Tule brood fall chinook were caught primarily in the British Columbia, Washington and northern Oregon ocean commercial fisheries. The up-river bright fall chinook contributed primarily to the Alaska and British Columbia ocean commercial fisheries and the Columbia River gillnet fishery. Contribution of Rogue fall chinook released in the lower Columbia River system occurred primarily in the Oregon ocean commercial and Columbia river gillnet fisheries Willamette spring chinook salmon contributed primarily to the Alaska andmore » British Columbia ocean commercial, Oregon freshwater sport and Columbia River gillnet fisheries. Restricted ocean sport and commercial fisheries limited contribution of the Columbia coho released in the Umatilla River that survived at an average rate of 1.05% and contributed primarily to the Washington, Oregon and California ocean sport and commercial fisheries and the Columbia River gillnet fishery. The 1987 to 1991 brood years of coho released in the Yakima River survived at an average rate of 0.64% and contributed primarily to the Washington, Oregon and California ocean sport and commercial fisheries and the Columbia River gillnet fishery. Survival rates of salmon and steelhead are influenced, not only by factors in the hatchery, disease, density, diet and size and time of release, but also by environmental factors in the river and ocean. As part of the Hanford Federal Facility Agreement and Consent Order negotiations (Ecology et al. 1994), the US Department of Energy, Richland Operations Office, the US Environmental Protection Agency, and the Washington State Department of Ecology agreed that liquid effluent discharges to the ground on the Hanford Site which affect groundwater or have the potential to affect ground would be subject to permitting under the structure of Chapter 173-216 (or 173-218 where applicable) of the Washington Administrative Code, the State Waste Discharge Permit Program. As a result of this decision, the Washington State Department of Ecology and the US Departmentmore » of Energy, Richland Operations Office entered into Consent Order No. DE 91NM-177, (Ecology and DOE-RL 1991). The Consent Order No. DE 91NM-177 requires a series of permitting activities for liquid effluent discharges. This document presents the State Waste Discharge Permit (SWDP) application for the 200-E Chemical Drain Field. Waste water from the 272-E Building enters the process sewer line directly through a floor drain, while waste water from the 2703-E Building is collected in two floor drains, (north and south) that act as sumps and are discharged periodically. The 272-E and 2703-E Buildings constitute the only discharges to the process sewer line and the 200-E Chemical Drain Field.« less

Introduction This report and accompanying map (Plate 1) presents information on the Defense Minerals Administration (DMA), Defense Minerals Exploration Administration (DMEA), and Office of Minerals Exploration (OME) mineral exploration programs in Washington. Under these programs, the federal government participated in the exploration costs for certain strategic and critical minerals. Federal funds for mineral exploration under the programs were available from 1950 to 1974, although limited funds for OME administrative work were continued until 1979. The report reviews the three programs, associated regulations, administrative procedures, and operational techniques. It also describes the various types of informative reports generated by the programs, lists mining properties in Washington that were involved in the exploration programs, and advises on location of compiled exploration information that resulted from the work.

The Hanford Site, located northwest of the city of Richland, Washington, houses reactors, chemical-separation systems, and related facilities used for the production of special nuclear materials, as well as for activities associated with nuclear energy development. The 300 Area of the Hanford Site contains reactor fuel manufacturing facilities and several research and development laboratories. The 3718-F Alkali Metal Treatment and Storage Facility (3718-F Facility), located in the 300 Area, was used to store and treat alkali metal wastes. Therefore, it is subject to the regulatory requirements for the storage and treatment of dangerous wastes. Closure will be conducted pursuant tomore » the requirements of the Washington Administrative Code (WAC) 173-303-610 (Ecology 1989) and 40 CFR 270.1. Closure also will satisfy the thermal treatment facility closure requirements of 40 CFR 265.381. Discount medical cards have come under increasing scrutiny by regulators and law enforcement officials as a result of mounting consumer-reported problems. For their study, the authors tested five cards available in the Washington, D.C., metro area; interviewed card company representatives, state attorneys general insurance regulators, and insurance agents; and reviewed court and administrative actions. While some cards provide a measure of value, other cards were found to have serious drawbacks, including: high-pressure sales tactics; misleading or inaccurate promotion; exaggerated claims of savings; difficulty finding participating doctors; and providers who failed to give cardholders promised discounts. Some discount card companies are seeking to reform the market through a trade association and voluntary code of conduct. Still, legislative and regulatory interventions will be needed to protect consumers in an unregulated and growing market.

This report is a compilation of previously published miscellaneous streamflow measurements made in Washington State by the U.S. Geological Survey between January 1961 and September 1985. It is a supplement to a volume of similar data for the period 1890 to January 1961. The data include stream name and stream to which it is tributary, latitude and longitude, county code, hydrologic unit code, land-line location, drainage area, and measurement dates and discharges. In general, the data sites are not at gaging stations; however, some data are given for gaging station sites during periods when the stations were not in operation. All data in this report have been entered into a computerized data base that includes the data for the period 1890 to January 1961. The data can be retrieved in a variety of ways, such as by county, by hydrologic unit code, by river basin , or by size of drainage area. (USGS)

This report documents radionuclide air emissions from the Hanford Site in 1998 and the resulting effective dose equivalent to the maximally exposed individual (MEI) member of the public. The report has been prepared in accordance with the Code of Federal Regulations, Title 40, Protection of the Environment, Part 61, National Emission Standards for Hazardous Air Pollutants (40 CFR 61), Subpart H: ''National Emission Standards for Emissions of Radionuclides Other than Radon from Department of Energy Facilities,'' and with the Washington Administrative Code Chapter 246247, Radiation Protection - Air Emissions. The federal regulations in 40 CFR 61, Subpart H, require themore » measurement and reporting of radionuclides emitted from Department of Energy facilities and the resulting offsite dose from those emissions. A standard of 10 mrem/yr effective dose equivalent (EDE) is imposed on them. The EDE to the MEI due to routine emissions in 1998 from Hanford Site point sources was 1.3 E-02 mrem (1.3 E-04 mSv). which is 0.13 percent of the federal standard. Chapter 246-247 of the Washington Administrative Code (WAC) requires the reporting of radionuclide emissions from all Department of Energy Hanford Site sources. The state has adopted into these regulations the 40 CFR 61 standard of 10 mrem/yr EDE. The EDE to the MEI attributable to diffuse and fugitive radionuclide air emissions from the Hanford Site in 1998 was 2.5 E-02 mrem (2.S E-04 mSv). This dose added to the dose from point sources gives a total for all sources of 3.8 E-02 mrem/yr (3.8 E-04 mSv) EDE. which is 0.38 percent of the 10 mrem/yr standard. An unplanned release on August 26, 1998, in the 300 Area of the Hanford Site resulted in a potential dose of 4.1 E-02 mrem to a hypothetical individual at the nearest point of public access to that area. This hypothetical individual was not the MEI since the wind direction on the day of the release was away from the MEI residence. The potential dose from the unplanned event was similar in magnitude to that from routine releases during 1998. Were the release from this unplanned event combined with routine releases, the total dose would be less than 1 percent of the 10 mrem/yr standard.« less

In 2002, the EPA amended 40 CFR 61 Subpart H and 40 CFR 61 Appendix B Method 114 to include requirements from ANSI/HPS N13.1-1999 Sampling and Monitoring Releases of Airborne Radioactive Substances from the Stack and Ducts of Nuclear Facilities for major emission points. Additionally, the WDOH amended the Washington Administrative Code (WAC) 246-247 Radiation protection-air emissions to include ANSI/HPS N13.1-1999 requirements for major and minor emission points when new permitting actions are approved. A result of the amended regulations is the requirement to prepare a written technical basis for the radiological air emission sampling and monitoring program. Genetic variation in steelhead (Salmo gairdneri) from the north coast of Washington

Steelhead (Salmo gairdneri) collected from various sites in nine drainages in northwestern Washington were genetically characterized at 65 protein-coding loci by starch-gel electrophoresis. Genetic differentiation within and among drainages was not significant, and genetic variation among drainages was much less than that reported in British Columbia; these results may be the consequence of gene flow from hatchery stocks that have been released in Washington since the 1940's. Allele frequencies varied significantly among year-classes (hence, genetic characterization studies must include data from several year-classes), and also between hatchery fish (including a stock developed with local wild fish) and wild fish, indicating that few wild fish have been successfully and routinely included in hatchery brood stocks. Conservation of genetic diversity along the north coast of Washington should be facilitated by reducing the numbers of hatchery fish that spawn in streams and by including wild fish in hatchery brood stocks.

Scanning for safety: an integrated approach to improved bar-code medication administration.

This is a review of lessons learned in the postimplementation evaluation of a bar-code medication administration technology implemented at a major tertiary-care hospital in 2001. In 2006, with a bar-code medication administration scan compliance rate of 82%, a near-miss sentinel event prompted review of this technology as part of an institutional recommitment to a "culture of safety." Multifaceted problems with bar-code medication administration created an environment of circumventing safeguards as demonstrated by an increase in manual overrides to ensure timely medication administration. A multiprofessional team composed of nursing, pharmacy, human resources, quality, and technical services formalized. Each step in the bar-code medication administration process was reviewed. Technology, process, and educational solutions were identified and implemented systematically. Overall compliance with bar-code medication administration rose from 82% to 97%, which resulted in a calculated cost avoidance of more than $2.8 million during this time frame of the project.

The authors surveyed Washington state dentists to gain an understanding of their participation in the Medicaid dental program, their willingness to learn more about the program and the degree of importance they attached to preventive care for preschool-aged children. They found that concerns about fees and administrative aspects predominated and concerns about client behaviors were expressed less often. Many dentists indicated a willingness to learn more about the program. These findings will be used to develop a plan to market the Medicaid program to Washington state dentists.

Health advocates are facing a sea-change in Washington. Comprehensive federal health care reform eluded the nation under the Clinton Administration, but incremental progress through state-based Medicaid expansions and new CHIP programs was widespread, giving rise to some innovative state efforts to cover more of the uninsured. Washington's support was instrumental in those efforts, but the second Bush Administration seems intent on reversing that direction. To protect--and build upon--these nascent Medicaid efforts, advocates will have to mount careful strategies in defense of Medicaid. This States of Health looks at the dangers that may lie ahead and suggests how advocates can respond to keep Medicaid moving in the right direction.

Sandia National Laboratories has been involved in designing, installing and evaluating security systems for various applications during the past 15 years. A systems approach to security that evolved from this experience was applied to aviation security for the Federal Aviation Administration. A general systems study of aviation security in the United States was concluded in 1987. One result of the study was a recommendation that an enhanced security system concept designed to meet specified objectives be demonstrated at an operational airport. Baltimore-Washington International Airport was selected as the site for the demonstration project which began in 1988 and will bemore » completed in 1992. Nurses' attitudes toward the use of the bar-coding medication administration system.

This study determines nurses' attitudes toward bar-coding medication administration system use. Some of the factors underlying the successful use of bar-coding medication administration systems that are viewed as a connotative indicator of users' attitudes were used to gather data that describe the attitudinal basis for system adoption and use decisions in terms of subjective satisfaction. Only 67 nurses in the United States had the chance to respond to the e-questionnaire posted on the CARING list server for the months of June and July 2007. Participants rated their satisfaction with bar-coding medication administration system use based on system functionality, usability, and its positive/negative impact on the nursing practice. Results showed, to some extent, positive attitude, but the image profile draws attention to nurses' concerns for improving certain system characteristics. The high bar-coding medication administration system skills revealed a more negative perception of the system by the nursing staff. The reasons underlying dissatisfaction with bar-coding medication administration use by skillful users are an important source of knowledge that can be helpful for system development as well as system deployment. As a result, strengthening bar-coding medication administration system usability by magnifying its ability to eliminate medication errors and the contributing factors, maximizing system functionality by ascertaining its power as an extra eye in the medication administration process, and impacting the clinical nursing practice positively by being helpful to nurses, speeding up the medication administration process, and being user-friendly can offer a congenial settings for establishing positive attitude toward system use, which in turn leads to successful bar-coding medication administration system use.

Validation of Carotid Artery Revascularization Coding in Ontario Health Administrative Databases.

The positive predictive value (PPV) of carotid endarterectomy (CEA) and carotid artery stenting (CAS) procedure and post-operative complication coding were assessed in Ontario health administrative databases. Between 1 April 2002 and 31 March 2014, a random sample of 428 patients were identified using Canadian Classification of Health Intervention (CCI) procedure codes and Ontario Health Insurance Plan (OHIP) billing codes from administrative data. A blinded chart review was conducted at two high-volume vascular centers to assess the level of agreement between the administrative records and the corresponding patients' hospital charts. PPV was calculated with 95% confidence intervals (CIs) to estimate the validity of CEA and CAS coding, utilizing hospital charts as the gold standard. Sensitivity of CEA and CAS coding were also assessed by linking two independent databases of 540 CEA-treated patients (Ontario Stroke Registry) and 140 CAS-treated patients (single-center CAS database) to administrative records. PPV for CEA ranged from 99% to 100% and sensitivity ranged from 81.5% to 89.6% using CCI and OHIP codes. A CCI code with a PPV of 87% (95% CI, 78.8-92.9) and sensitivity of 92.9% (95% CI, 87.4-96.1) in identifying CAS was also identified. PPV for post-admission complication diagnosis coding was 71.4% (95% CI, 53.7-85.4) for stroke/transient ischemic attack, and 82.4% (95% CI, 56.6-96.2) for myocardial infarction. Our analysis demonstrated that the codes used in administrative databases accurately identify CEA and CAS-treated patients. Researchers can confidently use administrative data to conduct population-based studies of CEA and CAS.

Health administrative databases may provide rich sources of data for the study of outcomes following burn. We aimed to determine the accuracy of International Classification of Diseases diagnoses codes for burn in a population-based administrative database. Data from a regional burn center's clinical registry of patients admitted between 2006-2013 were linked to administrative databases. Burn total body surface area (TBSA), depth, mechanism, and inhalation injury were compared between the registry and administrative records. The sensitivity, specificity, and positive and negative predictive values were determined, and coding agreement was assessed with the kappa statistic. 1215 burn center patients were linked to administrative records. TBSA codes were highly sensitive and specific for ≥10 and ≥20% TBSA (89/93% sensitive and 95/97% specific), with excellent agreement (κ, 0.85/κ, 0.88). Codes were weakly sensitive (68%) in identifying ≥10% TBSA full-thickness burn, though highly specific (86%) with moderate agreement (κ, 0.46). Codes for inhalation injury had limited sensitivity (43%) but high specificity (99%) with moderate agreement (κ, 0.54). Burn mechanism had excellent coding agreement (κ, 0.84). Administrative data diagnosis codes accurately identify burn by burn size and mechanism, while identification of inhalation injury or full-thickness burns is less sensitive but highly specific. Serious medication errors continue to exist in hospitals, even though there is technology that could potentially eliminate them such as bar code medication administration. Little is known about the degree to which the culture of patient safety is associated with behavioral intention to use bar code medication administration. Based on the Technology Acceptance Model, this study evaluated the relationships among patient safety culture and perceived usefulness and perceived ease of use, and behavioral intention to use bar code medication administration technology among nurses in hospitals. Cross-sectional surveys with a convenience sample of 163 nurses using bar code medication administration were conducted. Feedback and communication about errors had a positive impact in predicting perceived usefulness (β=.26, P<.01) and perceived ease of use (β=.22, P<.05). In a multiple regression model predicting for behavioral intention, age had a negative impact (β=-.17, P<.05); however, teamwork within hospital units (β=.20, P<.05) and perceived usefulness (β=.35, P<.01) both had a positive impact on behavioral intention. The overall bar code medication administration behavioral intention model explained 24% (P<.001) of the variance. Identified factors influencing bar code medication administration behavioral intention can help inform hospitals to develop tailored interventions for RNs to reduce medication administration errors and increase patient safety by using this technology.

In 1917 the Geological Survey moved into the newly built Interior Building near the White House in Washington, D. C. Twenty years later a larger building was constructed nearby to house the expanding U. S. Department of the Interior. Height restrictions reduced the planned size of the new building, and the Geological Survey was compelled to remain in the older Interior Building now identified as the General Services Administration Building. The Light-Water-Reactor (LWR) Aerosol Containment Experiments (LACE) are being performed in Richland, Washington, at the Hanford Engineering Development Laboratory (HEDL) under the leadership of an international project board and the Electric Power Research Institute. These tests have two objectives: (1) to investigate, at large scale, the inherent aerosol retention behavior in LWR containments under simulated severe accident conditions, and (2) to provide an experimental data base for validating aerosol behavior and thermal-hydraulic computer codes. Aerosol computer-code comparison activities are being coordinated at the Oak Ridge National Laboratory. For each of the six LACE tests, ''pretest'' calculations (for code-to-code comparisons) andmore » ''posttest'' calculations (for code-to-test data comparisons) are being performed. The overall goals of the comparison effort are (1) to provide code users with experience in applying their codes to LWR accident-sequence conditions and (2) to evaluate and improve the code models.« less
