Overview -- Cadmium is a soft, low-melting-point metal that has many uses. It is similar in abundance to antimony and bismuth and is the 63d element in order of crustal abundance. Cadmium is associated in nature with zinc (and, less closely, with lead and copper) and is extracted mainly as a byproduct of the mining and processing of zinc. In 2000, it was refined in 27 countries, of which the 8 largest accounted for two-thirds of world production. The United States was the third largest refiner after Japan and China. World production in 2000 was 19,700 metric tons (t) and U.S. production was 1,890 t. In the United States, one company in Illinois and another in Tennessee refined primary cadmium. A Pennsylvania company recovered cadmium from scrap, mainly spent nickel-cadmium (NiCd) batteries. The supply of cadmium in the world and in the United States appears to be adequate to meet future industrial needs; the United States has about 23 percent of the world reserve base.

The United States is the largest producer and consumer of diatomite in the world. In 2001, the United States produced about a third of the estimated global production of 1.95 million metric tons (Mt) of diatomite (Dolley, 2003). In any given year, the United States accounts for at least 50 percent of all the diatomite exported in the world (Roskill, 1994). Seven diatomite companies operating in the United States produce diatomite in various grades for a range of applications, including filtration, absorbents, fillers, insulation, and cement manufacture. Economic deposits of diatomite within the United States depend on variations in the physical and chemical properties between and within deposits, potential end uses, and proximity to suitable markets. On the basis of historical production figures, estimated U.S. diatomite-production capacity is currently about 800,000 metric tons per year (t/yr).

Predictors of Homonegativity in the United States and the Netherlands Using the Fifth Wave of the World Values Survey.

Using data from the fifth wave of the World Values Survey (WVS), I investigated negative attitude toward homosexual individuals in two countries-the United States and the Netherlands-to determine how factors associated with homonegativity in the United States compare with factors associated with homonegativity in the Netherlands. Logistic regression of survey responses from 2,299 participants from the United States (n = 1,249) and the Netherlands (n = 1,050) supported findings from previous research suggesting that homonegativity is more likely to occur in the United States than in the Netherlands, and that negative attitudes toward persons with AIDS and immigrants predicted homonegativity in both countries. Predictors of homonegativity in the United States included being male and being unemployed; in the Netherlands, being unhappy predicted homonegativity. How these findings inform social work policy and practice related to the lesbian, gay, bisexual, and transgender (LGBT) population, as well as suggestions for future research, are discussed.

Conducting a study covering more than two decades of United States defense policy and the development of doctrine towards the Third World is an ambitious project. Nevertheless, there is a continuum from President Kennedy to President Reagan in the decision-making process and application of resources towards United States involvement in the less developed world. The incumbent president is perhaps closer to Kennedy in his world view of the root causes of instability and unrest in the Third World, specifically communist inspired and supported subversive insurgency. This calls for examination in view of the significant variations in the emphasis given tomore » US security interests in the Third World from Kennedy to Reagan. Each president since Kennedy has been faced with the problem of subversive insurgencies in the less developed world, inspired and supported by the communists both internally and externally. The Soviet Union is on record in vowing to support so called Wars of National Liberation in the Third World. Each president has had to deal with such threats to US security interests in the Third World. To quantify the proportion of academic neurosurgeons practicing in the United States who acquired residency training outside of the United States and compare their training backgrounds and academic success with those who received their residency training in the United States. We identified 1338 clinically active academic neurosurgeons from 104 programs that participated in the neurosurgery residency match in the United States in January-February 2015. Their training backgrounds, current academic positions, and history of National Institutes of Health (NIH) grant awards between 2005 and 2014 were retrieved from publicly accessible sources. Eighty-four U.S. academic neurosurgeons (6.3%) received their residency training in 20 different countries outside of the United States/Puerto Rico, representing all major regions of the world. The majority trained in Canada (n = 48). We found no major differences between the foreign-trained and U.S.-trained neurosurgeons in male:female ratio, year of starting residency, proportion with positions in medical schools ranked in the top 15 by the U.S. News and World Report, general distribution of academic positions, and proportion with an NIH grant. Compared with U.S.-trained academic neurosurgeons, foreign-trained academic neurosurgeons had a significantly higher proportion of Ph.D. degrees (32.1% vs. 12.3%; P < 0.0001) and held more associate professorships (34.5% vs. 23.1%; P = 0.02). The academic practices of the foreign-trained neurosurgeons were widely distributed throughout the United States. A small group of U.S. academic neurosurgeons (6.3%) have acquired residency training outside of the United States, representing all major regions of the world. Their general demographic data and academic accomplishments are comparable to those of U.S.-trained neurosurgeons. Copyright © 2017 Elsevier Inc. All rights reserved.

Over the past 140 years, the close academic and clinical interactions in Internal Medicine between German-speaking countries and the United States have been through three distinct stages. From 1870 until the First World War, German medical research, teaching, and university organization served as a model for U.S. medical schools and practice. However, after World War I, medical education reforms were implemented in the U.S., and due also to radical economic and political changes at home, German medicine lost its pioneering role. Furthermore, many scientists and clinicians were forced to emigrate in the face of racial and political persecution in Germany and Austria. Since the Second World War, American medicine has grown further to become the world leader in research, training, and clinical practice. The earlier trend of American physicians studying abroad was thus reversed, with many of today's foremost German physicians completing clinical and research training in the United States.

Bibliometric indicators are used to assess research performance. The goal of this study was to explore publication output to construct a picture of orthopedics that may be beneficial to researchers and orthopedic specialists. All orthopedics articles published in 61 journals from 2000 to 2011 were retrieved from the Science Citation Index Expanded database. The numbers of articles, citations, authors, institutions, and journals were analyzed and subjected to quantitative and qualitative comparisons. The number of published orthopedics articles increased between 2000 and 2011. Articles published by authors from the United States always ranked first in number, although the United States' share is decreasing in the world literature. Authors from the United States published the most-cited articles and the most articles in journals with top-10 impact factors; moreover, the United States also had the greatest share of experts and highly ranked institutions. The United Kingdom, Germany, and Japan were always within the world's top 4 in terms of numbers of articles and citations. The shares of Germany, South Korea, and China among total orthopedics articles increased, especially that of China. In 2011, China ranked the fifth in the world, with its world share increasing from 0.64% in 2000 to 5.05% in 2011. However, China lags behind in average citations per article, top research institutions, and most prolific authors. According to the total citations per article, the University of Pittsburgh, Harvard University, and the Hospital for Special Surgery were the most prolific institutions. A survey of dermatophytes isolated from patients seeking medical advice was made from 1979 to 1981 in the United States. The survey included 54 locations with data from 40 cities and 2 states. Correlations of these data with that of the other localities of the world were made to illustrate the dynamic epidemiology of several common dermatophytes. The most often isolated dermatophyte in this survey was Trichophyton rubrum having 53.66% of the total for these three calendar years. In a chronological listing of ringworm infections caused by this organism, many areas of the world have reported similar increased incidence of this pathogen. Trichophyton tonsurans was isolated 27.85% of the total. A dramatic increase of this pathogen as a cause of tinea capitis has been observed in most cities of the United States. It has been isolated in 25 different countries of the world. The percentage of isolation of Trichophyton mentagrophytes was 8.56%. This percentage may not be near the true incidence of infection by this dermatophyte because the infections are mild and respond to treatment without the individual seeking medical advice. Since the 1950s the percentage of isolations of the total has dropped for T. mentagrophytes in the United States. Epidermophyton floccosum accounted for 4.36% of the total. In a few areas of the world it causes over 30% of the total of dermatophytoses. Microsporum canis was isolated 3.72% of the total in the United States. It has recently been reported to be the dominant agent of tinea capitis in several South American countries, Tucson, Arizona and Kuwait. Once the dominant pathogen of tinea capitis in children in the United States, it was replaced by Microsporum audouinii before 1960. Today in the United States, M. audouinii only accounts for 0.30% of the total. It is considered eliminated as a pathogen in England. In this survey, isolated less than 1.0% of the total were Microsporum gypseum. Microsporum ferrugineum , Microsporum nanum , Microsporum fulvum and Trichophyton schoenleinii . Trichophyton meginii and Trichophyton terrestre were reported isolated but no numerical data were available.

Consumption of iron and steel scrap and the health of the scrap industry depend directly on the health of the steelmaking industry. The United States, as well as most of the world, is expected to consume increasing amounts of scrap as a steadily increasing population demands more steel products. World resources of scrap should be sufficient for the foreseeable future. An estimated 75 million metric tons (Mt) of scrap was generated during 1998 in the United States, and 35 Mt of old scrap and 18 Mt of new scrap was consumed. The United States is not only the world's largest economy, but it is also one of the world's largest consumers of natural resources. The country, which is inhabited by some 5% of the world's population, uses roughly one-fifth of the global primary energy supply and 15% of all extracted materials. This article explores long-term trends and patterns of material use in the United States. Based on a material flow account (MFA) that is fully consistent with current standards of economy-wide MFAs and covers domestic extraction, imports, and exports of materials for a 135-year period, we investigated the evolution of the U.S. industrial metabolism. This process was characterized by an 18-fold increase in material consumption, a multiplication of material use per capita, and a shift from renewable biomass toward mineral and fossil resources. In spite of considerable improvements in material intensity, no dematerialization has happened so far; in contrast to other high-income countries, material use has not stabilized since the 1970s, but has continued to grow. This article compares patterns and trends of material use in the United States with those in Japan and the United Kingdom and discusses the factors underlying the disproportionately high level of U.S. per capita resource consumption. The Unites States Department of Defense (DoD) is viewed by many in the general public as a monolithic government entity whose primary purpose is to coordinate this country's ability to make war and maintain a military presence around the world. However, the DoD is in fact a multidimensional organization whose global impact is as expansive as it is varying and is responsible for far-reaching global health interventions. The United States has worked toward providing long-term care among host nation populations by providing training in several areas related to medicine, with positive results. These efforts can be built upon with substantial positive effects. Building health infrastructure and capacity around the world is essential. The DoD is the most generously funded agency in the world, and the resources at its disposal provide the opportunity to make great gains in the long term in terms of both health and security worldwide. With efficient and careful use of DoD resources, and partnerships with key non-governmental organizations with specialized knowledge and great passion, partnerships can be forged with communities around the world to ensure that public health is achieved in even the most underserved communities. A move toward creating sustainable health systems with long-term goals and measurable outcomes is an essential complement to the already successful disaster and emergency relief that the United States military already provides. By ensuring that communities around the world are both provided with access to the sustainable health care they need and that emergency situations can be responded to in an efficient way, the United States can serve its duty as a leader in sharing expertise and resources for the betterment and security of all humankind. Can Academic Medicine Lead the Way in the Refugee Crisis?

The world is currently in the midst of the largest refugee crisis since World War II, with the highest interval of mass displacement in recorded history according to the United Nations. The United States has pledged to maintain its position as one of the world's top resettlement countries in response to this crisis. These new immigrants will arrive with exceptional chronic and acute medical needs, including higher rates of behavioral health disorders. The author describes the health care challenges experienced by refugees seeking asylum in the United States and outlines the ways in which our health care system is currently deficient in helping refugee patients to overcome these challenges. He argues that the academic medical community can change this dynamic by standardizing and expanding instruction in cross-cultural competence and behavioral health screenings throughout the spectrum of medical education. Ensuring the long-term well-being of refugees in the United States, including meeting their mental health needs, will be the best inoculation against the risks of violent extremism which so many fear. With the absence of national leadership on this issue, academic medicine can and should lead the way.

Organ shortage is the major limitation to kidney transplantation in the developed world. Conversely, millions of patients in the developing world with end-stage renal disease die because they cannot afford renal replacement therapy-even when willing living kidney donors exist. This juxtaposition between countries with funds but no available kidneys and those with available kidneys but no funds prompts us to propose an exchange program using each nation's unique assets. Our proposal leverages the cost savings achieved through earlier transplantation over dialysis to fund the cost of kidney exchange between developed-world patient-donor pairs with immunological barriers and developing-world patient-donor pairs with financial barriers. By making developed-world health care available to impoverished patients in the developing world, we replace unethical transplant tourism with global kidney exchange-a modality equally benefitting rich and poor. We report the 1-year experience of an initial Filipino pair, whose recipient was transplanted in the United states with an American donor's kidney at no cost to him. The Filipino donor donated to an American in the United States through a kidney exchange chain. Follow-up care and medications in the Philippines were supported by funds from the United States. We show that the logistical obstacles in this approach, although considerable, are surmountable. © 2016 The American Society of Transplantation and the American Society of Transplant Surgeons.

Nested taxa-area curves for eastern United States floras

The slopes of log-log species-area curves have been studied extensively and found to be influenced by the range of areas under study. Two such studies of eastern United States floras have yielded species-area curve slopes which differ by more than 100%: 0.251 and 0.113. The first slope may be too steep because the flora of the world was included, and both may be too steep because noncontiguous areas were used. These two hypotheses were tested using a set of nested floras centered in Ohio and continuing up to the flora of the world. The results suggest that this set of eastern United States floras produces a log-log species-area curve with a slope of approximately 0.20 with the flora of the world excluded, and regardless of whether or not the floras are from nested areas. Genera- and family-area curves are less steep than species-area curves and show similar patterns. Taxa ratio curves also increase with area, with the species/family ratio showing the steepest slope.

Homicide, suicide, and unintentional firearm fatality: comparing the United States with other high-income countries, 2003.

Violent death is a major public health problem in the United States and throughout the world. A cross-sectional analysis of the World Health Organization Mortality Database analyzes homicides and suicides (both disaggregated as firearm related and non-firearm related) and unintentional and undetermined firearm deaths from 23 populous high-income Organization for Economic Co-Operation and Development countries that provided data to the World Health Organization for 2003. The US homicide rates were 6.9 times higher than rates in the other high-income countries, driven by firearm homicide rates that were 19.5 times higher. For 15-year olds to 24-year olds, firearm homicide rates in the United States were 42.7 times higher than in the other countries. For US males, firearm homicide rates were 22.0 times higher, and for US females, firearm homicide rates were 11.4 times higher. The US firearm suicide rates were 5.8 times higher than in the other countries, though overall suicide rates were 30% lower. The US unintentional firearm deaths were 5.2 times higher than in the other countries. Among these 23 countries, 80% of all firearm deaths occurred in the United States, 86% of women killed by firearms were US women, and 87% of all children aged 0 to 14 killed by firearms were US children. The United States has far higher rates of firearm deaths-firearm homicides, firearm suicides, and unintentional firearm deaths compared with other high-income countries. The US overall suicide rate is not out of line with these countries, but the United States is an outlier in terms of our overall homicide rate.

Before World War 1 both the United States of America and Australia had rather rudimentary medical systems. Enterprising practitioners in both countries tended to look to continental Europe (especially Germany) and the United Kingdom as places of medical renown. The outbreak of World War 1 changed this but also enabled doctors from both the United States and Australia to work in military hospitals in Europe. The interactions that occurred were important for their professional development and led to some close associations between American and antipodean doctors. Examples of these were the experiences of Harvey Cushing, a burgeoning American neurosurgeon, which he recounted in his diary. His commentary merits close inspection and analysis. Campaigns for state nursing registration in the United States and Great Britain have a prominent place in the historical scholarship on nursing professionalization; the closely related German campaign has received less scholarly attention. Applying a transnational perspective to these three national movements highlights the collaborative and interrelated nature of nursing reform prior to World War I and recognizes the important contribution of German nurses to this dialogue and agenda. Focusing particularly on the years 1909-12, this article depicts a generation of German, American, and British nurses who organized national and international nursing associations to realize state registration as a stepping stone to other markers of professional recognition, such as collegiate education, full political citizenship, social welfare, and labor legislation. However, the consequent reliance of these strategies on nation-states as arbiters of citizenship and professional status undermined the shared ideological foundation of international and national nursing leaders. This article contributes to a more multinational understanding of how these international nursing leaders transcended and were confined by the limits of their nation-states in the years leading up to World War I.

This paper analyses approaches to global education in the United States, the United Kingdom and Japan. The paper begins by looking at movements that preceded global education, such as education for international understanding, development education, multicultural education, and peace education. The rise and fall of these earlier movements is analysed in terms of the interplay between the international and domestic politics of particular countries. To identify the world views which underpinned these pedagogic forms, the author discusses various discontinuities between the period up to the 1990s and thereafter. It is suggested that fresh forms of global education are emerging in - and because of - the changed world of the late 20th and early 21st century.

With the collapse of the former Soviet Union and the emergence of the United Nations actively pursuing a peace keeping role in world affairs, the United States has been forced into a position as the world's leading peace enforcer. It is still a very dangerous world with seemingly never ending ideological, territorial, and economic disputes requiring the U.S. to maintain a credible deterrent posture in this uncertain environment. This has created an urgent need to rapidly transport large numbers of troops and equipment from the continental United States (CONUS) to any potential world trouble spot by means of a global range/mobility transport aircraft. The most recent examples being Operation Desert Shield/Storm and Operation Restore Hope. To meet this challenge head-on, a request for proposal (RFP) was developed and incorporated into the 1992/1993 AIAA/McDonnell Douglas Corporation Graduate Team Aircraft Design Competition. The RFP calls for the conceptual design and justification of a large aircraft capable of power projecting a significant military force without surface transportation reliance.

Overview -- Silver is one of the eight precious, or noble, metals; the others are gold and the six platinum-group metals (PGM). World mine production in 2001 was 18,700 metric tons (t) and came from mines in 60 countries; the 10 leading producing countries accounted for 86 percent of the total. The largest producer was Mexico, followed by Peru, Australia, and the United States. Placer mines yielded less than 1 percent of the national silver production. Silver was mined in 12 States, of which Nevada was by far the largest producer; it accounted for nearly one-third of the national total. The production of silver at domestic mines generated employment for about 1,100 mine and mill workers. The value of mined domestic silver was estimated to be $290 million. Of the nearly 27,000 t of world silver that was fabricated in 2001, about one-third went into jewelry and silverware, one-fourth into the light-sensitive compounds used in photography, and nearly all the remainder went for industrial uses, of which there were 7 substantial uses and many other small-volume uses. By comparison, 85 percent of the silver used in the United States went to photography and industrial uses, 8 percent to jewelry and silverware, and 7 percent to coins and medals. The United States was the largest consumer of silver followed by India, Japan, and Italy; the 13 largest consuming countries accounted for nearly 90 percent of the world total. In the United States, about 30 companies accounted for more than 90 percent of the silver fabricated. The consumption of silver for all fabrication uses is expected to grow slowly through the decade ending in 2010 at about 1.3 percent per year for the world and 2.4 percent per year for the United States. World and U.S. reserves and reserve bases are more than adequate to satisfy the demand for newly mined silver through 2010. The other components of supply will be silver recovered from scrap, silver from industrial stocks, and silver bullion that is sold into the market from commodity exchange and private stocks.

During the May 2016 World Health Assembly of 194 member states, the World Health Organization (WHO) announced the process of developing and launching emergency medical teams as a critical component of the global health workforce concept. Over 64 countries have either launched or are in the development stages of vetting accredited teams, both international and national, to provide surge support to national health systems through WHO Regional Organizations and the delivery of emergency clinical care to sudden-onset disasters and outbreak-affected populations. To date, the United States has not yet committed to adopting the emergency medical team concept in funding and registering an international field hospital level team. This article discusses future options available for health-related nongovernmental organizations and the required educational and training requirements for health care provider accreditation. The U.S. Geological Survey estimated volumes of technically recoverable, conventional petroleum resources resulting from reserve growth for discovered fields outside the United States that have reported in-place oil and gas volumes of 500 million barrels of oil equivalent or greater. The mean volumes were estimated at 665 billion barrels of crude oil, 1,429 trillion cubic feet of natural gas, and 16 billion barrels of natural gas liquids. These volumes constitute a significant portion of the world's oil and gas resources.

USGS world petroleum assessment 2000; new estimates of undiscovered oil and natural gas, including reserve growth, outside the United States

Oil and natural gas account for approximately 63 percent of the world’s total energy consumption. The U.S. Geological Survey periodically estimates the amount of oil and gas remaining to be found in the world. Since 1981, each of the last four of these assessments has shown a slight increase in the combined volume of identified reserves and undiscovered resources. The latest assessment estimates the volume of technically recoverable conventional oil and gas that may be added to the world's reserves, exclusive of the United States, in the next 30 years. The USGS World Petroleum Assessment 2000 reports an increase in global petroleum resources, including a 20-percent increase in undiscovered oil and a 14-percent decrease in undiscovered natural gas compared to the previous assessment (table 1). These results have important implications for energy prices, policy, security, and the global resource balance.

Less than two weeks after the launch of Sputnik I, the United States Information Agency conducted an informal analysis of public opinion on this subject. The analysis yielded four clear conclusions: (1) Soviet claims of scientific and technological superiority were widely accepted in the United States; (2) U.S. allies were concerned about a shift in the balance of military power; (3) the overall credibility of Soviet propaganda was greatly strengthened; and (4) American prestige was dealt a severe blow. The report also concluded that the near-hysteria in the United States in turn increased the level of concern in countries friendly to the United States. An evaluation is presented.

World War I catapulted the United States from traditional isolationism to international involvement in a major European conflict. Woodrow Wilson envisaged a permanent American imprint on democracy in world affairs through participation in the League of Nations. Amid these defining events, Wilson suffered a major ischemic stroke on October 2, 1919, which left him incapacitated. What was probably his fourth and most devastating stroke was diagnosed and treated by his friend and personal physician, Admiral Cary Grayson. Grayson, who had tremendous personal and professional loyalty to Wilson, kept the severity of the stroke hidden from Congress, the American people, and even the president himself. During a cabinet briefing, Grayson formally refused to sign a document of disability and was reluctant to address the subject of presidential succession. Wilson was essentially incapacitated and hemiplegic, yet he remained an active president and all messages were relayed directly through his wife, Edith. Patient-physician confidentiality superseded national security amid the backdrop of friendship and political power on the eve of a pivotal juncture in the history of American foreign policy. It was in part because of the absence of Woodrow Wilson's vocal and unwavering support that the United States did not join the League of Nations and distanced itself from the international stage. The League of Nations would later prove powerless without American support and was unable to thwart the rise and advance of Adolf Hitler. Only after World War II did the United States assume its global leadership role and realize Wilson's visionary, yet contentious, groundwork for a Pax Americana. The authors describe Woodrow Wilson's stroke, the historical implications of his health decline, and its impact on United States foreign policy.

In England during the late nineteenth century, intellectuals, especially Francis Galton, called for a variety of eugenic policies aimed at ensuring the health of the human species. In the United States, members of the Progressive movement embraced eugenic ideas, especially immigration restriction and sterilization. Indiana enacted the first eugenic sterilization law in 1907, and the US Supreme Court upheld such laws in 1927. State programs targeted institutionalized, mentally disabled women. Beginning in the late 1930s, proponents rationalized involuntary sterilization as protecting vulnerable women from unwanted pregnancy. By World War II, programs in the United States had sterilized approximately 60,000 persons. After the horrific revelations concerning Nazi eugenics (German Hereditary Health Courts approved at least 400,000 sterilization operations in less than a decade), eugenic sterilization programs in the United States declined rapidly. Simplistic eugenic thinking has faded, but coerced sterilization remains widespread, especially in China and India. In many parts of the world, involuntary sterilization is still intermittently used against minority groups.

Cases of plague continue to occur in various parts of the world, including the western United States, where plague is endemic among the wild rodent population. In 1980, a case of plague acquired in New Mexico and hospitalized in Nebraska illustrated the problem of plague occurring in persons traveling from the state in which they become infected to another state. Nine cases of plague in travelers were identified among the 166 cases of plague reported in the United States from 1950 to 1980. Physicians should be aware of natural plague foci in the western United States and should obtain a travel history from patients with an illness clinically compatible with plague.

Few charitable organizations have achieved the status of global recognition enjoyed by UNICEF, the United Nations Children's Fund, which embodies the international effort to provide for needy children the world over. Created because of its synchronicity with the United Nations' stated purpose—to maintain peace in the world—UNICEF launched its operations in 1946. Its founding, early operations and eventual restructuring reveal a great deal about concurrent political and economic events, but also provide keen insight into international ideas about who qualified for full citizenship in the post-war world. The consequences of UNICEF's policies, procedures and practices posed challenges to notions of citizenship for both women and children. It challenged citizenship not by questioning sex-specific gender roles, but by judiciously adhering to the United Nations' promise to create equality for men and women alike. UNICEF found itself in the unique position to be able to globalize definitions of what constituted full citizenship in any nation, due to its rapid expansion throughout the world. Through its programs, especially those related to health care, it not only challenged these roles in the West, but began over several decades to complicate the definition of citizenship as it became a forceful presence in Asia and Africa throughout the 1970s.

Enabling Housing Cooperatives: policy lessons from Sweden, India and the United States.

Housing cooperatives became active in urban areas in Sweden, India and the United States during the interwar period. Yet, after the second world war, while housing cooperatives grew phenomenally nationwide in Sweden and India, they did not do so in the United States. This article makes a comparative institutional analysis of the evolution of housing cooperatives in these three countries. The analysis reveals that housing cooperatives' relationship with the state and the consequent support structures explain the divergent evolution. Although the relationships between cooperatives and the state evolved over time, they can be characterized as embedded autonomy, overembeddedness and disembeddedness in Sweden, India and the United States respectively. Whereas the consequent support structures for housing cooperatives became well developed in Sweden and India, such structures have been weak in the United States. The article highlights the need for embedded autonomy and the need for supportive structures to enable the growth of housing cooperatives.

In the United States during the 1950's, polio was on the forefront of every provider and caregiver's mind. Today, most providers in the United States have never seen a case. The Global Polio Eradication Initiative (GPEI), which began in 1988 has reduced the number of cases by over 99%. The world is closer to achieving global eradication of polio than ever before but as long as poliovirus circulates anywhere in the world, every country is vulnerable. The global community can support the polio eradication effort through continued vaccination, surveillance, enforcing travel regulations and contributing financial support, partnerships and advocacy. UNAIDS executive director Peter Piot called for the United States to do more to support HIV prevention efforts internationally. Speaking at the United States Conference on AIDS, he said that U.S. groups will need to offer more technical assistance and advice to community-based organizations around the world if they ever hope to manage the epidemic. UNAIDS will issue model legislation which other countries can use to pattern their own anti-discrimination laws. Health Canada recently published a guide in which ways that community-based organizations in industrialized countries can assist their counterparts in the developing world are outlined. Overview -- Selenium, which is one of the chalcogen elements in group 16 (or 6A) of the periodic table, is a semiconductor that is chemically similar to sulfur for which it substitutes in many minerals and synthetic compounds. It is a byproduct of copper refining and, to a much lesser extent, lead refining. It is used in many applications, the major ones being a decolorizer for glass, a metallurgical additive to free-machining varieties of ferrous and nonferrous alloys, a constituent in cadmium sulfoselenide pigments, a photoreceptor in xerographic copiers, and a semiconductor in electrical rectifiers and photocells. Refined selenium amounting to more than 1,800 metric tons (t) was produced by 14 countries in 2000. Japan, Canada, the United States, and Belgium, which were the four largest producers, accounted for nearly 85 percent of world production. An estimated 250 t of the world total is secondary selenium, which is recovered from scrapped xerographic copier drums and selenium rectifiers; the selenium in nearly all other uses is dissipated (not recoverable as waste or scrap). The present selenium reserve bases for the United States and the world (including the United States), which are associated with copper deposits, are expected to be able to satisfy demand for selenium for several decades without difficulty.

The National Academy of Sciences recently published a detailed study of technical factors related to the Comprehensive Nuclear Test Ban Treaty (CTBT), with emphasis on those issues that arose when the Senate declined to ratify the Treaty in 1999. The study considered (1) the capacity of the United States to maintain confidence in the safety and reliability of its nuclear weapons without nuclear testing; (2) the capabilities of the international nuclear-test monitoring system; and (3) the advances in nuclear weapons capabilities that other countries might make through low-yield testing that might escape detection. Excluding political factors, the committee considered three possible future worlds: (1) a world without a CTBT; (2) a world in which the signatories comply with a CTBT; and (3) a world in the signatories evade its strictures within the limits set by the detection system. The talk and ensuing discussion will elaborate on the study. The principal conclusion of the report, based solely on technical reasons, is that the national security of the United States is better served with a CTBT in force than without it, whether or not other signatories conduct low level but undetected tests in violation of the treaty. Moreover, the study finds that nuclear testing would not add substantially to the US Stockpile Stewardship Program in allowing the United States to maintain confidence in the assessment of its existing nuclear weapons.

Monitoring the World Health Organization Global Target 2025 for Exclusive Breastfeeding: Experience From the United States.

Exclusive breastfeeding under 6 months, calculated from a single 24-hour recall among mothers of children 0 to 5 months of age, is a World Health Organization (WHO) indicator used to monitor progress on the 2025 global breastfeeding target. Many upper-middle-income and high-income countries, including the United States, do not have estimates for this indicator. Research aim: To describe the prevalence of exclusive breastfeeding under 6 months in the United States. We used a single 24-hour dietary recall from the National Health and Nutrition Examination Survey 2009-2012 to calculate the prevalence of exclusive breastfeeding under 6 months. We discuss our results in the context of routine breastfeeding surveillance, which is reported from a national survey with different methodology. Among children younger than 6 months, 24.4%, 95% confidence interval [17.6, 31.1], were exclusively breastfed the previous day. To our knowledge, this is the first estimate of the WHO indicator of exclusive breastfeeding under 6 months for the United States. This study supports the global surveillance and data strategy for reporting to the WHO on the 2025 target for exclusive breastfeeding.

HIV Transmission Dynamics Among Foreign-Born Persons in the United States.

In the United States (US), foreign-born persons are disproportionately affected by HIV and differ epidemiologically from US-born persons with diagnosed HIV infection. Understanding HIV transmission dynamics among foreign-born persons is important to guide HIV prevention efforts for these populations. We conducted molecular transmission network analysis to describe HIV transmission dynamics among foreign-born persons with diagnosed HIV. Using HIV-1 polymerase nucleotide sequences reported to the US National HIV Surveillance System for persons with diagnosed HIV infection during 2001-2013, we constructed a genetic distance-based transmission network using HIV-TRACE and examined the birth region of potential transmission partners in this network. Of 77,686 people, 12,064 (16%) were foreign born. Overall, 28% of foreign-born persons linked to at least one other person in the transmission network. Of potential transmission partners, 62% were born in the United States, 31% were born in the same region as the foreign-born person, and 7% were born in another region of the world. Most transmission partners of male foreign-born persons (63%) were born in the United States, whereas most transmission partners of female foreign-borns (57%) were born in their same world region. These finding suggests that a majority of HIV infections among foreign-born persons in our network occurred after immigrating to the United States. Efforts to prevent HIV infection among foreign-born persons in the United States should include information of the transmission networks in which these individuals acquire or transmit HIV to develop more targeted HIV prevention interventions.

In the days of the post 9/11 attacks in the United States and the response against terrorism, one must question whether the source and supply of medicines in America is safe and adequate. This article reviews a common thread that runs through the periods of national emergency brought on by war, beginning with the Revolutionary War and repeated in the Civil War, World War 1, and World War 2: access to sufficient safe medicines. The shortage of medicines challenged the birth of the new nation in the Revolutionary War and helped thwart its destruction by the withdrawal of the southern states in the Civil War. Similarly, access to natural medicine and patented synthetic chemicals created major challenges in both World War 1 and World War 2. While not the only factor in victory, medicines did play an important role. Two separate but intertwined questions are evident. The first is whether the United States can control and provide the supply of medicines needed for the health and safety of it citizens-military and civilian. The second is whether the quality, that is the safety and efficacy, can be assured whether the source of the medicine is domestic or imported.

Although insufficiently studied, schools of complementary and alternative medicine (CAM) provide substantial insight into the transformation of medicine in the United States. Scholars have suggested that the increasing acceptance of CAM is due to its alignment with biomedical models of professionalization, education, research, and practice. At West Coast University, students of acupuncture and Oriental medicine learn to straddle both Western and Eastern medical worlds through an increasingly science-oriented curriculum and the inculcation of professional values associated with West Coast University's emphasis upon integration with Western medicine as a means of achieving professional status and legitimacy vis-à-vis the dominant biomedical paradigm. The implications of integration with biomedicine for the identity of Chinese medicine are discussed: from the perspective of critical medical anthropology, integration reproduces biomedical hegemony; paving the way toward co-optation of Chinese medicine, the subordination of its practitioners, and, ultimately, the constraint of medical pluralism in the United States.

Investors view the opening of managed care in Latin America as a lucrative business opportunity. As public-sector services and social security funds are cut back, privatized, and reorganized under managed care, with the support of international lending agencies such as the World Bank, the effects of these reforms on access to preventive and curative services will hold great importance throughout the developing world. Many groups in Latin America are working on alternative projects that defend health as a public good, and similar movements have begun in Africa and Asia. Increasingly, this organizing is being recognized not only as part of a class struggle but also as part of a struggle against economic imperialism--which has now taken on the new appearance of rescuing less developed countries from rising health care costs and inefficient bureaucracies through the imposition of neoliberal managed-care solutions exported from the United States.

Japan has re-emerged in 2013 as one of the world's fastest-growing and largest photovoltaic (PV) markets (Renewable Energy World 2013). Year-over-year growth in Japanese installed capacity in the first half of 2013 was about 270%. Japan's PV market was on pace to triple in 2013 compared with 2012, with approximately 2.7 GW installed during the first half of 2013 (compared to about 1.6 GW in the United States). This places Japan's market among the world's largest PV markets, along with China, Germany, and the United States. The U.S. government has brought a new trade case against China over rare earth elements (REE) as well as tungsten and molybdenum, President Barack Obama announced on 13 March. Japan and the European Union also have taken similar actions against China about REEs, which are a group of 17 chemically similar metallic elements that are used in a variety of electronic, optical, magnetic, and catalytic applications. REEs are plentiful in the Earth's crust, although China currently has about 37% of the world's reserves and accounts for more than 95% of the world's production of the elements, according to the British Geological Survey. The United States has requested consultations with China at the World Trade Organization (WTO) concerning "China's unfair export restraints on rare earths, as well as tungsten and molybdenum," the Office of the United States Trade Representative announced in a 13 March statement.

This paper considers the macroscopic and microscopic statistical features of the top 500 firms in China, the United States and the world, denoted as China 500 (CH500), Fortune 500 (US500) and Fortune Global 500 (FG500). From a macroscopic perspective, the firm size distribution of each category, when measured by revenue, is steadily distributed over the observed period, even during periods of financial crises. As is evidenced by the Gini coefficient, divergences between firm scales are most significant for the CH500. From a microscopic perspective, the underlying micro-dynamics are volatile and often turbulent due to the exit and entry of firms as well as shifts in their revenues and ranks. Such fluctuations, or mobility, are visualized in rank/revenue/share clocks. We also propose a revenue/rank/share mobility index that is a quantitative measurement of mobility. Among these, we find that the share mobility acts as an effective indicator of economic status; where there is a share mobility spike, there is an ailing economy. The share mobility indexes indicate that the 2008 Financial Crisis had little impact on the Chinese economy, while it triggered violent changes in the top 500 firms in the United States and the world.

The uncertain availability of (99m)Tc has become a concern for nuclear medicine departments across the globe. An issue for the United States is that currently it is dependent on a supply of (99m)Tc (from (99)Mo) that is derived solely by production outside the United States. Since the United States uses half the world's (99)Mo production, the U.S. (99)Mo supply chain would be greatly enhanced if a producer were located within the United States. The fragility of the old (99)Mo supply chain is being addressed as new facilities are constructed and new processes are developed to produce (99)Mo without highly enriched uranium. The conversion to low-enriched uranium is necessary to minimize the potential misuse of highly enriched uranium in the world for nonpeaceful means. New production facilities, new methods for the production of (99)Mo, and a new generator elution system for the supply of (99m)Tc are currently being pursued. In the United States, while immunization is recognized as one of the most effective primary prevention services that improves health and well-being, adult immunization rates remain low and large gaps exist between national adult immunization goals and actual adult immunization rates. Closing these gaps requires a commitment by national leaders to a multifaceted national strategy to: (1) establish the value of adult vaccines in the eyes of the public, payers, policy makers, and health care professionals; (2) improve access to recommended adult vaccinations by improving the adult vaccine infrastructure in the United States and developing public-private partnerships to facilitate effective immunization behaviors; and (3) ensure fair and appropriate payment for adult immunization. Many of the situations that result in low adult immunizations rates in the United States also exist in many other countries around the world. Successful strategies to improve adult immunization coverage rates will result in reductions in morbidity, mortality, and healthcare costs. All medical and public health stakeholders must now collaborate to realize the significant health benefits that come with a strong adult immunization program. PMID:26091249

A primary characteristic of transportation in the United States is its high per capita energy consumption. The average US citizen consumes nearly five times as much energy for transportation as the average Japanese and nearly three times as much as the average citizen of France, Britain, or West Germany. The energy efficiency of US transportation has improved substantially over the past two decades (both absolutely and in comparison to Europe), and US travel volume has grown more slowly than in most of the developed world. However, the United States still consumes more than one-third of the world`s transport energy. Also,more » 96 percent of US transport energy is in the form of oil products. This is more oil than the United States produces, despite its position as one of the world`s largest oil producers. With current problems and expectation of continued growth in travel and energy use, Congress has increasingly turned to transportation energy conservation - in the form of improvements in the technical efficiency of travel, increases in load factors, reductions in travel demand, shifting to alternative fuels, and shifts to more efficient travel modes - as an important policy goal. For example, the Clean Air Amendments of 1990 incorporate transportation demand management as a critical tool in reducing urban air pollution. Legislation proposed in the 102d Congress sought rigorous new automobile and light truck fuel economy standards. With continued increases in U.S. oil imports, urban traffic congestion, and greenhouse gas emissions, and the failure of many urban areas to meet air quality standards, strong congressional interest in new energy conservation initiates is likely to continue.« less

OBJECTIVE: To evaluate total bone mineral density (BMD) and body composition (% fat) in world class women field hockey players, members of the 1996 United States Olympic team. METHODS: Whole body BMD (g/cm2) and relative body fatness (% fat) were assessed by dual energy x ray absorptiometry using a Lunar DPX-L unit with software version 1.3z. Body composition was also estimated by hydrostatic weighing and the sum of seven skinfolds. Results: Mean (SD) BMD was 1.253 (0.048) g/cm2 which is 113.2 (4.0)% of age and weight adjusted norms. Estimates of body composition from the three methods were similar (statistically non- significant): 16.1 (4.4)% fat from dual energy x ray absorptiometry, 17.6 (3.2)% from hydrostatic weighing, and 16.9 (2.6)% from the sum of seven skinfolds. Mean fat free mass was approximately 50 kg. CONCLUSIONS: The mean whole body BMD value for members of the 1996 United States Olympic women's field hockey team is one of the highest reported for any women's sports team. Moreover, the mean fat free mass per unit height was quite high and % fat was low. In this group of world class sportswomen, low % fat was not associated with low BMD.

A review of traffic-related air pollution exposure assessment studies in the developing world.

Exposure assessment studies in the developing world are important. Although recent years have seen an increasing number of traffic-related pollution exposure studies, exposure assessment data on this topic are still limited. Differences among measuring methods and a lack of strict quality control in carrying out exposure assessment make it difficult to generalize and compare findings between studies. In this article, exposure assessment studies carried out in the developing world on several traffic-related air pollutants are reviewed. These pollutants include particulate matter (PM), carbon monoxide (CO), nitrogen dioxide (NO(2)), volatile organic compounds (VOCs), and polycyclic aromatic hydrocarbons (PAHs). In addition, it discusses advantages and disadvantages of various monitoring methods (ambient fixed-site monitoring, microenvironment monitoring, and personal exposure assessment using portable samplers) for these pollutants in exposure assessment studies. Also included in this paper is a brief introduction of standards for these pollutants in ambient air or in occupational settings established by the United States Environmental Protection Agency (USEPA), the United States Occupational Safety and Health Administration (OSHA) and the World Health Organization (WHO). The review ends with a summary of the limitations and gaps in recent studies and suggestions for future research in the developing world.

The United States is the only nation in the world that continues to execute its youth. The use of the death penalty against those who committed crimes as children is an act contrary to American standards of decency and fairness, as well as international law. The adolescent brain has not fully developed before the age of 18 years of age. Thus children do not have the same emotional and mental capacity as adults. Although juveniles should be held accountable for their crimes, the United States must not impose this most extreme punishment. The medical profession must take a stand to stop the execution of juvenile offenders in the United States.

Two decades since the World Health Organization Regional Office for Europe published a report on health promotion in prison that stimulated further debate on the concept of the "health-promoting prison," this article discusses the extent to which the concept has translated to the United States. One predicted indicator of success for the health-promoting prison movement was the expansion of activity beyond European borders; yet 2 decades since the European model was put forward, there has been very limited activity in the United States. Science literacy is a concern of educators and policy makers in the United States and all over the world. Science literacy is defined by society and includes important knowledge for individuals that varies with culture and local knowledge systems. The technological societies of the western world have delegated the knowledge that underpins their everyday world to mechanics who know how their cars work, technicians who know how their computers work, and policy wonks who know how their individual choices and actions will affect the environment and their health. The scientific principles that frame and sculpt the technological world are invisible and mysterious to most people. A question for debate is whether or not this is a healthy situation or not, and if not, what to do about it. The panelists shared their prospects and challenges of building science literacy with individuals in the United States and with Tibetan monks. As they discussed their efforts working with these different populations, they shared lessons based on common issues and unique solutions based on local knowledge systems and communities of learners.
