Patients at Kenyan public hospitals are detained if their families cannot pay their medical bills. Access to health insurance and waiving procedures to prevent detention may be limited. This study explores the perspectives of health-care providers (HCP) on health-insurance access, waiving procedures, and hospital detention practices. A self-administered structured questionnaire was completed by 104 HCP (response rate 78%) involved in childhood cancer care. The perspectives of respondents were as follows: all children with cancer should have health insurance according to 96% of HCP. After parents apply for health insurance, it takes too long before treatment costs are covered (67% agree). Patients with childhood cancer without health insurance have a higher chance of abandoning treatment (82% agree). Hospitals should waive bills of all children with cancer when parents have payment difficulties (69% agree). Waiving procedures take too long (75%). Parents are scared by waiving procedures and may decide never to return to the hospital again (68%). Poor families delay visiting the hospital because they fear hospital detention and first seek alternative treatment (92%). When poor families finally come to the hospital, the disease is in advanced stage already (94%). Parents sometimes have to abandon their detained child at the hospital if they cannot pay hospital bills (68%). Detention of children at the hospital if parents cannot pay their medical bills is not approved by 84% of HCP. HCP acknowledge that access to health insurance needs improvement and that waiving procedures contribute to treatment abandonment. By far, most HCP disapprove of hospital detention practices. These factors warrant urgent attention and adjustment. © 2018 Wiley Periodicals, Inc.

Overcoming Innocents' Naiveté: Pre-interrogation Decision-making Among Innocent Suspects.

Suspects, especially innocent ones, are highly susceptible to waiving their interrogation rights. This research tested the ability of two strategies to overcome innocent suspects' willingness to waive their rights. One strategy was based on the social influence of scarcity (i.e., not constraining the pre-interrogation time limit). The other strategy focused on disrupting individuals' cognitive fluency during the decision-making process (i.e., violating their induced expectation of offering a waiver). Disrupting innocent individuals' cognitive fluency increased their willingness to invoke their rights and, notably, was not qualified by interactions with any other factors. However, scarcity did not influence individuals' pre-interrogation decision-making. Results also further established the association between innocent individuals' naïve mindset and their willingness to waive their rights - specifically, innocents' willingness to waive their rights increased with the strength of their just world beliefs. The theoretical and applied implications of these findings are discussed. The importance and benefit of reforming pre-interrogation protocols using fair and feasible strategies that would disrupt suspects' cognitive fluency are emphasized. In an age of high deductibles, many surgical candidates are requesting that practices reduce or waive the self-pay portion of the professional fee. These requests come with a risk to the practice. Third-party payer agreements often prohibit discounting of fees to patients. Claiming breach of contract and interference with actuarial calculations, some third-party payers have sued practices for waiving fees owed by their insureds. Only by having the proper policies in place may a practice safely engage in fee reductions for patients insured by an entity with whom the practice has a contractual relationship.

GCP inspections in Germany and Europe following the implementation of the Directive 2001/20/EC

Background: The implementation of the Clinical Trials Directive 2001/20/EC and the Good Clinical Practice Directive 2005/28/EC fundamentally restructured and harmonized the conduct of clinical trials in Europe. GCP inspections – which affect study sites, laboratories, sponsors and contract research organizations (CRO) alike – make up an important part of these regulations. A common understanding of how these regulations apply in daily life is however not always ensured. Methods: A working group of the Clinical Research/Quality Assurance subcommittee of the German Association of Research-Based Pharmaceutical Companies (VFA) was established to outline the regulatory requirements, the experience gathered with inspections by means of a survey and to set up guidance on how to manage an inspection. Results and conclusions: The survey, conducted with the help of 15 pharmaceutical companies within the VFA, included a total of 224 inspections (74 inspections in Germany, 150 from other European countries). Most frequent findings in and outside Germany were related to “documentation” (40.5% vs. 21.3%), “investigational new drugs” (16.2% vs. 14.7%), “drug safety” (13.5% vs. 8%) and “application for a clinical trial authorization” (5.4% vs. 12%). From a German perspective, key findings of this working group were the necessity for a clear differentiation of responsibilities between national and federal as well as international authorities, a harmonization of inspection procedures and topics, and a clarification of whether pre-study/on-study and pre-approval/post-approval GCP inspections of the federal higher authority are included in the “Zentralstelle der Länder für Gesundheitsschutz bei Arzneimitteln und Medizinprodukten” (ZLG) requirements. The survey illustrated, that inspections usually are conducted at the investigational site, and that most of the findings are well known and thus could be prevented by communicating and discussing audit results

Waiving away the chance of freedom: exploring why prisoners decide against applying for parole.

Little is known about inmates' decisions regarding seeking release on parole, even though many choose to waive their parole hearings. Using semistructured, in-depth interviews with 25 adult male parole-eligible inmates who had waived a parole hearing, we seek to better understand the reasons inmates forgo the possibility of parole. We frame the study in rational choice theory, which suggests that inmates balance the perceived costs and benefits of remaining in prison versus returning to the community in reaching their decisions. A large majority of inmates attributed the decision to waive their hearings, in part, to concerns about the hearing process (e.g., parole denial likely, fear of negative experience), and most also listed various reasons that made remaining in prison a better outcome compared with release on parole (e.g., prison easier than parole, fear of revocation). The findings suggest that inmates' parole waiver decisions involve multiple factors that merit further examination.

The Software Formal Inspections Guidebook is designed to support the inspection process of software developed by and for NASA. This document provides information on how to implement a recommended and proven method for conducting formal inspections of NASA software. This Guidebook is a companion document to NASA Standard 2202-93, Software Formal Inspections Standard, approved April 1993, which provides the rules, procedures, and specific requirements for conducting software formal inspections. Application of the Formal Inspections Standard is optional to NASA program or project management. In cases where program or project management decide to use the formal inspections method, this Guidebook provides additional information on how to establish and implement the process. The goal of the formal inspections process as documented in the above-mentioned Standard and this Guidebook is to provide a framework and model for an inspection process that will enable the detection and elimination of defects as early as possible in the software life cycle. An ancillary aspect of the formal inspection process incorporates the collection and analysis of inspection data to effect continual improvement in the inspection process and the quality of the software subjected to the process.

Steam generator tube inspection was first carried out in 1971 at Mihama Unit-1 that is first PWR plant in Japan, when the plant was brought into the first annual inspection. At that time, inspection was made on sampling basis, and only bobbin coil probe was used. After experiencing various kinds of tube degradations, inspection method was changed from sampling to all number of tubes, and various kinds of probes were used to get higher detectability of flaw. At present, it is required that all the tubes shall be inspected in their full length at each annual inspection using standard bobbinmore » coil probe, and some special probes for certain plants that have susceptibility of occurrence of flaw. Sleeve repaired portion is included in this inspection. As a result of analyses of eddy current testing data, all indications that have been evaluated to be 20% wall thickness or deeper shall be repaired by either plugging or sleeving, where flaw morphology is to be a wastage or wear. Other types of flaw such as IGA/SCC are not allowed to be left inservice when those indications are detected. These inspections are performed according to inspection procedures that are approved by regulatory authority. Actual inspections are witnessed by the Japan Power engineering and inspection corporation (JAPEIC)`s inspectors during data acquisition and analysis, and they issue inspection report to authority for review and approval. It is achieved high safety performance of steam generator through this method of inspections, however. some tube leakage problems were experienced in the past. To prevent recurrence of such events, government is conducting development and verification test program for new eddy current testing technology.« less

One use of pulsed thermography that has shown promise in aircraft inspection for some time is an inspection for disbonds in metallic structures. The FAA has funded research at Wayne State University in this area and Boeing identified a specific inspection requirement for disbonds on Boeing 747 aircraft. Laboratory and subsequent field testing monitored by the AANC has demonstrated the reliability of this type of inspection. As a result Boeing expects to approve a general fuselage disbond inspection procedure using pulsed thermography in the 2nd Quarter of 2001.

Bacillus anthracis, the causative agent of anthrax, is a spore forming and toxin producing rod-shaped bacterium that is classified as a category A bioterror agent. This pathogenic microbe can be transmitted to both animals and humans. Clinical presentation depends on the route of entry (direct contact, ingestion, injection or aerosolization) with symptoms ranging from isolated skin infections to more severe manifestations such as cardiac or pulmonary shock, meningitis, and death. To date, anthrax is treatable if antibiotics are administered promptly and continued for 60 days. However, if treatment is delayed or administered improperly, the patient's chances of survival are decreased drastically. In addition, antibiotics are ineffective against the harmful anthrax toxins and spores. Therefore, alternative therapeutics are essential. In this review article, we explore and discuss advances that have been made in anthrax therapy with a primary focus on alternative pre-approved and novel antibiotics as well as anti-toxin therapies. A literature search was conducted using the University of Manitoba search engine. Using this search engine allowed access to a greater variety of journals/articles that would have otherwise been restricted for general use. In order to be considered for discussion for this review, all articles must have been published later than 2009. The alternative pre-approved antibiotics demonstrated high efficacy against B. anthracis both in vitro and in vivo. In addition, the safety profile and clinical pharmacology of these drugs were already known. Compounds that targeted underexploited bacterial processes (DNA replication, RNA synthesis, and cell division) were also very effective in combatting B. anthracis. In addition, these novel compounds prevented bacterial resistance. Pre-fab buildings, which are modularly fully-built in factories, are a good example of this. Although this kind of building is quite new, the in situ inspection is documented using traditional tools, mainly based on paper annotations. Thus, the inspection process is not taking advantage of new technologies. In this paper, we present the preliminary results of the SIRAE project that aims to provide an Augmented Reality (AR) tool that can seamlessly aid in the regular processes of pre-fab building inspections to detect and eliminate the possible existing quality and energy efficiency deviations. In this regards, we show a description of the current inspection process and how an interactive tool can be designed and adapted to it. Our first results show the design and implementation of our tool, which is highly interactive and involves AR visualizations and 3D data-gathering, allowing the inspectors to quickly manage it without altering the way the inspection process is done. We report our experience at the University of Florida in which residents and fellows served as the inspection team for a College of American Pathologists (CAP) self-inspection. We aimed to determine whether the CAP self-inspection could serve as a learning opportunity for pathology residents and fellows. To prepare for the inspection, we provided a series of 4 lunchtime seminars covering numerous laboratory management topics relating to inspections and laboratory quality. Preparation for the inspection began approximately 4 months prior to the date of the inspection. The intent was to simulate a CAP peer inspection, with the exception that the date was announced. The associate residency program director served as the team leader. All residents and fellows completed inspector training provided by CAP, and the team leader completed the team leader training. A 20 question pre- and posttest was administered; additionally, an anonymous survey was given after the inspection. The residents' and fellows' posttest scores were an average of 15% higher than on the pretest ( P < .01). The surveys as well as subjective comments were overwhelmingly positive. In conclusion, the resident's and fellow's experience as an inspector during a CAP self-inspection was a useful tool to learn accreditation and laboratory management.

Quality inspection of parts is a major component to any project that requires hardware implementation. Keeping track of all of the inspection jobs is essential to having a smooth running process. By using HTML, the programming language ColdFusion, and the MySQL database, I created a web-based job management system for the 170 Mechanical Inspection Group that will replace the Microsoft Access based management system. This will improve the ways inspectors and the people awaiting inspection view and keep track of hardware as it is in the inspection process. In the end, the management system should be able to insert jobs into a queue, place jobs in and out of a bonded state, pre-release bonded jobs, and close out inspection jobs.

Aqueous radioactive wastes from Savannah River Site (SRS) separations and vitrification processes are contained in large underground carbon steel tanks. The 2007 inspection program revealed that the structural integrity and waste confinement capability of the Savannah River Site waste tanks were maintained. A very small amount of material had seeped from Tank 12 from a previously identified leaksite. The material observed had dried on the tank wall and did not reach the annulus floor. A total of 5945 photographs were made and 1221 visual and video inspections were performed during 2007. Colorectal cancer (CRC) screening with colonoscopy often requires expensive copayments from patients. The 2010 Patient Protection and Affordable Care Act mandated elimination of copayments for CRC screening, including colonoscopy, but little is known about the effects of copayment elimination on use. The University of Texas employee, retiree, and dependent health plan instituted and promoted a waiver of copayments for screening colonoscopies in fiscal year (FY) 2009; we examined the effects of removing cost sharing on colonoscopy use. We conducted a retrospective cohort study of 59,855 beneficiaries of the University of Texas employee, retiree, and dependent health plan, associated with 16 University of Texas health and nonhealth campuses, ages 50-64 years at any point in FYs 2002-2009 (267,191 person-years of follow-up evaluation). The primary outcome was colonoscopy incidence among individuals with no prior colonoscopy. We compared the age- and sex-standardized incidence ratios for colonoscopy in FY 2009 (after the copayment waiver) with the expected incidence for FY 2009, based on secular trends from years before the waiver. The annual incidence of colonoscopy increased to 9.5% after the copayment was waived, compared with an expected incidence of 8.0% (standardized incidence ratio, 1.18; 95% confidence interval, 1.14-1.23; P < .001). After adjusting for age, sex, and beneficiary status, the copayment waiver remained significantly associated with greater use of colonoscopy, with an adjusted hazard ratio of 1.19 (95% confidence interval, 1.12-1.26). Waiving copayments for colonoscopy screening results in a statistically significant, but modest (1.5%), increase in use. The DoD Counterdrug Technology Development Program has addressed the development and demonstration of technology to enhance nonintrusive inspection of small packages such as passenger baggage, commercially delivered parcels, and breakbulk cargo items. Within the past year they have supported several small package-class nonintrusive inspection system performance assessment activities. All performance assessment programs involved the use of a red/blue team concept and were conducted in accordance with approved assessment protocols. This paper presents a discussion related to the systematic performance assessment of small package-class nonintrusive inspection technologies, including transmission, backscatter and computed tomography x-ray imaging, and protocol-related considerations for the assessment of these systems.

A "NASA Tech Briefs" article describing an inspection tool and technique known as Optically Stimulated Electron Emission (OSEE) led to the formation of Photo Acoustic Technology, Inc. (PAT). PAT produces sensors and scanning systems which assure surface cleanliness prior to bonding, coating, painting, etc. The company's OP1000 series realtime pre-processing detection capability assures 100 percent surface quality testing. The technique involves brief exposure of the inspection surface to ultraviolet radiation. The energy interacts with the surface layer, causing free electrons to be emitted from the surface to be picked up by the detector. When contamination is present, it interferes with the electron flow in proportion to the thickness of the contaminant layer enabling measurement by system signal output. OP1000 systems operate in conventional atmospheres on all types of material and detect both organic and inorganic contamination.

The 2011 Varsity Medical Debate, between Oxford and Cambridge Universities, brought students and faculty together to discuss the waiving of patents for antiretroviral therapies in the developing world. With an estimated 29.5 million infected by Human Immunodeficiency Virus (HIV) in low- and middle-income countries and only 5.3 million of those being treated, the effective and equitable distribution of anti-retroviral therapy (ART) is an issue of great importance. The debate centred around three areas of contention. Firstly, there was disagreement about whether patents were the real barrier to the access of anti-retroviral therapy in the developing world. Secondly, there were differing views on the effectiveness of a patent pool. Thirdly, concerns were raised over the impact of waiving patents on research to produce new and better anti retro-viral drugs. PMID:21740573

Could FDA approval of pre-exposure prophylaxis make a difference? A qualitative study of PrEP acceptability and FDA perceptions among men who have sex with men.

The FDA has approved tenofovir-emtricitabine for use as HIV pre-exposure prophylaxis, but it is unknown how approval may affect PrEP acceptability among US men who have sex with men. We conducted 8 focus groups among 38 Rhode Island MSM, including 3 groups among 16 male sex workers and 5 groups among 22 men in the general MSM community. Participants reported wide-ranging beliefs regarding consequences and meanings of FDA approval. Some participants would not use PrEP without approval, while others perceived approval as irrelevant or less significant than other sources of information. Our results suggest that FDA approval sends a signal that directly shapes PrEP acceptability among some MSM, while indirect influences of approval may affect uptake by others. Efforts to educate MSM about PrEP can increase acceptability by incorporating information about FDA approval, and outreach strategies should consider how this information may factor into personal decisions about PrEP use.

The principal mechanism for developing orbital debris environment models, is to make observations of larger pieces of debris in the range of several centimeters and greater using radar and optical techniques. For particles that are smaller than this threshold, breakup and migration models of particles to returned surfaces in lower orbit are relied upon to quantify the flux. This reliance on models to derive spatial densities of particles that are of critical importance to spacecraft make the unique nature of the EFT-1's return surface a valuable metric. To this end detailed post-flight inspections have been performed of the returned EFT-1 backshell, and the inspections identified six candidate impact sites that were not present during the pre-flight inspections. This paper describes the post-flight analysis efforts to characterize the EFT-1 mission craters. This effort included ground based testing to understand small particle impact craters in the thermal protection material, the pre- and post-flight inspection, the crater analysis using optical, X-ray computed tomography (CT) and scanning electron microscope (SEM) techniques, and numerical simulations.

Under-vehicle autonomous inspection through undercarriage signatures

Increased threats to gate security have caused recent need for improved vehicle inspection methods at security checkpoints in various fields of defense and security. A fast, reliable system of under-vehicle inspection that detects possibly harmful or unwanted materials hidden on vehicle undercarriages and notifies the user of the presence of these materials while allowing the user a safe standoff distance from the inspection site is desirable. An autonomous under-vehicle inspection system would provide for this. The proposed system would function as follows: A low-clearance tele-operated robotic platform would be equipped with sonar/laser range finding sensors as well as a video camera. As a vehicle to be inspected enters a checkpoint, the robot would autonomously navigate under the vehicle, using algorithms to detect tire locations for weigh points. During this navigation, data would be collected from the sonar/laser range finding hardware. This range data would be used to compile an impression of the vehicle undercarriage. Once this impression is complete, the system would compare it to a database of pre-scanned undercarriage impressions. Based on vehicle makes and models, any variance between the undercarriage being inspected and the impression compared against in the database would be marked as potentially threatening. If such variances exist, the robot would navigate to these locations and place the video camera in such a manner that the location in question can be viewed from a standoff position through a TV monitor. At this time, manual control of the robot navigation and camera control can be taken to imply further, more detailed inspection of the area/materials in question. After-market vehicle modifications would provide some difficulty, yet with enough pre-screening of such modifications, the system should still prove accurate. Also, impression scans that are taken in the field can be stored and tagged with a vehicles's license plate number, and future

Inspections are widely used in health care as a means to improve the health services delivered to patients. Despite their widespread use, there is little evidence of their effect. The mechanisms for how inspections can promote change are poorly understood. In this study, we use a national inspection campaign of sepsis detection and initial treatment in hospitals as case to: (1) Explore how inspections affect the involved organizations. (2) Evaluate what effect external inspections have on the process of delivering care to patients, measured by change in indicators reflecting how sepsis detection and treatment is carried out. (3) Evaluate whether external inspections affect patient outcomes, measured as change in the 30-day mortality rate and length of hospital stay. The intervention that we study is inspections of sepsis detection and treatment in hospitals. The intervention will be rolled out sequentially during 12 months to 24 hospitals. Our effect measures are change on indicators related to the detection and treatment of sepsis, the 30-day mortality rate and length of hospital stay. We collect data from patient records at baseline, before the inspections, and at 8 and 14 months after the inspections. We use logistic regression models and linear regression models to compare the various effect measurements between the intervention and control periods. All the models will include time as a covariate to adjust for potential secular changes in the effect measurements during the study period. We collect qualitative data before and after the inspections, and we will conduct a thematic content analysis to explore how inspections affect the involved organisations. The study has obtained ethical approval by the Regional Ethics Committee of Norway Nord and the Norwegian Data Protection Authority.
