The issue of discharge from hospital ward to the streets is seldom explored in the literature, but all too commonly experienced by individuals experiencing psychiatric disorders. The Community University Research Alliance on Housing and Mental Health sought to determine how frequently people were discharged from psychiatric wards to shelters or the street in London, Ontario, Canada. A number of data sources were accessed to determine instances of discharges to shelters or the street. Data were analysed to determine the number of moves occurring between hospital and shelter or no fixed address. All datasets revealed the problem of discharge to shelters or the street occurred regularly. All data sources used have the difficulty of likely underestimating the extent of the problem. This type of discharge occurred at least 194 times in 2002 in London, Ontario, Canada. Policies that contribute to this problem include income-support policies, the reduction in psychiatric hospital beds and the lack of community supports. Without recognition, this problem is at risk of remaining invisible with no further improvements to the situation.

Background About 10 million children worldwide live or work on the street. International reports estimate the prevalence of substance use among street children to be between 25% - 90%, which is who were referredntal disorders and high-risk behaviors. Objectives The objective of this study was to report the outcomes of assisted withdrawal of opioid-dependent vulnerable children and adolescents who were referred to child and adolescent psychiatric ward of Ali Ebne Abitaleb hospital, an academic hospital in Zahedan city. Methods Clinical chart abstractions were performed on a convenience sample of 40 serial opioid-dependent street children and adolescents (mean age: 11.14 ± 3.6 years) who were referred to child and adolescent psychiatric ward of Ali Ebne Abitaleb treatment and research center from November 2014 to May 2015. The demographic data, drug use history, comorbid physical and psychiatric conditions, symptomatology of opioid withdrawal syndrome, pharmacotherapies and psychosocial services, length of hospital stay, and any adverse events were extracted from the patients’ files using a checklist developed by the authors. Results Twenty-four (60%) patients were male, and 16 (40%) were female. The main drug used by all patients was opioids. Heroin Kerack (which has a street name of crystal in southeast Iran) was the most common (75%) drug of use, followed by opium (10%) and opium residue (7.5%). None of the participants self-reported using injected drugs. The high rate of a lack of eligibility for guardianship was documented among parents (87.5%) mainly due to their use of illegal drugs. Musculoskeletal pain and diarrhea were the most common withdrawal symptoms of the patients upon admission. The mean length of stay was 10.8 (± 7.30) days, and no significant adverse events were reported during the symptomatic treatment of opioid withdrawal syndrome. Bandung’s temperature has been higher since last years. This phenomenon affects the level of thermal comfort in open space. One indicator that determines the thermal comfort level is the type of activity performed by the open space user. Riau Street and Dago Street are corridors that are often used by the people for strolling, jogging, shopping. Dago Street has special event every Sunday namely car free day. Both corridors have different orientation; Dago Street is North to South corridor while Riau Street’s is West to East. The goal of the study is to compare people’s perception of thermal comfort in both corridors. This research uses two methods, namely qualitative method and quantitative method. Based on the results of qualitative analysis found that the thermal conditions in Dago Street more comfortable than the Riau Street. The result of quantitative analysis found that the average PET (thermal comfort indices) value of Dago Street was at 27.5 °C PET and Riau Street 28.6 °C PET. Dago Street is considered more convenient because it has a lower PET value than Riau Street. The people perception of thermal comfort is very important to start the steps for designing the orientation of street in urban design.

Harm reduction and law enforcement in Vietnam: influences on street policing

Background and rationale The HIV epidemic in Vietnam has from its start been concentrated among injecting drug users. Vietnam instituted the 2006 HIV/AIDS Law which includes comprehensive harm reduction measures, but these are unevenly accepted and inadequately implemented. Ward police are a major determinant of risk for IDUs, required to participate in drug control practices (especially meeting quotas for detention centres) which impede support for harm reduction. We studied influences on ward level police regarding harm reduction in Hanoi to learn how to better target education and structural change. Methods After document review, we interviewed informants from government, NGOs, INGOs, multilateral agencies, and police, using semi-structured guides. Topics covered included perceptions of harm reduction and the police role in drug law enforcement, and harm reduction training and advocacy among police. Results Police perceive conflicting responsibilities, but overwhelmingly see their responsibility as enforcing drug laws, identifying and knowing drug users, and selecting those for compulsory detention. Harm reduction training was very patchy, ward police not being seen as important to it; and understanding of harm reduction was limited, tending to reflect drug control priorities. Justification for methadone was as much crime prevention as HIV prevention. Competing pressures on ward police create much anxiety, with performance measures based around drug control; recourse to detention resolves competing pressures more safely. There is much recognition of the importance of discretion, and much use of it to maintain good social order. Policy dissemination approaches within the law enforcement sector were inconsistent, with little communication about harm reduction programs or approaches, and an unfounded assumption that training at senior levels would naturally reach to the street. Background Sexual violence is a major public health concern as well as human rights violation. Homeless women are far more likely to experience violence of all sorts than women in general. The objective of this study is to assess the prevalence and consequence of rape, and explore the reasons and factors associated with rape among street females in Bahir-Dar town, North West Ethiopia. Methods This is a mixed method study which included: a survey of 395 street females age 15 – 49; key informant interview with 4 stakeholders; 5 case studies; one focus group of 10 street females and one focus group of 10 street males. Street females are those who spend most of their time on the street and who depend on the street for their life. Qualitative and quantitative data were assessed separately with thematic and statistical analysis respectively. Quantitative data was analyzed using SPSS version 16.0. Bivariate and Multivariate analysis were determined. Results Life time prevalence of rape was 24.3% and the prevalence of rape in the last year was 11.4%. Factors like females “off” the street [OR (95% CI) =6.2 (3.0, 12.9)], being a prostitute [OR (95% CI) = 4.1 (1.5, 11.1)] and age 15–29 [OR (95% CI) =3.5 (1.1, 11.2)] were significantly associated with rape. Most, 93.8% of the rapes were not reported to legal bodies. None of the victims used condom during the rape event. Only, 4 (4.2%) of the victims used emergency contraceptive method following the rape event. Out of the total of 96 victims of rape, 13 (19.1%) and 9 (13.2%) experienced unwanted pregnancy and induced abortion respectively. Beside, 38 (41%) and 15 (22%) victims claimed genital injury and unusual vaginal discharge respectively. Psychological consequence like, hating others by 34 (35.8%), fear and concern for HIV/AIDS by 44 (46.6%), guilt feeling by 28 (29.4%) and loss of interest in sexual activity by 28 (29.4%) of victims were reported. Field-measurements were conducted in an urban street canyon with an east-west orientation, and a height-to-width ratio H/W = 1 during cloudless summer weather in 2003 in Freiburg, Germany. This experimental work adds to the knowledge available on the microclimate of an urban canyon and its impact on human comfort. Air temperature T a , air humidity VP, wind speed v and direction dd were measured continuously. All short-wave and long-wave radiation fluxes from the 3D surroundings were also measured. The degree of comfort was defined in terms of physiologically equivalent temperature (PET). Furthermore, the data gathered within the canyon were compared to data collected by a permanent urban climate station with the aim of furthering the understanding of microclimatic changes due to street geometry. Changes in the meteorological variables T a , v and dd in the canyon in comparison to an unobstructed roof level location were found to be in good agreement with previous studies, i.e., a small increase of T a in the canyon adjacent to irradiated surfaces, and a good correlation of v and dd between canyon and roof levels. The daily dynamics of canyon facet irradiances and their impacts on the heat gained by a pedestrian were strongly dependent on street geometry and orientation. Thermal stress was mostly attributable to solar exposure. Under cloudless summer weather, a standing body was found to absorb, on average, 74% of heat in the form of long-wave irradiance and 26% as short-wave irradiance. Shading the pedestrian as well as the surrounding surfaces is, hence, the first strategy in mitigating heat stress in summer under hot conditions.

Ensuring the health of street-involved children is a growing public health challenge. These children are vulnerable, neglected, and rarely a priority for basic service providers and governments. Sizable populations of street-involved children are present in major urban areas worldwide and current trends in urbanization suggest these populations will grow in the coming years. Although migration offers employment and training opportunities, the health and wellbeing of children is negatively impacted by their interactions with the streets. However, systemic barriers may also prevent these children from achieving an adequate health status. The situation of street-involved children in Ghana, West Africa will be discussed. Concerns had been raised at clinical governance regarding the safety of our inpatient ward rounds with particular reference to: documentation of clinical observations and National Early Warning Score (NEWS), compliance with Trust guidance for venous thromboembolism (VTE) risk assessment, antibiotic stewardship, palliative care and treatment escalation plans (TEP). This quality improvement project was conceived to ensure these parameters were considered and documented during the ward round, thereby improving patient care and safety. These parameters were based on Trust patient safety guidance and CQUIN targets. The quality improvement technique of plan-do-study-act (PDSA) was used in this project. We retrospectively reviewed ward round entries to record baseline measurements, based on the above described parameters, prior to making any changes. Following this, the change applied was the introduction of a ward round template to include the highlighted important baseline parameters. Monthly PDSA cycles are performed, and baseline measurements are re-examined, then relevant changes were made to the ward round template. Documentation of baseline measurements was poor prior to introduction of the ward round template; this improved significantly following introduction of a standardised ward round template. Following three cycles, documentation of VTE risk assessments increased from 14% to 92%. Antibiotic stewardship documentation went from 0% to 100%. Use of the TEP form went from 29% to 78%. Following introduction of the ward round template, compliance improved significantly in all safety parameters. Important safety measures being discussed on ward rounds will lead to enhanced patient safety and will improve compliance to Trust guidance and comissioning for quality and innovation (CQUIN) targets. Ongoing change implementation will focus on improving compliance with usage of the template on all urology ward rounds.

Cloud Streets over the Bering Sea

NASA image captured January 4, 2012 Most of us prefer our winter roads free of ice, but one kind of road depends on it: a cloud street. Such streets formed over the Bering Sea in early January 2012, thanks to snow and ice blanketing the nearby land, and sea ice clinging to the shore. The Moderate Resolution Imaging Spectroradiometer (MODIS) on NASA’s Terra satellite captured this natural-color image of the cloud streets on January 4, 2012. Air blowing over frigid ice then warmer ocean water can lead to the development of parallel cylinders of spinning air. Above the upward cycle of these cylinders (rising air), small clouds form. Along the downward cycle (descending air), skies are clear. The resulting cloud formations resemble streets. This image shows that some of the cloud streets begin over the sea ice, but most of the clouds hover over the open ocean water. These streets are not perfectly straight, but curve to the east and west after passing over the sea ice. By lining up along the prevailing wind direction, the tiny clouds comprising the streets indicate the wind patterns around the time of their formation. To assess whether there is a difference in ENT knowledge amongst nurses caring for patients on a dedicated ENT ward and nurses caring for ENT patients in a similar hospital without a dedicated ENT ward. A test of theoretical knowledge of ENT nursing care was devised and administered to nurses working on a dedicated ENT ward and then to nurses working on generic non-subspecialist wards regularly caring for ENT patients in a hospital without a dedicated ENT ward. The test scores were then compared. A single specialist ENT/Maxillo-Facial/Opthalmology ward in hospital A and 3 generic surgical wards in hospital B. Both hospitals are comparable district general hospitals in the south west of England. Nursing staff working in hospital A and hospital B on the relevant wards were approached during the working day. 11 nurses on ward 1, 10 nurses on ward 2, 11 nurses on ward 3 and 10 nurses on ward 4 (the dedicated ENT ward). Each individual test score was used to generate an average score per ward and these scores compared to see if there was a significant difference. The average score out of 10 on ward 1 was 6.8 (+/-1.6). The average score on ward two was 4.8 (+/-1.6). The average score on ward three was 5.5 (+/-2.1). The average score on ward 4, which is the dedicated ENT ward, was 9.7 (+/-0.5). The differences in average test score between the dedicated ENT ward and all of the other wards are statistically significant. Nurses working on a dedicated ENT ward have an average higher score in a test of knowledge than nurses working on generic surgical wards. This difference is statistically significant and persists despite banding or training. © 2014 John Wiley & Sons Ltd.

A street nurse position in the rural and small-town interior of British Columbia has been addressing the needs of street-involved or otherwise marginalized client populations by bringing healthcare services to wherever those clients are, rather than waiting for the clients to seek care. The primary reason for a street outreach approach is that marginalized populations face a variety of barriers to accessing traditional healthcare services--barriers such as homelessness, mental health problems, criminal involvement, lack of transportation, lack of ability to pay for prescriptions, lack of specialized or knowledgeable providers and provider discrimination. In the rural street nurse program, the target population includes the usual street nurse populations of illegal drug users and sex trade workers, which are more hidden in small communities than in larger urban centres, creating the community denial that is a barrier to healthcare access. Yet another barrier is the co-locaton of services common in small communities, where public health clinics might share a building with police services, making marginalized clients reluctant to attend clinics. The rural street nurse collaborates with public health nurses and other care providers (mental health workers, social workers, etc) with collegial advice and support, making and receiving referrals, and generally assisting one another--the street nurse through his rapport with the marginalized individuals and the others with their specialized knowledge. Rural street services are delivered whereverthe clientsfeel comfortable: a school, a drop-in centre, a mall, a youth centre or simplythe street. Services provided include sexually transmitted infection testing, chlamydia treatments, pregnancy testing emergency contraception pills and assistance with filling out forms for financial support. Communication plays a crucial role in the management of medications. Ward rounds are sites where health professionals from different disciplines and patients come together to exchange medication information and make treatment decisions. This article examines power relations and spatial practices surrounding medication communication between patients and health professionals including doctors, nurses and pharmacists during ward rounds. Data were collected in two medical wards of a metropolitan teaching hospital in Melbourne, Australia. Data collection methods involved participant observations, field interviews, video-recordings, together with individual and group reflexive interviews. A critical discourse analysis was undertaken to identify the location sites where power relations were reproduced or challenged in ward rounds. Findings demonstrated that traditional medical hierarchies constructed the ways in which doctors communicated about medications during ward rounds. Nurses and pharmacists ventured into the ward round space by using the discourse of preparation and occupying a peripheral physical position. Doctors privileged the discourse of medication rationalization in their ward round discussions, competing with the discourse of inquiry taken up by patients and families. Ward rounds need to be restructured to provide opportunities for nurses and pharmacists to speak at dedicated times and in strategic locations. By critically reflecting upon the complex process of medication communication during ward rounds, greater opportunities exist for enhanced team communication among health professionals.

Residents of one street expressed concern about the number of incident cancers, following the installation of a nearby mobile phone base station. The investigation explored whether the base station could be responsible for the cancers. Data were collected from residents' medical records. GPs and oncologists provided further information. Ward-level cancer incidence and mortality data were also obtained, over four three-year time periods. A total of 19 residents had developed cancer. The collection of cancers did not fulfil the criteria for a cancer cluster. Standardized mortality ratios (SMRs) for all malignant neoplasms (excluding non-melanoma skin cancers) in females (1.38 (95% CI, 1.08-1.74)) and all persons (1.27 (CI, 1.06-1.51)) were significantly higher than in the West Midlands during 2001-3. There were no significant differences for colorectal, female breast and prostate cancers, for any time period. Standardized incidence ratios (SIRs) for non-melanoma skin cancers in males and all persons was significantly lower than in the West Midlands during 1999-2001, and significantly lower in males, females and all persons during 2002-4. We cannot conclude that the base station was responsible for the cancers. It is unlikely that information around a single base station can either demonstrate or exclude causality.

Little is known about how safe nurses feel on psychiatric wards across different European countries. This paper is aim to evaluate how ward safety is perceived by ward managers in Great Britain, Germany and Switzerland. We replicated a Swiss questionnaire study in Germany and Britain, which asked ward managers on adult psychiatric wards to give details about their ward including data on the management of aggression, staffing levels, staff training, standards and type of restraint used, alarm devices, treatment and management of aggression and the existence and perceived efficacy of standards (protocols, guidelines). The British sample had by far the highest staffing levels per psychiatric bed, followed by Switzerland and Germany. The British ward managers by far perceived violence and aggression least as a problem on their wards, followed by Germany and then Switzerland. British ward managers are most satisfied with risk management and current practice dealing with violence. German managers were most likely to use fixation and most likely to have specific documentation for coercive measures. Swiss wards were most likely to use non-specific bedrooms for seclusion and carry alarm devices. British wards were far more likely to have protocols and training for the treatment and management of violence, followed by Switzerland and Germany. British ward managers by far perceived violence and aggression to be a small problem on their wards compared with Swiss and German ward managers. This was associated with the availability of control and restraint teams, regular training, clear protocols and a lesser degree risk assessments, but not staffing levels.

Ward rounds are the traditional process by which clinical information is interpreted and management plans made in the inpatient setting and the only time during which patient-doctor interaction can reliably occur. Efforts to improve quality and safety have started looking at the ward round but this has mainly been in the acute medical setting. To begin the quality improvement process for Urological ward rounds. Twenty indicators thought to relate to quality were recorded for every weekday ward round by the Urology team for one month. Twenty ward rounds, 93 patient encounters, were reviewed. A consultant was present for 37% of the patient encounters. 84% of observation charts were reviewed; drug charts 28% and antibiotics 70%. Plans were communicated to the doctors, patient and nursing staff. All notes were typed directly onto the electronic system, 20% of notes were checked by the lead clinician. Mean time per patient was 6 minutes. By starting a discussion about ward rounds we aim to align the process with the broader values of the organisation. Ward rounds can be the cornerstone of delivering safe, clean and personal care and measuring this process is vital to understanding efforts to improve them.

Consumer perspectives in adolescent ward design.

The aim of the study was to gain an understanding of the environment and facilities that adolescents require in a purpose-built adolescent ward. People who are hospitalized have limited control over their care and environment. The experience of hospitalization is also considered a highly stressful event. It is essential that patients have an opportunity to participate in the planning and design of ward environments in order to reduce their stress and better accommodate their treatment needs. An interpretative qualitative design was used. Seven chronically ill adolescents were asked to design their own ward in order to obtain consumer input about what the perceived necessary facilities for adolescent wards. The adolescents provided clear information about the facilities that should be incorporated into an adolescent ward to maintain their privacy and independence. Further research needs to be conducted seeking consumer input, especially if it can enhance the stay of adolescent patients while they are hospitalized. Adolescent voice is more often than not represented by nurses, or other healthcare professionals in the patients' best interest. This practice acts on the assumption that the adult knows best and/or that adolescents are unable to articulate their own needs. Adolescent consumers contribute worthwhile recommendations to how a ward is run providing insight about their needs in the ward environment.

Objective To understand the meanings, through the description of experiences that street merchants have of their interactions with children in a street situation in downtown of Medellin in 2013. Methodology Qualitative investigation with ethnographic approach. Observations were made in the zone at different times of the day. Twelve interviews with street merchants were held in their work places. Records were kept in a field journal. Results The street merchants and children in street situations share the same space. Both groups try to survive and their relationship is mediated by the trust-distrust that is established between them. The coexistence generates ambivalent feelings. For the street merchants, children in street situations are the result of abandonment by the family, the state and society. They live in a hard world in which they are exposed to a series of risks that they must face. Conclusion The interaction between street merchants and children in street situations is good to the extent that conflicts are avoided, establishing norms of coexistence. The element that determines this relationship is trust-distrust. In trust, strong affective ties are generated, considering each other family. Distrust generates a preventative attitude.

The design characteristics of street canyons were investigated in Guangzhou in the hot-humid area of China, and the effects of the design factors and their interactions on pedestrian thermal comfort were studied by numerical simulations. The ENVI-met V4.0 (BASIC) model was validated by field observations and used to simulate the micrometeorological conditions and the standard effective temperature (SET) at pedestrian level of the street canyons for a typical summer day of Guangzhou. The results show that the micrometeorological parameters of mean radiant temperature (MRT) and wind speed play key roles in pedestrian thermal comfort. Street orientation has the largest contribution on SET at pedestrian level, followed by aspect ratio and greenery, while surface albedo and interactions between factors have small contributions. The street canyons oriented southeast-northwest or with a higher aspect ratio provide more shade, higher wind speed, and better thermal comfort conditions for pedestrians. Compared with the east-west-oriented street canyons, the north-south-oriented street canyons have higher MRTs and worse pedestrian thermal comfort due to their wider building spacing along the street. The effects of greenery change with the road width and the time of the day. Street canyon design is recommended to improve pedestrian thermal comfort. This study provides a better understanding of the effects of street canyon design on pedestrian thermal comfort and is a useful guide on urban design for the hot-humid area of China.

Kuala Lumpur has envisioning in becoming World Class City by the year 2020. Essential elements of form and function of the urban environment are streets. Streets showcase the community and connect people. It’s one of the most comfortable social environment that provides aesthetical and interaction pleasure for everyone. Classified as main shopping streets in the local Kuala Lumpur urban design guidelines, Jalan Masjid India (JMI) has its uniqueness of shopping experience and social interaction. This conceptual paper will study the physical and cultural characteristics of the street that will generate the street character by mapping its original characters. The findings will focus on strengthening the methodology applied to promote improvements in evaluating it as a great public space. Results will also contribute to understanding the overall site context, the street connectivity, and urban dynamics. This paper is part of a larger study that addresses on transforming the sociability of public space.

The role of simulation in surgical education, initially confined to technical skills and procedural tasks, increasingly includes training nontechnical skills including communication, crisis management, and teamwork. Research suggests that many preventable adverse events can be attributed to nontechnical error occurring within a ward context. Ward rounds represent the primary point of interaction between patient and physician but take place without formalized training or assessment. The simulated ward should provide an environment in which processes of perioperative care can be performed safely and realistically, allowing multidisciplinary assessment and training of full ward rounds. We review existing literature and describe our experience in setting up our ward simulator. We examine the facilities, equipment, cost, and personnel required for establishing a surgical ward simulator and consider the scenario development, assessment, and feedback tools necessary to integrate it into a surgical curriculum. Purpose - The purpose of this paper is to classify and describe the purpose of ward rounds, who attends each round and their role, and participants' perception of each other's role during the respective ward rounds. Design/methodology/approach - A literature review of face-to-face ward rounds in medical wards was conducted. Peer reviewed journals and government publications published between 2000 and 2014 were searched. Articles were classified according to the type of round described in the study. Purposes were identified using keywords in the description of why the round was carried out. Descriptions of tasks and interactions with team members defined participant roles. Findings - Eight round classifications were identified. The most common were the generalised ward; multidisciplinary; and consultant rounds. Multidisciplinary rounds were the most collaborative round. Medical officers were the most likely discipline to attend any round. There was limited reference to allied health clinicians and patient involvement on rounds. Perceptions attendees held of each other reiterated the need to continue to investigate teamwork. Practical implications - A collaborative approach to care planning can occur by ensuring clinicians and patients are aware of different ward round processes and their role in them. Originality/value - Analysis fulfils a gap in the literature by identifying and analysing the different ward rounds being undertaken in acute medical wards. It identifies the complexities in the long established routine hospital processes of the ward round.

Living in the cities poses many challenges for the vulnerable group of user especially women where they are exposed to many issues related to safety. With the changing of lifestyle and demands, women are expected to play multiple roles in the society and working is one of the tasks. When women are expected to be working as men do, they are no longer occupied at one place. Women nowadays travel on a daily basis and being in the streets is one of the important activities. With the influx of diverse group of people into the country, our streets are dominated by different types of people from different background. Due to these factors, there are possibilities of challenges and threats for users especially women. Therefore, city spaces especially the street become an important public realm for women. The design of the street should be able to make women feel safe as these are the public space where they spend time getting to and from work. The way women perceived their environment might be different from men especially when they fear of crime. Perception of safety will affect the quality of life where fear is an important psychological factor in human life. Living in fear will restrict human's freedom. Therefore, this study aimed to explore women's perception of safety in the streets of Kuala Lumpur. The study adopted a mixed-method approach of qualitative and quantitative in order to understand the safety perception among women that will later establish the relationship between built environment and human psychology. 120 respondents were selected randomly around Jalan Benteng, Jalan Tun Perak, Jalan Melaka and Jalan Melayu. Questionnaire survey forms were distributed and structured observation was conducted at interval period at these streets to examined and assess women's behavior. Finding shows that fear does affect women's perception and physical design of the streets are important in affecting their behavior.

To explore the experiences of participants involved in the implementation of the Productive Ward: Releasing Time to Care™ initiative in Ireland, identifying key implementation lessons. A large-scale quality improvement programme Productive Ward: Releasing Time to Care™ was introduced nationwide into Ireland in 2011. We captured accounts from ward-based teams in an implementation phase during 2013-14 to explore their experiences. Semi-structured, in-depth interviews with a purposive sample of 24 members of ward-based teams from nine sites involved in the second national phase of the initiative were conducted. Interviews were analysed and coded under themes, using a seven-stage iterative process. The predominant theme identified was associated with the implementation and management of the initiative and included: project management; training; preparation; information and communication; and participant's negative experiences. The most prominent challenge reported related to other competing clinical priorities. Despite the structured approach of Productive Ward: Releasing Time to Care™, it appears that overstretched and busy clinical environments struggle to provide the right climate and context for ward-based teams to engage and interact actively with quality improvement tools, methods and activities. Findings highlight five key aspects of implementation and management that will help facilitate successful adoption of large-scale, ward-based quality improvement programmes such as Productive Ward: Releasing Time to Care™. Utilising pre-existing implementation or quality frameworks to assess each ward/unit for 'readiness' prior to commencing a quality improvement intervention such as Productive Ward: Releasing Time to Care™ should be considered. © 2017 John Wiley & Sons Ltd.

Nysa, is one of the most important ancient cities of the region of Caria in Anatolia (Asia Minor), was built on the slopes on the sides of the stream called Tekkecik, in a 3 km North of Sultanhisar, East of Aydın city of Turkey. The buildings, streets and public squares of the ancient city were supported by vaulted substructures adapted to the topographic conditions. There are important ruins on the site from the Hellenistic and the Roman periods and the Byzantine era. Now the Greek theatre and its walled entrance are intact. Other important remains are the gymnasium, stadium, library, temple, nymphaeum, bridge a tunnel like structure and the Byzantine churches in the West; the agora, Bouleuterion / Gerontikon, and the Roman public baths in the East. The first aim of the study was to determine the locations of the buried streets thought grid system according to the some excavated results already in the east and west side of the Nysa. Therefore the GPR studies were realized selected areas, which could include the streets, around the Bouleuterion / Gerontikon and at the South of the library. Two dimensional (2D) GPR data were measured on the parallel GPR profiles spaced 1m apart on each part of the study areas using 250 MHz shielded antennas. The processed parallel profile data set was formed through a solid 3D view by aligning the profiles respectively. A simplified amplitude-color range and appropriate opacity function were constructed to activate buried remains. Interactive interpretation was done with time slices and transparent sub-blocks of the 3D volume by arranging an opaque function. It was known that the maximum amplitudes represented the remains. So, weak amplitude range around the zero amplitude was eliminated by giving zero opacity value and transparent 3D imaging was obtained. Street dust: implications for stormwater and air quality, and environmental through street sweeping.

Street dust represents a source of dual potential risk to stormwater and air quality. It has been well documented that street dust washes into local watersheds and can degrade water quality. Research has also demonstrated that ambient particulate matter (PM10) , which is associated with adverse health outcomes, can arise from resuspension of accumulated street dust. Furthermore, many contaminants, including metals, are present at higher concentrations in the smallest available particles, which are more likely to be resuspended in air and stormwater runoff. Although street cleaning is listed as a best management practice for storm water quality by the EPA, data are limited on the critical parameters (technology, environment, usage), which determine the effectiveness of any street cleaning program, particularly in the peer-reviewed literature. The purpose of the present study was to develop a comprehensive understanding of the efficacy of various street cleaning technologies and practices to protect both water quality and public health. Few studies have compared the effectiveness of street sweeping technologies to remove street dust. Unfortunately, the dearth of comprehensive data on exposure, contaminant concentrations, and efficacy of various sweeping technologies and strategies precludes developing quantitative estimates for potential risk to humans and the environment. Based on the few studies available, regenerative air street sweepers appear to provide the most benefit with regard to collection of small particles and prevention of re-entrainment. It is also clear from the available data that local conditions, climate, and specific needs are critical determinants of the ideal street sweeping strategy (technology, frequency, speed, targeted areas, etc.). Given the critical need for protection of water and air quality in rapidly expanding urban regions (e.g., megacities), further research is necessary to develop best practices for street dust management. Street-running LRT may not affect a neighbour's sleep

A comprehensive dynamic finite difference model and analysis was conducted simulating LRT running at the speed of 24 km/h on a city street. The analysis predicted ground borne vibration (GBV) to remain at or below the FTA criterion of a RMS velocity of 72 VdB (0.004 in/s) at the nearest residence. In the model, site-specific stratography and dynamic soil and rock properties were used that were determined from in situ testing. The dynamic input load from LRT vehicle running at 24 km/h was computed from actual measured data from Portland, Oregon's West Side LRT project, which used a low floor vehicle similar to the one proposed for the NJ Transit project. During initial trial runs of the LRT system, vibration and noise measurements were taken at three street locations while the vehicles were running at about the 20-24 km/h operating speed. The measurements confirmed the predictions and satisfied FTA criteria for noise and vibration for frequent events. This paper presents the analytical model, GBV predictions, site measurement data and comparison with FTA criterion.

Simulated ward round: reducing costs, not outcomes.

Distractions and interruptions on the ward pose substantial patient safety risks, but medical students receive little training on their management. Although there is some evidence that medical students can be taught how to manage distractions and interruptions in a simulated ward environment, the only model to date is based on individual feedback, which is resource-expensive, mitigating curricular integration. Our aim was to assess the educational utility of a cost-efficient approach to a patient safety-focused simulated ward round. Twenty-three of 55 final-year medical students took part in a cost-reduced simulated ward round. Costs were minimised by providing group rather than individualised feedback, thereby shortening the duration of each simulation and reducing the number of interruptions. The utility of the simulation was assessed via student evaluation and performance on a patient safety station of an objective structured clinical examination (OSCE). The direct costs of the simulation were more than 50 per cent lower per student compared with the original study, mostly as a result of a reduction in the time that faculty members took to give feedback. Students managed distractions better and received higher scores in the OSCE station than those who had not undergone the ward round. Group feedback was evaluated positively by most participants: 94 per cent of those who provided feedback agreed or strongly agreed that the simulation would make them a safer doctor and would improve their handling of distractions. Our aim was to assess the educational utility of a cost-efficient approach to a patient safety-focused simulated ward round DISCUSSION: The costs of a simulated ward round can be significantly reduced whilst maintaining educational utility. These findings should encourage medical schools to integrate ward simulation into curricula. © 2016 John Wiley & Sons Ltd.

Street racing: a neglected research area?

To review: (1) the extent and frequency of street racing and its consequences; (2) the characteristics of street racers; (3) explanatory theories for street racing; (4) the legal issues; and (5) the best methods of preventing street racing. Review of academic and other literature. Very limited official statistics are available on street racing offenses and related collisions, in part because of the different jurisdictional operational definitions of street racing and the ability of police to determine whether street racing was a contributing factor. Some data on prevalence of street racing have been captured through social surveys and they found that between 18.8 and 69.0 percent of young male drivers from various international jurisdictions have reported street racing. Moreover, street racing is found to be associated with other risky behaviors, substance abuse, and delinquent activities. The limited evidence available on street racing suggests that it has increased in the last decade. Street racing is a neglected research area and the time has come to examine the prevalence and causes of street racing and the effectiveness of various street racing countermeasures.

Gender differences in Brazilian street youth's family circumstances and experiences on the street.

It has been proposed that homeless street girls are more likely to be from dysfunctional families and exhibit psychological distress than homeless street boys, reflecting cultural factors that result in differential norms for male and female behavior. The current analysis examined whether male and female street youth in a mid-sized Brazilian city differed in their family circumstances and day-to-day functioning on the street. The opportunity sample consisted of 33 male (mean age 14.3, range 10-17 years) and 33 female (mean age 14.6, range 11-18 years) street youth who participated in a sentence completion task and structured interview examining family experiences and ability to meet daily needs on the street. Consistent with the cultural hypothesis, girls described more negative family backgrounds than boys (e.g., they were more likely to have left home because of conflict or abuse, and reported more negative relationships with their parents). However, girls' functioning on the street was similar to, and in some cases more positive than, that of boys (e.g., fewer girls reported police violence and begging to earn money). The few gender differences that emerged in this study offer little support for the cultural hypothesis. The findings make sense if local factors, such as services available for male and female youth and public response to street youth, are considered. It appears that street youth differ in their experiences depending on local circumstances, highlighting the need for caution in generalizing about the situation of street youth in different countries.

Patient violence against nurses in their work environments is a widespread global concern, particularly in the field of mental health care. A high prevalence of violent events impacts the well-being of nurses and may also impair overall ward climate. However, it has been proposed that nurses' use limited techniques to prevent patient violence, and, therefore, more comprehensive methods for dealing with patient violence are needed. There is still restricted understanding of the ward climate during the occurrence of a violent event as well as how these incidents could be more effectively prevented. This study aimed to explore nurses' experiences of violent events in psychiatric wards, give insight into ward climates and examine suggestions for violence prevention. This study employed a descriptive, exploratory design including focus groups (n = 5) and open-ended questions. The participants were registered and enrolled nurses (n = 22) working on three closed psychiatric in-patient wards in one Finnish hospital district. Focus groups were tape-recorded, transcribed and analyzed with inductive content analysis. Nurses' experiences of violent events included a variety of warning signs and high-risk situations which helped them to predict forthcoming violence. Patient-instigated violent events were described as complicated situations involving both nurses and patients. When the wards were overloaded with work or emotions, or if nurses had become cynical from dealing with such events, well-being of nurses was impaired and nursing care was complicated. Suggestions for violence prevention were identified, and included, for example, more skilled interaction between nurses and patients and an increase in contact between nurses and patients on the ward. This study revealed the complexity of violent events on psychiatric wards as well as the implications of these events on clinical practice development and training, administration and policy.
