Abstract The importance of discovering, describing and cataloguing poorly known species in herbarium collections is discussed. It is a spur to efforts at rediscovery and consequent conservation efforts. The problems faced in describing species from limited material are discussed and our methods and criteria in making a decision are described. Prospects for future novelties are briefly assessed. Fifteen new species are described and illustrated with line drawings and distribution maps: Ipomoea attenuata J.R.I. Wood & Scotland, I. cuscoensis J.R.I. Wood & P. Muñoz, I. dasycarpa J.R.I. 2005-08-01

Since 1996, Japan Research Institute Limited (JRI) has been providing a sheet metal forming simulation system called JSTAMP-Works packaged the FEM solvers of LS-DYNA and JOH/NIKE, which might be the first multistage system at that time and has been enjoying good reputation among users in Japan. To match the recent needs, "faster, more accurate and easier", of process designers and CAE engineers, a new metal forming simulation system JSTAMP-Works/NV is developed. The JSTAMP-Works/NV packaged the automatic healing function of CAD and had much more new capabilities such as prediction of 3D trimming lines for flanging or hemming, remote control of solver execution for multi-stage forming processes and shape evaluation between FEM and CAD. On the other way, a multi-stage multi-purpose inverse FEM solver HYSTAMP is developed and will be soon put into market, which is approved to be very fast, quite accurate and robust. Lastly, authors will give some application examples of user defined ductile damage subroutine in LS-DYNA for the estimation of material failure and springback in metal forming simulation.

James Ross Island (JRI) exposes a Miocene-Recent alkaline basaltic volcanic complex that developed in a back-arc, east of the northern Antarctic Peninsula. JRI has been the focus of several geological studies because it provides a window on Neogene magmatic processes and paleoenvironments. However, little is known about its internal structure. New airborne gravity data were collected as part of the first high-resolution aerogeophysical survey flown over the island and reveal a prominent negative Bouguer gravity anomaly over Mt Haddington. This is intriguing as basaltic volcanoes are typically associated with positive Bouguer anomalies, linked to underlying mafic intrusions. The negative Bouguer anomaly may be associated with a hitherto unrecognised low-density sub-surface body, such as a breccia-filled caldera, or a partially molten magma chamber.

This is a Final Report for a Joint Research Interchange (JRI) between NASA Ames Research Center and San Jose State University, Department of Meteorology. We present below a summary of progress made during the duration of this JRI. The focus of this JRI has been to investigate seasonal water vapor transport in the atmosphere of Mars and its effects on the planet's present climate. To this end, the primary task has been to adapt a new dynamical processor for the adiabatic tendencies of the atmospheric circulation into the NASA Ames Mars general circulation model (MGCM). Using identical boundary and initial conditions, several comparative tests between the new and old MGCMs have been performed and the nature of the simulated circulations have been diagnosed. With confidence that the updated version of the Ames MGCM produces quite similar mean and eddy circulation statistics, the new climate model is well poised as a tool to pursue fundamental questions related to the spatial and seasonal variations of atmospheric water vapor on Mars, and to explore exchanges of water with non-atmospheric reservoirs and transport within its atmosphere. In particular, the role of surface sources and sinks can be explored, the range of water-vapor saturation altitudes can be investigated, and plausible precipitation mechanisms can be studied, for a range of atmospheric dust loadings. Such future investigations can contribute to a comprehensive study of surface inventories, exchange mechanisms, and the relative importance of atmospheric transport Mars' water cycle. A listing of presentations made and manuscripts submitted during the course of this project is provided.

  8. Seven big strike-slip earthquakes

    2003-12-01

We examine seven large (Mw > 7) strike-slip earthquakes that occurred since the beginning of ERS 1 and 2 missions. We invert GPS observations and InSAR interferograms and azimuth offsets for coseismic slip distributions. We explore two refinements to the traditional least-squares inversion technique with roughness constraints. First, we diverge from the usual definition of ``roughness'' as the average roughness over the entire fault plane, and allow ``variable smoothing'' constraints. Variable smoothing allows our inversion to select models that are more complex in regions that are well-resolved by the data, while still damping regions that are poorly resolved. Second, we choose our smoothing parameters using the jR_i criterion. The jR_i criterion draws on the theory behind cross-validation and the bootstrap method. We examine the theoretical basis behind such methods and use an analytical approximation technique for linear problems. We provide maps of model variance and spatial averaging scale over the fault plane, to explicitly show which features in our slip models are robust. We examine the 1992 Landers (CA), 1995 Sakhalin (Russia), 1995 Kobe (Japan), 1997 Ardekul (Iran), 1997 Manyi (Tibet), 1999 Hector Mine (CA), and 2001 Kunlun (Tibet) earthquakes. We compare features of the slip distributions such as the depth distribution of slip, the inferred magnitude and the degree of heterogeneity of slip over the fault plane, as resolved by the available InSAR and GPS data. We end with a brief description of the data coverage required for future earthquakes of similar size if we want to infer some of the above quantities to within a given confidence interval. We describe both the number of InSAR scenes and the distribution of GPS points that would be required, based on theoretical treatments of the fault plane/data point geometry using the jR_i method.

  9. Insights into accumulation variability over the last 2000 years at James Ross Island, Antarctic Peninsula

    2016-12-01

The James Ross Island ice core, drilled to 364 m on the northern tip of the Antarctic Peninsula, preserves a climate record that spans beyond the Holocene period to the end of the last glacial maximum (LGM). Reanalysis of the ice core using high-resolution continuous flow analysis (CFA) highlighted errors in the identification of events of known age that had been used to constrain the earlier chronology. The new JRI2 chronology is annual layer counted to 300 years, with the remaining profile reconstructed using a new age-depth model that is tied to age horizons identified in the annual-layer counted WAIS Divide ice core record. An accurate age-depth profile requires reliable known-age horizons along the ice core profile. In addition, these allow us to determine a solution for the accumulation history and rate of compaction due to vertical strain. The accuracy of the known-age constraints used in JRI2 allows only a small uncertainty in the reconstruction of the most recent 2000 years of accumulation variability. Independently, the surface temperature profile has been estimated from the stable water isotope profile and calibrated to borehole temperature observations. We present the accumulation, vertical thinning and temperature history interpreted from the James Ross Island ice core for the most recent 2000 years. JRI2 reconstructions show accumulation variability on a decadal to centennial timescale up to 20% from the present-day mean annual accumulation rate of 0.63 m yr-1. Analysis of the accumulation profile for James Ross Island offers insight into the sensitivity of accumulation to a change in surface temperature, as well as the reliability of the assumed relationship between accumulation and surface temperature in climate reconstructions using stable water isotope proxies.

  10. An Intercomparison of the Dynamical Cores of Global Atmospheric Circulation Models for Mars

    1998-01-01

This is a Final Report for a Joint Research Interchange (JRI) between NASA Ames Research Center and San Jose State University, Department of Meteorology. The focus of this JRI has been to evaluate the dynamical 'cores' of two global atmospheric circulation models for Mars that are in operation at the NASA Ames Research Center. The two global circulation models in use are fundamentally different: one uses spherical harmonics in its horizontal representation of field variables; the other uses finite differences on a uniform longitude-latitude grid. Several simulations have been conducted to assess how the dynamical processors of each of these circulation models perform using identical 'simple physics' parameterizations. A variety of climate statistics (e.g., time-mean flows and eddy fields) have been compared for realistic solstitial mean basic states. Results of this research have demonstrated that the two Mars circulation models with completely different spatial representations and discretizations produce rather similar circulation statistics for first-order meteorological fields, suggestive of a tendency for convergence of numerical solutions. Second and higher-order fields can, however, vary significantly between the two models.

  12. Intraseasonal and Interannual Variability of Mars Present Climate

    NASA Astrophysics Data System (ADS)

    1996-01-01

This is a Final Report for a Joint Research Interchange (JRI) between NASA Ames Research Center and San Jose State University, Department of Meteorology. The focus of this JRI has been to investigate the nature of intraseasonal and interannual variability of Mars'present climate. We have applied a three-dimensional climate model based on the full hydrostatic primitive equations to determine the spatial, but primarily, the temporal structures of the planet's large-scale circulation as it evolves during a given seasonal advance, and, over multi-annual cycles. The particular climate model applies simplified physical parameterizations and is computationally efficient. It could thus easily be integrated in a perpetual season or advancing season configuration, as well as over many Mars years. We have assessed both high and low-frequency components of the circulation (i.e., motions having periods of Omicron(2-10 days) or greater than Omicron(10 days), respectively). Results from this investigation have explored the basic issue whether Mars' climate system is naturally 'chaotic' associated with nonlinear interactions of the large-scale circulation-regardless of any allowance for year-to-year variations in external forcing mechanisms. Titles of papers presented at scientific conferences and a manuscript to be submitted to the scientific literature are provided. An overview of a areas for further investigation is also presented.

  15. Soil thermal properties at two different sites on James Ross Island in the period 2012/13

    NASA Astrophysics Data System (ADS)

    2015-04-01

James Ross Island (JRI) is the largest island in the eastern part of the Antarctic Peninsula. Ulu Peninsula in the northern part of JRI is considered the largest ice free area in the Maritime Antarctica region. However, information about permafrost on JRI, active layer and its soil properties in general are poorly known. In this study, results of soil thermal measurements at two different sites on Ulu Peninsula are presented between 1 April 2012 and 30 April 2013. The study sites are located (1) on an old Holocene marine terrace (10 m a. s. l.) in the closest vicinity of Johann Gregor Mendel (JGM) Station and (2) on top of a volcanic plateau named Johnson Mesa (340 m a. s. l.) about 4 km south of the JGM Station. The soil temperatures were measured at 30 min interval using platinum resistance thermometers Pt100/8 in two profiles up to 200 cm at JGM Station and 75 cm at Johnson Mesa respectively. Decagon 10HS volumetric water content sensors were installed up 30 cm at Johnson Mesa to 50 cm at JGM Station, while Hukseflux HFP01 soil heat flux sensors were used for direct monitoring of soil physical properties at 2.5 cm depth at both sites. The mean soil temperature varied between -5.7°C at 50 cm and -6.3°C at 5 cm at JGM Station, while that for Johnson Mesa varied between -6.9°C at 50 cm and -7.1°C at 10 cm. Maximum active layer thickness estimated from 0 °C isotherm reached 52 cm at JGM Station and 50 cm at Johnson Mesa respectively which corresponded with maximum observed annual temperature at 50 cm at both sites. The warmest part of both profiles detected at 50 cm depth corresponded with maximum thickness of active layer, estimated from 0°C isotherm, reached 52 cm at JGM Station and 50 cm at Johnson Mesa respectively. Volumetric water content at 5 cm varied around 0.25 m3m-3 at both sites. The slight increase to 0.32 m3m-3 was observed at JGM Station at 50 cm and at Johnson Mesa at 30 cm depth. Soil texture analysis showed distinctly higher share of coarser fraction >2 mm at Johnson Mesa than at JGM Station. Comparison of both sites indicated that mean ground temperature at 50 cm depth was higher by 1.2 °C at JGM station, although the active layer was thicker by 2 cm only. It can therefore be concluded that soil physical properties like texture and moisture may significantly affect thermal regime at boundary between AL and permafrost table during individual thawing seasons.

Correct estimation of the firn lock-in depth is essential for correctly linking gas and ice chronologies in ice core studies. Here, two approaches to constrain the firn depth evolution in Antarctica are presented over the last deglaciation: outputs of a firn densification model, and measurements of δ15N of N2 in air trapped in ice core, assuming that δ15N is only affected by gravitational fractionation in the firn column. Since the firn densification process is largely governed by surface temperature and accumulation rate, we have investigated four ice cores drilled in coastal (Berkner Island, BI, and James Ross Island, JRI) and semi-coastal (TALDICE and EPICA Dronning Maud Land, EDML) Antarctic regions. Combined with available ice core air- δ15N measurements from the EPICA Dome C (EDC) site, the studied regions encompass a large range of surface accumulation rates and temperature conditions. Our δ15N profiles reveal a heterogeneous response of the firn structure to glacial-interglacial climatic changes. While firn densification simulations correctly predict TALDICE δ15N variations, they systematically fail to capture the large millennial-scale δ15N variations measured at BI and the δ15N glacial levels measured at JRI and EDML - a mismatch previously reported for central East Antarctic ice cores. New constraints of the EDML gas-ice depth offset during the Laschamp event (41 ka) and the last deglaciation do not favour the hypothesis of a large convective zone within the firn as the explanation of the glacial firn model- δ15N data mismatch for this site. While we could not conduct an in-depth study of the influence of impurities in snow for firnification from the existing datasets, our detailed comparison between the δ15N profiles and firn model simulations under different temperature and accumulation rate scenarios suggests that the role of accumulation rate may have been underestimated in the current description of firnification models.

This work aimed to engineer a multi-station shoulder simulator in order to wear test shoulder prostheses using recognized shoulder activities of daily living. A bespoke simulator was designed, built and subject to commissioning trials before a first wear test was conducted. Five JRI Orthopaedics Reverse Shoulder VAIOS 42 mm prostheses were tested for 2.0 million cycles and a mean wear rate and standard deviation of 14.2 ± 2.1 mm(3)/10(6) cycles measured for the polymeric glenoid components. This result when adjusted for prostheses diameters and test conditions showed excellent agreement with results from hip simulator studies of similar materials in a lubricant of bovine serum. The Newcastle Shoulder Simulator is the first multi-station shoulder simulator capable of applying physiological motion and loading for typical activities of daily living. © IMechE 2016.

  5. Continued rapid glacier recession following the 1995 collapse of the Prince Gustav Ice Shelf on the Antarctic Peninsula (Invited)

    NASA Astrophysics Data System (ADS)

    Glasser, N. F.; Scambos, T. A.

    2009-12-01

We use optical satellite imagery (ASTER and Landsat) to document changes in the Prince Gustav Ice Shelf (PGIS) and its tributary glaciers before and after its 1995 collapse. Interpretation of a pre-collapse Landsat 4-5 TM image acquired in February 1988 shows that the ice shelf was fed primarily by Sjogren Glacier from the Antarctic Peninsula and by Rhoss Glacier from James Ross Island (JRI). In 1988, the PGIS contained numerous structural discontinuities (rifts and crevasses), which collectively indicate that ice-shelf break-up had commenced at least seven years before collapse. Meltwater ponds and streams were also common across its surface. After the ice shelf collapsed, Rhoss Glacier became a tidewater glacier and has since experienced rapid and continued recession. Between January 2001 and December 2006 (six to eleven years after the collapse of the PGIS), the front of Rhoss Glacier receded a total of 13.6 km. We conclude that where tributary glaciers become tidewater glaciers they react to ice-shelf removal by rapid and continued recession and that the response time of glaciers on the Antarctic Peninsula to ice-shelf removal is measured on annual to decadal timescales. This rapid recession, coupled with previously documented tributary glacier thinning and acceleration, indicates that Antarctic Peninsula glaciers are extremely sensitive to ice-shelf collapse.

  7. Micro-Pressure Sensors for Future Mars Missions

    1996-01-01

The joint research interchange effort was directed at the following principal areas: u further development of NASA-Ames' Mars Micro-meteorology mission concept as a viable NASA space mission especially with regard to the science and instrument specifications u interaction with the flight team from NASA's New Millennium 'Deep-Space 2' (DS-2) mission with regard to selection and design of micro-pressure sensors for Mars u further development of micro-pressure sensors suitable for Mars The research work undertaken in the course of the Joint Research Interchange should be placed in the context of an ongoing planetary exploration objective to characterize the climate system on Mars. In particular, a network of small probes globally-distributed on the surface of the planet has often been cited as the only way to address this particular science goal. A team from NASA Ames has proposed such a mission called the Micrometeorology mission, or 'Micro-met' for short. Surface pressure data are all that are required, in principle, to calculate the Martian atmospheric circulation, provided that simultaneous orbital measurements of the atmosphere are also obtained. Consequently, in the proposed Micro-met mission a large number of landers would measure barometric pressure at various locations around Mars, each equipped with a micro-pressure sensor. Much of the time on the JRI was therefore spent working with the engineers and scientists concerned with Micro-met to develop this particular mission concept into a more realistic proposition.

The authors further analyzed results from the Women's Health Initiative randomized trials (1993–2004) of conjugated equine estrogens, with or without medroxyprogesterone acetate, focusing on health benefits versus risks among women who initiated hormone therapy soon after menopause. Data from the Women's Health Initiative observational study (1993–2004) were included in some analyses for additional precision. Results are presented here for incident coronary heart disease, stroke, venous thromboembolism, breast cancer, colorectal cancer, endometrial cancer, or hip fracture; death from other causes; a summary global index; total cancer; and total mortality. Hazard ratios for breast cancer and total cancer were comparatively higher (P < 0.05) among women who initiated hormone therapy soon after menopause, for both regimens. Among these women, use of conjugated equine estrogens appeared to produce elevations in venous thromboembolism and stroke and a reduction in hip fracture. Estrogen plus progestin results among women who initiated use soon after menopause were similar for venous thromboembolism, stroke, and hip fracture but also included evidence of longer-term elevations in breast cancer, total cancer, and the global index. These analyses provide little support for the hypothesis of favorable effects among women who initiate postmenopausal estrogen use soon after menopause, either for coronary heart disease or for health benefits versus risk indices considered. 1999-01-01

This report describes a three-month research program undertook jointly by the Robotics Institute at Carnegie Mellon University and Ames Research Center as part of the Ames' Joint Research Initiative (JRI.) The work was conducted at the Ames Research Center by Mr. Liam Pedersen, a graduate student in the CMU Ph.D. program in Robotics under the supervision Dr. Ted Roush at the Space Science Division of the Ames Research Center from May 15 1999 to August 15, 1999. Dr. Martial Hebert is Mr. Pedersen's research adviser at CMU and is Principal Investigator of this Grant. The goal of this project is to investigate and implement methods suitable for a robotic rover to autonomously identify rocks and minerals in its vicinity, and to statistically characterize the local geological environment. Although primary sensors for these tasks are a reflection spectrometer and color camera, the goal is to create a framework under which data from multiple sensors, and multiple readings on the same object, can be combined in a principled manner. Furthermore, it is envisioned that knowledge of the local area, either a priori or gathered by the robot, will be used to improve classification accuracy. The key results obtained during this project are: The continuation of the development of a rock classifier; development of theoretical statistical methods; development of methods for evaluating and selecting sensors; and experimentation with data mining techniques on the Ames spectral library. The results of this work are being applied at CMU, in particular in the context of the Winter 99 Antarctica expedition in which the classification techniques will be used on the Nomad robot. Conversely, the software developed based on those techniques will continue to be made available to NASA Ames and the data collected from the Nomad experiments will also be made available.

We report the clinical and radiological outcome of 86 revisions of cemented hip arthroplasties using JRI-Furlong hydroxyapatite-ceramic-coated acetabular and femoral components. The acetabular component was revised in 62 hips and the femoral component in all hips. The mean follow-up was 12.6 years and no patient was lost to follow-up. The mean age of the patients was 71.2 years. The mean Harris hip and Oxford scores were 82 (59 to 96) and 23.4 (14 to 40), respectively. The mean Charnley modification of the Merle d'Aubigné and Postel score was 5 (3 to 6) for pain, 4.9 (3 to 6) for movement and 4.4 (3 to 6) for mobility. Migration of the acetabular component was seen in two hips and the mean acetabular inclination was 42.6 degrees. The mean linear polyethylene wear was 0.05 mm/year. The mean subsidence of the femoral component was 1.9 mm and stress shielding was seen in 23 (28%) with bony ingrowth in 76 (94%). Heterotopic ossification was seen in 12 hips (15%). There were three re-revisions, two for deep sepsis and one for recurrent dislocation and there were no re-revisions for aseptic loosening. The mean EuroQol EQ-5D description scores and health thermometer scores were 0.69 (0.51 to 0.89) and 79 (54 to 95), respectively. With an end-point of definite or probable loosening, the probability of survival at 12 years was 93.9% and 95.6% for the acetabular and femoral components, respectively. Overall survival at 12 years, with removal or further revision of either component for any reason as the end-point, was 92.3%. Our study supports the continued use of this arthroplasty and documents the durability of hydroxyapatite-ceramic-coated components.

    1998-01-01

Kim Pil Soon was born at Sorae Village of Hwang Hye Province, the birth place of the Protestantism in Korea. He was brought up under the strong influence of Christianity and received modern education at Pae Chae School according to the recommendation of Rev. Underwood. In 1899, Kim Pil Soon, who had been working at Je Joon Won as an assistant and interpreter of Dr. Sharrocks, was employed by Dr. Avison to help prepare medical textbooks and was asked to participate in the medical education. He acquired medical knowledge through his work of translating various medical texts, which enabled him to teach other medical students. He participated in the administration of the Hospital, taking charge of the provision of meals for in-patients as well as directing the construction of Severance Hospital buildings. His experience of treating soldiers wounded during the turmoil of the forced dismission of the Korean Army by the Japanese led him to reflect seriously on Korea's fate in peril. In addition, he became a member of Sinmin Society, a secret political association, to engage in the independence movement. In 1908, Kim Pil Soon graduated from Severance Hospital Medical School as one of the first seven graduates. On graduation, he was appointed as a professor and took the charge of school affairs in 1910. At first, he worked as an assistant physician of ward and surgery, then he took the responsibility of the outpatient clinic in 1911. But suddenly, in December 1911, he exiled to China to escape from the Japanese police who was in pursuit of him on account of his involvement in the so-called 105-Person Affair, a fabricated affair served as a pretext for the persecution of the independence movement. He continued the independence movement in the form of an ideal village movement and in the training of the Independence Army. In 1919, however, he was poisoned to death in a mysterious way. Kim Pil Soon dedicated himself to the independence movement that demanded personal sacrifice: giving up his prospective career as a doctor, professor, and hospital administrator. He no longer remained as an ordinary clinician who treats only diseased persons, but transformed himself to the Great Doctor, a time-old ideal type of doctor in the East Asian countries who treats and cures the diseased nation, by dedicating himself to the independence movement.

  18. Born Toon Soon: Preterm birth matters

    PubMed Central

    2013-01-01

Urgent action is needed to address preterm birth given that the first country-level estimates show that globally 15 million babies are born too soon and rates are increasing in most countries with reliable time trend data. As the first in a supplement entitled "Born Too Soon", this paper focuses on the global policy context. Preterm birth is critical for progress on Millennium Development Goal 4 (MDG) for child survival by 2015 and beyond, and gives added value to maternal health (MDG 5) investments also linking to non-communicable diseases. For preterm babies who survive, the additional burden of prematurity-related disability may affect families and health systems. Prematurity is an explicit priority in many high-income settings; however, more attention is needed especially in low- and middle-income countries where the invisibility of preterm birth as well as its myths and misconceptions have slowed action on prevention and care. Recent global attention to preterm birth hit a tipping point in 2012, with the May 2 publication of Born Too Soon: The Global Action Report on Preterm Birth and with the 2nd annual World Prematurity Day on November 17 which mobilised the actions of partners in many countries to address preterm birth and newborn health. Interventions to strengthen preterm birth prevention and care span the continuum of care for reproductive, maternal, newborn and child health. Both prevention of preterm birth and implementation of care of premature babies require more research, as well as more policy attention and programmatic investment. Declaration This article is part of a supplement jointly funded by Save the Children's Saving Newborn Lives programme through a grant from The Bill & Melinda Gates Foundation and March of Dimes Foundation and published in collaboration with the World Health Organization (WHO). The original article was published in PDF format in the WHO Report "Born Too Soon: the global action report on preterm birth (ISBN 978 92 4 150343 30). Just as notebook computers once freed us to take our computers with us, smartphones freed us to walk around with computers in our pockets, and wearables will soon free us from needing to hold a screen at all. Today, as high-quality virtual and augmented reality begins to become available at consumer prices, the "screen" will soon be all around us. But the largest long-term impact here may not merely be one of form factor, but rather one of language itself. Once wearables become small enough, cheap enough, and therefore ubiquitous enough to be accepted as part of our everyday reality, our use of language will evolve in important ways.

    1991-04-01

The purpose of this study was to determine whether American psychiatrists believe that DSM-IV is being published too soon after DSM-III-R. The authors conducted a mail survey of the attitudes of practicing psychiatrists (N = 454), residency program directors (N = 128), residents (N = 1,331), and researchers (N = 196) toward the scheduled publication of DSM-IV in the early 1990s. They found that the majority of all four groups believed that DSM-IV is being published prematurely. In contrast to respondents who believed that the timing of DSM-IV is appropriate, those who indicated that it is being published too soon had more recently completed their residency training and also believed that DSM-III-R was published prematurely. There was no association between the psychiatrists' responses and their theoretical orientation, Board certification status, ownership of the DSM manuals, the length of time they had used DSM-III, and the diagnostic manual (DSM-III or DSM-III-R) they were currently using. The belief that DSM-IV is being published too soon could contribute to underuse of DSM-IV by substantial numbers of psychiatrists. Thus, to foster compliance with it, APA must preserve in its efforts to demonstrate that the advantages of publishing it in 1993 outweigh the disadvantages of adopting yet another manual.

The efficacy of enteral nutrition in postoperative nutritional management is known, but the effects on gastrointestinal motility and nutrition have not yet been elucidated. The purpose of this study was to compare the effects of enteral and parenteral nutrition soon after open abdominal surgery on gastrointestinal motility and nutritional status. A partial resection of rectum models was prepared to compare two types of nutrient administration: enteral nutrition and total parenteral nutrition. The differences between the effects of nutrition types in terms of gastrointestinal motility and nutritional status were investigated. Enteral nutrition contributed to recovery of gastrointestinal motility and maintenance of nutritional status. Enteral nutrition should therefore be initiated soon after surgery if the gastrointestinal tract is available.

Modeling of planetary climate and weather has followed the development of tools for studying Earth, with lags of a few years. Early Earth climate studies were performed with 1-dimensionalradiative-convective models, which were soon fol-lowed by similar models for the climates of Mars and Venus and eventually by similar models for exoplan-ets. 3-dimensional general circulation models (GCMs) became common in Earth science soon after and within several years were applied to the meteorology of Mars, but it was several decades before a GCM was used to simulate extrasolar planets. Recent trends in Earth weather and and climate modeling serve as a useful guide to how modeling of Solar System and exoplanet weather and climate will evolve in the coming decade.

Here, two approaches to constrain the firn depth evolution in Antarctica are presented over the last deglaciation: output of a firn densification model and measurements of δ15N of N2 in air trapped in ice core. Since the firn densification process is largely governed by surface temperature and accumulation rate, we have investigated four ice cores drilled in coastal (Berkner Island, BI, and James Ross Island, JRI) and semi coastal (TALDICE and EPICA Dronning Maud Land, EDML) Antarctic regions. Combined with available δ15N measurements performed from the EPICA Dome C (EDC) site, the studied regions encompass a large range of surface accumulation rate and temperature conditions. While firn densification simulations are able to correctly represent most of the δ15N trends over the last deglaciation measured in the EDC, BI, TALDICE and EDML ice cores, they systematically fail to capture BI and EDML δ15N glacial levels, a mismatch previously seen for Central East Antarctic ice cores. Using empirical constraints of the EDML gas-ice depth offset during the Laschamp event (~ 41 ka), we can rule out the existence of a large convective zone as the explanation of the glacial firn model-δ15N data mismatch for this site. The good match between modelled and measured δ15N at TALDICE as well as the lack of any clear correlation between insoluble dust concentration in snow and δ15N records in the different ice cores suggest that past changes in loads of impurities are not the only main driver of glacial-interglacial changes in firn lock-in depth. We conclude that firn densification dynamics may instead be driven mostly by accumulation rate changes. The mismatch between modelled and measured δ15N may be due to inaccurate reconstruction of past accumulation rate or underestimated influence of accumulation rate in firnification models.

The US operates a diverse, evolving constellation of research and operational environmental satellites, principally in polar and geosynchronous orbits. Our current and enhanced future domestic remote sensing capability is complemented by the significant capabilities of our current and potential future international partners. In this analysis, we define "success" through the data customers' "eyes": participating in the sufficient and continuously improving satisfaction of their mission responsibilities. To successfully fuse together observations from multiple simultaneous platforms and sensors into a common, self-consistent, operational environment requires that there exist a unified calibration and validation approach. Here, we consider develop a concept for an integrating framework for absolute accuracy; long-term stability; self-consistency among sensors, platforms, techniques, and observing systems; and validation and characterization of performance. Across all systems, this is a non-trivial problem. Simultaneous Nadir Overpasses, or SNO's, provide a proven intercomparison technique: simultaneous, collocated, co-angular measurements. Many systems have off-nadir elements, or effects, that must be calibrated. For these systems, the nadir technique constrains the process. We define the term "SOON," for simultaneous overpass off nadir. We present a target architecture and sensitivity analysis for the affordable, sustainable implementation of a global SOON calibration/validation network that can deliver the much-needed comprehensive, common, self-consistent operational picture in near-real time, at an affordable cost.

There have been a number of spontaneous reports of sudden unexpected death soon after the administration of Infanrix hexa (combined diphtheria, tetanus, acellular pertussis, hepatitis B, inactivated poliomyelitis and Haemophilus influenza type B vaccine). The manufacturer, GlaxoSmithKline (GSK), submits confidential periodic safety update reports (PSURs) on Infanrix hexa to the European Medicines Agency (EMA). The latest is the PSUR 19. Each PSUR contains an analysis of observed/expected sudden deaths, which shows that the number of observed deaths soon after immunisation is lower than that expected by chance. This commentary focuses on that aspect of the PSUR which has a bearing on policy decisions. We analysed the data provided in the PSURs. It is apparent that the deaths acknowledged in the PSUR 16 were deleted from the PSUR 19. The number of observed deaths soon after vaccination among children older than one year was significantly higher than that expected by chance once the deleted deaths were restored and included in the analysis. The manufacturer must explain the figures that have been submitted to the regulatory authorities. The procedures undertaken by the EMA to evaluate the manufacturer's claims in the PSUR need to be reviewed. The Drugs Controller General of India nearly automatically accepts drugs and vaccines approved by the EMA. There is a need to reappraise the reliance on due diligence by the EMA.

During previous research expeditions to Siberia, Alaska and Antarctica, it was observed that glaciers and ice wedges contained bacterial cells that became motile as soon as the ice melted. This phenomenon of live bacteria in ice was first documented for microbes in ancient ice cores from Vostok, Antarctica. The first validly published species of Pleistocene bacteria alive on Earth today was Carnobacterium pleistocenium. This extremophile had remained for 32,000 years, encased in ice recently exposed in the Fox Tunnel of Alaska. These frozen bacteria began to swim as soon as the ice was thawed. Dark field microscopy studies revealed that large numbers of bacteria exhibited motility as soon as glacial ice was melted during our recent Expeditions to Alaska and Antarctica led to the conclusion that microbial life in ice was not a rare phenomenon. The ability of bacteria to remain alive while frozen in ice for long periods of time is of great significance to Astrobiology. In this paper, we describe the recent observations and advance the hypothesis that life in ice provides valuable clues to how we can more easily search for evidence of life on the Polar Caps of Mars, comets and other icy bodies of our Solar System. It is suggested that cryopanspermia may have played a far more important role in Origin of Life on Earth and the distribution of Life throughout the Cosmos and than previously thought possible.

Detecting astronomical gravitational waves will soon open a new window on the universe. The effects of gravitational waves have already been seen indirectly, but a direct observation of these waves will test Einstein's theory of general relativity under the most extreme conditions. The Laser Interferometer Gravitational-Wave Observatory, or LIGO, will soon begin searching for gravitational waves, and the first direct detections are likely in the next few years. To help LIGO detect as many gravitational waves as possible, a major research effort is underway to accurately predict the expected waves. In this presentation, I will discuss new supercomputer simulations of merging black holes--some of the brightest sources of gravitational waves--that I have completed using the Spectral Einstein Code (http://www.black-holes.org/SpEC.html).

In 1965, just four years after the first planar integrated circuit (IC) was discovered, Cordon Moore observed that the number of transistors per integrated circuit had grown exponentially. He predicted that this would continue, and the media soon began to call his prophesy "Moore's Law" For nearly forty years, Moore's Law has been validated by the technological progress achieved in the semiconductor industry. Now, however, industry experts are warning of a "Red Brick Wall" that may soon block the continued scaling predicted by by Moore's Law. The "red bricks" in the wall are those areas of technical challenge for which no known manufacturable solution exists. One such "brick" is the challenge of finding a new material and processing technology to replace the metals used today to interconnect transistors on a chip.

Algorithms for failure detection and identification for redundant noncolinear arrays of single degree of freedom gyros and accelerometers are described. These algorithms are optimum in the sense that detection occurs as soon as it is no longer possible to account for the instrument outputs as the outputs of good instruments operating within their noise tolerances, and identification occurs as soon as it is true that only a particular instrument failure could account for the actual instrument outputs within the noise tolerance of good instruments. An estimation algorithm is described which minimizes the maximum possible estimation error magnitude for the given set of instrument outputs. Monte Carlo simulation results are presented for the application of the algorithms to an inertial reference unit consisting of six gyros and six accelerometers in two alternate configurations.

  19. Changes of Serum Calcium Concentration, Frequency of Ruminal Contraction and Feed Intake Soon after Parturition of Dairy Cows Fed Difructose Anhydride III.

Requirements to control the large decrease in serum calcium (Ca) due to parturition and to increase the feed intake soon after parturition have been well accepted in dairy cows. This study was aimed to investigate the feed intake affected by serum Ca concentration with difructose anhydride (DFA) III supplement in dairy cows soon after parturition. Fourteen transition Holstein cows were divided into DFA and control (CONT) groups within 1 to 5 parity variations in each group. Measurement schedule for an individual cow was from 14 d before parturition to 7 d following parturition. The cows in DFA group were supplied 0.2 kg/head/d of DFA III feed containing 40 g of pure DFA III while the cows in CONT group received no DFA III. Other feeding procedures were the same for all cows in both groups. At parturition (d 0), serum Ca concentration sharply declined in both groups (p<0.05). Time interval for recovery from decreased serum Ca to its normal range (>9.0 mg/dL) tended to be faster in DFA group (12 h) than in the CONT group (48 h), but the differences were not significant. Active ruminal contraction was observed in DFA group at following parturition of d 1 (p<0.05), d 3 (p<0.05), and d 5 (p<0.01). Dry matter (DM) intake did not differ between the groups. However, positive correlations were observed between serum Ca concentration and ruminal contraction (p<0.001), and between ruminal contraction and DM intake (p<0.001) during following parturition. According to multiple regression analysis (R(2) = 0.824, p<0.001), the DM intake was positively affected by serum Ca concentration and ruminal contraction. These results suggest that feed intake soon after parturition in dairy cows can be increased by improvement of serum Ca concentration and active ruminal contraction, but DFA III supplementation in this study did not improve the lower serum Ca concentration due to parturition.

  1. History of the Buttonhole Technique.

    PubMed

    Misra, Madhukar

    2015-01-01

The constant side method of access cannulation in hemodialysis, popularly known as the 'buttonhole' method, has an interesting history. Dr. Zbylut J. Twardowski, a Polish nephrologist, discovered this technique by pure serendipity in 1972. A patient with a complicated vascular access history and limited options for cannulation was repeatedly 'stuck' at the same sites by a nurse. Soon it was noticed that the cannulation at the same spot became easier with time. Since the needles were being reused, the sharpness of the needles decreased with time and the bluntness of the needle seemed to minimize the damage to the cannulation tract (another serendipity!). This method soon became popular among patients, and many patients started using this technique. This chapter traces the invention of this technique and its subsequent development following Dr. Twardowski's emigration to the USA. © 2015 S. Karger AG, Basel.

  2. Cancer-related fatigue: can exercise physiology assist oncologists?

    PubMed

Lucía, Alejandro; Earnest, Conrad; Pérez, Margarita

    2003-10-01

Most patients with cancer experience fatigue, a severe activity-limiting symptom with a multifactorial origin. To avoid cancer-related fatigue, patients are frequently advised to seek periods of rest and to reduce their amount of physical activity. This advice is reminiscent of that formerly given to patients with heart disease. However, such recommendations can paradoxically compound symptoms of fatigue, since sedentary habits induce muscle catabolism and thus cause a further decrease in functional capacity. By contrast, there is scientific evidence that an exercise programme of low to moderate intensity can substantially reduce cancer-related fatigue and improve the quality of life of these patients. Current knowledge, combined with findings soon to be published, could launch new opportunities for patients with cancer. In this new century, exercise physiology could soon prove to be very useful for oncologists.

  3. Indirect effects of immunological tolerance to a regular dietary protein reduce cutaneous scar formation.

    PubMed

Cantaruti, Thiago Anselmo; Costa, Raquel Alves; de Souza, Kênia Soares; Vaz, Nelson Monteiro; Carvalho, Cláudia Rocha

    2017-07-01

Oral tolerance refers to the specific inhibition of immune responsiveness to T-cell-dependent antigens contacted through the oral route before parenteral immunization. Oral tolerance to one protein does not inhibit immune responses to other unrelated proteins, but parenteral injection of tolerated antigens plus adjuvant into tolerant, but not normal, mice inhibits immune responses to antigens injected concomitantly or soon thereafter. The inhibitory effect triggered by parenteral injection of tolerated proteins is known as bystander suppression or indirect effects of oral tolerance. Intraperitoneal injection of ovalbumin (OVA) plus alum adjuvant in OVA-tolerant mice soon before skin injury inhibits inflammation and improves cutaneous wound healing. However, as OVA is not a regular component of mouse chow, we tested whether indirect effects could be triggered by zein, the main protein of corn that is regularly present in mouse chow. We show that intraperitoneal injection of a single dose (10 μg) of zein plus alum adjuvant soon before skin injury in mice reduces leucocyte infiltration but increase the number of T cells and the expression of resistin-like molecule-α (a marker of alternatively activated macrophages) in the wound bed, increases the expression of transforming growth factor-β 3 in the newly formed epidermis and reduces cutaneous scar formation. These results suggest that indirect effects of oral tolerance triggered by parenteral injection of regular dietary components may be further explored as one alternative way to promote scarless wound healing. © 2017 John Wiley & Sons Ltd.

    2008-08-01

The Swift spacecraft detects and autonomously observes ˜100 Gamma Ray Bursts (GRBs) per year, ˜96% of which are detected by the X-ray telescope (XRT). GRBs are accompanied by optical transients and the field of ground-based follow-up of GRBs has expanded significantly over the last few years, with rapid response instruments capable of responding to Swift triggers on timescales of minutes. To make the most efficient use of limited telescope time, follow-up astronomers need accurate positions of GRBs as soon as possible after the trigger. Additionally, information such as the X-ray light curve, is of interest when considering observing strategy. The Swift team at Leicester University have developed techniques to improve the accuracy of the GRB positions available from the XRT, and to produce science-grade X-ray light curves of GRBs. These techniques are fully automated, and are executed as soon as data are available.

Lewis M. Rutherfurd (1816-1892) was a wealthy amateur practitioner of science, making notable contributions to the optics of astronomy and spectroscopy. He graduated from Williams College at the age of 18 and soon began a career in law. Not long thereafter he was fortunate enough to acquire sufficient wealth through marriage to permit him to pursue scientific activity, for which he had gained interest. He soon became interested in photographing the heavens to obtain quantitative data regarding star motions. A few years later he developed a spectrometer with which he obtained some of the best attainable stellar spectra of the time. Rutherfurd is not widely known because he did not fully report his work; but through correspondence and instrument development he had a profound influence on his contemporaries. He was made a trustee of Columbia College in 1858, and in 1881 he helped to found a department of Geodesy and Practical Astronomy there.

A 42-year-old patient with epilepsy was admitted to the hospital for fever and generalized skin rash. He has known allergy to phenytoin. Valproate was started in 2012, but failed to control his seizure despite gradual increase in dosage. Phenobarbitone was added 16 days before admission and was stopped on admission. He was treated with beta-lactam antibiotics. The rash subsided gradually after the cessation of phenobarbitone. Lacosamide was subsequently added for seizure control. Unfortunately, he developed drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome soon after introduction of lacosamide that required the use of systemic steroid for acute hepatitis. A cross-reactivity with lacosamide was suspected in view of the rapid onset of DRESS syndrome after the initial rash resolution and soon after the introduction of lacosamide. We postulated that the rapid onset of DRESS syndrome may be related to the aromatic ring that is in common among phenytoin, phenobarbitone, and lacosamide.

Americans were stunned when pandemic influenza hit the United States in 1918. Recent advances in bacteriology and public health allowed Americans to imagine a future free of infectious disease, even as their familiarity with influenza tempered their fears of it. They soon realized this influenza was something unprecedented, as it shocked them with its pace, virulence, mortality patterns, and symptoms. Patients endured and frequently succumbed to a miserable illness, their suffering often made worse by the chaotic circumstances the epidemic produced in families and communities and shaped in significant and sometimes discriminatory ways by their gender, class, and race. While the nation's public culture soon forgot the epidemic, it lived on in lives changed irrevocably by its consequences. As they face present and future influenza pandemics, Americans can learn from this earlier experience, guarding against identity-based discrimination and acknowledging and remembering the grief and loss fellow citizens suffered.

  18. Does timing of surgery matter in fragility hip fractures?

    2010-01-01

The effect of delay of surgery on the geriatric hip fractures has been a subject of interest in the past two decades. While the elderly patients will not tolerate long periods of immobilization, it is still unclear how soon these surgeries need to be performed. A review of existing literature was performed to examine the effect of timing of surgery on the different outcome parameters of these patients. Although there is conflicting evidence that early surgery would improve mortality, there is widespread evidence in the literature that other outcomes including morbidity, the incidence of pressure sores, and the length of hospital stay could be improved by shortening the waiting time of hip fracture surgery. We concluded that it is beneficial to the elderly patients to receive surgical treatment as an urgent procedure as soon as the body meets the basic anesthetic requirements. PMID:21057992

    Wells, D. C.

    1984-01-01

During the next 5-10 years it is likely that the bit density available in digital mass storage systems (magnetic tapes, optical and magnetic disks) will be increased to such an extent that it will greatly exceed that of the conventional photographic emulsions like IIIaJ which are used in astronomy. These developments imply that it will soon be advantageous for astronomers to use microdensitometers to completely digitize all photographic plates soon after they are developed. Distribution of digital copies of sky surveys and the contents of plate vaults will probably become feasible within ten years. Copies of other astronomical archieves (e.g., Space Telescope) could also be distributed with the same techniques. The implications for designers of future microdensitometers are: (1) there will be a continuing need for precision digitization of large-format photographic imagery, and (2) that the need for real-time analysis of the output of microdensitometers will decrease.

    NASA Astrophysics Data System (ADS)

    Biasi, J.; Kirschvink, J. L.

    2017-12-01

Here we present paleointensity results from within the South Atlantic Anomaly (SAA), which is a large non-dipole feature of the geomagnetic field. Within the area of the SAA, anomalous declinations, inclinations, and intensities are observed. Our results suggest that the SAA has been present for at least 5 Ma. This is orders-of-magnitude greater than any previous estimate, and suggests that some non-dipole features do not `average out' over geologic time, which is a fundamental assumption in all paleodirectional studies. The SAA has been steadily growing in size since the first magnetic measurements were made in the South Atlantic, and it is widely believed to have appeared 400 years ago. Recent studies from South Africa (Tarduno et al. (2015)) and Tristan da Cunha (Shah et al. (2016)) have suggested that the SAA has persisted for 1 ka and 96 ka respectively. We conducted paleointensity (PI) experiments on basaltic lavas from James Ross Island, on the Antarctic Peninsula. This large shield volcano has been erupting regularly over the last 6+ Ma (dated via Ar/Ar geochronology), and therefore contains the most complete volcanostratigraphic record in the south Atlantic. Our PI experiments used the Thellier-Thellier method, the IZZI protocol, and the same selection criteria as the Lawrence et al. (2009) study of Ross Island lavas (near McMurdo Station), which is the only comparable PI study on the Antarctic continent. We determined an average paleointensity at JRI of 13.8±5.2 μT, which is far lower than what we would expect from a dipole field (55 μT). In addition, this is far lower than the current value over James Ross Island of 36 μT. These results support the following conclusions: The time-averaged field model of Juarez et al. (1998) and Tauxe et al. (2013) is strongly favored by our PI data. The SAA has persisted over James Ross Island for at least 5 Ma, and has not drifted significantly over that time. The strength of non-dipole features such as the SAA scales with the dipole moment of the earth. Non-dipole features like the SAA can survive geomagnetic reversals. The fundamental assumption that non-dipole features of the geomagnetic field are `averaged out' over geologic timescales needs to be reconsidered.

We tested the hypothesis that an increase in the anti-inflammatory cholinergic pathway, when induced by pyridostigmine (PY), may modulate subtypes of lymphocytes (CD4+, CD8+, FOXP3+) and macrophages (M1/M2) soon after myocardial infarction (MI) in rats. Wistar rats, randomly allocated to receive PY (40 mg·kg−1·day−1) in drinking water or to stay without treatment, were followed for 4 days and then were subjected to ligation of the left coronary artery. The groups—denominated as the pyridostigmine-treated infarcted (IP) and infarcted control (I) groups—were submitted to euthanasia 3 days after MI; the heart was removed for immunohistochemistry, and the peripheral blood and spleen were collected for flow cytometry analysis. Noninfarcted and untreated rats were used as controls (C Group). Echocardiographic measurements were registered on the second day after MI, and heart rate variability was measured on the third day after MI. The infarcted groups had similar MI areas, degrees of systolic dysfunction, blood pressures, and heart rates. Compared with the I Group, the IP Group showed a significant higher parasympathetic modulation and a lower sympathetic modulation, which were associated with a small, but significant, increase in diastolic function. The IP Group showed a significant increase in M2 macrophages and FOXP3+ cells in the infarcted and peri-infarcted areas, a significantly higher frequency of circulating Treg cells (CD4+CD25+FOXP3+), and a less extreme decrease in conventional T cells (CD25+FOXP3−) compared with the I Group. Therefore, increasing cholinergic modulation with PY induces greater anti-inflammatory cell recruitment soon after MY in rats. PMID:26791829

Therefore, increasing cholinergic modulation with PY induces greater anti-inflammatory cell recruitment soon after MY in rats. Copyright © 2016 the American Physiological Society.

The Scottish Women's Hospitals (SWH), a unique health institution in the history of medicine, staffed entirely by women, was founded soon after the outbreak of the First World War, August 12, 1914 in Edinburgh, by the National Union of Women's Suffrage Societies. The founder and the main driving force behind this organisation was Dr. Elsie Inglis (1864-1917). Although her proposition to the British War Office had been rejected, she offered her services to the Allies (France, Belgium, Russia and Serbia). The first 200 bed SWH unit was sent to France in November 1914, and soon after followed other units, so at the end there were 13 very well equipped SWH units working in the various theatres of war in Belgium, Serbia, Russia, Rumania and Greece. The first unit of SWH came to Serbia in early January 1915, and was located at Kragujevac. Soon after, three other SWH units arrived to Serbia and were stationed at Mladenovac, Valjevo and Lazarevac. It was an enormous help to Serbia, full of wounded and sick people, due to the dreadful typhus epidemic which was devastating the country. A large SWH unit, attached to the Southern Slav Volunteer Division, had worked on the Dobrudja front, and there were three hospitals and a special transport unit on the Salonika Front, which were all engaged in the treatment of Serbian wounded soldiers until the end of the First World War. Two other SWH units, located in France, were treating the Serbian refugees. Serving bravely and honorably on the various theatres of war, the legendary Scottish Women's Hospitals made enormous contributions to the allied war efforts, and helped Serbian people a great deal.

On January 17, 2001, a 39-year old female with sudden abdominal pain was admitted to her neighboring outpatient clinic and diagnosed as suspicious of infectious enteritis. However, on the next day (January 18, 2001) she was soon transferred to Toyoshina Red Cross Hospital with the chief complaint of severe abdominal pain, high fever, and of conspicuous leucocytosis. Laboratory data on her admission demonstrated apparent signs of inflammation and she was soon undergone an emergency operation. Neisseria gonorrhoeae was recovered from her ascetic fluid, otherwise Chlamydia EIA was negative. The antibiotic chemotherapy of minocycline (200 mg/day) was continued for the first 9 days and sulbactam/cefoperazone (2 g/day) had been administered for the first 5 days. Her symptoms were discontinued on her 10th hospital day, and she was discharged on the 14th hospital day. There have been few reported cases in Japan of bacterial peritonitis due to N. gonorrhoeae. This is, to the best of our knowledge, the first reported case of N. gonorrhoeae peritonitis in Japan.

The traditional implementation of the risk-adjusted Bernoulli cumulative sum (CUSUM) chart for monitoring surgical outcome quality requires waiting a pre-specified period of time after surgery before incorporating patient outcome information. We propose a simple but powerful implementation of the risk-adjusted Bernoulli CUSUM chart that incorporates outcome information as soon as it is available, rather than waiting a pre-specified period of time after surgery. A simulation study is presented that compares the performance of the traditional implementation of the risk-adjusted Bernoulli CUSUM chart to our improved implementation. We show that incorporating patient outcome information as soon as it is available leads to quicker detection of process deterioration. Deterioration of surgical performance could be detected much sooner using our proposed implementation, which could lead to the earlier identification of problems. © The Author 2017. PubMed

Kelly, Aaron S; Fox, Claudia K

    2017-08-01

This review provides a rationale for the use of pharmacotherapy in pediatric weight management, summarizes results of some of the key pediatric clinical trials of approved and "off-label" obesity medications, introduces new options in the pediatric pipeline, and offers a glimpse into the future of pediatric obesity medicine. Despite the need for adjunctive treatments to enhance the outcomes of lifestyle modification therapy among youth with obesity, none of the obesity medications evaluated to date have been shown to meaningfully reduce BMI or cardiometabolic risk factors. Promising medications recently approved for the treatment of obesity in adults will soon be tested in pediatric trials, offering hope that new therapeutic options will soon be available. As new medications are approved to treat pediatric obesity, it will be important to evaluate the safety and efficacy of combination pharmacotherapy and investigate predictors of response. Application of precision medicine approaches to the field of pediatric obesity management will improve the long-term outlook for the tens of millions of youth afflicted with this serious and recalcitrant disease.

  3. Born Too Soon: The global epidemiology of 15 million preterm births

    PubMed Central

    2013-01-01

This second paper in the Born Too Soon supplement presents a review of the epidemiology of preterm birth, and its burden globally, including priorities for action to improve the data. Worldwide an estimated 11.1% of all livebirths in 2010 were born preterm (14.9 million babies born before 37 weeks of gestation), with preterm birth rates increasing in most countries with reliable trend data. Direct complications of preterm birth account for one million deaths each year, and preterm birth is a risk factor in over 50% of all neonatal deaths. In addition, preterm birth can result in a range of long-term complications in survivors, with the frequency and severity of adverse outcomes rising with decreasing gestational age and decreasing quality of care. The economic costs of preterm birth are large in terms of immediate neonatal intensive care, ongoing long-term complex health needs, as well as lost economic productivity. Preterm birth is a syndrome with a variety of causes and underlying factors usually divided into spontaneous and provider-initiated preterm births. Consistent recording of all pregnancy outcomes, including stillbirths, and standard application of preterm definitions is important in all settings to advance both the understanding and the monitoring of trends. Context specific innovative solutions to prevent preterm birth and hence reduce preterm birth rates all around the world are urgently needed. Strengthened data systems are required to adequately track trends in preterm birth rates and program effectiveness. These efforts must be coupled with action now to implement improved antenatal, obstetric and newborn care to increase survival and reduce disability amongst those born too soon. Declaration This article is part of a supplement jointly funded by Save the Children's Saving Newborn Lives programme through a grant from The Bill & Melinda Gates Foundation and March of Dimes Foundation and published in collaboration with the Partnership for Maternal, Newborn and Child Health and the World Health Organization (WHO). The original article was published in PDF format in the WHO Report "Born Too Soon: the global action report on preterm birth" (ISBN 978 92 4 150343 30), which involved collaboration from more than 50 organizations. The article has been reformatted for journal publication and has undergone peer review according to Reproductive Health's standard process for supplements and may feature some variations in content when compared to the original report. This co-publication makes the article available to the community in a full-text format. PMID:24625129
