Progress along developmental tracks for electronic health records implementation in the United States

The development and implementation of electronic health records (EHR) have occurred slowly in the United States. To date, these approaches have, for the most part, followed four developmental tracks: (a) Enhancement of immunization registries and linkage with other health records to produce Child Health Profiles (CHP), (b) Regional Health Information Organization (RHIO) demonstration projects to link together patient medical records, (c) Insurance company projects linked to ICD-9 codes and patient records for cost-benefit assessments, and (d) Consortia of EHR developers collaborating to model systems requirements and standards for data linkage. Until recently, these separate efforts have been conducted in the very silos that they had intended to eliminate, and there is still considerable debate concerning health professionals access to as well as commitment to using EHR if these systems are provided. This paper will describe these four developmental tracks, patient rights and the legal environment for EHR, international comparisons, and future projections for EHR expansion across health networks in the United States. Observations and interpretation of fundamental mode Rayleigh wavefields recorded by the Transportable Array (USArray)

Broadband recordings of the dense Transportable Array (TA) in the western United States provide unparalleled detailed images of long-period seismic surface wavefields. With 400 stations spanning most of the western United States, wavefronts of fundamental mode Rayleigh waves may be visualized coherently across the array at periods ???40 s. In order to constrain the Rayleigh wave phase velocity structure in the western United States, I assemble a data set of vertical component seismograms from 53 teleseismic events recorded by the TA from April 2006 to October 2007. Complex amplitude spectra from these recordings at peni ods 27-100 s are interpreted using the multiplane wave tomographic method of Friederich and Wielandt (1995) and Pollitz (1999). This analysis yields detailed surface wave phase velocity and three-dimensional shear wave velocity patterns across the North American plate boundary zone, elucidating the active processes in the highly heterogeneous western U.S. upper mantle.

Aedes (Stegomyia) aegypti (L.) and Aedes (Stegomyia) albopictus (Skuse) are potential vectors of Zika, dengue, and chikungunya viruses in the United States. A Zika virus outbreak in Florida in the summer of 2016, driven by Ae. aegypti and resulting in > 200 locally acquired cases of human illness, underscored the need for up-to-date information on the geographic distribution of Ae. aegypti and Ae. albopictus in the United States. In early 2016, we conducted a survey and literature review to compile county records for presence of Ae. aegypti and Ae. albopictus in the United States from 1995 to 2016. Surveillance for these vectors was intensified across the United States during the summer and fall of 2016. At the end of 2016, we therefore conducted a follow-up survey of mosquito control agencies, university researchers, and state and local health departments to document new collection records for Ae. aegypti and Ae. albopictus. The repeated survey at the end of the year added Ae. aegypti collection records from 38 new counties and Ae. albopictus collection records from 127 new counties, representing a 21 and 10 percent increase, respectively, in the number of counties with reported presence of these mosquitoes compared with the previous report. Moreover, through our updated survey, 40 and 183 counties, respectively, added additional years of collection records for Ae. aegypti and Ae. albopictus from 1995 to 2016. Our findings underscore the continued need for systematic surveillance of Ae. aegypti and Ae. albopictus. Published by Oxford University Press on behalf of Entomological Society of America 2017. New technology in dietary assessment: a review of digital methods in improving food record accuracy.

Methods for conducting dietary assessment in the United States date back to the early twentieth century. Methods of assessment encompassed dietary records, written and spoken dietary recalls, FFQ using pencil and paper and more recently computer and internet applications. Emerging innovations involve camera and mobile telephone technology to capture food and meal images. This paper describes six projects sponsored by the United States National Institutes of Health that use digital methods to improve food records and two mobile phone applications using crowdsourcing. The techniques under development show promise for improving accuracy of food records.

In flood frequency analysis (FFA), sufficiently long data series are important to get more reliable results. Compared to return periods of interest, at-site FFA usually needs large data sets. Generally, the precision of at site estimators and time-sampling errors are associated with the length of a gauged record. In this work, we quantify the difference with various record lengths. we use generalized extreme value (GEV) and Log Pearson type III (LP3), two traditional methods on annual maximum stream flows to undertake FFA, and propose quantitative ways, relative difference in median and interquartile range (IQR) to compare the flood frequency performances on different record length from selected 350 USGS gauges, which have more than 70 years record length in Continuous United States. Also, we group those gauges into different regions separately based on hydrological unit map and discuss the geometry impacts. The results indicate that long record length can avoid imposing an upper limit on the degree of sophistication. Working with relatively longer record length may lead accurate results than working with shorter record length. Furthermore, the influence of hydrologic unites for the watershed boundary dataset on those gauges also be presented. The California region is the most sensitive to record length, while gauges in the east perform steady.

A solid-state digital temperature recorder for space use

A solid-state, digital, temperature recorder has been developed for use in space experiments. The recorder is completely self-contained and includes a temperature sensor; all necessary electronics for signal conditioning, processing, storing, control and timing; and a battery power supply. No electrical interfacing with the particular spacecraft on which the unit is used is required. The recorder is small, light, and sturdy, and has no moving parts. It uses only biocompatible materials and has passed vibration and shock spaceflight qualification tests. The unit is capable of storing 2048, -10 to +45 C, 8-bit temperature measurements taken at intervals selectable by factors of 2 from 1.875 to 240 min; data can be retained for at least 6 months. The basic recorder can be simplified to accommodate a variety of applications by adding memory to allow more data to be recorded, by changing the front end to permit measurements other than temperature to be made, and by using different batteries to realize various operating periods. Stored flight data are read out from the recorder by means of a ground read-out unit.

Using chronically implanted tetrodes into primary visual cortex (V1) of rats, we conducted extracellular recordings of single units and followed the same cell ensembles in the awake and anesthetized states. We found that the transition from wakefulness to anesthesia involves unpredictable changes in temporal response characteristics. The latency of single-unit responses to visual stimulation was delayed in anesthesia, with large individual variations between units. Pair-wise correlations between units increased under anesthesia, indicating more synchronized activity. Further, the units within an ensemble show reproducible temporal activity patterns in response to visual stimuli that is changed between states, suggesting state-dependent sequences of activity. The current dataset, with recordings from the same neural ensembles across states, is well suited for validating and testing computational network models. This can lead to testable predictions, bring a deeper understanding of the experimental findings and improve models of neural information processing. Here, we exemplify such a workflow using a Brunel network model.

Comparison of the historical record of earthquake hazard with seismic-hazard models for New Zealand and the continental United States

We compare the historical record of earthquake hazard experienced at 78 towns and cities (sites) distributed across New Zealand and the continental United States with the hazard estimated from the national probabilistic seismic-hazard (PSH) models for the two countries. The two PSH models are constructed with similar methodologies and data. Our comparisons show a tendency for the PSH models to slightly exceed the historical hazard in New Zealand and westernmost continental United States interplate regions, but show lower hazard than that of the historical record in the continental United States intraplate region. Factors such as non-Poissonian behavior, parameterization of active fault data in the PSH calculations, and uncertainties in estimation of ground-motion levels from historical felt intensity data for the interplate regions may have led to the higher-than-historical levels of hazard at the interplate sites. In contrast, the less-than-historical hazard for the remaining continental United States (intraplate) sites may be largely due to site conditions not having been considered at the intraplate sites, and uncertainties in correlating ground-motion levels to historical felt intensities. The study also highlights the importance of evaluating PSH models at more than one region, because the conclusions reached on the basis of a solely interplate or intraplate study would be very different.

The USGS National Earthquake Information Center (NEIC) has planned and is developing a broadband digital seismograph network for the United States. The network will consist of approximately 150 seismograph stations distributed across the contiguous 48 states and across Alaska, Hawaii, Puerto Rico and the Virgin Islands. Data transmission will be via two-way satellite telemetry from the network sites to a central recording facility at the NEIC in Golden, Colorado. The design goal for the network is the on-scale recording by at least five well-distributed stations of any seismic event of magnitude 2.5 or greater in all areas of the United States except possibly part of Alaska. Trends in record-breaking temperatures for the conterminous United States

In an unchanging climate, record-breaking temperatures are expected to decrease in frequency over time, as established records become increasingly more difficult to surpass. This inherent trend in the number of record-breaking events confounds the interpretation of actual trends in the presence of any underlying climate change. Here, a simple technique to remove the inherent trend is introduced so that any remaining trend can be examined separately for evidence of a climate change. As this technique does not use the standard definition of a broken record, our records* are differentiated by an asterisk. Results for the period 1961-2010 indicate that the number of record* low daily minimum temperatures has been significantly and steadily decreasing nearly everywhere across the United States while the number of record* high daily minimum temperatures has been predominantly increasing. Trends in record* low and record* high daily maximum temperatures are generally weaker and more spatially mixed in sign. These results are consistent with other studies examining changes expected in a warming climate.

Unprecedented declines in mortality among the very old have led to the emergence of "true" supercentenarians (persons aged 110 and over). The ages of these individuals have been well-documented in European countries with a history of birth registration, but have not been systematically studied in the United States, which lacks similar documentation and where the inaccuracy of age reporting has been an issue. To verify age, we linked records from the Social Security Administration for close to 700 individuals who died from 1980 to 1999 purportedly at ages 110 and older to records of the U.S. censuses of 1880 and 1900, conducted when these individuals were children. This group was a residual group from an earlier file that was reduced by the SSA after data checks that eliminated incorrect records. The results of the matched records for the residual file indicate that over 90% of the whites were accurately reported as supercentenarians, but only half of the blacks appeared to have attained age 110. The verification of age shows that the United States has more "true" supercentenarians than do other nations.

Water resources data for the 1999 water year for Georgia consists of records of stage, discharge, and water quality of streams; and the stage and contents of lakes and reservoirs published in one volume in a digital format on a CD-ROM. This volume contains discharge records of 121 gaging stations; stage for 13 gaging stations; stage and contents for 18 lakes and reservoirs; continuous water quality records for 10 stations; and the annual peak stage and annual peak discharge for 75 crest-stage partial-record stations. These data represent that part of the National Water Data System collected by the U.S. Geological Survey and cooperating State and Federal agencies in Georgia. Records of discharge and stage of streams, and contents or stage of lakes and reservoirs were first published in a series of U.S. Geological water-supply papers entitled, 'Surface-Water Supply of the United States.' Through September 30, 1960, these water-supply papers were in an annual series and then in a 5-year series for 1961-65 and 1966-70. Records of chemical quality, water temperature, and suspended sediment were published from 1941 to 1970 in an annual series of water-supply papers entitled, 'Quality of Surface Waters of the United States.' Records of ground-water levels were published from 1935 to 1974 in a series of water-supply papers entitled, 'Ground-Water Levels in the United States.' Water-supply papers may be consulted in the libraries of the principal cities in the United States or may be purchased from the U.S. Geological Survey, Branch of Information Services, Federal Center, Box 25286, Denver, CO 80225. For water years 1961 through 1970, streamflow data were released by the U.S. Geological Survey in annual reports on a State-boundary basis prior to the two 5-year series water-supply papers, which cover this period. The data contained in the water-supply papers are considered the official record. Water-quality records for water years 1964 through 1970 were similarly released either in separate reports or in conjunction with streamflow records. Beginning with the 1971 water year, water data for streamflow, water quality, and ground water are published in official Survey reports on a State-boundary basis. These official Survey reports carry an identification number consisting of the two-letter State abbreviation, the last two digits of the water year, and the volume number. For example, this volume is identified as 'U.S. Geological Survey Water-Data Report GA-99-1.' These water-data reports are for sale in various formats, by the National Technical Information Service, U.S. Department of Commerce, Springfield, VA 22161.

Solid state recording current meter conversion

The authors describe the conversion of an Endeco-174 current meter to a solid-state recording current meter. A removable solid-state module was designed to fit in the space originally occupied by an 8-track tape cartridge. The module contains a CPU and 128 kilobytes of nonvolatile CMOS memory. The solid-state module communicates with any terminal or computer using an RS-232C interface at 4800 baud rate. A primary consideration for conversion was to keep modifications of the current meter to a minimum. The communication protocol was designed to emulate the Endeco tape translation unit, thus the need for a translation unit was eliminated and the original data reduction programs can be used without any modification. After conversion, the data recording section of the current meter contains no moving parts; the storage capacity of the module is equivalent to that of the original tape cartridge.

The surface-water records for the 1964 water year for gaging stations, partialrecord stations, and miscellaneous sites within the State of New Mexico are given in this report. For convenience there are also included records for a few pertinent gaging stations in bordering States. The records were collected and computed by the Water Resources Division of the U. S. Geological Survey, under the direction of W. L. Heckler, district engineer, Surface Water Branch. This report is the fourth in a series presenting, annually, basic data on surfacewater records by States. Through September 30, 1960, the records of discharge and stage of streams and contents and stage of lakes or reservoirs were published in an annual series of U. S. Geological Survey water-supply papers entitled "Surface Water Supply of the United States." Since 1951 there have been 20 volumes in the series; each volume covered an area whose boundaries coincided with those of certain natural drainage areas. The records in New Mexico were contained in Parts 7, 8, and 9 of that series. Beginning with the 1961 water year, streamflow records and related data will be released by the Geological Survey in annual reports on a State-boundary basis. Distribution of these basic-data reports will be limited and primarily for local needs. The records later will be published in Geological Survey water-supply papers at 5-year intervals. These 5-year water-supply papers will show daily discharge and will be compiled on the same geographical areas previously used for the annual series; however, some of the 14 parts of conterminous United States will be further subdivided.

Challenges with secondary use of multi-source water-quality data in the United States

Combining water-quality data from multiple sources can help counterbalance diminishing resources for stream monitoring in the United States and lead to important regional and national insights that would not otherwise be possible. Individual monitoring organizations understand their own data very well, but issues can arise when their data are combined with data from other organizations that have used different methods for reporting the same common metadata elements. Such use of multi-source data is termed “secondary use”—the use of data beyond the original intent determined by the organization that collected the data. In this study, we surveyed more than 25 million nutrient records collected by 488 organizations in the United States since 1899 to identify major inconsistencies in metadata elements that limit the secondary use of multi-source data. Nearly 14.5 million of these records had missing or ambiguous information for one or more key metadata elements, including (in decreasing order of records affected) sample fraction, chemical form, parameter name, units of measurement, precise numerical value, and remark codes. As a result, metadata harmonization to make secondary use of these multi-source data will be time consuming, expensive, and inexact. Different data users may make different assumptions about the same ambiguous data, potentially resulting in different conclusions about important environmental issues. The value of these ambiguous data is estimated at \\$US12 billion, a substantial collective investment by water-resource organizations in the United States. By comparison, the value of unambiguous data is estimated at \\$US8.2 billion. The ambiguous data could be preserved for uses beyond the original intent by developing and implementing standardized metadata practices for future and legacy water-quality data throughout the United States.

We examine the ratio of record-breaking highs to record-breaking lows with respect to extent of time-series for monthly mean temperatures within the continental United States (1900-2006) and ask the following question. How are record-breaking high and low surface temperatures in the United States affected by time period? We find that the ratio of record-breaking highs to lows in 2006 increases as the time-series extend further into the past. For example: in 2006, the ratio of record-breaking highs to record-breaking lows is ≈ 13 : 1 with 1950 as the first year and ≈ 25 : 1 with 1900 as the first year; both ratios are an order of magnitude greater than 3-σ for stationary simulations. We also find record-breaking events are more sensitive to trends in time-series of monthly averages than time-series of corresponding daily values. When we consider the ratio as it evolves with respect to a fixed start year, we find it is strongly correlated with the ensemble mean. Correlation coefficients are 0.76 and 0.82 for 1900-2006 and 1950-2006 respectively; 3-σ = 0.3 for pairs of uncorrelated stationary time-series. We find similar values for globally distributed time-series: 0.87 and 0.92 for 1900-2006 and 1950-2006 respectively. However, the ratios evolve differently: global ratios increase throughout (1920-2006) while continental United States ratios decrease from about 1940 to 1970. The United States Historical Climatology Network (USHCN) is a high-quality data set of daily and monthly records of basic meteorological variables from 1218 observing stations across the 48 contiguous United States. Daily data include observations of maximum and minimum temperature, precipitation amount, snowfall amount, and snow depth; monthly data consist of monthly-averaged maximum, minimum, and mean temperature and total monthly precipitation. Most of these stations are U.S. Cooperative Observing Network stations located generally in rural locations, while some are National Weather Service First-Order stations that are often located in more urbanized environments. The USHCN has been developed over the years at the National Oceanic and Atmospheric Administration's (NOAA) National Climatic Data Center (NCDC) to assist in the detection of regional climate change. Furthermore, it has been widely used in analyzing U.S. climte. The period of record varies for each station. USHCN stations were chosen using a number of criteria including length of record, percent of missing data, number of station moves and other station changes that may affect data homogeneity, and resulting network spatial coverage. Collaboration between NCDC and CDIAC on the USHCN project dates to the 1980s (Quinlan et al. 1987). At that time, in response to the need for an accurate, unbiased, modern historical climate record for the United States, the Global Change Research Program of the U.S. Department of Energy and NCDC chose a network of 1219 stations in the contiguous United States that would become a key baseline data set for monitoring U.S. climate. This initial USHCN data set contained monthly data and was made available free of charge from CDIAC. Since then it has been comprehensively updated several times [e.g., Karl et al. (1990) and Easterling et al. (1996)]. The initial USHCN daily data set was made available through CDIAC via Hughes et al. (1992) and contained a 138-station subset of the USHCN. This product was updated by Easterling et al. (1999) and expanded to include 1062 stations. In 2009 the daily USHCN dataset was expanded to include all 1218 stations in the USHCN.

Update on Electronic Dental Record and Clinical Computing Adoption Among Dental Practices in the United States

This study sought to re-characterize trends and factors affecting electronic dental record (EDR) and technologies adoption by dental practices and the impact of the Health Information Technology for Economic and Clinical Health (HITECH) act on adoption rates through 2012. A 39-question survey was disseminated nationally over 3 months using a novel, statistically-modeled approach informed by early response rates to achieve a predetermined sample. EDR adoption rate for clinical support was 52%. Adoption rates were higher among: (1) younger dentists; (2) dentists ≤ 15 years in practice; (3) females; and (4) group practices. Top barriers to adoption were EDR cost/expense, cost-benefit ratio, electronic format conversion, and poor EDR usability. Awareness of the Federal HITECH incentive program was low. The rate of chairside computer implementation was 72%. Adoption of EDR in dental offices in the United States was higher in 2012 than electronic health record adoption rates in medical offices and was not driven by the HITECH program. Patient portal adoption among dental practices in the United States remained low. PMID:29229631

Fifty-Year Record of Glacier Change Reveals Shifting Climate in the Pacific Northwest and Alaska, USA

Fifty years of U.S. Geological Survey (USGS) research on glacier change shows recent dramatic shrinkage of glaciers in three climatic regions of the United States. These long periods of record provide clues to the climate shifts that may be driving glacier change. The USGS Benchmark Glacier Program began in 1957 as a result of research efforts during the International Geophysical Year (Meier and others, 1971). Annual data collection occurs at three glaciers that represent three climatic regions in the United States: South Cascade Glacier in the Cascade Mountains of Washington State; Wolverine Glacier on the Kenai Peninsula near Anchorage, Alaska; and Gulkana Glacier in the interior of Alaska (fig. 1).

The Water Resources Division of the U.S. Geological Survey, in cooperation with State, county, municipal, and other Federal agencies, obtains a large amount of data pertaining to the water resources of Iowa each water year. These data, accumulated during many water years, constitute a valuable data base for developing an improved understanding of the water resources of the State. To make this data readily available to interested parties outside of the Geological Survey, the data is published annually in this report series entitled “Water Resources Data - Iowa” as part of the National Water Data System. Water resources data for water year 2001 for Iowa consists of records of stage, discharge, and water quality of streams; stage and contents of lakes and reservoirs; and water levels and water quality of ground water. This report, in two volumes, contains stage or discharge records for 132 gaging stations; stage records for 9 lakes and reservoirs; water-quality records for 4 gaging stations; sediment records for 13 gaging stations; and water levels for 163 ground-water observation wells. Also included are peak-flow data for 92 crest-stage partial-record stations, water-quality data from 86 municipal wells, and precipitation data collected at 6 gaging stations and 2 precipitation sites. Additional water data were collected at various sites not included in the systematic data-collection program, and are published here as miscellaneous measurements and analyses. These data represent that part of the National Water Data System operated by the U.S. Geological Survey and cooperating local, State, and Federal agencies in Iowa.Records of discharge or stage of streams, and contents or stage of lakes and reservoirs were first published in a series of U.S. Geological Survey water-supply papers entitled “Surface Water Supply of the United States.” Through September 30, 1960, these water-supply papers were published in an annual series; during 1961-65 and 1966-70, they were published in 5- year series. Records of chemical quality, water temperatures, and suspended sediment were published from 1941 to 1970 in an annual series of water-supply papers entitled “Quality of Surface Waters of the United States.” Records of ground-water levels were published from 1935 to 1974 in a series of water-supply papers entitled “Ground-Water Levels in the United States.” Water-supply papers may be consulted in the libraries of the principal cities in the United States, or they may be purchased from Books and Open-File Reports Section, Federal Center, Box 25425, Denver, Colorado 80225. For water years 1961 through 1970, streamflow data were released by the Geological Survey in annual reports on a State-boundary basis. Water-quality records for water years 1964 through 1970 were similarly released either in separate reports or in conjunction with streamflow records. Beginning with the 1971 water year, water data for streamflow, water quality, and ground water is published in official U.S. Geological Survey reports on a State-boundary basis. These official reports carry an identification number consisting of the two-letter State postal abbreviation, the last two digits of the water year, and the volume number. Onset of snowmelt and streamflow in 2004 in the Western Unites States: How shading may affect spring streamflow timing in a warmer world

Historic streamflow records show that the onset of snowfed streamflow in the western United States has shifted earlier over the past 50 yr, and March 2004 was one of the earliest onsets on record. Record high temperatures occurred throughout the western United States during the second week of March, and U.S. Geological Survey (USGS) stream gauges throughout the area recorded early onsets of streamflow at this time. However, a set of nested subbasins in Yosemite National Park, California, told a more complicated story. In spite of high air temperatures, many streams draining high-elevation basins did not start flowing until later in the spring. Temperatures during early March 2004 were as high as temperatures in late March 2002, when streams at all of the monitored Yosemite basins began flowing at the same time. However, the March 2004 onset occurred before the spring equinox, when the sun was lower in the sky. Thus, shading and solar radiation differences played a much more important role in 2004, leading to differences in streamflow timing. These results suggest that as temperatures warm and spring melt shifts earlier in the season, topographic effects will play an even more important role than at present in determining snowmelt timing. ?? 2006 American Meteorological Society.

Nineteenth Century Long-Term Instrumental Records, Examples From the Southeastern United States

Early instrumental records in the United States, defined as those operating before 1892 which is regarded the period prior to the modern climate record, provide a longer perspective of climatic variability at decadal and interannual timescales. Such reconstructions also provide a means of verification for other proxy data. This paper provides a American perspective of historical climatic research, emphasizing the urgent need to properly evaluate data quality and provide necessary corrections to make them compatible with the modern record. Different fixed observation times, different practices of weather instrument exposures, and statistical methods for calibration are the main issues in applying corrections and conducting proper climatic interpretations. I illustrate several examples on methodologies of this historical climatic research, focusing on the following in the Southeastern United States: daily reconstructed temperature time-series centered on Charleston SC and Natchez MS back to the late eighteenth century, and precipitation frequency reconstructions during the antebellum period for the Gulf Coast and coastal Southeast Atlantic states. Results indicate several prominent extremes unprecedented as compared to the modern record, such as the widespread warm winter of 1827-28, and the severe cold winters of 1856 and 1857. The reconstructions also yield important information concerning responses to past ENSO events, the PNA, NAO, and the PDO, particularly when compared with instrumental data from other regions. A high potential also exists for applying the climate reconstructions to assess historical climatic impacts on society in the Southeast, such as to understand climatic linkages to famous case studies of Yellow Fever epidemics and severe drought.

Wind energy is a rapidly growing form of renewable energy in the United States. While summary information on the total amounts of installed capacity are available by state, a free, centralized, national, turbine-level, geospatial dataset useful for scientific research, land and resource management, and other uses did not exist. Available in multiple formats and in a web application, these public domain data provide industrial-scale onshore wind turbine locations in the United States up to March 2014, corresponding facility information, and turbine technical specifications. Wind turbine records have been collected and compiled from various public sources, digitized or position verified from aerial imagery, and quality assured and quality controlled. We assess the spatial distribution of the largest rainfall-generated streamflows from a database of 35,663 flow records composed of the largest 10% of annual peak flows from each of 14,815 U.S. Geological Survey stream gaging stations in the United States and Puerto Rico. High unit discharges (peak discharge per unit contributing area) from basins with areas of 2.6 to 26,000 km2 (1-10,000 mi2) are widespread, but streams in Hawaii, Puerto Rico, and Texas together account for more than 50% of the highest unit discharges. The Appalachians and western flanks of Pacific coastal mountain systems are also regions of high unit discharges, as are several areas in the southern Midwest. By contrast, few exceptional discharges have been recorded in the interior West, northern Midwest, and Atlantic Coastal Plain. Most areas of high unit discharges result from the combination of (1) regional atmospheric conditions that produce large precipitation volumes and (2) steep topography, which enhances precipitation by convective and orographic processes and allows flow to be quickly concentrated into stream channels. Within the conterminous United States, the greatest concentration of exceptional unit discharges is at the Balcones Escarpment of central Texas, where maximum U.S. rainfall amounts apparently coincide with appropriate basin physiography to produce many of the largest measured U.S. floods. Flood-related fatalities broadly correspond to the spatial distribution of high unit discharges, with Texas having nearly twice the average annual flood-related fatalities of any other state.

The annual exceedance probability (AEP) for 327 streamgages in the Northeastern United States were computed using annual peak streamflow data through 2011 and are included in this report. The 2011 peak streamflow for 129 of those streamgages was estimated to have an AEP of less than or equal to 1 percent. Almost 100 of these peak streamflows were a result of the flooding associated with Hurricane Irene in late August 2011. More extreme than the 1-percent AEP, is the 0.2-percent AEP. The USGS recorded peak streamflows at 31 streamgages that equaled or exceeded the estimated 0.2-percent AEP during 2011. Collectively, the USGS recorded peak streamflows having estimated AEPs of less than 1 percent in Connecticut, Delaware, Maine, Maryland, Massachusetts, Ohio, Pennsylvania, New Hampshire, New Jersey, New York, and Vermont and new period-of-record peak streamflows were recorded at more than 180 streamgages resulting from the floods of 2011.

Corisella tarsalis, Sigara lineata, Trichocorixa borealis, and Trichocorixa kanza were recently identified from Michigan and constitute new state records. These four species were collected from two of the Great Lakes or their connecting rivers and increase the number of corixids for Michigan to 47 species. We newly report the genus Corisella for Michigan. Although most abundant in the western United States and Canada, scattered Corisella records in the Midwest (Wisconsin, Ohio and Ontario, Canada) indicated there was a good probability of its occurrence in Michigan. One of the most severe outbreaks of yellow fever, a viral disease transmitted by the Aedes aegypti mosquito, affected the southern United States in the summer of 1878. The economic and human toll was enormous, and the city of Memphis, Tennessee, was one of the most affected. The authors suggest that as a consequence of one of the strongest El Niño episodes on record-that which occurred in 1877-78-exceptional climate anomalies occurred in the United States (as well as in many other parts of the world), which may have been partly responsible for the widespread nature and severity of the 1878 yellow fever outbreak.This study documents some of the extreme climate anomalies that were recorded in 1877 and 1878 in parts of the eastern United States, with particular emphasis on highlighting the evolution of these anomalies, as they might have contributed to the epidemic. Other years with major outbreaks of yellow fever in the eighteenth and nineteenth centuries also occurred during the course of El Niño episodes, a fact that appears not to have been noted before in the literature.

A recording device allows wireless interrogation to determine its identity and its state. The state indicates whether one or more physical or chemical events have taken place. In effect, the one or more physical or chemical events are recorded by the device. The identity of the device allows it to be distinguished from a number of similar devices. The recording device may be used in an array of devices that allows wireless probing by an interrogation unit. When probed, each device tells the interrogator who it is and what state it is in. The devices allow multiple use and the interrogator may use a logical reset to determine the state of each device. The interrogator can thus easily identify particular items in an array that have reached a particular condition. The device may record the status of each device in a database to maintain a history for each.

The National Geochronological Data Base (NGDB) was established by the United States Geological Survey (USGS) to collect and organize published isotopic (also known as radiometric) ages of rocks in the United States. The NGDB (originally known as the Radioactive Age Data Base, RADB) was started in 1974. A committee appointed by the Director of the USGS was given the mission to investigate the feasibility of compiling the published radiometric ages for the United States into a computerized data bank for ready access by the user community. A successful pilot program, which was conducted in 1975 and 1976 for the State of Wyoming, led to a decision to proceed with the compilation of the entire United States. For each dated rock sample reported in published literature, a record containing information on sample location, rock description, analytical data, age, interpretation, and literature citation was constructed and included in the NGDB. The NGDB was originally constructed and maintained on a mainframe computer, and later converted to a Helix Express relational database maintained on an Apple Macintosh desktop computer. The NGDB and a program to search the data files were published and distributed on Compact Disc-Read Only Memory (CD-ROM) in standard ISO 9660 format as USGS Digital Data Series DDS-14 (Zartman and others, 1995). As of May 1994, the NGDB consisted of more than 18,000 records containing over 30,000 individual ages, which is believed to represent approximately one-half the number of ages published for the United States through 1991. Because the organizational unit responsible for maintaining the database was abolished in 1996, and because we wanted to provide the data in more usable formats, we have reformatted the data, checked and edited the information in some records, and provided this online version of the NGDB. This report describes the changes made to the data and formats, and provides instructions for the use of the database in geographic information system (GIS) applications. The data are provided in .mdb (Microsoft Access), .xls (Microsoft Excel), and .txt (tab-separated value) formats. We also provide a single non-relational file that contains a subset of the data for ease of use.

A wireless recording device can be interrogated to determine its identity and its state. The state indicates whether a particular physical or chemical event has taken place. In effect, the physical or chemical event is recorded by the device. The identity of the device allows it to be distinguished from a number of similar devices. Thus the sensor device may be used in an array of devices that can be probed by a wireless interrogation unit. The device tells the interrogator who it is and what state it is in. The interrogator can thus easily identify particular items in an array that have reached a particular condition.

Prairie rattlesnake envenomation in 27 New World camelids.

Morbidity and case fatality from rattlesnake envenomation is regionally specific because of variability in relative toxicity of the species of snake encountered. A previous report of rattlesnake envenomation in New World camelids (NWC) from the western coastal United States documented high case fatality rates and guarded prognosis for survival. To describe clinical findings, treatments, and outcome of NWC with prairie rattlesnake (Crotalus viridis viridis) envenomation in the Rocky Mountain region of the United States. Twenty-seven NWC admitted to the Colorado State University Veterinary Teaching Hospital for evaluation of acute rattlesnake envenomation between 1992 and 2012. Medical records of NWC evaluated for rattlesnake envenomation as coded by the attending clinician and identified by a database search were reviewed retrospectively. Month of admission, signalment, area of bite, clinical and clinicopathologic data, treatments, and outcome were recorded. Twenty-five llamas and 2 alpacas were admitted for envenomation. Llamas were overrepresented compared to hospital caseload. The face was the most common site of envenomation, observed in 96% of recorded cases. Presenting clinical signs included fever, tachypnea, tachycardia, and respiratory distress. Nine animals required a tracheotomy. Median hospitalization time was 3 days and overall survival rate was 69%. Case fatality rate for prairie rattlesnake envenomation in NWC was lower than that reported in the Western coastal region of the United States and similar to that reported for prairie rattlesnake envenomation in horses. Records of streamflow can provide an account of climatic variation over a hydrologic basin. The ability to do so is conditioned on the absence of confounding factors that diminish the climate signal. A national data set of streamflow records that are relatively free of confounding anthropogenic influences has been developed for the purpose of studying the variation in surface-water conditions throughout the United States. Records in the U.S. Geological Survey (USGS) National Water Storage and Retrieval System (WATSTORE) data base for active and discontinued streamflow gaging stations through water year 1988 (that is, through September 30, 1988) were reviewed jointly with data specialists in each USGS District office. The resulting collection of stations, each with its respective period of record satisfying the qualifying criteria, is called the Hydro-Climatic Data Network, or HCDN. The HCDN consists of 1,659 sites throughout the United States and its territories, totaling 73,231 water years of daily mean discharge values. For each station in the HCDN, information necessary for its identification, along with any qualifying comments about the available record and a set of descriptive watershed characteristics are provided in tabular format in this report, both on paper and on computer disk (enclosed). For each station in the HCDN, the appropriate daily mean discharge values were compiled, and statistical characteristics, including monthly mean discharges and annual mean, minimum and maximum discharges, were derived. The discharge data values are provided in a companion report.

Recent studies indicate again that there is a deficit in the use of electronic health records (EHR) in German hospitals. Despite good arguments in favour of their use, such as the rapid availability of data, German hospitals shy away from a wider implementation. The reason is the high cost of installing and maintaining the EHRs, for the benefit is difficult to evaluate in monetary terms for the hospital. Even if a benefit can be shown it is not necessarily evident within the hospital, but manifests itself only in the health system outside. Many hospitals only manage to partly implement EHR resulting in increased documentation requirements which reverse their positive effect.In the United States, electronic medical records are also viewed in light of their positive impact on patient safety. In particular, electronic medication systems prove the benefits they can provide in the context of patient safety. As a result, financing systems have been created to promote the digitalisation of hospitals in the United States. This has led to a large increase in the use of IT systems in the United States in recent years. The Universitätsklinikum Eppendorf (UKE) introduced electronic patient records in 2009. The benefits, in particular as regards patient safety, are numerous and there are many examples to illustrate this position. These positive results are intended to demonstrate the important role EHR play in hospitals. A financing system of the ailing IT landscape based on the American model is urgently needed to benefit-especially in terms of patient safety-from electronic medical records in the hospital.
