2012

Results indicate that higher percentages of women in state legislatures are associated with reduced IMRs, both between states and within-states over time. According to model predictions, if women were at parity with men in state legislatures, the expected number of infant deaths in the U.S. in 2012 would have been lower by approximately 14.6% (3,478 infant deaths). Leveraging EarthScope USArray with the Central and Eastern United States Seismic Network

Recent earthquakes, such as the 2011 M5.8 Mineral, Virginia earthquake, raised awareness of the comparative lack of knowledge about seismicity, site response to ground shaking, and the basic geologic underpinnings in this densely populated region. With this in mind, the National Science Foundation, United States Geological Survey, United States Nuclear Regulatory Commission, and Department of Energy supported the creation of the Central and Eastern United States Seismic Network (CEUSN). These agencies, along with the IRIS Consortium who operates the network, recognized the unique opportunity to retain EarthScope Transportable Array (TA) seismic stations in this region beyond the standard deployment duration of two years per site. The CEUSN project supports 159 broadband TA stations, more than 30 with strong motion sensors added, that are scheduled to operate through 2017. Stations were prioritized in regions of elevated seismic hazard that have not been traditionally heavily monitored, such as the Charlevoix and Central Virginia Seismic Zones, and in regions proximal to nuclear power plants and other critical facilities. The stations (network code N4) transmit data in real time, with broadband and strong motion sensors sampling at 100 samples per second. More broadly the CEUSN concept also recognizes the existing backbone coverage of permanently operating seismometers in the CEUS, and forms a network of over 300 broadband stations. This multi-agency collaboration is motivated by the opportunity to use one facility to address multiple missions and needs in a way that is rarely possible, and to produce data that enables both researchers and federal agencies to better understand seismic hazard potential and associated seismic risks. In June 2015, the CEUSN Working Group (www.usarray.org/ceusn_working_group) was formed to review and provide advice to IRIS Management on the performance of the CEUSN as it relates to the target scientific goals and objectives. Map shows the 159 CEUSN stations (yellow) that will be operated and maintained by the IRIS Consortium through 2017. The CEUSN stations were selected for proximity to nuclear power plants (black squares) and other critical infrastructure as well as to more evenly distribute seismic stations across the central and eastern United States.

Non-accidental trauma (NAT) is a leading cause of childhood traumatic injury and death in the United States. It is estimated that 1,400 children died from maltreatment in the United States in 2002 and abusive head trauma (AHT) accounted for 80% of these deaths. This review examines the epidemiology and risk factors for NAT as well as the general presentation and required medical work up of abused children. In addition, potential algorithms for recognizing cases of abuse are reviewed as well as outcomes in children with NAT and potential neurosurgical interventions which may be required. Finally, the evidence for seizure prophylaxis in this population is addressed. PMID:26835337

An examination of the ethical code of the ASME International shows that its provisions pose many problems of application, especially in societies outside the United States. In applying the codes effectively in the international environment, two principal issues must be addressed. First, some Culture Transcending Guidelines must be identified and justified. This study aims at building an efficient word recognition system resulting from the combination of three handwriting recognizers. The main component of this combined system is an HMM-based recognizer which considers dynamic and contextual information for a better modeling of writing units. For modeling the contextual units, a state-tying process based on decision tree clustering is introduced. Decision trees are built according to a set of expert-based questions on how characters are written. Questions are divided into global questions, yielding larger clusters, and precise questions, yielding smaller ones. Such clustering enables us to reduce the total number of models and Gaussians densities by 10. We then apply this modeling to the recognition of handwritten words. Experiments are conducted on three publicly available databases based on Latin or Arabic languages: Rimes, IAM, and OpenHart. The results obtained show that contextual information embedded with dynamic modeling significantly improves recognition.

This is a supplement to the report of the International Board of Inquiry relative to the preservation and development of the Great Lakes fisheries as provided by an exchange of notes between Canada and the United States on February 29, 1940. The report aimed to present in a very brief manner the background of the establishment of the Board, its program, and the fishery problems that confronted it with particular reference to the international point of view. (See Contribution #100.) The Board, however, recognized the significance and import of those problems and data more or less peculiar or applicable largely to each country. It was, therefore, agreed that the representatives of each country should, if they deem it desirable, submit to their respective governments a supplementary report to cover the basis of action in greater detail than it was possible in a joint report. The United States members believed it advisable not only to publish detailed statistics of catch, but also other information deemed pertinent for a full understanding of the Great Lakes fishery problems from the United States point of view.

Avian wildlife mortality events due to salmonellosis in the United States, 1985-2004

Infection with Salmonella spp. has long been recognized in avian wildlife, although its significance in causing avian mortality, and its zoonotic risk, is not well understood. This study evaluates the role of Salmonella spp. in wild bird mortality events in the United States from 1985 through 2004. Analyses were performed to calculate the frequency of these events and the proportional mortality by species, year, month, state, and region. Salmonellosis was a significant contributor to mortality in many species of birds; particularly in passerines, for which 21.5% of all mortality events involved salmonellosis. The proportional mortality averaged a 12% annual increase over the 20-yr period, with seasonal peaks in January and April. Increased salmonellosis-related mortality in New England, Southeastern, and Mountain-Prairie states was identified. Based on the results of this study, salmonellosis can be considered an important zoonotic disease of wild birds. ?? Wildlife Disease Association 2008.

Compositional trends in aeolian dust along a transect across the southwestern United States

Aeolian dust strongly influences ecology and landscape geochemistry over large areas that span several desert ecosystems of the southwestern United States. This study evaluates spatial and temporal variations and trends of the physical and chemical properties of dust in the southwestern United States by examining dust deposited in natural depressions on high isolated surfaces along a transect from the Mojave Desert to the central Colorado Plateau. Aeolian dust is recognized in these depressions on the basis of textural, chemical, isotopic, and mineralogical characteristics and comparisons of those characteristics to the underlying bedrock units. Spatial and temporal trends suggest that although local dust sources are important to the accumulated material in these depressions, Mojave Desert dust sources may also contribute. Depth trends in the depressions suggest that Mojave sources may have contributed more dust to the Colorado Plateau recently than in the past. These interpretations point to the important roles of far-traveled aeolian dust for landscape geochemistry and imply future changes to soil geochemistry under changing conditions in far-distant dust source areas. Energy security, environmental concerns, and unstable oil prices have been the driving trifecta of demand for alternative fuels in the United States. The United States' dependence on energy resources, often from unstable oil-producing countries has created political insecurities and concerns. As we try to gain energy security, unconventional oil becomes more common, flooding the market, and causing the major downshift of the usual unstable oil prices. Meanwhile, consumption of fossil fuels and the consequent CO 2 emissions have driven disruptions in the Earth's atmosphere and are recognized to be responsible for global climate change. While the significance of each of these three factors may fluctuate with global politics or new technologies, transportation energy will remain the prominent focus of multi-disciplined research. Bioenergy future depends on the price of oil. Current energy policy of the United States heavily favors petroleum industry. Emerging zoonoses in the southern United States: toxocariasis, bovine tuberculosis and southern tick-associated rash illness.

The majority of emerging diseases in humans have been linked to zoonotic pathogens originating in domestic animals or wildlife. This is a public health concern because zoonotic infections affect several aspects of the society. The complex interactions among pathogen, host and environment also pose challenges in estimating the true burden of those infections. However, the recent development of new molecular diagnostic tools has allowed for better diagnosis of zoonotic infections. This review focuses on 3 emerging zoonoses, namely toxocariasis, bovine tuberculosis and southern tick-associated rash illness, and demonstrates that these infections may be more prevalent in the southern United States than previously recognized. This review places special emphasis on the recent epidemiologic trends, intra/interspecies transmission and clinical features of each of these zoonoses. In addition, treatment and prevention for each zoonotic pathogen are discussed. Clinicians working in the southern United States should be aware of the presence of those zoonotic infections.

Cryptococcus gattii has emerged as a human and animal pathogen in the Pacific Northwest. First recognized on Vancouver Island, British Columbia, Canada, it now involves mainland British Columbia, and Washington and Oregon in the United States. In Canada, the incidence of disease has been one of the highest worldwide. In the United States, lack of cryptococcal species identification and case surveillance limit our knowledge of C. gattii epidemiology. Infections in the Pacific Northwest are caused by multiple genotypes, but the major strain is genetically novel and may have emerged recently in association with unique mating or environmental changes. C. gattii disease affects immunocompromised and immunocompetent persons, causing substantial illness and death. Successful management requires an aggressive medical and surgical approach and consideration of potentially variable antifungal drug susceptibilities. We summarize the study results of a group of investigators and review current knowledge with the goal of increasing awareness and highlighting areas where further knowledge is required. Assessing Implementation of Cultural Competency Content in the Curricula of Colleges of Pharmacy in the United States and Canada

Objectives To assess the presence of curricular and organizational content related to cultural competency within colleges of pharmacy in the United States and Canada. Methods Curriculum committee chairs (n = 87) and student leaders (n = 54) in colleges of pharmacy in the United States and Canada were surveyed via an e-mailed assessment tool. Results Forty-nine (56.3%) curriculum committee chairs and 27 (50%) student leaders returned usable responses. Respondents reported that cultural competency was mentioned in 61.2% of their mission statements, and half had made curricular changes with respect to diversity within the past 5 years. Almost 94% felt the necessity to add cultural competency topics to required courses in the curriculum, and 42.9% wanted to add a course specific to cultural competency into the curriculum. Conclusion Curriculum committee chairs recognize the need to add curricular content related to cultural competency, but not all of the respondents have implemented changes in their college's curriculum. Pattern and frequency of use of complementary and alternative medicine among patients with epilepsy in the midwestern United States.

Complementary and alternative medicine (CAM) is recognized to be commonly used by patients, yet there have been few studies regarding the scope of CAM use by patients with epilepsy. This study assessed usage and perceptions of CAM by patients with epilepsy in the midwest of the United States. A 25-item survey was administered to adult patients with epilepsy, and data were collected from 228 patients. The survey collected demographics, specific CAM usage, adverse effects of CAM therapy, and perceptions of the effectiveness of CAM. Thirty-nine percent reported using CAM; 25% reported using CAM specifically for their epilepsy. Prayer/spirituality was the most commonly used form of CAM (46%), followed by "mega" vitamins (25%), chiropractic care (24%), and stress management (16%). CAM use is common among midwestern patients with epilepsy, although the pattern of use may be slightly different than in other regions of the United States and elsewhere.

As their expansion slows in the United States, managed care organizations will continue to enter new markets abroad. Investors view the opening of managed care in Latin America as a lucrative business opportunity. As public-sector services and social security funds are cut back, privatized, and reorganized under managed care, with the support of international lending agencies such as the World Bank, the effects of these reforms on access to preventive and curative services will hold great importance throughout the developing world. Many groups in Latin America are working on alternative projects that defend health as a public good, and similar movements have begun in Africa and Asia. Increasingly, this organizing is being recognized not only as part of a class struggle but also as part of a struggle against economic imperialism--which has now taken on the new appearance of rescuing less developed countries from rising health care costs and inefficient bureaucracies through the imposition of neoliberal managed-care solutions exported from the United States.

Michael Tuomey's 1848 geological survey of South Carolina

One hundred and fifty years ago, Michael Tuomey completed his 'Report on the Geology of South Carolina,' the result of four years of arduous labor. The report is the first detailed and comprehensive geological description of the entire state, and it includes a geological map that shows the distribution of Coastal Plain and Piedmont-Blue Ridge units. In the sesquicentennial of Tuomey's survey, it is fitting that we recognize his important early contribution to the geology of South Carolina and the southeast. Tuomey's report is a 293-page volume with a 48-page appendix and an index. Although he gave a complete depiction of Coastal Plain geology and delineated Cretaceous, Lower Eocene, Eocene, Miocene, Post-Pliocene, and alluvial units on his map, the emphasis herein is on his mapping of the Piedmont and Blue Ridge. The metamorphic units he delineated are clay slate, mica slate, talcose slate, hornblende slate, gneiss, and lime rock. Gneiss is the most extensive unit on the map. His map shows many elements of the geologic framework we recognize today. The distribution of his clay slate unit corresponds closely with the Carolina slate and Bel Air belts as we know them now. The gneiss between the two clay slate areas matches the Kiokee belt. Areas of mica slate approximate the northern part of the Kings Mountain belt and the Chauga belt. He also recognized that his talcose slate unit was associated with gold deposits. Granitic and basaltic intrusive rocks are also delineated on the map. It shows the Newberry, Columbia, and Liberty Hill granites we recognize today. Basaltic intrusives outlined include the Bush River of western Newberry County, Dutchmans Creek, Big Wateree Creek, and Ogden gabbros. He described the regional extent of diabase dikes as occuring from Virginia to Alabama, noted their preferred direction and diagrammed their near-vertical orientation. He also referred to the distinctive soil and topography that develops on the large gabbros. Michael Tuomey's report is truly a benchmark publication, for sixty years passed before the next statewide survey was done. Upon completing the report, he left South Carolina to become director of the Alabama Geological Survey.

To realize the maximum potential benefits of remote sensing, the technology must be applied by personnel responsible for the management of natural resources and the environment. In Canada and the United States, these managers are often in local offices and are not those responsible for the development of systems to acquire, preprocess, and disseminate remotely sensed data, nor those leading the research and development of techniques for analysis of the data. However, the latter organizations have recognized that the technology they develop must be transferred to the management agencies if the technology is to be useful to society. Problems of motivation and communication associated with the technology transfer process, and some of the methods employed by Federal, State, Provincial, and local agencies, academic institutions, and private organizations to overcome these problems are explored.
